MORPHOMETRIC STUDY OF SCAPULAR LATERAL BORDER FOR INSTALLATION OF DENTAL IMPLANT. (치과용 임프란트 매식을 위한 견갑골외연의 형태학적 연구)

  • The scapular flap, described by dos Santos in 1986, has been used successfully for the reconstruction of a variety of defects of oro-mandible. Some have defined the gross and vascular anatomy of the lateral border of the scapula, yet useful anatomical information and a complete description of area and contour of each cut surface of lateral border of scapula, which is very important for esthetic and functional reconstruction using dental implants, are missing. These prompted us to clarify the cross-sectional area of lateral border of scapula. Twenty three scapulas of 15 fixed adult Caucasian cadavers were sectioned in every 1cm interval along the lateral border of scapular, and the metric relations and the shape of cut surface were assessed. The lateral border of the scapula, consisting of cortico-cancellous bone measuring $7.86{\pm}0.97mm$ in width, $19.6{\pm}2.86mm$ in height and $12{\pm}1.78cm$ in length, could be harvested as an osteocutaneous scapular flap or as a single vascularized bone flap. The mean thickness of cortical bone of lateral, medial, dorsal and costal surface was $0.46{\pm}1.48mm$, $1.78{\pm}1.34mm$, $1.54{\pm}1.11mm\;and\;1.35{\pm}0.87mm$, respectively. So we have thought that all scapular transplants could be supported osseointegrated implants for fixation of dental prosthesis.

Effects of Imyo-san Treatment on the Monosodium Iodoacetate-induced Osteoarthritis in Rats (이묘산(二妙散)이 흰쥐의 Monosodium Iodoacetate 유발 골관절염에 미치는 영향)

  • Objectives : This study was to investigate the effects of Imyo-san treatment on the monosodium iodoacetate-induced osteoarthritis in rats. Methods : Arthritis was induced by injection of monosodium iodoacetate(MIA)(0.5 mg) into both knee joint cavities of rats. Arthritic rats were divided into control(n=8) and treated(n=8) group. Control group was taken distilled water and treated group was taken extracts of Imyo-san by orally for 20 days. At the end of experiment(20day after MIA injection), gross and histopathological examination on the articular structures of knee joints were performed. Blood cell counts and proteoglycan(PG) contents in articular cartilages were analysed. And also, tumor necrosis factor-$\alpha$($TNF-{\alpha}$) and interleukin-$1{\beta}$($IL-1{\beta}$) contents synovial fluids were measured by enzyme-liked immunosorbent assay(ELISA) method. Results : 1. Body weight(g) of the treated group were increased significantly compared with control group at 15 and 20 days after injection. 2. Grossly, degree of osteoarthritis in the treated group was alleviated compared with the control group. 3. PG content in articular cartilage of the treated group was increased significantly compared with the control group. 4. Histopathologically, osteoarthritic scores of the treated group was decreased significantly compared with the control group. 5. $TNF-{\alpha}$ content in synovial fluid of the treated group was decreased significantly compared with the control group. Conclusions : On the basis of these results, we suggested that Imyo-san has inhibiting effects on the progression of arthritis in MIA-induced osteoarthritis model.

Daily Setup Uncertainties and Organ Motion Based on the Tomoimages in Prostatic Radiotherapy (전립선암 치료 시 Tomoimage에 기초한 Setup 오차에 관한 고찰)

  • Purpose: The patient's position and anatomy during the treatment course little bit varies to some extend due to setup uncertainties and organ motions. These factors could affected to not only the dose coverage of the gross tumor but over dosage of normal tissue. Setup uncertainties and organ motions can be minimized by precise patient positioning and rigid immobilization device but some anatomical site such as prostate, the internal organ motion due to physiological processes are challenge. In planning procedure, the clinical target volume is a little bit enlarged to create a planning target volume that accounts for setup uncertainties and organ motion as well. These uncertainties lead to differences between the calculated dose by treatment planning system and the actually delivered dose. The purpose of this study was to evaluate the differences of interfractional displacement of organ and GTV based on the tomoimages. Materials and Methods: Over the course of 3 months, 3 patients, those who has applied rectal balloon, treated for prostatic cancer patient's tomoimage were studied. During the treatment sessions 26 tomoimages per patient, Total 76 tomoimages were collected. Tomoimage had been taken everyday after initial setup with lead marker attached on the patient's skin center to comparing with C-T simulation images. Tomoimage was taken after rectal balloon inflated with 60 cc of air for prostate gland immobilization for daily treatment just before treatment and it was used routinely in each case. The intrarectal balloon was inserted to a depth of 6 cm from the anal verge. MVCT image was taken with 5 mm slice thickness after the intrarectal balloon in place and inflated. For this study, lead balls are used to guide the registration between the MVCT and CT simulation images. There are three image fusion methods in the tomotherapy, bone technique, bone/tissue technique, and full image technique. We used all this 3 methods to analysis the setup errors. Initially, image fusions were based on the visual alignment of lead ball, CT anatomy and CT simulation contours and then the radiation therapist registered the MVCT images with the CT simulation images based on the bone based, rectal balloon based and GTV based respectively and registered image was compared with each others. The average and standard deviation of each X, Y, Z and rotation from the initial planning center was calculated for each patient. The image fusions were based on the visual alignment of lead ball, CT anatomy and CT simulation contours. Results: There was a significant difference in the mean variations of the rectal balloon among the methods. Statistical results based on the bone fusion shows that maximum x-direction shift was 8 mm and 4.2 mm to the y-direction. It was statistically significant (P=<0.0001) in balloon based fusion, maximum X and Y shift was 6 mm, 16mm respectively. One patient's result was more than 16 mm shift and that was derived from the rectal expansions due to the bowl gas and stool. GTV based fusion results ranging from 2.7 to 6.6 mm to the x-direction and 4.3$\sim$7.8 mm to the y-direction respectively. We have checked rotational error in this study but there are no significant differences among fusion methods and the result was 0.37$\pm$0.36 in bone based fusion and 0.34$\pm$0.38 in GTV based fusion.

Effects of Gamisoyeoum-tang(Jiaweixiaoyan-tang) on the Monosodium Iodoacetate-induced Osteoarthritis in Rats (가미소염탕(加味消炎湯)이 Monosodium Iodoacetate로 유발된 흰쥐의 골관절염에 미치는 영향)

  • Objectives : This study was to investigate the effects of Gamisoyeoum-tang(Jiaweixiaoyan-tang) on the monosodium iodoacetate-induced osteoarthritis in rats. Methods : Monosodium iodoacetate induced arthritic rats were divided into control and treated group. Control group was taken distilled water for 20 days. Treated group was taken extracts of Gamisoyeoum-tang(Jiaweixiaoyan-tang) by orally for same duration. Normal group was injected with normal saline and was taken distilled water. Body weights were measured at 0, 5, 10, 15, 20 days after injection. At the end of experiment, gross and histopathological examination on the articular cartilages of the knee joints were performed. Contents of $TNF-\alpha$, $IL-1\beta$, IL-6 in synovial fluids and proteoglycan contents of articular cartilages were analysed. COX-2 and iNOS immunohistochemical examination on the knee joints were performed. Results : 1. Body weights of the treated group were significantly increased compared with control group at 20 days after injection. 2. Grossly, the severity of osteoarthritis in the treated group were alleviated compared with control group. 3. PG contents in articular cartilages of the treated group were significantly increased compared with control group. 4. Histopathologically, degenerative and necrotic lesion of articular cartilages in the treated group were alleviated compared with those of the control group. 5. $IL-1\beta$ contents in synovial fluids of the treated group were significantly decreased compared with control group. 6. Positive reactions of COX-2 in chondrocytes and synovial membranes of the treated group were decreased compared with the control group. Conclusions : On the basis of these results, we concluded that Gamisoyeoum-tang(Jiaweixiaoyan-tang) has anti-arthritic effects on the monosodium iodoacetate-induced osteoarthritis in rats. And it's effects were related with reduced secretion of $IL-1\beta$ and COX-2 from osteoarthritic chondrocytes and synovial membranes.

The Effect of Injinsammul-tang(IJS) on the Hepatocellular Carcinogenesis Induced by Diethylnitrosamine(DENA) in Rats (인진삼물탕(茵蔯三物湯)이 DENA로 유발된 흰쥐의 간암에 미치는 영향)

  • Objectives : This study was designed to investigate the effect of Injinsammul-tang (IJS) on hepatocellular carcinogenesis in rats. Methods : Sprague Dawley (SD) rats of control and treatment groups received intraperitoneal injection of 50 mg/kg/day diethylnitrosamine (DENA) weekly for 8 weeks. Experimental rats were classified into 3 groups; normal group (Nor), hepatic cancer induced control group (Con), and IJS extract 250 mg/kg administered group (IST) after being injected with DENA. Thereafter the changes of body weight, liver weight and weight of liver/100g body weight, the activities of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), lactate dehydrogenase (LDH), and superoxide dismutase (SOD) were measured. Gross anatomy and optical microscopy were also observed. Results : The body weight decreased in Con and IST compared with the Nor. The weight of liver and the weight of liver/100g body weight increased significantly in Con and IST compared with the Nor. The activities of AST, ALT, ALP, LDH increased in the Con compared with Nor, but decreased in IST compared with Con. The activities of SOD increased in the Con and IST compared with Nor. Upon naked eye and light microscopic examination, IJS improved the morphological and histopathological changes of liver caused by DENA-induced hepatic neoplasm. The number of hepatic p53 positive cells decreased in the IST compared with Con. Conclusions : Most of the results did not show a significant effect, but some of the results showed a significant effect. It can be estimated that IJS has some effects on hepatocellular carcinogenesis induced by DENA in rats, and further studies will be needed.

Immunohistochemical Study on Hair Growth Promoting Effect of Yonnyuniksoogobon-dan (연년익수고본단(延年益壽固本丹)의 육모효능(育毛效能)에 대한 면역조직화학적 연구)

  • Objectives: Yonnyuniksoogobon-dan (Yan Nian Yi Shou Gu Ben Dan 延年益壽固本丹) is composed of 11 herbs (Polygoni Mutiflori Radix, Lycii Radicis Cortex, Polia, Rehmanniae Radix, Rehmanniae Radix Preparat, Asparagi Radix, Liriopis Tuber, Lycii Fructus, Acori Graminei Rhizoma, Angelicae Acutiloba Radix, and Pini Folium) based on Yonryunggobon-dan (Yan Ling Gu Ben Dan 延齡固本丹) and Yonnyuniksoobulrho-dan (Yan Nian Yi Shou Bu Lao Dan 延年益壽不老丹). This study evaluated hair growth promoting effect of Yonnyuniksoogobon-dan on the shaved C57BL/6 mice. Methods: Yonnyuniksoogobon-dan was treated by oral administration (Sample I) and oral administration plus skin application (Sample II) once a day for 12 days. Hair regrowth was photographically and histologically determined during the experimental period. Hair growth cycle related factors (EGF, TGF-${\beta}1$) and vascular factors (VEGF, iNOS) were also determined with immunohistochemistry. Results: 1. On gross observation of hair regrowth, Sample I and Sample II groups demonstrated acceleration of hair regrowth. 2. The hair regrowth index of the Sample I group increased significantly from 7 days (P<0.05) to 12 days (P<0.01) after the shave while that of the Sample II group significantly increased at 12 days (P<0.05). 3. On histological observation, both Sample I and Sample II groups demonstrated histological improvement and increases of number and diameter of the hair follicles. 4. EGF expressions on the root sheath of hair follicles were up-regulated in both Sample I and Sample II groups. 5. TGF-${\beta}1$ expressions on the root sheath of hair follicles were not regulated in Sample I or Sample II groups. 6. VEGF expressions in the surrounding tissues of hair follicles were up-regulated in both Sample I and Sample II groups. 7. iNOS expressions in the surrounding tissues of hair follicles were down-regulated in both Sample I and Sample II groups. Conclusions: These results suggest that Yonnyuniksoogobon-dan has hair growth-promoting activity and these effects relate to up-regulations of EGF and VEFG expressions and down-regulations of TGF-${\beta}1$ and iNOS expressions on hair roots.

Effects of Gagamsosokmyeong-tang(Jiajianxiaoxuming-tang) Treatment on the Monosodium Iodoacetate-induced Early Stage Osteoarthritis in Rats (가감소속명탕이 Monosodium Iodoacetate로 유발된 골관절염의 초기변화에 미치는 영향)

  • Objectives: This study was performed to investigate the effects of Gagamsosokmyeong-tang(Jiajianxiaoxuming-tang) on the monosodium iodoacetate(MIA) induced early state osteoarthritis in rats. Methods: Osteoarthritis was induced by injection of MIA(0.25 mg) into knee joints of rats. Osteoarthritis rats were divided into control(n=8) and treated=8 group respectively, Control group was taken distilled water and treated group was taken extracts of Gagamsosokmyeong-tang(Jiajianxiaoxuming-tang) by orally for 20 days. Body weight was measured at 0, 5, 10, 15, 20 days after MIA injection. At the end of experiment, gross and histopathological examination on the articular cartilages of the knee joints were performed. Proteoglycan(PG) content of articular cartilages were analysed by safranine O staining method. The content of tumor necrosis $factor-{\alpha}(TNF-{\alpha})$, $interleukin-1{\beta}(IL-1{\beta})$ in synovial fluids were analysed by enzyme-inked inmunosorbent assay(ELISA) method. And also cycloxygenase-2(COX-2), matrix metalloproteinase 3(MMP-3), inducible nitric oxide synthase(iNOS), calpain immunochistochemical examination on the knee joints were performed. Results: PG content in articular cartilages of the treated group was significantly increased compared with control group. Histopathological osteoarthritic score of the treated group was significantly decreased compared with control group. $TNF-{\alpha}$ content in synovial fluids, expression of iNOS and calpain in synovial membrane of the treated group were significantly decreased compared with control group. But body weight, $1L-1{\beta}$ content in synovial fluids, expression of iNOS and MMP-3 of the treated group were not significantly changed compared with control group. Conclusions: On the basis of these results, we conclude that Gagamsosokmyeong-tang(Jiajianxiaoxuming-tang) has anti-arthritic effects on the MIA induced early stage osteoarthritis in rats.

Powdered Wound Dressing Materials Made from wild Silkworm Antheraea pernyi Silk Fibroin on Full-skin Thickness Burn Wounds on Rats

  • Purpose: This study evaluated powdered burn wound dressing materials from wild silkworm fibroin in an animal model. Methods: Fifteen rats were used in this experiment. Full-thickness $2{\times}2cm$ burn wounds were created on the back of rats under anesthesia. In the two experimental groups, the wounds were treated with two different dressing materials made from silkworm fibroin. In the Control Group, natural healing without any dressing material was set as control. The wound surface area was measured at five days, seven days and 14 days. Wound healing was evaluated by histologic analysis. Results: By gross observation, there were no infections or severe inflammations through 14 days post-injury. The differences among groups were statistically significant at seven days and 14 days, postoperatively (P<0.037 and 0.001, respectively). By post hoc test, the defect size was significantly smaller in experimental Group 1 compared with the Control Group and experimental Group 2 at seven days postoperatively (P=0.022 and 0.029, respectively). The difference between Group 1 and Group 2 was statistically significant at 14 days postoperatively (P<0.001). Group 1 and control also differed significantly (P=0.002). Group 1 showed a smaller residual scar than the Control Group and Group 2 at 14 days post-injury. Histologic analysis showed more re-epithelization in Groups 1 and 2 than in the Control Groups. Conclusion: Burn wound healing was accelerated with silk fibroin spun by wild silkworm Antheraea pernyi. There was no atypical inflammation with silk dressing materials. In conclusion, silk dressing materials can be used for treatment of burn wound.

Changes of lateral cephalometric values according to the rotation of head (두부회전에 따른 측모두부방사선 계측치의 변화)

  • This study was performed to find out the effect of projection errors on cephalometric linear and angular measurements according to head rotation during taking lateral cephalometric radiographs. Seventeen skulls with permanent dentition and no gross asymmetry were obtained from the Department of Anatomy, Medical School, Chosun University. Total 527 x-ray films were taken with $1^{\circ}$ interval from the reference position($0^{\circ}$) to ${\pm}15^{\circ}$ around the vertical axis (Z axis) which is perpendicular to the midpoint of the line connecting the center of two ear rods in submento-vertex direction. Statistical analysis was performed by paired t-test if there were statistically significant differences between the mean of the reference position($0^{\circ}$) and that of each rotation angle. The following results were obtained. 1. The projection errors of angular measurements were smaller than those of linear measurements. 2. The projection errors of angular measurements including midline landmarks were smaller than those including bilateral landmarks. 3. The horizontal linear measurements were gradually decreased when the stroll was rotated toward the film, but slightly increased and then decreased when the skull was rotated toward the focal spot. However, the changes were smaller in focal direction. 4. The projection errors of horizontal linear measurements were larger than those of vertical linear measurements. 5. The projection errors of vertical linear measurements were increased with increased distance from the rotation axis to vertical measurements. It is concluded that the use of angular measurements rather than linear measurements is recommended to minimize the projection errors.
