Background: Breast cancer is commonly diagnosed at late stages in countries with limited resources. In Morocco, breast cancer is ranked the first female cancer (36.1%) and screening methods could reduce the proportion presenting with a late diagnosis. Morocco is currently adopting a breast cancer screening program based on clinical examination at primary health facilities, diagnosis at secondary level and treatment at tertiary level. So far, there is no systematic information on the performance of the screening program for breast cancer in Morocco. The aim of this study was to analyze early performance indicators. Materials and Methods: A retrospective evaluative study conducted in Temara city. The target population was the entire female population aged between 45-70 years. The study was based on process and performance indicators collected at the individual level from the various health structures in Tamara between 2009 and 2011. Results: A total of 2,350 women participated in the screening program; the participation rate was 35.7%. Of these, 76.8% (1,806) were married and 5.2% (106) of this group had a family history of breast cancer. Of the women who attended screening, 9.3% (190) were found to have an abnormal physical examination findings. A total of 260 (12.7%) were referred for a specialist consultation. The positive predictive value of clinical breast examination versus mammography was 23.0%. Forty four (35.5%) of the lesions found on the mammograms were classified as BI-RADs 3; 4 or 5 category. Cancer was found in 4 (1.95%) of the total number of screened women and benign cases represented 0.58%. Conclusions: These first results of the programme are very encouraging, but there is a need to closely monitor performance and to improve programme procedures with the aim of increasing both the participation rate and the proportion of women eligible to attend screening.
Purpose: To study the prevalence of CIN2+ diagnosis in women with atypical Papaniculoau (Pap) smears to suggest appropriate management option for Thai health care. Materials and Methods: Data from all patients with liquid based cytology with human papillomavirus (HPV) testing between May 2013 - May 2016 were collected from medical records. Women with atypical cervical Pap smears were recruited. Results for age, HPV testing, HPV 16, 18, 45 and other genotypes tested, colposcopic examination and histopathological assessment were all collected. Atypical smears were defined as atypical squamous cells of undetermined significance (ASC-US) and atypical squamous cells cannot be exclude high grade squamous intraepithelial lesion (ASC-H). Results: A total of 2,144 cases were recruited. Twenty six women with ASC-US on cytology had high risk (HR) HPV detection while eight cases with ASC-H had HR-HPV (40.0% VS 72.7%, p=0.005). Among the 26 women with ASC-US cytology and positive HR-HPV, HPV type 16 (n=8, 30.8%), type 18 (n=1, 3.8%), type 45 (n=1, 3.8%) and other HPV types (n=17, 65.4%) were found. Eight women with ASC-H and positive HR-HPV demonstrated type 16 (n=6, 75%) and other HPV types (n=2, 25%). Fifty seven women with ASC-US had normal colposcopy, CIN1 and CIN2+ at percentages of 80.7 (46/57), 14.0 (8/57) and 5.3 (3/57), respectively. In the ASC-H group, 7 out of 10 women had normal colposcopy and three (30%) had CIN2+ results. Conclusions: In women with ASC-US cytology, immediate colposcopy is highly recommended. HPV testing can be performed if colposcopy is not an available option because there was high prevalence (5.3%) of CIN2+ in our findings. ASCCP recommendations for ASC-H that colposcopy should be performed on all ASC-H cases regardless of HPV result are thereby supported by the findings of this investigation.
Pinus densiflora stands are common secondary forest communities on infertile soils in Korea. The stands are results of long severe past biotic pressure such as cutting, burning and grazing. These could be regarded as biotic climax in Korea. Because of their prevalent occurrence, relatively simple species and age composition, and their domestic economic importance, study of their distributional patterns may give some basic knowledge for better utilization of land resources in Korea. To detect distributional patterns and interspecific associations ten pine stands, each of which was homogenious with respect to topography and physiognomy, were subjectively selected from pine stands in Kyunggi Province near Seoul in 1969 and were made object of this study. Four contiguous systematic samples of count for trees, shrubs and seedlings from belt transects were collected from homogeneous areas within ten natural pine stands. The belt transect was 64m or 128m in length, and 1m, 2m or 4m in width. Basic units within the transect ranged from 64 to 256. The data from the contiguous transects were analysed in terms of multiple split-plot experiment. Departure from randomness of stem distribution, i.e., pattern, was tested in terms of variance mean ratio. For the detection of association between species, correlation coefficient was calculated for different block sizes. The values of ${\gamma}$ were tested by the usual t-test. Fine trees within one of the stands showed significant regular distribution through out the blocks. Within other eight stands pines were randomly distributed at basic unit with 4$\times$4m, 2$\times$2m, 2$\times$1m and 1$\times$1m. One significantly clumped distribution at basic unit 2$\times$2m, however, was observed from one of the pine stands. These randomly distributed groups were themselves significantly regularly distributed throughout the blocks for four pine stands. For the other four pine stands, in addition to the random distribution at the basie unit(the primary random group), randomly distributed groups with 32m dimension(the secondary random groups) were also observed. Both the primary and the secondary random groups were significantly regularly distributed at the rest of blocks. Pine seedlings were not distributed randomly thoughout the blocks. Within three of the ten stands they were contagiously distributed. Important shrub species underneath pines such as Querus serrata, Q. acutissima, Leapedeza intermedia, Rhododendron Yedoense var. poukhanenae, Juniperus utilis, Rhododendron mucronulatum var. ciliatum shnwed consistently similar distributional pattern with the pine at each stand. The shrub species pairs; Rhododendron Yedoense var. poukhanenae/Quercus serrata, Rhododendron mucronulatum var. ciliatum/Lespedeza intermedia were significantly negatively associated from 1m to 4m dimensional block sizes but became significantly positively associated from 8m sized blocks on. On the other hand the shrub species pairs; Lespedeza intermedia/Robinia Pseudoacacia, and Lespedeza bicolor var, japonica/Lespedeza intermedia were also significnalty negatively associated from 1m to 8m sized blocks but became significantly positively associated from 16m sized blocks on. The associational pattern between Rhododendron mucronul tun var. poukhanenae and Lespedeza intermedia was not consistent throughout the stands. In some stands negative associations were observed throughout the blocks except NS 32. From these observatons micro-edaphic variation within the pine stands seems not to be great enough to cause distributinal difference of pine trees within the ten pine stands. Among each species and pine seedings, however, the edaphic variation within the pine stands may be great enought to cause distributional variation.
Respiratory Syncytial virus (RSV) is an important cause of acute lower respiratory tract infections in human, with infants and young children being particularly susceptible. In the temperate zones, sharp annual outbreaks of RSV occur during the colder months, in both the northern and the southern hemisphere. RSV is unusual in that it can repeatedly reinfect individuals throughout life and infect babies in the presence of maternal antibody. RSV isolates can be divided into two subgroups, A and B, on the basis of their reactions with monoclonal antibodies, and the two subgroups are also distinct at the nucleotide sequence level. The specific diagnosis of RSV infection was best made by isolation of virus in tissue culture, identification of viral antigen, or by specific serologic procedures. Recently, rapid detection of RSV and analysis of RSV strain variation became possible by development of methods of reverse transcription and polymerase chain reaction amplification. In this study, to determine the genetic diversity of RSV found in Korea, 173 bp and 164 bp spanning selected regions of the RSV F and SH genes were enzymatically amplified and sequenced, respectively. Eight for F gene and three for SH gene were detected in 66 nasopharyngeal swap samples tested. Two major antigenic subgroups, A and B were confirmed from Korean samples (seven for subgroup A and one for subgroup B). At the nucleotide level of the F gene region, Korean subgroup A strains showed 95-99% homologies compared to the prototype A2 strain of subgroup A and 93-100% homologies among Korean subgroup A themselves. For the SH gene region, Korean subgroup A strain showed 97.5% homology compared to the prototype A2 strain of subgroup A, and Korean subgroup B strain showed 97% homology compared to the prototype 18537 strain of subgroup B. Most of base changes were transition and occured in codon position 3, which resulted in amino acid conservation. Using the maximum parsimony method, phylogenetic analysis indicated that Korean RSV strains formed a group with other RSV strains isolated from the United States, Canada, the Great Britain and Australia.
Purpose: Patients with chronic gastrointestinal disease are at risk for trace element deficiency due to impaired absorption and gastrointestinal loss. The aim of this study was to evaluate the trace element status of patients with gastrointestinal disease by blood and hair analysis, and to determine the usefulness of hair mineral analysis for diagnosing trace element deficiency not detected by a blood test. Methods: An analysis of hair minerals was performed and compared with blood mineral analysis in 13 patients with chronic gastrointestinal disease. The concentration of each element in the hair and blood was compared in the subgroups based on parenteral nutritional support or clinical symptoms. Results: Almost all patients had trace element deficiency. The trace elements deficient in the blood or hair analysis included zinc, selenium and copper. The hair zinc concentration was significantly lower in the group receiving parenteral nutritional support. The hair selenium concentration was statistically associated with the clinical symptoms of hair loss, brittle hair and loss of hair pigmentation. Conclusion: The results of this study suggest that patients with chronic gastrointestinal disease should receive adequate zinc and selenium replacement to avoid trace element deficiency especially when treated with long-term parenteral nutrition. Hair mineral analysis is useful as a complementary tool for the detection of a trace element deficiency.
The object of this study is to propose a study on the Private Investigator usage for Cyber Crime. The latest trend of cyber crime is being evolve in sophisticated and complex way over the global, like internet fraud, cyber gambling, hacking and etc. Hence national investigative authority mobilize high specialized skills and method of criminal investigation by each nation. But it is hard to respond in rapid and effective way because of propoor, distribution of group and insufficient of related legal system. Already in other countries, not considerable amount of services are given to private investigators in detection and tracking part which is inefficient by nation. So it has significantly meaningful to compensate the defect and study about private investigator usage as companion of cooperation policing for effectively respond to cyber-crime. The way to effectively deal with the cyber-crime is reevaluate meaning of partnership policing and need of private investigator usage. Also it is to analyze the main issue about introduction of a system and suggest the effective way of introduction. First, legislation of private investigator usage which is based upon partnership policing should be made up. Moreover, to establish the range of private investigator's business and enhance the reliability, it is to propose introduction of leading professional global certificate and license system with sufficient education and test. We are expecting introduction of private investigator usage can improve efficiency of investigation and promote effective countermeasures of cyber-crime.

The objective of this study is to improve the optical sensitivity of laser fluorescence for detection of incipient enamel caries. An incipient carious lesion was formed in various stages by placing an enamel specimen of a bovine tooth in STPP demineralization solution. After measuring the optical density of the lesion surface by laser fluorescence induced by argon laser and various alter of yellow(500-520nm), amber(520-540nm), orange(540-560nm), and red(560-580nm), the specimen was cut vertically to measure the depth of the lesion using a polarizing microscope. SAS statistical program was used to analyze the relationship between the optical density of the lesion suface and the depth of the lesion. The results were as follows: 1. The optical density of early carious lesion, measured by laser fluorescence with amber and orange filter, and lesion depth observed by polarizing microscope, were increased as demineralization time increased. 2. The correlation coefficient between optical density of the lesion surface and the histological depth of the lesion was the highest in orange filter(r=0.49), followed by amber(r=0.32), yellow(r=0.13) and red(0.01). 3. Regression analysis showed that the most linear relationship between the optical density and the lesion depth was existed in orange filter group. In regard above results, laser fluorescence could be considered to be reliable for optical diagnosis of dental caries.
Background: Gastric cancer is the most common gastrointestinal cancer and a leading cause of cancer mortality in Nepal. Survival of gastric cancer patients depends on the stage at which diagnosis is made. The aim of this study was to analyze the presentation and outcomes of gastric cancer patients treated at a tertiary care hospital in Nepal. Materials and Methods: A retrospective analysis of 140 consecutive histologically proven gastric adenocarcinoma cases managed at the Department of Surgery, Tribhuvan University Teaching Hospital, Kathmandu, Nepal for the period of January 2009 to December 2013 was carried out. Results: One hundred forty out of the total 186 patients with histologically proven gastric adenocarcinoma, were admitted for surgery. The mean age was $59.6{\pm}12.4yrs$ (range 29 to 78 yrs) and the male: female ratio was 2:1. Sixty three (45%) patients featured Tibeto-Burman descent though this ethnic group accounts for only 18% of the Nepalese population. Two-thirds or more patients presented with abdominal pain, anorexia, weight loss and/or vomiting. In 86 (61.5%) of the patients the tumor was located in the lower $3^{rd}$ of the stomach and in only 15% of the patients the tumor was located at the upper $3^{rd}$. Early gastric cancer was diagnosed postoperatively in only 4%. In 54%, the disease was locally advanced and metastatic lesions were found in 14% of the patients. Subtotal (73) or total (11) curative gastrectomies (D1, D1+ or D2) were performed in 84 (60%) patients with average lymph node retrieval of $16.6{\pm}8.2$. Palliative gastrectomies or procedures were performed in 23% of the patients and no intervention (open & close/biopsy) was employed in 15% of the patients. Perioperative morbidity was seen in 10% and mortality in 4%. Three, four and five year survival rates up to the recent follow-up were 17.9%, 11.9% and 8.3%, respectively. Conclusions: Gastric cancer in Nepal is usually diagnosed at an advanced stage and has a poor prognosis. Thus, early detection is the key to improve the survival of gastric cancer patients.
In the intestinal mucosa, mast cells are thought to be responsible for the expulsion of parasites. We investigated the relationship of worm expulsion and mast cells in C3H/HeN and BALB/c mice infected with Echinostoma hortense. In addition, we examined whether the worm recovery rate was associated with the strain of mice, and whether a toluidine stain and immunohistochemistry using the c-kit antibody was effective in the detection of mast cells. In order to investigate the mucosal immune response of C3H/HeN and BALB/c mice, each mouse was infected orally with 30 E. hortense metacercariae. Then, the number of mucosal mast cells and worm recovery rates was observed in experimentally infected mouse strains between 1 week and 8 weeks post infection (PI). Mucosal mast cells were increased in 3 weeks P.I. in C3H/HeN and BALB/c mice. On the other hand, only mucosal goblet cells and worm recovery rates correlated in C3H/HeN mice (P=0.0482). Worm recoveries in C3H/HeN mice were 65.7$\pm$5.6, 53.3$\pm$5.4 and 6.7$\pm$0.6 in week 1, 2, and 3 P.I. and strongly decreased in week 3 P.I. Worm recoveries in BALB/c mice were 23.0$\pm$2.5, 10.0$\pm$1.0, and 6.7$\pm$0.6% in week 1, 2, and 3 P.I. and gradually decreased from week 1 P.I. to week 3 P.I. Worm recoveries in C3H/HeN mice were significantly higher than in BALB/c mice (P<0.00l). The number of mast cells in C3H/HeN and BALB/c mice using the anti-c-kit antibody reached to a peak in week 3 P.I. and recovered as normal level in week 5 P.I. and 6 P.I. The number in E. hortense-infected C3H/HeN mice (P=0.0015) was higher than in E. hortense-infected BALB/c mice (P=0.01) compared with the control group. There were significant differences in the number of mast cells among regions of the intestine in in C3H/HeN mice (P<0.05) but not in BALB/c mice (P>0.05). Immunohistochemistry using the anti-c-kit antibody was significant method as an examination of the number of mast cells (P=0.0002). In conclusion, the present study demonstrated that mast cells play an important role in worm recovery, and immunohistochemistry using the anti-c-kit antibody was superior to toluidine stain as an examination of mast cells.

In this paper, we will introduce the system that can control multiple vehicles on the road through Single Board Computers and V2I (Vehicle-To-Infrastructure). Also, we will propose the group evasive maneuver decision algorithm, which plays a critical role in deciding whether the vehicles in the system have to conduct evasive maneuvers to avoid obstacles on the road. In order to test this system, we have utilized Wi-Fi and TCP/IP for establishing the communication between multiple vehicles and the relay server, and observed their driving states on the road with obstacles. During the experiments, we have discovered that our original decision algorithm possesses high failure rate when there is frequency interference in ISM (Industrial Scientific Medical) band. In order to reduce this failure rate, we have implemented the data transition detector. This paper will focus on how the use of data transition detector can affect the reliability of the system under the frequency interference of ISM band. If this technology is improved and applied in the field, we will effectively deal with such dangerous situations as multiple collision accidents through vehicle-to-vehicle communication or vehicle-to-infrastructure communication. Furthermore, this can be applied to the autonomous driving technologies. This can be used as the reference data for the development of the similar system.
