Protective and Healing Effects of both Jiguyangwi-tang and Gamijiguyangwi-tang on Gastric Mucosa Injuries induced by Cyclophosphamide in Mice (지구양위탕과 가미지구양위탕의 항암제에 의해 손상된 위장관의 회복효과에 관한 연구)

  • This research was to find out the protective and healing effects of both the Jiguyangwi-tang and the Gamijiguyangwi-tang on the gastric mucosa injuries by cyclophosphamide. At first, Jiguyangwi-tang and Gamijiguyangwi-tang extract were administered to the mice before one week, and then integral administration of those two drugs(each herbal extract and cyclophosphamide) were followed for another one week, respectively. After finishing those treatments, I have examined the state of the both ulcer and inflammation on the damaged gastric mucosa cell and watched the level of proliferating cell nuclear antigen(PCNA), Bcl-2, and apoptosis. These results were as follows, 1. Gastric mucosa inflammation have more significantly reduced in groups of integral administration of Jiguyangwi-tang plus cyclophosphamide, Gamijiguyangwi-tang plus cyclophosphamide and cyclophosphamide then in group of single cyclophosphamide. 2. Gastric ulcer have been reduced in groups of integral administration of Jiguyangwi-tang plus cyclophosphamide, Gamijiguyangwi-tang plus cyclophosphamide and cyclophosphamide then in group of single cyclophosphamide. But the significance have not shown. 3. PCNA level have more significantly elevated in integral administration of Jiguyangwi-tang plus cyclophosphamide and Gamijiguyangwi-tang plus cyclophosphamide and cyclophosphamide then in group of single cyclophosphamide. 4. The significance of both apoptosis induction and bcl-2level have not noticed among all groups. 5. Between Jiguyangwi-tang and Gamijiguyangwi-tang, the differance of effect was not admitted in statistically From these results, it is suggested that Jiguyangwi-tang and Gamijiguyangwi-tang are useful medicines in protecting gastric inflammation and ulcer, that is gastrointestinal side-effect of cyclophosphamide. The preventing effect of Jiguyangwi-tang and Gamijiguyangwi-tang may be through the 'affecting the period of cell division', but not the inhibition of apoptosis.

  • Endoscopic diagnosis and medical management from K43 and C45 patients, whose sex, occupation, character and life patterns are quite different each other, were discussed. The K43 was patient with erosive gastritis, which may caused by maldietary habits and life stresses. Although the effective drugs such as H2-receptor antagonists with atacids, H+/K+-pump inhibitors, prokinetics, colloidal bismuth, and sucralfates, were administrated for long time, symptomatic relief of abdominal pain was not improved. The tests of clinical phathology and abdominal sonogram were normal, visceral hypersensitivity was appeared from barostat test of stomach. However C45 had taken headache during every day life, and endoscopically confirmed as a gastric·duodenal ulcer patiant, which may caused by chronic use of NSAIDs. The her ulceric symptoms were suppressed from active stage to healing stage by using combination therapy for H. pylori with traditional antipeptic ulcer drugs, where as amitriptyline was administrated to the K43 but not effective. Nonulcer dyspepsia and irritable bowel syndrome is pooly defined, and affect about 15% of the population annually in U. S. A. but don't seek medical care. Author would like to point out that statistics for visceral pains are absent in Korea.

Spasmolytic and Anti-peptic Ulcer Activities of Crude Drugs Acting on Gastrointestinal Tract in Rats (흰쥐에서 위장관에 작용하는 생약의 진경 및 항위궤양 효능)

  • The water extracts of ten crude drugs were tested for the spasmolytic and anti-peptic ulcer activities on rat ileum smooth muscle contraction and aspirin-induced acute hemorrhag ic erosive gastritis respectively. The water extract of Aurantii immaturi pericarpium(AIP)($IC_{50}=1.5{\times}1O^{-2}$g/l), Aurantii nobilis pericarpium(ANP)($IC_{50}=2.5{\times}1O^{-2}$g/l), Cyperi rhizoma(CR)($IC_{50}=3.3{\times}1O^{-2}$g/l). Linderae radix(LR) ($IC_{50}=6.8{\times}1O^{-2}$), Aurantii fructus immaturus(AFI)($IC_{50}=11.8{\times}1O^{-2}$), Saussureae radix(SR)($IC_{50}=13.2{\times}1O^{-2}$g/l) and Ponciri fructus(PF)($IC_{50}=23.3{\times}1O^{-2}$g/l) showed inhibitory activity on the isometric contraction of rat ileum smooth muscle induced by electrical stimulation in a concentration-dependent manner, whereas the water extracts of Arecae pericarpium(AP), Agastachis herba(AH) and Magnoliae cortex(MC) potentiated the isometric contraction. In the aspirin-induced acute gastritis, the water extracts of MC, AP and CR reduced significantly the gastric juice secretion, gastric juice acidity and pepsin activity. They also showed protective activity of gastric mucosal layer from erosion and petichial hemorrhage in gross and histological examination.

  • Crohn's disease is an inflammatory bowel disease which affects whole gastrointestinal tract from mouth to anus. Crohn's disease may present both oral manifestation and gastrointestinal symptom-abdominal pain, diarrhea, weight loss, anorexia, fever, and growth failure. The prevalence rate of oral manifestation is approximately between 0.5% and 20%. The oral lesion could be the first sign of Crohn's disease. We present a case of Crohn's disease in a patient who did not show typical oral manifestations but had nonspecific aphthous like ulceration and burning sensation for many years. Through this case, we suggest approaches for the diagnosis and treatment of the oral lesion of Crohn's disease.

Colonic cryptococcosis presenting with chronic diarrhea in a person with advanced human immunodeficiency virus disease: a case report

  • Cryptococcus neoformans infection usually occurs in patients with advanced human immunodeficiency virus (HIV) infection or with a CD4 T lymphocyte count of <100 cells/µL. Pulmonary and central nervous system infections are the most frequently encountered forms of cryptococcosis; however, colonic cryptococcosis is uncommon. We describe the case of a 41-year-old antiretroviral-naïve man with HIV infection diagnosed eight years prior and intermittent diarrhea for 4 months who presented to the emergency department with a 1-day history of low-grade fever and confusion. Brain magnetic resonance imaging and cerebrospinal fluid analysis revealed normal results; however, he was diagnosed with Pneumocystis jirovecii pneumonia based on chest computed tomography and bronchoalveolar lavage analysis. Trimethoprim-sulfamethoxazole administration was initiated followed by antiretroviral treatment. Although his condition gradually improved, he developed fever and abdominal discomfort, and the diarrhea worsened. Endoscopy revealed a small ulcer in the distal transverse colon. Histopathological examination of a colon tissue sample revealed cryptococcal infection. He improved substantially during liposomal amphotericin B and fluconazole treatment. We encountered a rare case of colonic cryptococcosis that caused chronic diarrhea in a patient with advanced HIV infection. Colonic cryptococcosis should be considered when patients with acquired immune deficiency syndrome present with gastrointestinal symptoms.

The Safety and Usefulness of Synthetic Absorbable Monofilament, Glycoside-$\varepsilon$-caprolactonetrimethylene Carbonate Interpolymer, in Gastrointestinal Anastomosis and Closure (위장관문합 및 봉합 시 인공 흡수성 단사인 Glycoside-$\varepsilon$- caprolactone-trimethylene Carbonate 혼성중합체의 안정성 및 유용성)

  • Purpose: Synthetic absorbable monofilaments offer excellent glide characteristics and cause minimal tissue trauma as a result of their smooth monofilament structure and gradual absorption within the healing tissues. For these reasons, these suture materials are commonly used in various surgical fields such as gastroenterology, urology, gynecology, and plastic surgery. The aim of this study was to evaluate the safety and usefulness of a new synthetic absorbable monofilament, Glycoside-..-caprolactone-trimethylene carbonate interpolymer (GCT), in gastrointestinal anastomosis and closure. Materials and Methods: We evaluated 55 gastrointestinal anastomoses and closures using GCT $MONOSYN^{R}$, B. Braun, Germany) in 47 patients who underwent gastric surgery between December 2001 and May 2002 at Seoul National University Hospital. Patient's characteristics, operative procedure, surgeon's opinion of handling properties of GCT, and suture-related complications were analyzed. Results: There were 34 males and 13 females (M:F= 2.6:1) with an average age of 54.2 years old. Forty-five cases of gastrointestinal anastomosis (20 gastrojejunostomies and 25 jejunojejunostomies) and 10 cases of intestinal closure (7 gastrostomy closures and 3 duodenal stump closures) were performed in 41 cases of stomach cancer, three of peptic ulcer disease, two of GIST, and one MALToma. The handling properties of GCT according to the criteria of knot breaking load, knot security, and placing property were always scored with 7 to 9 points (10=excellent, 1=very poor). Two cases of postoperative complications ($3.6\%$) were noted. One was a leak of the gastrojejunostomy site which was successfully managed conservatively, and the other was a stricture of the gastrojejunostomy site which was managed by reoperation (side-to-side jejunojejunostomy). Conclusion: GCT seems to be an applicable suture material for various gastrointestinal anastomoses and closures.

Characteristics of Upper Gastrointestinal Tract Involvement in Korean Pediatric Crohn's Disease: A Multicenter Study

  • Purpose: Crohn's disease (CD) can involve any site of the gastrointestinal tract (GIT). However, the characteristics of upper GIT involvement in CD are unclear, especially in the Eastern pediatric population. This study aimed to estimate the prevalence of upper GIT involvement and identify the clinical features of Korean children with CD. Methods: This was a retrospective multicenter cohort study that included 52 pediatric patients with CD who underwent esophagogastroduodenoscopy and biopsy. The clinical symptoms and endoscopic and histologic features of the upper GIT were identified according to the presence or absence of upper gastrointestinal symptoms. Results: Among the 52 patients, upper GIT involvement was noted in 50.0% (26/52). The mean age at CD diagnosis was $14.1{\pm}2.1$ years. Gastric ulcer was the most common lesion (19.2%) found on upper GIT endoscopy, followed by duodenal ulcers (15.4%). Chronic inflammation was the most common histopathologic feature (75.0%), followed by gastric erosion (17.3%). Granuloma was found in 9.6% of patients. Helicobacter pylori infection was identified in 5.8% of patients. Endoscopic and histologic findings were not significantly different, but the mean values of erythrocyte sedimentation rate ($60.7{\pm}27.1$ vs. $43.0{\pm}27.6mm/h$, p=0.037) and C-reactive protein ($16.5{\pm}28.2$ vs. $6.62{\pm}13.4mg/dL$, p=0.014) were significantly different between patients with and without upper gastrointestinal CD symptoms. Conclusion: Upper GIT involvement was relatively common in pediatric patients with CD irrespective of upper gastrointestinal symptoms, and H. pylori infection was relatively uncommon. The results of this study should aid the establishment of regional guidelines for upper GIT examination.

Gastric Tuberculosis Presenting as a Subepithelial Mass: A Rare Cause of Gastrointestinal Bleeding (위장관 출혈을 일으킨 상피하 종양으로 나타난 위 결핵)

  • Gastric tuberculosis accounts for approximately 2% of all cases of gastrointestinal tuberculosis. Diagnosis of gastric tuberculosis is challenging because it can present with various clinical, endoscopic, and radiologic features. Tuberculosis manifesting as a gastric subepithelial tumor is exceedingly rare; only several dozen cases have been reported. A 30-year-old male visited emergency room of our hospital with hematemesis and melena. Abdominal CT revealed a 2.5 cm mass in the gastric antrum, and endoscopy revealed a subepithelial mass with a visible vessel at its center on gastric antrum. Primary gastric tuberculosis was diagnosed by surgical wedge resection. We report a rare case of gastric tuberculosis mimicking a subepithelial tumor with acute gastric ulcer bleeding.

Experimental Studies on the Effects of Kamikwakjeongtang (가미곽정탕(加味藿正湯)의 효능(效能)에 관(關)한 실험적(實驗的) 연구(硏究))

  • In order to investigate the effects of Kamikwakjeongtang by using the experimental animals. the action on gastrointestinal smooth muscles. the action of gastric juice secretion, the action of antiulcer. the transport ability of intestinal contents. the action of anticatharsis and the actions on the central nervous system were studied. The results were as follow: 1. Spontaneous motility of the isolated ileum of mice was suppressed and antiacetylcho-line chloride action was recognized. 2. Anti-action on barium chloride of fundus-strip of white rat was recognized. 3. Supression effects on gastric juice secretion. free acidity was recognized. 4. Preventive effect on the ulcer induced by pylorus-ligated was recognized. 5. Supression effects of large intestinal transport ability was recognized. 6. Anti-carthartic action was shown but was not recognized. 7. Analgesic effect by the acetic-acid method and prolonged effect of the total sleep time by pentobarbital-Na were recognized. According to the above results, effects based on oriental medical reference were consistent with the actual experimental effects.

Heterotopic Pancreas Presented as Duodenal Tumor with Obstruction

  • Heterotopic pancreas (HP) is defined as pancreatic tissue lacking anatomic and vascular continuity with the main body of the pancreas. Most are asymptomatic, but can cause ulcer, bleeding, intussusception, and mechanical obstruction. Herein, we presented one case of HP presented as duodenal tumor causing duodenal obstruction. A 7-year-old girl visited the emergency room for abdominal pain with vomiting for 24 hours. Computed tomography and upper gastrointestinal series revealed a polypoid mass with short stalk in the 2nd portion of duodenum. We attempted an endoscopic removal. However, the lumen was nearly obstructed by the mass and the stalk was too broad and hard to excise. The mass was surgically removed via duodenotomy. It was confirmed as a HP with ductal and acini components (type 2 by Heinrich classification). Postoperatively, the patient has been well without any complication and recurrence.
