세기변조방사선치료의 품질관리를 위한 이온전리함 매트릭스의 유용성 고찰 (Feasibility of MatriXX for Intensity Modulated Radiation Therapy Quality Assurance)

  • 목 적: 세기변조방사선치료 시 선행되는 품질관리에서 선량분포와 절대선량 값을 비교하여 2차원 이온전리함의 유용성을 알아보고자 한다. 대상 및 방법: 본 실험은 선형가속기(CL 21EX, Varian, Palo Alto, USA)의 6 MV 광자선을 이용하여 비인두암 환자를 대상으로 하였다. 세기변조방사선치료를 위한 품질관리에는 2차원 이온전리함 배열(MatriXX, Wellhofer Dosimetrie, Germany)을 사용하였다. 비인두암 환자의 치료를 위해 역방향치료계획을 시행하였다. 고체팬톰에 삽입된 MatriXX를 전산화단층 촬영하여 환자 치료계획과 동일한 플루언스로 하이브리드(갠트리 각도 0$^\circ$) 치료계획을 실시하였다. 실험적 선량분포의 측정은 하이브리드 치료계획과 동일한 기하학적 조건으로 MatriXX와 고체팬톰을 이용하여 측정하였다. 선량분포에서 고선량 저변위(High Dose Low Gradient: HDLG) 점을 선정하여 절대선량을 비교하였으며, 선량분포 일치성분석을 위해 감마 인덱스 히스토그람($\gamma$-index, Dose difference: 3%, Distance to agreement: 3 mm)을 이용하여 정량화하였다. 결 과: 감마 인덱스 히스토그람 분석결과 인정허용범위(${\gamma}{\leq}$1) 비율을 모든 조사면에서 각각 확인하였다. 갠트리 각도 0$^\circ$에서 95.08%, 55$^\circ$에서 97.52%, 110$^\circ$에서 96.28%, 140$^\circ$에서 98.2%, 220$^\circ$에서 97.78%, 250$^\circ$에서 96.64%, 305$^\circ$에서 92.7%로 나타났다. HDLG에서 절대선량은 $\pm$3% 이내의 일치도를 보였다. 결 론: MatriXX를 이용한 세기변조방사선치료의 품질관리는 필름이나 단일 이온전리함을 이용한 일반적인 세기변조방사선치료 품질관리에 비해 시간, 인력을 최소화하면서 보다 효율적인 접근이 가능하였으며, 선량분포일치성 분석 및 절대선량확인에 매우 유용한 장치임을 알 수 있었다.

동아시아의 인구사상: 홍량길과 맬서스의 비교 (Population Thoughts in East Asia: A Comparison of Hung Liang-Chi and Malthus)

  • 오늘날 전 세계 학문에 절대적인 영향을 미치고 있는 서구 학문의 기초는 그리스와 로마 문명임은 잘 알려진 사실이다. 그 결과 이집트와 페르시아를 포함하는 아랍세계의 훌륭한 업적과, 인도대륙의 심오한 사상, 그리고 중국을 포함하는 동아시아의 탁월한 업적이 등한시되거나 도외시되어 왔다. 인구에 관한 사상과 이론도 예외는 아니다. 본 연구는 우리가 간과(看過)한 과거 인구사상 중 동아시아 부분을 중국학자들을 중심으로 고대, 중세, 근세 등 시기로 나누어 유럽학자들과의 비교를 통해 재조명해 보았다. 중국을 위시한 동아시아에서는 고대로부터 과잉 및 과소 인구에 대한 우려가 자주 거론되었다. 춘추시대 인구에 대한 주요 관심은 인구와 자원과의 균형관계였으며 인구가 국부의 원천임은 대체로 인정하고 있었으나 과소인구에 대한 대책뿐만 아니라 과잉인구에 대한 단점도 심각하게 거론되고 있어 적정인구와 근대 맬서스주의의 기초는 이미 고대 춘추전국시대에 마련됐다고 볼 수 있다. 서양고대 그리스와 로마시기의 인구 사상의 발전은 플라톤과 아리스토텔레스의 적정인구론과 로마시기 팽창주의적 인구증가 장려책으로 대표된다. 천년이란 장기간의 중세 시기 유럽의 발전은 정체되어있었으며, 동아시아의 발전 역시 부진했으나 아랍세계의 발전은 주목할만한 것으로 알려져 있다. 근세에 들어와 유럽 특히 서구에서는 눈부신 학문적 발전을 이룩한데 비해 동아시아의 발전은 매우 저조한 것이었다. 그러나 맬서스가 그의 인구이론을 발표하기 수년전 중국의 홍량길은 불완전하나마 인구의 기하급수적 증가율의 문제를 논하고 인구증가에 대한 규제로서 맬서스의 적극적 규제 및 궁극적 규제에 해당하는 천지조제법과, 소극적 규제 및 즉각적 규제에 해당되는 군상조제법을 들고 있다. 특히 그가 상대적 과잉인구에 대해 논의를 전개하고 있음이 주목된다. 마지막으로 고대 탁월한 동아시아의 학자들과 근세 맬서스 직전 홍량길과 같은 학자도 있었으나 동아시아에서의 학문발전은 근세 이후 서양에 비해 비교될 수 없을 정도로 부진하게 된 이유를 학문내적 요인과 학문외적 요인으로 나누어 검토했다.

입체조준장치를 이용한 중추신경계의 방사선 입체조형치료 계획 (3-D Conformal Radiotherapy for CNS Using CT Simulation)

  • 목적 : 척수에 전이가 가능한 뇌종양 치료를 위한 두개척수 방사선치료를 전산화단층촬영장치(volumetric spiral CT)와 입체조준장치(CT simulator) 및 3차원 조형치료계획장치(3D conformal planning system)를 이용한 두개척수 방사선치료계획 방법을 개발하고 기하학적 검증을 통하여 유용성과 정확성을 평가한다. 방법 : 연세암센터 방사선종양학과에서 두개척수 방사선치료를 받은 환자 11명을 대상으로 전산화 단층촬영을 이용한 입체 모의치료계획과 3차원 방사선조형치료를 시행하였다. 중증의 뇌종양 환자의 두경부는 두부고정틀(thermoplastic mask, $Aquaplast^{R}$)로 고정시키고, 전신은 $Vac-Loc$ $Aquaplast^{R}$ (전성물산, 한국)으로 고정한 후 전산화단층촬영장치(Spiral CT)를 이용하여 전신체적영상(volumetric image)을 얻었다. 환자자세의 재현성 확인 및 검증을 위해 두부에 세 개의 점과 전신에 기준선 및 기준점 등을 표시하였다. 이후 입체조준장치(CT-simulator)의 가상현실영상(virtual fluoroscopy)에서 인체의 크기와 방향에 제약이 없고 치료 침대와 고정기구에 대한 시각장애를 제거함으로써 자유롭게 모의치료를 할 수 있었으며, 조사면과 선속을 결정하고 디지털화재구성사진(digitally reconstructed radiography, DRR)과 디지털화합성사진(digitally composited radiography, DCR)을 통하여 분해능이 좋은 화질의 투시 및 모의치료영상을 획득하였다. 기하학적 검증은 치료중심점 이동시 얻은 모의치료영상과 첫 치료 시에 얻은 조사면 검증 사진(port verification film) 등을 전산화단층촬영영상으로부터 재구성한 DRR 영상과 시각적, 정량적으로 비교, 분석 였다. 결론 : 입체조준장치와 3차원 방사선치료계획 장치 등을 이용하여 두개척수 방사선 치료계획을 신속하고 정확하게 원활히 수행할 수 있었다. 가상현실영상에서 대부분의 설계작업이 이루어지므로 환자의 자세고정을 요하는 시간은 전신체적 영상을 얻는 10분 이내이므로 환자의 불편을 줄일 수 있을 뿐 아니라 모의치료과정 중의 체위 변동 변수를 제거할 수 있었다. 또한 전산화단층촬영영상을 얻음으로써 중요정상조직인 안구, 척수 등을 정확하게 설정할 수 있었고, 조사면 결정과 차폐의 정확성을 증진시킬 수 있었다. 환자자세오차는 디지털화재구성사진과 치료 시마다 얻은 포트필름에서 치료중심점과 척수 사이의 거리를 측정하여 3 mm 이내의 정확성을 얻을 수 있었다. CT조준장치를 이용한 중추신경계의 방사선 입체조형치료는 가상현실모의치료계획으로 두개척수의 방사선치료를 정확하고 용이하게 실현할 수 있었다.

경상분지 의성지괴 길안면지역에서 청송화강암의 단열 발달사 및 운동성에 대한 기하학적 해석 (Geometrical Interpretation on the Development Sequence and the Movement Sense of Fractures in the Cheongsong Granite, Gilan-myeon Area, Uiseong Block of Gyeongsang Basin, Korea)

  • 경상분지 의성지괴의 중앙 북부에 위치하는 안동시 길안면 지역은 선캠브리아기 변성암류, 트라이아스기 청송화강암, 백악기 초 하양층군, 백악기 말-고제3기 화성암류 등으로 구성되어 있다. 이 지역에는 다양한 방향의 단층(남북 내지 북북서 방향의 옥산단층, 북서 방향의 길안단층, 서북서 방향의 황학산단층, 그리고 동서 방향의 임봉산단층)들이 발달하고, 경상분지의 기반암류에 해당하는 청송화강암내에는 다수 단열조의 상대적 시간관계(선후관계 및 공존관계)와 전단단열의 운동감각을 결정하는데 이용되는 기하학적 지시자가 잘 관찰된다. 본 논문은 이들 단열조의 기하학적 특성(연결, 종료, 교차형상 및 절단관계)에 대한 정밀한 분석을 통하여 경상분지 의성지괴 길안면 지역에 발달하는 인장단열의 발달사와 전단단열의 운동성을 연구하였다. 그 결과, 길안면 지역에 발달하는 단열계는 적어도 7회의 변형단계(Dn 이전 단계에서 Dn+5 단계로 명기)를 걸쳐 형성되었고, 단열조의 우세 방향성은 길안면 주변지역에 발달하는 지질도 규모의 단층 우세 방향성과 거의 일치하는 (서)북서, 북북서, 북북동, 동서, 북동 순서로 나타난다. 단열조의 발달사와 운동성를 요약하면 다음과 같다. (1) Dn이전 단계: 남북 내지 북북서 또는 서북서 내지 동북동 방향의 인장단열 형성기. 이후 응력장 변화와 함께 남북 내지 북북서 방향의 절리조는 좌수향 ${\rightarrow}$우수향${\rightarrow}$좌수향 전단단열운동으로 그리고 서북서 내지 동북동 방향의 절리조는 (우수향${\rightarrow}$)좌수향 전단단열운동으로 각각 재활동하였다. (2) Dn 단계: 북서 방향의 인장단열 형성기. 이 단계의 절리조는 이후 좌수향${\rightarrow}$우수향 순서의 전단단열운동을 경험한다. (3) Dn+l 단계:북북동 내지 북동 방향의 인장단열 형성기. 이후, 좌수향 전단단열운동으로 활동하게 된다. (4) Dn+2 단계: 동북동 내지 동서 방향의 인장단열 형성기. (5) Dn+3 단계: 서북서 내지 북서 방향의 인장단열 형성기. (6) Dn+4 단계: 북북서 방향의 인장단열 형성기. 이후, 우수향 전단단열운동으로 활동하였다. (7) Dn+5 단계: 북북동 방향의 인장단열 형성기.

임플란트 나사산 디자인이 변연골 응력에 미치는 영향 (Effect of thread design on the marginal bone stresses around dental implant)

  • 연구 목적: 본 연구의 목적은 치과용 임플란트 나사산 디자인이 변연골 응력에 미치는 영향에 정량적인 분석을 하고자 한다. 연구 재료 및 방법: 외경 4.1 mm (경부직경 3.5 mm), 매식부 길이 10 m인 표준형 ITI 임플란트 시스템(ITI Dental Implant System; Straumann AG, Waldenburg, Switzerland)을 기본모델(대조모델)로 채택하고, 그 몸체의 나사산은 다른 임플란트 시스템에 채택되고 있는 삼각형, 사각형, buttres형 디자인을 가지는 가상의 해석모델을 4종 만들었다. 해석모델은 나사산 형태와 크기에 따라 (1) 모델 A (작은 삼각형 나사산), (2) 모델 B (큰 삼각형 나사산), (3) 모델 C (buttres형 나사산), 및 (4) 모델 D (사각형 나사산)로 구분하였다. 유한요소 모델링과 해석에는 NISA II/DISPLAY III (Engineering Mechanics Research Corporation, Troy, MI, USA) 프로그램을 사용하였다. Mesh 구성에는 NKTP type 34형 solid 요소(4각형 축대칭 요소, 요소당 절점수 8개)를 사용하여 임플란트 장축과 평행한 축대칭 하중은 물론 장축과 경사각을 갖는 비축대칭 하중조건을 모두 해석할 수 있도록 하였다. 임플란트의 표면으로부터 각각 0.2, 0.4, 0.6, 0.8, 1.0 mm 떨어진 위치에 5개의 응력관찰점(stress monitoring point)을 설정 하여 기록된 응력 값으로부터 회귀분석을 통하여 변연골 응력 최대값(peak stress)을 정량화하였다. 해석에 사용한 하중 조건은 2가지로, 임플란트 축에 평행한 수직하중 100 N과 임플란트 축과 $30^{\circ}$를 이루는 경사력 100 N 조건이었다. 결과: 임플란트 경부와 접하고 있는 인접 변연골에 응력집중현상이 보이고 있었으며, 그 양상은 임플란트 나사산 디자인과 무관하게 거의 유사하게 관찰되었다. 수직력 100 N 조건에서 산출된 변연골 최대응력값은 대조모델과 실험모델 A, B, C, D에서 7.84, 6.45, 5.96, 6.85, 5.39 MPa이었고, 경사력 조건에서는 각각 29.18, 26.45, 25.12, 27.37, 23.58 MPa이었다. 결론: 임플란트 나사산의 디자인은 변연골의 응력에 영향을 미치는 중요한 요소이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases.
