과제 지향적 활동이 수부손상환자의 손 기능에 미치는 효과 (Effects of task-oriented activities on hand functions in patients with hand injuries)

  • 본 연구는 수부손상환자를 대상으로 과제 지향적 활동이 손 기능에 미치는 효과를 알아보고자 하였다. 2011년 12월부터 2012년 10월까지 경남 창원에 위치하는 C병원에 내원한 16명의 수부손상환자를 두 그룹으로 무작위 할당한 후 실험군에 과제 지향적 활동을 1일 1회, 주 3회, 4주 동안 적용하고 그룹 간 비교를 실시하였다. 과제 지향적 활동군은 캐나다 작업수행측정(Canadian Occupational Performance Measure; COPM)을 통해 대상자가 선호하는 일상생활활동영역에 대한 과제를 제공하였다. 이때 중재의 효과를 검증하기 위해 손 기능 평가로 장악력 검사, 파악력 검사, 엄지의 맞섬, 손가락의 벌림 범위와, Purdue pegboard test, Disabilities of the Arm, Shoulder, and Hand(DASH)를 사용하였다. 중재 전후 비교에서 과제 지향적활동군은 모든 손 기능 평가에 유의하게 향상되었고(p<.05), 대조군의 손 기능도 유의하게 향상되었다(p<.05). 그룹 간 비교에서 과제 지향적 활동군은 손끝집기, 측면집기, 세점집기, Purdue pegboard test, DASH에서 대조군과 유의한 차이가 있었으나(p<.05), 장악력 검사와 엄지의 맞섬 및 손가락의 벌림 범위는 그룹 간 유의한 차이가 없었다. 본 연구의 결과를 통해 과제지향적 활동이 수부손상환자의 손 기능 향상에 더 효과적임을 알 수 있었다.

  • Even though the automation of industrial machinery has dramatically increased, most industries still require human hands for the production of goods. Our hands are essential to our existence. For this reason, hand injuries caused by industrial accidents have become an important issue lately. Most hand injuries are external wounds and constitute 38.6% of disabling industrial accidents. This significant statistic cannot be ignored by rehabilitation programs. The rate of success in operations of finger and hand injuries has increased, but relatively less consideration has been given to physical therapy, rehabilitation, research, or systematic installment in terms of welfare, which would help the injured carry on the nominal life they lived before their injury. Therefore, it is necessary to study systematically all related aspects to provide patients with physical therapy, rehabilitation, and social welfare, in order to restore their social, professional, and economical capabilities. Physical considerations, functional usability, and cosmetic restoration, as well as the patients' mental state must be addressed. The results of the study shows several problems. Medical appliances, manpower, and the environment of the treatment rooms are very poor. The patients cannot receive treatment early enough nor enough treatment because of too many patients for too few physical therapists. Close cooperation between physical therapists and doctors, nurses, or related departments is lacking. Furthermore, it is irrational that industrial accident premiums for the exert; e treatment of hand injury patients are not itemized. Lack of recognition of the importance of specialty in hand injury therapists leads to the lack of professionalization of systematic hand injury treatment In order to solve these problems, the professonalism of physical therapy should be acknowledged and particular treatment courses should be available and/or required. Based on the understanding of the hand injury patients' needs, new theories of physical therapy should be developed and modem medical appliances and facilities should be available, in order to provide the patients with qualitative medical treatment, which then will facilitate the patients' recovery.

American Football Injuries In Korea

  • American football is currently played in 29 Universities in Korea, It is a contact sport where injuries are inevitable. This study was undertaken to determine the incidence and the anatomical distribution of injury based on evaluation of Yonsei University American football team. A total 74 injuries was reported among 77 players Knee, ankle and hand were vulnerable to injury. Operations were performed on $9(12.2\%)$ of the injuries. Catastrophic injuries can occur in this sport, but none has happened in Yonsei University team during the 20 years for which the sports has been practised. The frequency of injuries and their profile resemble those in US-based studies so it is suggested that the precautions and preventive measures recommended in the USA should be applied and followed in Korea.

Severity of grinder injuries and related factors compared with other high-rotation cutting tool injuries: a multicenter retrospective study from 2011 to 2018

  • Purpose: This study aimed to identify the characteristics of patients injured by high-rotation cutting tools and the factors related to the severity of their injuries. Methods: Adult patients (≥18 years), who presented to the emergency department (ED) after a high-rotation cutting tool injury and who were registered in the Korean Emergency Department-based Injury In-Depth Surveillance (2011-2018) database, were included. Patients' demographic characteristics, injury-related factors, and Injury Severity Scores were collected. All included cases were categorized into two groups according to the tool that caused the injury: grinder versus nongrinder. The characteristics of the two groups were compared, and the factors associated with the severity of injuries were investigated. Results: Among 8,697 ED visits, 4,603 patients had been using a grinder and 4,094 had been using a nongrinder tool. The most frequently injured body part while using a grinder was the hand (46.4%), followed by the head (23.0%). While using a nongrinder tool, the most frequently injured body part was also the hand (64.0%), followed by the lower leg (11.4%). The odds of a severe injury were affected by patient age (odds ratio [OR], 1.024; 95% confidence interval [CI], 1.020-1.028) and using a grinder (OR, 2.073; 95% CI, 1.877-2.290). The odds of a severe injury using a grinder were higher in arm injuries (OR, 1.60; 95% CI, 1.40-1.83) and multiple-part injuries (OR, 1.998; 95% CI, 1.639-2.437). The odds of a severe injury using a grinder were lower for head injuries (OR, 0.481; 95% CI, 0.297-0.781). Conclusions: Injuries from grinders were more likely to affect the head and neck than nongrinder injuries, despite the lower severity. The current lack of regulations on grinders in occupational safety and health standards warrants relevant legislation and the development of applicable safety equipment.

급성 수부 손상시 즉시 족지 전이술 (Immediate Toe-to-Hand Transfer in Acute Hand Injuries)

  • From 1994-2001, 25 mutilated digits were reconstructed with immediate toe-to-hand transfer in acute hand injury in 21 patients. There were 15 cases of great toe-to-hand transfer(partial great toe transfer 8 cases, modified wrap-around procedure 2 cases, and trimmed great toe transfer 2 cases) for thumb reconstruction, 2 cases of second toe transfer for index reconstruction, and 4 cases of simultaneous two toe-to-hand transfer(great toe & second toe transfer 1 case, bilateral second-toe transfer 2 cases, combined second & third toe transfer 1 case) for reconstruction of multiple digit amputations. Two cases of emergency exploration(2/25, 8%) were successfully salvaged. The incidence of emergency exploration and postoperative infection was not significantly different from that of the elective toe-to-hand transfer cases. Duration of industrial insurance coverage was 225 days, which is much shorter than that of elective cases. Among 43% of patients maintained their original job even after injury and immediate toe-to-hand transfer. The subjective satisfaction self- assessment scores of aesthetic appearance and function on the new reconstructed thumb were 80 and 88 in average, respectively, over a total score of 100. These were higher than those of reconstruction of other digits, but lower than those of elective reconstruction. The donor site after harvest of the great toe was mostly unsatisfied in a view of appearance. Immediate toe-to-hand transfer provides many advantages over elective procedure in acute hand injuries such as single stage reconstruction, shortened convalescent period, early return to work and efficient socio-economic factor. Furthermore because there were no significant differences in success rate, frequency of complications or ultimate functional result, immediate toe-to-hand transfer is a safe and reliable procedure in case of limited indications for acute digits loss.

  • Objectives : This study is to applied effectively to the acupuncture treatment for disorders following whiplash injuries. Methods : This study suggests five various ways and principtes of treating disorders following whiplash injuries by means of acupuncture. First principle is that acupuncture treatment is carried out on acupoint far from cervical spine at acute stage, on the other hand, Extraordinary Vessel and other acupuncture treatment such as five element acupuncture treatment is applied at chronic stage in order to control balance disoders due to cervical pathology. Second is acupuncture treatment will be performed in group of three part of Meridian System-Yangmyung, Taeyang, Soyang-according to cervical kinetic, its characteristic and location of pain. Third is acupuncture treatment could be performed on basis of theory of Eohyel-disorders and disturbance of blood circulation. Fourth is to decrease tension of SCM muscle, because disorders following whiplash injuries has much association to injury of CM muscle and is simillar to tension syndroms of SCM muscle. fifth is taping therapy could control and decrease disorders following whiplash injuries based on spiral balance taping theory. Conclusions : These results suggest that acupuncture treatment for disorders following whiplash injuries will significally decrease pain and control syndrom caused by cervical disorder.

  • 손가락은 일상생활과 스포츠 활동 중에 가장 흔하게 손상될 수 있는 구조물이다. 손가락 손상은 손가락의 뼈와 인대, 연골을 포함한 연부조직까지 다양한 구조물의 손상을 포함한다. 특수 표면코일을 이용한 고해상도 3 테슬라 자기공명영상을 이용하여 복잡한 연부조직 구조물들을 평가할 수 있다. 손가락과 손의 기본 해부학과 흔한 병변의 자기공명영상 소견들에 대한 기존의 보고들이 있다. 손가락의 인대, 건, 도르래 등 여러 구조물의 기본 해부학과 흔한 외상성 병변의 영상학적 소견의 이해는 다양한 손의 외상성 병변에 대한 정확한 평가를 가능하게 한다. 본 눈문에서는 손가락의 기본 해부학적 구조 및 흔한 외상의 자기공명영상 소견을 복습하고, 실제 수술장 소견과 연관 지어 이해를 돕고자 한다.

성장기 태권도 선수의 경기 손상 (Match Injuries Related to Adolescent Taekwondo Athletes)

  • 목적: 성장기 태권도 선수들의 경기 손상을 분석하고 그 예 방법을 알아보고자 하였다. 대상 및 방법: 2005년 5월과 2006년 5월의 광주광역시에서 열린 전국 중 고교 태권도 대회에서 발생한 경기 손상 162예를 분석하였다. 결과: 경기 손상 162예에서 족부와 수부 손상이 많았고 골절 및 개방성 탈구, 개방성 골단판 골절 등의 심한 손상도 35예 있었다. 태권도 선수의 빠른 속도와 파괴력은 족부 무지가 접질릴 때 개방성 탈구를 가져 올 수 있고 가격 족부와 피격 상지에 골절을 가져 올 수 있다. 결론: 부상 선수들의 적절한 치료와 함께 훈련 손상 및 경기 손상이 누적되지 않도록 충분한 휴식이 요구되며, 경기 손상의 예방을 위해 경기장 매트, 발등 보호대 손 장갑의 보완과 팔꿈치 보호대의 착용 등이 필요할 것으로 사료된다.

Aging Workers and Trade-Related Injuries in the US Construction Industry

  • The study was designed to identify any trends of injury type as it relates to the age and trade of construction workers. The participants for this study included any individual who, while working on a heavy and highway construction project in the Midwestern United States, sustained an injury during the specified time frame of when the data were collected. During this period, 143 injury reports were collected. The four trade/occupation groups with the highest injury rates were laborers, carpenters, iron workers, and operators. Data pertaining to injuries sustained by body part in each age group showed that younger workers generally suffered from finger/hand/wrist injuries due to cuts/lacerations and contusion, whereas older workers had increased sprains/strains injuries to the ankle/foot/toes, knees/lower legs, and multiple body parts caused by falls from a higher level or overexertion. Understanding these trade-related tasks can help present a more accurate depiction of the incident and identify trends and intervention methods to meet the needs of the aging workforce in the industry.

전방십자인대 손상의 진단 (Diagnosis of Anterior Cruciate Ligament Injury)

  • Recently, the incidence of anterior cruciate ligament injury is rapidly increasing because many young Korean are actively participating in sports activities including basketball, skiing and soccer. Sometimes acute ACL injuries arc misdiagnosed as sprain because of pain and muscle spasm. An accurate physical examination and careful history taking are very important to diagnose the ACL injuries. Therefore author described the skill of history taking and the technique of physical examination to diagnose ACL injuries. The most sensitive test is the Lachman's test, which is performed with the knee in 200 of flexion, the femur stabilized with full flexed examiner's knee placed behind patient's distal thigh and one hand of examiner, and the proximal tibia grasped with the other. The pivot shift test is a passive motion test that attempts to subluxate the tibia anteriorly, then have it reduce rapidly. The pivot shift test is very sensitive test to diagnose the chronic ACL injuries.
