Cyclosporin A 혈중농도와 백서 치은증식과의 관계 (The Srudy on the Relationship between blood Cyclosporin A level and Gingival Overgrowth in rats)

  • The purpose of this study was to evaluate clinically and histopathologically the effects to the periodontal tissue in rats after Cyclosporin A (CsA) administration and to determine whether there is a relationship between dosage of CsA or blood CsA level and the seventy of gingival overgrowth in rats. Twenty 6-week-old Sprauge-Dawley Fats were randomized into 4groups. The control group received olive oil only and the test group received daily CsA in olive oil via gastric feeding for 6weeks at a 3,10, and 30mg/kg. Rats were weighed to evaluate the systemic effect of drug and stone models were made from alginate impressions of upper and lover anterior region at 2 week interval. On completion of offal CsA administration, blood were collected and blood CsA levels were quantitated by TDxFLx analyzer. Rats were sacrificed an6 their upper and lower jaws were removed together with the surrounding gingiva and soft tissue for light microscopic examination. The results were as follows : 1. The weight gain of GsA-treated rats was much less than of the control group and central incisors were gradually displaced and separated in the test groups. 2. The extensive fibrovascular proliferation and scattered inflammatoy infiltrates in an edematous stroma were observed in enlarged gingiva of CsA-treated rats. 3. The increase in buccolingual, mesiodistal dimension of the anterior teeth and vertical height of the interdental papilla showed dose-dependent manner in CsA-treated rats. 4. Significant positive correlation exists between blood CsA level and the severity of gingival overgrowth in anterior teeth. This result indicates that the severity of gingival enlargement in CsA treated rats is correlated with dosage of CsA administration and blood CsA level.

Cyclosporine A에 의해 과증식된 치은 조직에서 배양된 섬유아세포의 성장인자 발현에 관한 연구 (THE mRNA EXPRESSION OF GROWTH FACTORS IN FIBROBLAST FROM GINGIVAL HYPERPLASIA INDUCED BY CYCLOSPORINE A)

  • Cyclosporine A (CsA) is a powerful immunosuppresive agent used to prevent graft rejection of organ and treat autoimmune disease. One of the major side effects associated with CsA treatment is the development of gingival overgrowth. The purpose of this study was to investigate the mRNA expression and association of the several growth factors in gingival overgrowth induced by CsA, respectively. Gingival fibroblasts were obtained from gingival tissues of healthy donor and the patients treated with CsA. The cultured gingival fibroblasts were incubated with increasing concentrations of CsA for 24 hours, and the expression of MMP-1, TIMP-1, $TGF-{\beta}_1$, p21 were determined by reverse transcription-polymerase chain reaction (RT-PCR). The expressions of MMP-1 was slightly increased according to the concentration of treated CsA, but there was no statistical significance. TIMP-1 showed the increased expression at the CsA concentration of 250 and 500 ng/ml and significantly decreased at the CsA concentration of 750ng/ml. $TGF-{\beta}_1$ showed the increased expression at the CsA concentration of 500 and 750 ng/ml. The expression of p21 was not changed significantly. We concluded that the gingival hyperplasia induced by CsA was more related with $TGF-{\beta}_1$ than MMP-1 or TIMP-1 on gingival collagen metabolism in patients treated with CsA.

  • Generalized gingival enlargement, also known as gingival hyperplasia or hypertrophy, is defined as an abnormal diffuse overgrowth of gingival tissues. There are several causes of generalized gingival enlargement and they can be grouped into four categories: hereditary gingival fibromatosis, medication-induced, inflammatory, and systemic or neoplastic causes of gingival enlargement. This paper reviews the clinical features, differential diagnosis and significance of generalized gingival enlargements.

치은비대에서 비외과적 치료의 효과 (The effect of non-surgical treatment in gingival enlargement)

  • Purpose: It is well recognized that gingival enlargement is induced by unwanted effect associated with three major drugs/drug groups - phenytoin, cyclosporine, and the calcium channel blockers. The present case report describes the effect and limitation of non-surgical treatment in gingival enlargement cases. Materials and methods: Three cases included 2 drug-influenced gingival enlargement patients and a idiopathic gingival fibromatosis patient. For the drug-influenced gingival enlargement patients, the medication was replaced with other medication. And then, all the patients were treated non-surgically. Results: Drug-influenced gingival enlargements had been reduced after non-surgical treatment and the results were well-maintained. In the idiopathic gingival fibromatosis case, non-surgical treatment resulted in only limited reduction of gingival enlargement, and surgical periodontal treatment was unavoidable. Conclusion: These case reports indicated that non-surgical periodontal treatment with change in medication was effective in the treatment of drug-influenced gingival enlargements. Non-surgical approach can be considered as the primary management to reduce the gingival enlargement. If non-surgical treatment encounters a limitation, surgical treatment should be considered.

유치열기에서 나타난 치은섬유종증 환자의 장기간 관리 (Long-term Management of a Gingival Fibromatosis Patient with the Primary Dentition)

  • 치은 섬유종증은 흔하지 않은 성장기 구강질환으로, 치은 변연과 치간 유두뿐만 아니라 부착치은의 전반에 걸친 섬유성 증식을 특징으로 한다. 본 증례의 환아는 전반적인 치은비대를 보이는 27개월 여아로 특별한 의학적 전신병력은 없으나 오빠에게서 같은 증상을 보이는 가족력이 존재하였다. 심미적 요구와 영구치 맹출 시기를 고려하여 만 5세경에 전신마취 하에 치은절제술 및 잔존 유치의 발치를 시행하였고 이후 1년 6개월의 추적검사 기간 동안 재발 양상은 관찰되지 않았다. 치은 섬유종증의 치료법으로 과증식된 치은 조직의 외과적 절제술을 고려할 수 있으며, 수 년 내에 재발하려는 경향이 있으므로 전문적이고 지속적인 구강 위생 관리가 필요하다. 외과적 치료를 지연하면 영구치의 맹출 지연으로 인한 유치의 잔존, 저작과 발음의 어려움, 부정교합 및 환자의 심리적인 문제 등을 초래하므로 치은 비대의 정도에 따라 유치열에서 외과적 치료를 할 수 있다.

흰쥐에서 nifedipine으로 유발된 치은 증식증 및 하악선 분비기능에 대한 작약 추출물 저해효과 (Relation of Paeonia lactiflora Pallas to Nifedipine-induced Gingival Hyperplasia and Impaired Submandibular Glands Function in Rats)

  • Calcium-channel blockers such as nifedipine could be associated with gingival overgrowth. The aim of this study was to examine the role of Paeonia lactiflora Pallas(PLP) on nifedipine-induced gingival hyperplasia along with submandibular secretory function in rats. Animals in divided groups received nifedipine (250 mg/kg) alone and in PLP(100, 200 mg/kg) in orally administration for 3 weeks. Pure submandibular saliva was collected intraorally by micropolyethylene cannula and the mandibular gingiva was examined by means of dissecting microscope for signs of redness, tickness, inflammation and exuda. Twenty-one days nifedipine treatment induced gingival hyperplasia accompanied with reduced salivary flow rate and concentrations total protein, epidermal growth factor(EGF) and calcium in comparison with normals. Co-treatment of animals with nifedifine and PLP protected from gingival hyperplasia and retained flow rate, and concentrations of total protein, EGF and calcium in normal levels.

Glycyrrhetinic acid와 oleanolic acid가 배양 치은 섬유모세포의 cyclosporine A 유도 세포활성에 미치는 영향 (THE EFFECTS OF GLYCYRRHETINIC ACID AND OLEANOLIC ACID TO CYCLOSPORINE A INDUCED CELL ACTIVITY OF CULTURED GINGIVAL FIBROBLASTS)

  • Cyclosporine A is an immunosuppressant commonly used for patients receiving organ transplants. Gingival overgrowth is an adverse side-effect seen in about 8-26% of patients taking cyclosporine A which have been shown to increase the DNA synthesis of gingival fibroblast at the concentration of $10^{-9}g/ml$ in vitro. Glycyrrhetinic acid is the active pharmacological ingredients of licorice which exerts steroid-like action and anti-viral activity. Oleanolic acid, which were isolated from Glechoma hederacea, has been shown to act as inhibitors of tumor promotion in vivo and to be less cytotoxic retinoic acid. This study has been performed to evaluate the effects of glycyrrhetinic acid and oleanolic acid on cyclosporine A induced cell activity in vitro. Human gingival fibroblasts were isolated from explant cultures of healthy gingiva of orthodontic patients. Gingival fibroblasts were trypsinized and transferred to the walls of microtest plates. Fibroblasts were cultured in growth medium added $10^{-9}g/ml$ cyclosporineA and $50{\mu}l/ml$ lipopolysaccharides. Cells between the 4th and 6th transfer in culture were used for this study. The morphology of gingival fibroblst were examined by inverted microscope. The effects of cyclosporine A on the time course of DNA sythesis by human gingival fibroblasts were assessed by $[^3H]-thymidine$ uptake assays. Cyclosporine A was found to stimulate DNA synthesis of human gingival fibroblast at a concentration of $10^{-9}g/ml$. In the presence of lipopolysaccharide derived from Fusobacterium nucleatum, addition of cyclosporine A results in reversal of inhibition at the concentration which normally inhibits gingival fibroblast proliferation. The cell acitivities in the presence of glycyrrhetinic acid and oleanolic acid were decreased, and increased cell acitivities by cyclosporine A were decreased by glycyrrhetinic acid and oleanolic acid at the concentration of $200{\mu}g/ml$. These results suggested that the increased cell activities by cyclosporine A modulated by glycyrrhetinic acid and oleanolic acid.
