Purpose: Peri-implant sulcular fluid (PISF) has a production mechanism similar to gingival crevicular fluid (GCF). However, limited research has been performed comparing their behavior in response to inflammation. Hence, the aim of the present study was to comparatively evaluate PISF and GCF volume with varying degrees of clinical inflammatory parameters. Methods: Screening of patients was conducted. Based on the perimucosal inflammatory status, 39 loaded implant sites were selected from 24 patients, with equal numbers of sites in healthy, peri-implant mucositis, and peri-implantitis subgroups. GCF collection was done from age- and sex-matched dentate patients, selected with gingival inflammatory status corresponding to the implant sites. Assessment of the inflammatory status for dental/implant sites was performed using probing depth (PD), plaque index/modified plaque index (PI/mPI), gingival index/simplified gingival index (GI/sGI), and modified sulcular bleeding index (BI). Sample collection was done using standardized absorbent paper strips with volumetric evaluation performed via an electronic volume quantification device. Results: Positive correlation of the PISF and GCF volume was seen with increasing PD and clinical inflammatory parameters. A higher correlation of GCF with PD (0.843) was found when compared to PISF (0.771). PISF expressed a higher covariation with increasing grades of sGI (0.885), BI (0.841), and mPI (0.734), while GCF established a moderately positive correlation with GI (0.694), BI (0.696), and PI (0.729). Conclusions: Within the limitations of this study, except for minor fluctuations, GCF and PISF volumes demonstrated a similar nature and volumetric pattern through increasing grades of inflammation, with PISF showing better correlation with the clinical parameters.
This investigation was undertaken to determine the relationship between the amount of polymorphonuclear leukocyte(PMN) enzyme myeloperoxidase(MPO) in gingival crevicular fluid(GCF) collected from active or control site and gingival disease status described by clinical indices(gingival index, papillary bleeding index, pocket depth, periotron unit). The results were as follows : 1. MPO activity/site was greater at active sites than at control sites. 2. According to increasing the clinical parameters, MPO/sites was higher statistically (P<0. 01, P<0.05). 3. High MPO(unit/site) groups was higher statistically than low MPO(unit/site) groups in various clinical parameters. 4. Correlation coefficients between MPO(unit/site) and GI, MPO($unit/{\mu}l$ GCF) and periotron unit were 0.4782, -0.5901, respectively.
Objectives : This study mean to confirm the antibacterial activity of a garlic extract widely culturing in our region and was to determine the effect of dentifrice containing 0.1% extracts of garlic on dental plaque and gingivitis in a double blind and clinical studies in 50 healthy adults aged from 20 to 22 years who provided a consent for their participation. Methods : The antibacterial activity was evaluated using triple distilled water and the Kirby-Bauer disk diffusion method and the minimal inhibitory concentration(MIC) against various pathogens for periodontal disease, such as P. gingivalis 381(ATCC33277), was estimated. The experimental groups classified according to the concentration of garlic extract used: 10,000ppm(A), 5,000ppm(B), 2,500ppm(C), 1,000ppm(D). Oral examination of subjects was performed through clinical periods and on day of baseline, 6, 12, 19, 25 days plaque index and gingival index were scored by Turesky' modified index and Loe & Silness index. After 12, 19, 25 days use of their respective dentifrices, statistically decreases of plaque index, gingival index were shown in both the experimental and the control group, respectively. Results : There was significant antibacterial activity in the "2,500ppm(C)" group against P. gingivalis 381. Experimental group exhibited significantly the lower plaque levels and the higher levels of gingival health by the use of the dentifrices contained extract of garlic from 6 days compare with control group(p<0.05). The degree of decrease was more significant on gingivitis level of the experimental group than the control group(p<0.05). Conclusions : This findings indicated that the oral products containing a garlic extract is effective in preventing and treating periodontal diseases, and has potential value in inhibiting periopathogens.
A comparative clinical study on the ordinary toothbrush($Buttler^{(R)}$, America) and the silicone toothbrush($Jefe^{(R)}$, Korea) was performed. The volunteers who took part in this study were students of Dental college of Yonsei University and patients attending Dental Hospital of Yonsei University. They were classified into two group, control and experimental group. Control group brushed with nylon toothbrush and experimental group did with silicone toothbrush under the researcher's guidances. Volunteers were examined on Plaque Index(PI), Gingival Index(GI), Probing Depth(PD), Bleeding on Probing(BP) and Recession(R) at base line, 1st. week, 2nd. week and 4th. week. According to the results, both group have the tendency of improvement in the degrees of GI, PI and the improvement degree of GI of both group has the significant differences from base line statistically, and there are not statistically significant differences between the silicone and nylon group in respect of PI, GI values. So based on the present study, it could be carefully ascertained that the silicone toothbrush has similar effect with nylon toothbrush in respect of PI and GI. If it is sure that the silicone toothbrush is seldom abrasive and possibly enough to massage the gingiva, this new brush is worth to be recommended by the dentists.
치열이 발육 중에 있는 소아는 치은 및 치주 질환에 매우 흔하게 이환된다. 이 시기의 치은염은 대개 경미하며 가역적인 경우가 많으나, 만성적으로 진행하여 성인기의 치주질환으로 이행되는 경우도 발생한다. 소아의 치은 평가를 위한 방법으로 흔히 임상적인 치은염 평가 지수나 구강 위생 지수를 측정하게 된다. 치은염 측정 지수의 하나로써 치은 열구액은 치주조직과 치은 열구로부터 유래되는 염증성 삼출물로, 세균성 치태의 점진적인 축적에 대한 반응으로 증가한다고 알려져 있다. 본 연구에서는 소아의 치은 상태 평가를 위해 전북대학교 병원 소아치과에 내원한 3-14세(평균 $8.5{\pm}3.1$세)의 어린이 50명과 23-33세 (평균 $26.1{\pm}3.3$세)의 성인 20명을 대상으로 육안적 지수인 치은지수와 치태지수를 조사하였다. 또한 이와 함께 치은 열구액을 채취하여 그 부피를 Periotron 8000(Oraflow Inc., USA)을 이용하여 측정하였고, 소아와 성인의 수치를 비교 분석한 결과 다음의 결론을 얻게 되었다. 1. 어린이에서 유치열기와 영구치 맹출기, 맹출 완료기의 각 군 간에 치은 열구액 부피를 비교했을 때, 평균치는 영구치 맹출기에서 가장 높게 나타났으나 통계적으로 유의한 차이를 보이지 않았다(p>0.1). 2. 어린이와 성인을 비교하였을 때 치은 열구액 부피는 어린이에서 유의성 있게 크게 나타났다(p<0.001). 3. 어린이와 성인 모두 임상적인 치은지수(GI)와 치태지수(PLI)는 치은 열구액의 부피와 유의성 있는 상관관계를 보였다(GI; r=0.394, p<0.001 PLI; r=0.642, p<0.001). 4. 치은 열구액을 채취한 치아의 교정 치료 유무는 치은 열구액의 부피와 유의성 있는 양의 상관관계를 보였다(p<0.001). 그러나 레진 치료 유무나 치아 우식의 유무는 관계가 없었다(p>0.05). 5. DMFT치는 유치열기에서 영구치열기보다 높게 나타났다(p<0.001). 또한 DMFT치와 치은 열구액 부피는 유의성 있는 상관관계를 보이지 않았다(p>0.1).
The purpose of this study was to investigate the comparative effects of subgingival irrigation using some oral mouth rinses on early healing process of periodontal inflammation. The study population consisted of 13 patients with periodontal inflammation and distributed into 4 groups. Oral hygiene instruction, delicate scaling and root planing were done and then irrigated per 3 days during 2 weeks in situ with 1 of 4 solutions ; normal saline, C31G, Benzotonium chloride and tetracycline. Examination regarding probing pocket depth, plaque index, sulcular bleeding index, gingival index, gingival recession and leukocytes differential count was performed. Evaluation was made at the baseline and 2 weeks after non-surgical periodontal therapy. The results were as follows : 1. Clinical indices including probing pocket depth, plaque index, sulcular bleeding index, gingival index and gingival recession were significantly improved from baseline to 2 weeks. But there was no significant differences among 4 groups. 2. PMNs percent on leukocytes differential count was significantly decreased from baseline to 2 weeks on all groups. Those of tetracycline and C31G were significantly decreased than those of normal saline group. These results suggest that clinical indices were not different, but the decrease of inflammation were significantly different among some mouth rinses.

Purpose: Periodontal diseases are inflammatory conditions that alter the host's response to microbial pathogens. Type 2 diabetes mellitus (T2DM) is a complex disease that affects the incidence and severity of periodontal diseases. This study investigated the gingival crevicular fluid (GCF) levels of colony-stimulating factor-1 (CSF-1) and interleukin-34 (IL-34) in patients with stage III grade C periodontitis (SIII-GC-P) and stage III grade C periodontitis with uncontrolled type 2 diabetes (SIII-GC-PD). Methods: In total, 72 individuals, including 24 periodontally healthy (PH), 24 SIII-GC-P, and 24 SIII-GC-PD patients, were recruited for this study. Periodontitis patients (stage III) had interdental attachment loss (AL) of 5 mm or more, probing depth (PD) of 6 mm or more, radiographic bone loss advancing to the middle or apical part of the root, and tooth loss (<5) due to periodontal disease. Radiographic bone loss in the teeth was also evaluated; grade C periodontitis was defined as a ratio of the percentage of root bone loss to age greater than 1.0. The plaque index (PI), gingival index (GI), presence of bleeding on probing (BOP), PD, and clinical AL were used for clinical periodontal assessments. GCF samples were obtained and analyzed using an enzyme-linked immunosorbent assay. Results: All clinical parameters-PD, AL, GI, BOP, and PI-were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups for both the full mouth and each sampling site (P<0.05). The total IL-34 and CSF-1 levels were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups (P<0.05), and there were significant differences between the periodontitis groups (P<0.05). Conclusions: These findings suggest that IL-34 and CSF-1 expression increases in patients with SIII-GC-PD. CSF-1 was associated with the inflammatory status of periodontal tissues and T2DM, while IL-34 was associated only with T2DM.
Purpose: This randomized controlled study aimed to evaluate the effects of an electric toothbrush with 3 colors of light-emitting diodes (LEDs) on antiplaque and bleeding control. Methods: This randomized, placebo-controlled, double-blinded, parallel-group clinical trial included 50 healthy adults with gingivitis, who were randomly assigned to 2 groups. The experimental group used electric toothbrushes with 3 colors of LEDs and the control group used the same electric toothbrush as the experimental group, but with LED sources with one-hundredth of the strength. The subjects used the electric toothbrush 3 times a day for 4 minutes each time. As clinical indices, bleeding on marginal probing (BOMP), the Löe-Silness gingival index (GI), and the Turesky-Quigley-Hein plaque index (QHI) were assessed at baseline, at 3 weeks, and at 6 weeks. Results: There were significant decreases in all clinical indices (BOMP, GI, QHI) in both the experimental and control groups compared to baseline at 3 weeks and at 6 weeks. In a comparison between the experimental and control groups, no statistically significant differences were observed for any clinical indices at 3 weeks (P>0.05). However, at 6 weeks, statistically significant differences were observed between the experimental and control groups in BOMP and GI, which are indicators of gingival inflammation (P<0.05). Conclusions: This study demonstrated that an electric toothbrush combined with 3-color LEDs reduced gingival bleeding and inflammation after 6 weeks.
This double-blind controlled clinical and microbiological study was carried out to determine the effects of mouthwash preparation containing the mixture of herbal extract on developing plaque and gingivitis in the experimental gingivitis model. Following a 2-week normalization period, 34 dental students were distributed randomly into 1 of 3 treatment groups. They rinsed, under supervision, two times daily for 3 weeks with either normal saline(CT), 0.1% chlorhexidine(CH), or the mixture of herbal extract (HT), but refrained from any oral hygiene measures. The Plaque Index(PlI), the Gingival Index(GI), and the amount of Gingival Crevicular Fluid(GCF) were measured at week 0,1,2, and 3 of the experimental period, while the assessment of total wet weight of plaque and the phase contrast microscopic examination of plaque were performed at the end of experimental period(3 weeks). Subjects using mouthrinse preparation containing the mixture of herbal extract demonstrated negligible, if any, changes in the accumulation and microbial composition of plaque compared to those using normal saline, while the reduction of gingival inflammation by this mixture was highly significant and comparable to that of chlorhexidine. The results of this study indicate that the preparation containing the mixture of herbal extract do not provide any antiplaque benefits but is very effective in inhibiting the development of and in reducing existing experimental gingivitis when used as mouthrinse. Further research is needed to determine whether a significant reduction of gingival inflammation without a concomitant decrease in plaque accumulation is of clinical importance.
목적: 이 논문의 목적은 발광 다이오드(Light Emitting Diode(LED))의 유무에 따른 전동칫솔의 항치태 및 항치은염 효과를 치은염 및 경도의 치주염 환자에서 관찰하는 것이었다. 연구 재료 및 방법: 30명의 환자를 실험군, 대조군으로 각각 15명씩 나누어 실험을 진행하였다. 실험군은 정상 작동의 LED 전동칫솔을 사용하였고 대조군은 LED가 나오지 않도록 개조된 동일 제품의 전동칫솔을 사용하였다. 기준(Baseline), 2주, 4주 후의 임상지표($L{\ddot{o}}e-Silness$ gingival index (GI), Quigley-Hein plaque index (PI))를 측정하여 Wilcoxon signed rank test, Mann-Whitney test를 통해 통계적 분석을 시행하였다. 결과: 실험군과 대조군 간의 시간에 따른 GI의 변화는 두 군 모두 시간에 따른 감소를 나타내었으나, 2주, 4주차에서 유의한 차이를 나타내었다. PI는 4주차에서 실험군에서 대조군보다 낮은 수치를 나타내었는데, 통계적으로 유의한 차이는 아니었다. 결론: 본 연구에서 LED가 장착된 전동 칫솔은 단기간의 연구에서 치은염의 감소효과를 보였고, 장기간의 관찰 시 치태의 축적을 감소시킬 수 있다 추측할 수 있었다.
