Impact of sarcopenia on biliary drainage during neoadjuvant therapy for pancreatic cancer

  • Background/Aims: Since the usefulness of neoadjuvant chemo(radiation) therapy (NAT) for pancreatic cancer has been demonstrated, recurrent biliary obstruction (RBO) in patients with pancreatic cancer with a fully covered self-expandable metal stent (FCSEMS) during NAT is expected to increase. This study investigated the impact of sarcopenia on RBO in this setting. Methods: Patients were divided into normal and low skeletal muscle index (SMI) groups and retrospectively analyzed. Patient characteristics, overall survival, time to RBO (TRBO), stent-related adverse events, and postoperative complications were compared between the two groups. A Cox proportional hazard model was used to identify the risk factors for short TRBO. Results: A few significant differences were observed in patient characteristics, overall survival, stent-related adverse events, and postoperative complications between 38 patients in the normal SMI group and 17 in the low SMI group. The median TRBO was not reached in the normal SMI group and was 112 days in the low SMI group (p=0.004). In multivariate analysis, low SMI was the only risk factor for short TRBO, with a hazard ratio of 5.707 (95% confidence interval, 1.148-28.381; p=0.033). Conclusions: Sarcopenia was identified as an independent risk factor for RBO in patients with pancreatic cancer with FCSEMS during NAT.

  • We developed novel molecular dynamics and quantum chemical molecular dynamics simulators for the design of MgO protecting layer in plasma display panel. These simulators were applied to the investigations on the destruction processes of the MgO protecting layer as well as the evaluation of its second electron emission ability. From the simulation results, we successfully proposed new guidelines for MgO protecting layer with high durability and high second electron emission ability.

  • Purpose: This research was conducted to explore the experiences of graduate student women with children fulfilling multiple roles and to generate a substantive theory. Methods: This study uses the grounded theory method based on Strauss and Corbin(1988)'s method. Interviews were conducted with 11 graduate student women who had children aged three of older. Results: The core category of this research is 'Role balancing for self-generativity'. Based on the core category, three types of married women graduate students' experiences were found. They are 'concentrating on graduate-work' 'postponing graduate-work' and 'adjustment between two roles' Conclusions: Through understanding the married women's multiple roles experience process, there can be a basis for making a women's health system and social support system for the married women graduate student. Also, this research is to generate a substantive theory which can helps to change social perspectives on the quality of life for the women who will contribute in Korean society as professionals.

Algorithm of an automated auditory brainstem response neonatal hearing screening method (신생아를 대상으로한 청성뇌간유발반응의 자동 판독 알고리즘)

  • In this paper, we propose an algorithm that applies Rolle's theorem to automatically detect and label peak III and V of the normal, suprathreshold auditory brainstem response (ABR). ABR waveform were recorded from 55 normal-hearing ears at screening levels varying from 30 to 60 dBnHL. For each ABR waveform, the peak-finding algorithm proceeded in fourth steps: (1) Select maximum and minimum values of the target ABR waveform, (2) divide this range into n equal parts, (3) effective candidate peaks in the ABR waveform are identified using Rolle's theorem (4) peak III and V are identified from these candidate peaks based on their latency and morphology. As a result, proposed auto dectection method showed high correlation and accuracy with manual detection method performed by clinician. By using proposed algorithm, clinician can detect and label peak III and V faster and more efficient than manual detection method.
