Bow Hunter's Syndrome Caused by Bilateral Dynamic Occlusion of the Subaxial Vertebral Arteries during Neck Extension (경추 신전 시 축하 척추동맥의 양측성 동적 폐쇄로 인해 발생한 보우 헌터 증후군)

  • Bow hunter's syndrome is a rare disease that shows the symptoms of vertebrobasilar insufficiency resulting from a dynamic obstruction or stenosis of the vertebral arteries during neck movement. This paper reports a case of a 59-year-old male who visited the emergency room with diplopia, tinnitus, and gait disturbance. Magnetic resonance imaging and angiography revealed a multiple cerebellar infarct, total obstruction of the right vertebral artery, and dynamic obstruction of the left vertebral artery during neck extension. As the infarction worsened, a thrombectomy was done. Posterior decompression and fusion at C5-6 were performed for the left vertebral artery. The left vertebral arterial patency was confirmed by intraoperative and postoperative angiography. No recurrence of the symptoms was observed for six months after surgery. Physicians need to pay attention to the diagnosis of vertebrobasilar insufficiency caused by an obstruction of the vertebral arteries during neck extension in cervical instability patients.

Operation Plan for a Customized Convergence Marine Healing Exercise Program for Metabolic Disease Patients (대사질환자를 위한 맞춤형 융합 해양치유 운동프로그램 운영방안)

  • Purpose : According to the Korean Diabetes Association, the number of metabolic disease patients in Korea is approximately 14.97 million as of 2020;, the prevalence of diabetes among adults over 30 years old is 44.3 %, and the prevalence of diabetes in the elderly over 65 years old is 50.4%. These individuals exposed to the risk of complications. Therefore, the purpose of this study was to determine a distribution method for a customized fusion exercise marine healing program for metabolic disease patients. Methods : We have searched numerous papers concerning artificial intelligence (AI), virtual reality (VR), augmented reality (AR), and marine resources related to marine healing programs that can be introduced in marine healing centers for people with metabolic diseases Results : Through the production of various marine resources and evidence-based exercise programs, the provision of exercise programs using AI, and the development of exercise platforms using AR and VR, we were able to establish guidelines for how to operate marine healing programs at marine healing centers. Conclusion : Korea has much more diverse marine healing resources than other advanced countries in the marine healing industry. However, the development of these resources has only just begun. It is hoped that the studied marine healing program will be of great help to metabolic patients by creating contents that will be implemented in marine healing centers by using the cutting-edge technologies and various marine resources that Korea possesses.

Frontotemporal Craniotomy for Clipping of Unruptured Aneurysm Using a Diamond-Coated Thread Wire Saw and Reconstruction Using Calcium Phosphate Cement without Metal Fixation

  • Metal fixation systems for cranial bone flaps cut by a drill are convenient devices for cranioplasty, but cause several complications. We use modified craniotomy using a fine diamond-coated threadwire saw (diamond T-saw) to reduce the bone defect, and osteoplasty calcium phosphate cement without metal fixation. We report our outcomes and tips of this method. A total of 78 consecutive patients underwent elective frontotemporal craniotomy for clipping of unruptured intracranial aneurysms between 2015 and 2019. The follow-up periods ranged from 13 to 66 months. The bone fixation state was evaluated by bone computed tomography (CT) and three-dimensional CT (3D-CT). The diamond T-saw could minimize the bone defect. Only one wound infection occurred within 1 week postoperatively, and no late infection. No pain, palpable/cosmetically noticeable displacement of the bone flap, fluid accumulations, or other complications were observed. The condition of bone fixation and the cosmetic efficacy were thoroughly satisfactory for all patients, and bone CT and 3D-CT demonstrated that good bone fusion. No complication typical of metal fixation occurred. Our method is technically easy and safety, and achieved good mid-term bone flap fixation in the mid-term course, so has potential for bone fixation without the use of metal plates.

Development of a Brain Phantom for Multimodal Image Registration in Radiotherapy Treatment Planning

  • In radiotherapy treatment planning, it is critical to deliver the radiation dose to tumor and protect surrounding normal tissue. Recent developments in functional imaging and radiotherapy treatment technology have been raising chances to control tumor saving normal tissues. A brain phantom which could be used for image registration technique of CT-MR and CT-SPECT images using surface matching was developed. The brain phantom was specially designed to obtain imaging dataset of CT, MR, and SPECT. The phantom had an external frame with 4 N-shaped pipes filled with acryl rods, Pb rods for CT, MR, and SPECT imaging, respectively. 8 acrylic pipes were inserted into the empty space of the brain phantom to be imaged for geometric evaluation of the matching. For an optimization algorithm of image registration, we used Downhill simplex algorithm suggested as a fast surface matching algorithm. Accuracy of image fusion was assessed by the comparison between the center points of the section of N-shaped bars in the external frame and the inserted pipes of the phantom and minimized cost functions of the optimization algorithm. Technique with partially transparent, mixed images using color on gray was used for visual assessment of the image registration process. The errors of image registration of CT-MR and CT-SPECT were within 2mm and 4mm, respectively. Since these errors were considered within a reasonable margin from the phantom study, the phantom is expected to be used for conventional image registration between multimodal image datasets..

  • One of the important ways to improve the performance of magnetometers in satellite exploration is to reduce magnetic noise from satellites. One of the methods to decrease magnetic noise is by extending the satellite boom. However, this approach is often not preferred due to its high cost and operational considerations. Therefore, in many cases, removing interference from the satellite platform in the measured dataset is widely utilized after data acquisition. In this study, we would like to introduce an algorithm for removing magnetic noise observed from magnetometers installed on two solar panels and one main body without a boom.

  • Purpose: The purpose of this paper is to present a service construction plan using multiple complex sensor information to detect abnormal situations in urban life safety that are difficult to identify on CCTV. Method: This study selected service scenarios based on actual testbed data and analyzed service importance for local government control center operators, which are main users. Result: Service scenarios were selected as detection of day and night dynamic object, Detection of sudden temperature changes, and Detection of time-series temperature changes. As a result of AHP analysis, walking and mobility collision risk situation services and fire foreshadowing detection services leading to immediate major disasters were highly evaluated. Conclusion: This study is significant in proposing a plan to build an anomaly detection service that can be used in local governments based on real data.   • In this study, electric field analysis was conducted for each process as a preliminary step in the design of the electrospinning device to apply the electrospinning PVDF nanofibers to increase the filtering effect of insect screens. In the electrospinning analysis using a single nozzle, it was confirmed that there was a decrease in the electrostatic field strength as the tip's size decreased, an increase in the voltage, and no effect depending on the TCD distance. In addition, it was confirmed that the closer the distance between tips, the more electric field interference occurs, and this was found to have a more significant effect on the tip located in the center with tips on both sides. Therefore, based on these analytical results, it is believed that an increase in production speed can be expected by establishing an efficient process line by confirming the radiating area of the collector and designing the spacing between multi-nozzles through actual experiments.

Multi-task Learning Based Tropical Cyclone Intensity Monitoring and Forecasting through Fusion of Geostationary Satellite Data and Numerical Forecasting Model Output (정지궤도 기상위성 및 수치예보모델 융합을 통한 Multi-task Learning 기반 태풍 강도 실시간 추정 및 예측)

  • The accurate monitoring and forecasting of the intensity of tropical cyclones (TCs) are able to effectively reduce the overall costs of disaster management. In this study, we proposed a multi-task learning (MTL) based deep learning model for real-time TC intensity estimation and forecasting with the lead time of 6-12 hours following the event, based on the fusion of geostationary satellite images and numerical forecast model output. A total of 142 TCs which developed in the Northwest Pacific from 2011 to 2016 were used in this study. The Communications system, the Ocean and Meteorological Satellite (COMS) Meteorological Imager (MI) data were used to extract the images of typhoons, and the Climate Forecast System version 2 (CFSv2) provided by the National Center of Environmental Prediction (NCEP) was employed to extract air and ocean forecasting data. This study suggested two schemes with different input variables to the MTL models. Scheme 1 used only satellite-based input data while scheme 2 used both satellite images and numerical forecast modeling. As a result of real-time TC intensity estimation, Both schemes exhibited similar performance. For TC intensity forecasting with the lead time of 6 and 12 hours, scheme 2 improved the performance by 13% and 16%, respectively, in terms of the root mean squared error (RMSE) when compared to scheme 1. Relative root mean squared errors(rRMSE) for most intensity levels were lessthan 30%. The lower mean absolute error (MAE) and RMSE were found for the lower intensity levels of TCs. In the test results of the typhoon HALONG in 2014, scheme 1 tended to overestimate the intensity by about 20 kts at the early development stage. Scheme 2 slightly reduced the error, resulting in an overestimation by about 5 kts. The MTL models reduced the computational cost about 300% when compared to the single-tasking model, which suggested the feasibility of the rapid production of TC intensity forecasts.

Factors for Survival and Complications of Malignant Bone Tumor Patients with a Total Femoral Replacement (대퇴골 전치환술 받은 악성 골종양 환자의 생존인자와 합병증)

  • Purpose: Total femoral replacement (TFR) is an extreme form of limb salvage. Considering the rarity of this procedure, reports have focused on the complications and a proper indication is unclear. This study analyzed 36 patients with TFR who were asked the following: 1) prognostic factors related to survival in patients who underwent TFR with a tumoral cause; 2) overall implant and limb survival; 3) complications, functional outcome, and limb status for patients surviving for more than 3 years. Materials and Methods: According to the causes for TFR, 36 patients were categorized into three groups: extensive primary tumoral involvement (group 1, 15 cases), tumoral contamination by an inadvertent procedure or local recurrence (group 2, 16 cases), and salvage of a failed reconstruction (group 3, 5 cases). The factors that may affect the survival of patients included age, sex, cause of TFR, and tumor volume change after chemotherapy. Results: The overall five-year survival of the 36 patients was 31.5%±16.2%. The five-year survival of 31 patients with tumoral causes was 21.1%±15.6%. The five-year survival of 50.0%±31.0% in patients with a decreased tumor volume after chemotherapy was higher than that of increased tumor volume (p=0.02). The five-year survival of 12 cases with a wide margin was 41.7%±27.9%, whereas that of the marginal margin was 0.0%±0.0% (p=0.03). The ten-year overall implant survival of 36 patients was 85.9%±14.1%. The five-year revision-free survival was 16.6%±18.2%. At the final follow-up, 12 maintained tumor prosthesis, three underwent amputation (rotationplasty, 2; above knee amputation, 1), and the remaining one had knee fusion. Among 16 patients with a follow-up of more than three years, 14 patients underwent surgical intervention and two patients had conservative management. Complications included infection in 10 cases, local recurrences in two cases, and one case each of hip dislocation, bushing fracture, and femoral artery occlusion. Conclusion: Patients showing an increased tumor volume after chemotherapy and having an inadequate surgical margin showed a high chance of early death. In the long-term follow-up, TFR showed a high infection rate and the functional outcome was unsatisfactory. Nevertheless, this procedure is an inevitable option of limb preservation in selected patients.

  • The purpose of this study was to develop index about the sixth industrial entrepreneurs to establish the spirit and measuring the sixth industrial entrepreneurship for groups successfully led to the sixth industry of agriculture, which is actively being conducted in recent years aimed at rural stimulate the economy. Enlighten the value of rural resources, production, processing, in a sixth industry establishment that has a characteristic of fusion distribution and tourism in addition to the elements of the entrepreneurship that has been emphasized in the traditional establishment, the more diverse entrepreneurship element has been requested. In this study, to derive the traditional entrepreneurship of the components and the sixth industry entrepreneurship component through literature research, through interviews with experts of the sixth industry, an important element of the entrepreneurship that is required in the field It was derived. Based on the derived results, set the index of the sixth industry entrepreneurship, it was conducted a questionnaire survey of sixth industry workers. Through analysis of the navigation factors, to evaluate the measurement and indicators of relevance, factors that have been set through the results literature study and interviews of exploratory factor analysis it was found that has been rationally constructed. The results of this study, education and consulting for the activation of the sixth industry, would be able to take advantage of, such as in the planning of education programs for whom decide to go back to the countryside(Agro migration). In addition, to diagnose the entrepreneurship of a conventional sixth industry progress mainly, it is expected to be able to help you to proceed with the custom capability development that meets the individual needs.
