Functional Electrical Stimulation with Augmented Feedback Training Improves Gait and Functional Performance in Individuals with Chronic Stroke: A Randomized Controlled Trial

  • Purpose: The purpose of this study was to compare the effects of the FES-gait with augmented feedback training to the FES alone on the gait and functional performance in individuals with chronic stroke. Methods: This study used a pretest and posttest randomized control design. The subjects who signed the agreement were randomly divided into 12 experimental groups and 12 control groups. The experimental groups performed two types of augmented feedback training (knowledge of performance and knowledge of results) together with FES, and the control group performed FES on the TA and GM without augmented feedback and then walked for 30 minutes for 40 meters. Both the experimental groups and the control groups received training five times a week for four weeks. Results: The groups that received the FES with augmented feedback training significantly showed a greater improvement in single limb support (SLS) and gait velocity than the groups that received FES alone. In addition, timed up and go (TUG) test and six minute walk test (6MWT) showed a significant improvement in the groups that received FES with augmented feedback compared to the groups that received FES alone. Conclusion: Compared with the existing FES gait training, augmented feedback showed improvements in gait parameters, walking ability, and dynamic balance. The augmented feedback will be an important method that can provide motivation for motor learning to stroke patients.

편마비 환자의 보행 특성과 기능적 보행지수 변화 (The Change of Gait Characteristics and FAP in Patients with Chronic Unilateral Stroke)

  • Purpose : Improved walking is a common goal after stroke. Although the neurodevelopmental intervention(PNF) is the most widely used approach in the walking training of hemiparetic subjects. There is little neurophysiological evidence for its presumed effects on gait symmetry and facilitation of paretic muscles during the therapeutic intervention. The study, therefore, investigated the immediate effects of gait entrainment by a PNF techniques. Methods : Included persons with stroke who were living in the community. Sixteen subjects were assigned to the experimental group participated in a measures design that evaluated the subjects with pre-treatment, post-treatment(8 weeks). Temporal-spatial parameter of gait were analysed for using the computerized GAITRite system. Intervention : Training for the experimental group was carried out 3 times a week for 8 weeks. The training sessions were comprised of 50 minutes of walking with pattern and techniques in PNF. Results : The experimental group had improvements in the functional walking ability after 8 weeks treatment and Post-treatment test scores were more significant than the pre-treatment score. The treatment group demonstrated significantly post-treatment test improvement in gait velocity, cadence and FAP. Post-treatment test scores were more significant than the pre-treatment score(p<0.05). Conclusion : The results of this study showed that the PNF exercise intervention can improve functional gait ability. This study provides evidence for the efficacy of PNF treatment at improving locomotor function in chronic stroke.

The Effects of Two Types of Trunk Stability Exercise on the Gait Factors of Stroke Patients

  • The purpose of this study is to identify the effects of two trunk stability exercise types on the gait factors of stroke patients. We randomly divided 24 old elderly patients with hemiplegia, who were hospitalized due to stroke, into a two groups, each with its own six-week exercise program: one that used of a dynamic trunk stability exercise using with physio-balls(n=12) and a group of one that used a static trunk stability exercise using on mats(n=12). After measuring the participants gait ability a sin a pre-test, we again measured their ability again as in a posttest after two-for both types of six-week exercise programs for each group. The analysis of the data analysis showed that both ball and mat exercise programs significantly improved the participants' gait velocity and stride length; cadence, however, was significantly changed only by the ball exercise program. In conclusion, both types of trunk stability exercise may be useful in improving the gait ability of stroke patients, and, in particular, the former can be used as an exercise method that effectively significantly affects more various other gait factors.

뇌졸중 환자의 탄력-비탄력 발목 테이핑 적용이 자세균형과 보행능력에 미치는 일시적 효과 (Immediate Effect of Elastic and Non-Elastic Ankle Taping on Postural Balance and Gait Ability in Subject with Stroke )

  • Background: The purpose of this study was to investigate the immediate effects of ankle elastic and non-elastic taping on postural balance and gait ability in subject with stroke. Design: Cross-sectional study Methods: Twenty-seven subjects with stroke participated in this study. The subjects performed to stand quietly for 30s on the balance platform and walking test with three different ankle taping conditions. The sway length, sway area and sway velocity of center of gravity (COG) displacement was measured to assess the postural balance and the timed up and go test, 10m walking test, 6 minutes walking test was measured to assess the gait ability. Repeated measured ANOVA was used to compare the postural balance parameters and gait ability according to three different ankle taping conditions. Results: Postural balance with non-elastic ankle taping was significantly improved compared to no ankle taping and elastic ankle taping condition(p<0.05). On the other hand, gait ability with elastic ankle taping was significantly increased compared to no ankle taping and non-elastic ankle taping condition(p<0.05). Conclusion: These findings suggest that an elastic ankle taping could effect to improve the gait ability, whereas a non-elastic ankle taping could effect to improve the postural balance in subject with stroke.

Before and after experiments, temporal-spatial gait parameters were measured. The date of 14 patients who carried out the whole experimental course were statistically analyzed. The results of the study were : 1. In the comparison of gait velocity before and after experiment, the gait velocity was significantly increased in the experimental group and the control group(p<.05). In the comparison of difference of the gait velocity between groups, there was not, significant difference between the experimental group and the control group(p>.05). 2. In comparison of gait cadence before and after experiment, the gait cadence was significantly increased in both groups(p<.05). In the comparison of difference of the gait cadence between groups, there was not significant difference between the experimental group and the control group(p>.05). 3. In the comparison of step length before and after experiment, the step length was significantly increased in the experimental group and the control group(p<.05). In the comparison of difference of the step length between groups, there was not significant difference between the experimental group and the control group(p>.05). 4. In the comparison of single support time asymmetry before and after experiment, the single support time asymmetry was no significant difference between groups(p>.05). In the comparison of difference of the single support time asymmetry between groups, there was not significant difference between the experimental group and the control group(p>.05). 5. In the comparison of step length asymmetry before and. after experiment, the step length asymmetry was not significant difference between the experimental group and the control group(p>.05). In the comparison of difference of the single step length asymmetry between groups, there was not significant difference between the experimental group and the control group(p>.05).

In the comparison of functional ambulation profile(FAP) before and after experiment, the FAP was not significant difference in the experimental group and the control group(p>.05). In the comparison of difference of the FAP between groups, there was not significant difference between the experimental group and the control group(p>.05).

궁둥신경 가동술이 여성노인의 뻗은다리올림 검사와 보행능력에 미치는 영향 (The Effect of Sciatic Nerve Mobilization on SLR Angle and Gait Ability of Elderly Women)

  • PURPOSE: The purpose of this study was to examine the effects of sciatic nerve mobilization techniques on straight leg raising (SLR) and walking ability in elderly women aged over 65. METHODS: Seventy women aged over 65 were examined using SLR test and forty women were selected as subjects. They were divided into Group I (under $70^{\circ}$ of SLR test of both legs, n=20) and Group II (over $70^{\circ}$ of SLR test of both legs, n=20). Nerve mobilization was applied to both groups (three times a week for 4 weeks). SLR angle was measured using digital goniometer and walking ability was measured by step length, stride length, velocity, double support, using GAITRite System. After 4 weeks, paired t-test was used to compare the changes of SLR test and walking ability within Group I and Group II. RESULTS: In Group I, SLR test, step length, stride length and gait velocity were significantly increased but double support was significantly difference. In Group II, SLR test, step length and stride length were significantly increased but gait velocity was not significantly increased and double support was significantly decreased. CONCLUSION: This study showed that sciatic nerve mobilization techniques had positive effects on the SLR angle and walking ability in elderly women.

Floor Slipperiness Effect on the Biomechanical Study of Slips and Falls

  • A study was conducted to find the possible relationship between slip distance and dynamic coefficient of friction (DCOF) through the biomechanical study of slips and falls using a broader variety of floors and levels of slipperiness than those used before. Four different floor surfaces covering the full range of floor slipperiness (with and without on oil contaminant) were prepared for ten subjects with each walking at a fixed velocity. The results showed that slip distance and heel velocity had a decreasing trend while stride length had a increasing trend as DCOF increased. The contaminant effect overpowered floor slipperiness effect because a higher DCOF surface with oil contaminant created longer slip distance than the lower DCOF with dry floor. Normal gait pattern and suggested heel velocity (10 to 20 cm/sec) were seen on dry floors but abnormally longer stride length and 5 to 10 times faster heel velocity were found an oily floors. In other words, faster heel velocity (greater than 10 to 20 cm/sec) is recommended to measure DCOF on oily floors because the assumption of normal gait was no longer valid.

시각적 되먹임을 이용한 골반경사 운동이 편마비 환자의 보행특성에 미치는 영향 (The Influence of Pelvic Tilt Exercise Using Visual Feedback upon the Gait Characteristics of Patients with Hemiplegia)

  • The purpose of this study was to investigate the influence of anterior, posterior, and lateral pelvic tilt exercise upon the gait characteristics of patients with hemiplegia including their gait velocity, cadence, stride length, step length of the non affected side, step length of the affected side, foot angle of the non affected side, foot angle of the affected side, base of support, and so on. The subject of this study was 24 men and women patients with hemiplegia. The patients, the subject of this study, were classified into 12 patients of treatment group applying pelvic tilt exercise using visual feedback and 12 patients of control group applying general pelvic tilt exercise, and then analyzed their gait before and after exercise. Temporal distance gait analysis (Boening, 1977) was used to analyze their gait, and ink foot-print was applied as on of measurement methods. To find out meaningful difference between control group and treatment group, this study carried out independent sample t-test for each item by utilizing SPSS/Win 10.0, compared changes in control group's and treatment group's gait characteristics before and after exercise as percentage, and applied paired t-test to conduct before-after test in same group. Statistical significance level was ${\alpha}$ < 0.05. The results of this study were as follows. As a result of independent sample t-test for control group's and treatment group's gait characteristics after exercise, it was not statistically significant so there was no meaningful difference between two groups. However, it was statistically significant in the change rate(%) of gait characteristics, and treatment group's patients with hemiplegia had been highly changed in gait characteristics in comparison with control group. From the above-mentioned results, could find that pelvic tilt exercise using visual feedback for patients with hemiplegia was effective to improve their gait ability and it could increase the ability in comparison with general pelvic tilt exercise. In the future, studies on the effect of pelvic tilt exercise using visual feedback shall be continued based on more quantitative methods.

시각차단 균형훈련이 뇌졸중 환자의 보행기능에 미치는 영향 (The Effects of Balance Training with Visual Cue Deprivation on Gait Function in Patients with Stroke)

  • PURPOSE: The purpose of this study was to investigate the effect of balance rehabilitation training with the visual cue deprivation on gait function in stroke patients in comparison with balance training without the visual cue deprivation. METHODS: Twenty two stroke patients participated in this study. Patients were randomly assigned to one of the two balance training program with and without the visual cue deprivation. Balance training session for each group lasted 50 minutes, 3 times a week for a total of 6 weeks. Gait function was measured with the Functional Gait Assessment (FGA), the self-selective comfortable gait speed (CGS), the maximal gait speed (MGS), and the Gait Analysis System. Temporal and spatial gait parameters of each evaluation were measured before and after the balance training program respectively. RESULTS: After the program, the visual cue deprivation group improved significantly in the FGA, the CGS, the gait velocity, the step time, the step length, the stride length, and the Functional Ambulation Performance (FAP) in comparison with the balance training group with the visual cue (p<.05). CONCLUSION: The gait function of the participants with the visual cue deprivation showed more improvement after the balance training program compared to the patients group without the visual cue deprivation, Therefore, the balance training program with the visual cue deprivation may be useful for rehabilitation of patients with chronic stroke.
