Assessing the EORTC QLQ-BM22 Module Using Rasch Modeling and Confirmatory Factor Analysis across Countries: a Comprehensive Psychometric Evaluation in Patients with Bone Metastases

  • Background: The European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire Bone Metastases Module (EORTC QLQ-BM22) is a recently designed supplement to EORTC Quality of Life Questionnaire-C30 (EORTC QLQ-C30). Additional psychometric properties, especially using confirmatory factor analysis (CFA) and the Rasch model, are warranted. Materials and Methods: A total of 573 patients with bone metastases were enrolled from eight countries with a mean${\pm}$SD age of $55.8{\pm}13.7years$. Slightly more than two thirds of them were female (n=383; 66.8%). CFA was used to examine the BM22 framework; Rasch models were applied to understand misfit items and differential item functioning (DIF). Results: The fit indices were satisfactory in CFA (comparative fit index=0.972, Tucker-Lewis index=0.964, root mean square error of approximation=0.076, and standardized root mean square residual=0.045). All items fit well in the Rasch models (mean square values were between 0.5 and 1.5), and only one item (number 17) displayed DIF across gender. However, there were six DIF items across Canada and Taiwan, ten across Canada and Iran, and six across Taiwan and Iran. Conclusions: The BM22 has satisfactory psychometric properties, and could assess the QoL of patients with bone metastases specifically focusing on their symptoms. Clinicians may want to use it to capture the underlying QoL for patients with bone metastases. However, the score of item 17 should be interpreted with caution when comparing male and female patients. In addition, researchers should note that variation in DIF items may occur when conducting an international study.

  • Background: The incidence of upper aero-digestive tract (UADT) cancers, including C00-C14, C30-C32, C15 and C16, is increasing rapidly in Kamrup Urban District (KUD) of Assam, North East (NE) India. According to the NCRP (2013) report 37.6% of all cancers in both sexes are UADT cancers in the NE region, accounting for 53.3% in males and about 27.5% in females of the total cases. Materials and Methods: A retrospective study was conducted for patient information from the period of 2008-2011. Age-standardized or age-adjusted rates (ASR or AAR) (per 100,000 person-years) were calculated using the World Standard Population as proposed by Segi and modified by Doll et al. The registry population area at risk was estimated using the 1991 and 2001 census population by sex, as well as the growth rate during that interval using the difference distribution method. Results: There were 5,638 cases registered during the last four years of the study (2008-2011) accounting for 56.7% (3,198/5,638) of the total in males and 43.3% (2,440/5,638) in females. The male: female ratio was 1.31:1.00. The overall age adjusted rates (AAR) were 179.4 and 153.8 per 100 000 males and females respectively. Cancer of the oesophagus was most common in both sexes, with most appreciable gender variation for tongue and hypopharynx, presumably reflecting differential expsoure to risk factors.

  • Narcotic drugs generally refer to serious and habitual hidden rash such as opium, heroin, methyphetamin, nabinol, cocaine, and so forth. At present, narcotic drugs are spread unchecked and are causing a big social problem. So many countries and narcotists are making every effort to set up a barricade against narcotic drugs. And there is a limit suitable treatment for them. Thus Tuo Yin Tang Jiang is developed. As indicated by Chinese letters, Tuo Yin Tang Jiang(TYTJ) is a crude drug. It is a traditional chinese medicine developed by the study done from June in 1998 to June in 1999 that Hubeisheng was entrusted with by People's Republic of China. This study is a treatise on etiology and syntomatology of narcotism. TYTJ is a medicine which is in accord with Pharmacopeia of the People's Republic of Chinal) in order to remove from the body the toxic materials resulting from narcotic drugs such as opium and heroin. According to the standard diagnosis on narcotism, 105 cases are studied and treated at the Rehabilitation Center attached to Enshi Autonomous Region Hospital in Hubeisheng. 105 cases are divided into 2 groups by double-blind method. One is the experimental group which has 56 cases. The other is the control group which has 49 ones.13 cases among 105 cases are addicted by intravenously injections. 9 cases are by oral takings. It took 10 days for this experiment to be performed. Two groups didn't show a striking individual variation based on the age, gender, period of taking drugs, withdrawal symptoms, complication, and state of health. The experimental group had a higher effect of treatment than the control group had. TYTJ treats diseases effectively and has no side effect, irrespective of the serious or slight addiction to opium and morphine.

소아에서 발생한 장중첩증에서 수술적 치료의 필요와 관련된 위험인자 (Risk Factors Associated with the Need for Operative Treatment of Intussusception in Children)

  • The aim of this study was to identify the risk factor related to the need for operative treatment and avoid unnecessary non-operative management for intussusception in children. We retrospectively reviewed medical records of patient treated for intussusception at our institution between January 2006 and January 2013. Clinical features such as gender, age, seasonal variation, symptoms and signs, treatment results were analyzed. Univariate and multivariate analyses including a chi-square test for categorical variables and logistic regression analysis were performed. During the study period, 356 patients were treated for intussusception. 328 (92.1%) was treated successfully by the non-operative pneumoreduction, and 28 (7.9%) required operative management. On univariate analysis, risk factors which were related to the need for operative treatment were age, vomiting, bloody stool, lethargy, and symptoms duration. A logistic regression analysis in order to assess for independent predictors of operative treatment was performed. Age (<6 vs ${\geq}12$ months) (OR 4.713, 95% CI 1.198~18.539, p=0.027) and symptoms duration longer than 48 hours (OR 4.534, 95% CI 1.846~11.137, p=0.001) were significantly associated with a requirement for operative treatment. We conclude that younger age and a longer duration of symptoms (${\geq}48$ hours) are the independent risk factor related to the need for operative treatment for intussusception. Early surgical intervention or transfer to a hospital with pediatric surgical capabilities should be considered for patients with these findings.

Estimation of Genetic Parameters of Body Weight Traits in Ghezel Sheep

  • The present study was carried out to estimate genetic parameters for body weight traits in Ghezel sheep. The data set used was records of 9,221 lambs from 180 sires and 5,060 dams for birth weight (BW), 7,206 lambs from 167 sires and 4,497 dams for weaning weight (WW) and 6,112 lambs from 157 sires and 3,841 dams for 6-months weight (6 MW), which were collected from 1999 to 2007 (9-years) at Ghezel sheep Breeding Station in west Azarbaijan. Variance components and corresponding genetic parameters were obtained with univariate analyses fitting animal models using restricted maximum likelihood (REML) methods. The most suitable model for each trait was determined based on log likelihood ratio tests. Birth year, lamb gender, type of birth, age of dam and herd were significant sources of variation on BW, WW and 6 MW (p<0.01). Direct estimate of heritability for BW, WW and 6 MW was 0.24, 0.29 and 0.37, respectively. The estimate of maternal permanent environmental variance as a proportion of phenotypic variance was 0.09 and 0.05 for BW and WW, respectively. The results of this study showed that genetic progress for growth traits is possible by selection.

Descriptive Epidemiology of Colorectal Cancer in University Malaya Medical Centre, 2001 to 2010

  • Background: Colorectal cancer is the second most frequent cancer in Malaysia. Nevertheless, there is little information on treatment and outcomes nationally. We aimed to determine the demographic, clinical and treatment characteristics of colorectal cancer patients treated at the University Malaya Medical Centre (UMMC) as part of a larger project on survival and quality of life outcomes. Materials and Methods: Medical records of 1,212 patients undergoing treatment in UMMC between January 2001 and December 2010 were reviewed. A retrospective-prospective cohort study design was used. Research tools included the National Cancer Patient Registration form. Statistical analysis included means, standard deviations (SD), proportions, chi square, t-test/ANOVA. P-value significance was set at 0.05. Results: The male: female ratio was 1.2:1. The mean age was 62.1 (SD12.4) years. Patients were predominantly Chinese (67%), then Malays (18%), Indians (13%) and others (2%). Malays were younger than Chinese and Indians (mean age 57 versus 62 versus 62 years, p<0.001). More females (56%) had colon cancers compared to males (44%) (p=0.022). Malays (57%) had more rectal cancer compared to Chinese (45%) and Indians (49%) (p=0.004). Dukes' stage data weres available in 67%, with Dukes' C and D accounting for 64%. Stage was not affected by age, gender, ethnicity or tumor site. Treatment modalities included surgery alone (40%), surgery and chemo/radiotherapy 32%, chemo and radiotherapy (8%) and others (20%). Conclusions: Significant ethnic differences in age and site distribution, if verified in population-based settings, would support implementation of preventive measures targeting those with the greatest need, at the right age.

Expression of Ki67 in Papillary Thyroid Microcarcinoma and its Clinical Significance

  • Purpose: To investigate the expression of Ki67 protein in papillary thyroid microcarcinoma(PTMC), and to analyze its clinical significance. Materials and Methods: Ki67 protein expression was evaluated in the tissues of 108 human PTMC and 50 other benign papillary hyperplasia of thyroid specimens using immunohistochemistry. Results: The expression intensity of Ki67 in PTMC and benign papillary hyperplasia of thyroid specimens were $1.45{\pm}1.83%$ and $0.46{\pm}0.46%$.The positive expression rates were 46.3% and 14%. There were significant differences between these two groups (p<0.01). There was no significant variation of the expression intensity and positive expression rates of Ki67 in PTMC with gender, age, position of the tumor and the level of TSH pre-operation (p>0.05), but these parameters varied with tumor size, invasion by membrane and cervical lymph node metastasis (p<0.05 or p<0.01). Conclusions: The expression of Ki67 in PTMC was related to tumor size, invasion by membrane and cervical lymph node metastasis, and could be the important indicator for judging clinical progress and estimating prognosis.

Global Adult Tobacco Survey (GATS): A Case for Change in Definition, Analysis and Interpretation of "Cigarettes" and "Cigarettes Per Day" in Completed and Future Surveys

  • Background: The Global Adult Tobacco Survey has 15 key indicators, cigarettes smoked per day (CPD) among daily smokers being one of them. The first wave of GATS in 14 countries indicated that mean CPD use is higher in women than men in India only, which is contrary to the current understanding of tobacco use globally. This study was undertaken to understand the unusual findings for mean CPD use in the GATS-India survey. Materials and Methods: Items B06a and B06b of the GATS India survey questionnaire that collected information on daily consumption of manufactured and rolled cigarettes were analyzed using SPSS software. Exclusive users were identified from these items after excluding the concurrent users of other tobacco products. Cigarette type, exclusive use and gender stratified analyses were made. Consumption of different types of cigarettes among the mixed users of manufactured and rolled cigarettes were correlated. Results: Higher mean number of CPD use among male daily-smokers was observed than their female counterparts in product specific analysis. Mean CPD as per GATS cigarette definition was higher in males than females for exclusive users but a reverse trend was observed in case of non-exclusive users. Use of manufactured cigarettes increased with increase in use of rolled cigarette among the mixed users and around half of these users reported equal CPD frequency for the both types of cigarettes. Conclusions: The anomaly in mean CPD estimate in GATS-India data was due to inclusion of two heterogeneous products to define cigarettes, variation in cigarette product specific user proportions contributing to the average and non-exclusive concurrent use of other tobacco products. The consumption pattern of cigarettes among the mixed users highlights bias in CPD reporting. Definition, analysis and interpretation of 'cigarettes per day' in the GATS India survey need to be improved by redefining cigarettes and making product specific analyses.

Towards Conservation of Omani Local Chicken: Phenotypic Characteristics, Management Practices and Performance Traits

  • Characterizing local chicken types and their mostly rural production systems is prerequisite for designing and implementing development and conservation programs. This study evaluated the management practices of small-scale chicken keepers and the phenotypic and production traits of their chickens in Oman, where conservation programs for local livestock breeds have currently started. Free-range scavenging was the dominant production system, and logistic regression analysis showed that socio-economic factors such as training in poultry keeping, household income, income from farming and gender of chicken owners influenced feeding, housing, and health care practices (p<0.05). A large variation in plumage and shank colors, comb types and other phenotypic traits within and between Omani chicken populations were observed. Male and female body weight differed (p<0.05), being $1.3{\pm}0.65$ kg and $1.1{\pm}0.86$ kg respectively. Flock size averaged $22{\pm}7.7$ birds per household with 4.8 hens per cock. Clutch size was $12.3{\pm}2.85$ and annual production $64.5{\pm}2.85$ eggs per hen. Egg hatchability averaged $88{\pm}6.0%$ and annual chicken mortality across all age and sex categories was $16{\pm}1.4%$. The strong involvement of women in chicken keeping makes them key stakeholders in future development and conservation programs, but the latter should be preceded by a comprehensive study of the genetic diversity of the Omani chicken populations.

정상 한국 성인의 상완골과 관절와의 후경각 및 개방적 수술의 비교 (Humeral, Glenoid Retroversion and Bicipital Groove Distance of the Normal Korean Adults)

  • Purpose : Average humeral head retroversion was showed significant wide range from literatures based on variable measuring technique, We performed computed tomography(CT) study in an effort to define the specific anatomy relationships and evaluate their use. Materials and Methods : Two hundreds shoulders and distal humeri CT scan with no known pathology were examined. The study population was divided to 10 groups by gender and age (from third to seventh decade). The number of each group was twenty. Retroversion of proximal humerus and glenoid were measured using the lines that were connected the central axis of humeral head, central points of the humeral epicondyles paralleling to the trochlea, paralled to the glenoid surface, midpoint between the transverse glenoid diameter and medial edge of the scapular. We also measured the bicipital groove distance from the humeral central axis and scapulothoracic angle. Results: Retroversion of proximal humerus was highly variable, ranging in this study from 13 to 58 degrees(mean 28.73) These values correlated with sex, not age, height or hand dominance. Glenoid retroversion at the inferior sections showed average 1.3 degree, did not signigicant differences. The central axis was an average of l0mm(5-15mm) posterior to the posterior margin of the bicipital groove. Scapulothoracic angle was average 43 degrees(25-53 degrees) Conclusion: Anatomical reconstruction of retroversion angle should be individualized and bicipital groove could be useful as landmark for the lateral fin of the prosthesis to be positioned an average of 10mm posteriorly.
