  • The forms of healthcare service delivery emerges from the interaction between technological progress and institutional changes. Building a healthcare system which enables effective and efficient patient care is a imperative to a sustainable operation of a society. Identifying of a promising medical technologies and diffusing them consists of the basic tasks of a good healthcare system. Inducing of a promising innovation in healthcare and utilization of the innovation requires a deep understanding of healthcare innovation system and delicate governmental intervention to the effective functioning of the system. Therefore, the support for R&D in healthcare field should be given to social and institutional technologies for the better organization of healthcare delivery and consumption system as well as basic and applied medical sciences.

Research trends in obesity & obesogenic environments in Korea

  • BACKGROUND/OBJECTIVES: Globally, it has been projected that there will be 2 billion overweight and 1 billion obese individuals by 2030. In Korea, the prevalence of adult obesity (BMI > 25) increased from 29.7% in 2009 to 32.4% in 2015. Moreover, childhood obesity, which leads to adulthood obesity, has increasingly become a social problem. The purpose of this review is to summarize the scientific basis for the development of effective models and policies aimed at preventing obesity over a lifetime based on research modeling obesogenic environments. MATERIALS/METHODS: The review focuses on the characteristics of obesity prevalence and trends in 3P analysis (papers, patents, and products) as well as government-funded projects in Korean obesity obesogenic environments over the last 10 years. RESULTS AND DISCUSSION: As a result of the 3P analysis, studies on obesity risk factors were frequently carried out, according to two data bases RISS (4.9%) and PubMed (24.7%). Since there were only 17% patents related to the mechanism of preventing obesity in 7,951 Korean patents related to obesity, new paradigms of technologies to dominate the global obesity markets are needed. After government-funded projects were analyzed, communication and cooperation in multi-governmental departments were suggested to elucidate the characteristics of Korean obesity. Government should also produce short- and long-term road maps to develop a practical, successful outcome. Although the rate of obesity in Korea is currently lower than in other developed countries according to WHO criteria, without adequate governmental intervention, obesity rates will approach those of the top countries with high incidence rates of obesity within the next 10 years.

Descriptive literature review on the evaluation of the youth smoking prevention program effectiveness (청소년 흡연중재 교육프로그램의 효과평가에 관한 설명적 문헌고찰)

  • The age of the very first smoking is a significant indicator of life-long health status. Smoking prevention intervention was actively conducted based in middle and high schools with the support of the Korean Act for Health Promotion since 1998. These governmental supports became large and various smoking prevention programs were developed and conducted in diverse standards and perspectives. This study made a comprehensive descriptive literature review on smoking prevention educational programs for youth in Korea to identify the intervention quality and effectiveness of them for future smoking prevention program development. A total of 28 peer-reviewed journal articles published between May, 1995 and May 2005 (for the last 10 years) were finally included in this review process. The study participants should be adolescences in middle or high schools in Korea and the intervention types of the review studies should be smoking prevention or cessation educational programs. The outcomes of the reviewed studies were reanalyzed by the participants' characteristics, theory basement, evaluation design, intervention period, evaluation outcomes, and program effectiveness Most smoking intervention programs were not specified by sex and smoking status. Largely most intervention programs focused on male students although female students' smoking percent is increasing including both smokers and nonsmokers. Based on the school grades, the large percents of smoking intervention were conducted to the first grade of middle school and the first grade of high school. Almost 70% of the reviewed studies did not apply any health behavior change theories and the Transtheoretical model and social cognitive theory were utilized in 7 studies among the last reviewed 30%. The theory-based intervention studies had greater effectiveness than the non-theory based studies. More than 90% had quasi-experimental evaluation design and the effectiveness of the non-experimental designed study seemed over estimated than the quasi-experimental or experimental designed studies. More than 60% of the reviewed studies made their education for less than j days, over a short period and the evaluation factors were knowledge and attitude in general which can be obtained in short intervention period. Therefore, smoking intervention programs for youth in Korea need to be modified in terms of research design such as the intervention period, intervention-evaluation design, theory-based approach, and population-focused intervention specification.

Effectiveness of school counseling based on "the AMPQ-II and administrative manual": Focusing on the counselor and the number of session factors ("AMPQ-II 및 관리 매뉴얼"에 따른 학교 상담의 효과: 상담자 요인 및 회기 수를 중심으로)

  • The Korean government has administered the 'AMPQ-II' to all middle school students to prevent psychological problems and make immediate interventions when necessary. On the other hand, there has been a paucity of studies of the effectiveness of this governmental policy, or the factors influencing the effectiveness of the intervention. Using the data from 48 counselees from two middle schools, the present study examined the effectiveness of the intervention programs, and whether the counselor's characteristics and the number of counseling sessions might matter in the effect. The results suggested that most students reported improvements after the interventions. The intervention effect was less pronounced when the counselors had governmental certification. The number of counseling sessions was not a significant factor for predicting the effectiveness of the interventions. Interpretations of the present study should be made cautiously because of the small sample size. The present results, however, suggest that efforts to improve the quality of the counseling teachers are necessary. In addition, increasing the number of counseling sessions is not necessarily effective.

Caring for older adults with dementia -focused on therapeutic environment- (치매환자 돌봄 전략에 관한 일 고찰)

  • Even though enormous governmental expenses and scientists' efforts to find out definite causes and treatment methods of senile dementia have been investigated, little has been known in this area. Along with knowledge development of the etiology and treatment of the dementia, researchers have started to focus on improving the quality of life of the older adults with dementia through psychosocial intervention. This study was designed to propose a theoretical framework for establishing therapeutic environment for the older adults with dementia and for developing principles and strategies of caring. The results of this study were expected to help family members of the older adults with dementia to understand behavioral problems of the demented persons. The results can be utilized for health professionals to provide nursing interventions to reduce family caregivers' burden and to improve the quality of life of the older adults with dementia and their family. Caring principles developed from this study were as follows: 1. To minimize the stressors that can stimulate older adults with dementia. 2. To assess demented person's needs for safety and provide intervention based on the assessment. 3. To provide therapeutic environment for older adults with dementia to reduce confusion and to improve orientation. 4. To organize simple regular daily activities that older adults can anticipate. 5. To enhance demented person's self-esteem and self-confidence by providing supportive care. 6. To promote social interaction of the older adults with dementia by utilizing adequate activity programs.

A Critical Analysis of the Perspectives on Health Care Reform in Korea (의료개혁 논의의 비교분석)

  • This paper analyzed four different perspectives on health care reform in Korea in terms of the basic values, formulated problems and reform plans, implementation methods, and supporting groups. The medical security plan was insisted by social security specialists and social activists focusing on the integration of medical insurance coops in order to enhancing equity and right of the people. However, its perspective was limited to promoting security instead of reforming health care system. The government proposed the health care reform plans in 1994 and in 1997, focusing on promoting efficiency by remedying many problems in health care delivery system. However, its implementation was not successful due to the lack of organizational and financial supporters. Recently, two opposite proposals were issued. The market reform plan paid attention to revitalizing the market function to promoting efficiency by allowing hospitals to treat private patients instead of applying the medical insurance regulation. The government reform plan focused on intensifying governmental planning and intervention in the health care sector in order to removing inefficiency and promoting equity with the supports of social activists and labor unions. Finally, this paper proposed an alternative plan to promote harmonious social relationship between actors in the health care system.

Historical Essay on the Growth of Modern Big Business Corporations and the Formation of Business Groups in Korea - With the Focus on the Government Intervention (한국의 근대적 대기업 및 기업집단 형성사 - 정부 개입(1960년대와 70년대)을 중심으로)

  • The miraculous growth of Korean economy and its business corporations during 1960' s and 1970's are mainly due to the government leadership and its market intervention. We can find the reasons why the government initiated economic growth plan was so successful in Korea in its efficient bureaucratic government system and fair discipline to the corporations based on its contribution to the economy. During 1960's, the primary factors for the growth of business entities and the formation of business groups were the financial special favor, the preferential treatment in the new industry entrance and the merge & acquisition, lavish export incentives from the government, and the export explosion to Vietnam. During 1970's, the substantial deduction of corporations' private debt, enormous support in heavy industry investment, special benefits to general trading companies by the government, and the construction export to the Middle-East were the main causes of the business growth and the business groups formation. Also, the economic rent for the big companies had still been effective since 1960's. However, the preferential benefit to the big companies made them to diversify into the unrelated business ares and to be in very vulnerable financial position. The governmental support brought about the monopoly as well.

Knowledge and Attitudes Not Strongly Related to COVID-19 Prevention, Behaviors, and Vaccination Among Filipino College Students

  • The effects of the COVID-19 pandemic across various sectors are far-reaching, and the education sector is not an exemption. Approved and recognized vaccines are clinically proven to provide protection and have been considered one of the primary mitigations against the virus. Knowledge, attitude, and related practices (henceforth KAP) toward the virus is widely considered to be an essential cognitive key in public health for health intervention, prevention, and promotion. Previous studies revealed a positive correlation between knowledge and attitude. This study investigated the KAP and their relationship among the 857 college students in one state college in the Philippines using an online survey. Using descriptive statistics, the findings showed that the majority of the respondents have been fully vaccinated against COVID-19. Respondents were shown to be knowledgeable about COVID-19, specifically about its transmission, clinical symptoms, treatment, risk groups, isolation, prevention, and control. Positive attitudes toward individual and governmental mitigation and control of the virus were shown. Their scores on practices revealed that they always exhibit protective and preventive behavioral practices. Pearson correlation revealed a weak significant relationship between knowledge and practice, and a fair significant relationship between attitude and practice. No significant relationship is found between knowledge and attitude. This study hopes to find its place in the literature worthy of consideration as empirical support toward understanding a specific group's perceptive risk and preventive behavior toward the virus, policy making, and health intervention and promotion.

Musculoskeletal Ultrasound Findings of Shoulder Pain Patients: A Retrospective Observational Study (근골격계 초음파 진단기기를 활용한 견관절 통증 환자의 초음파 소견: 후향적 관찰 연구)

  • Objectives The aim of this study is to retrospectively evaluate the shoulder lesions in patients experiencing shoulder pain through the use of musculoskeletal ultrasound during Korean medicine intervention treatments. Methods A total of 20 cases were collected, including biceps tendinitis (n=4), calcification (n=3), SASD bursitis (n=4), partial tear (n=2), full-thickness tear (n=1), tendinopathy (n=5), and impingement syndrome (n=1). Musculoskeletal ultrasound was used by Korean medicine doctors to perform real-time scanning and to explain the patient's condition during treatment sessions. Results The use of musculoskeletal ultrasound allowed Korean medicine doctors to perform treatments such as pharmacoacupuncture, acupotomy, and acupuncture more safely and effectively. Patients were able to better understand their conditions through real-time imaging and explanations provided by the doctors. Conclusions Musculoskeletal ultrasound has the potential to enhance the safety and efficacy of Korean medicine intervention treatments for patients with shoulder pain. Institutional or governmental support is needed to further promote the use of medical devices by Korean medicine doctors, ultimately leading to an increase in cases and clinical evidence.

Elderly Women′s Grief (여성 노인의 슬픔)

  • This study sought to find a nursing intervention tool for enhancing elderly women's lives by investigating the causes and the meaning of their grief. Method: This research was derived from a phenomenological tool such as qualitative research design. The data collection took place from December of 2000 until April of 2001 Through systematic interviews and participatory observations of five elderly women attending C welfare center located in downtown Seoul the data was collected. Each interview lasted an hour and a half and was arranged five times. The analysis of this research was conducted using the Giorgi method. Results: \circled1 There was obvious physical and physiological decline caused by aging old; as well as there being spouse health problems, additional physical suffering, signs of senility, and insomnia, \circled2 Further grief was imposed by unpleasant memories of the spouse; infidelity, incapability, and even disregard of her own well-being, \circled3 Then there was pity for children; unfaithful children, uncertain futures of the children, and early death of a child, \circled4 Also, regrettable fate, painful daily acttrities, unreliable factors, bad circumstances, and feelings emptiness were reported, \circled5 Finally, anxiety for the future; ac sense of despair, loneliness, economic hardship, and the fear of imminent death increased grief levels. Conclusion: A variety of programs and social meetings for the elderly to overcome their physiological or psychological crisis should be substantially developed and supported by the government. In order to implement the social welfare for the elderly women, special consideration whether on the governmental level or the personal level, should be devoted to the elderly who live without any financial support or social concern.
