A serious problem after cardiovascular surgery known as Multiple Organ Failure[MOF] whereby several vital organs successively demonstrate dysfunction in spite of intensive postoperative treatment has recently arisen. We have made a retrospective study of the clinical records of 137 patients who underwent cardiovascular surgery during past two years [1987-1988]. Fourteen patients [10%] developed multi-organ failure postoperatively with the results of seven death [50%]. In fatal group, preoperative poor cardiac function [Cardiac Index<2.0L/min/m2] was considered important prognostic factor and infection 5 disseminated intravascular coagulation complicating gastrointestinal bleeding were the leading cause of death. In conclusion, evaluation of multiple factors concerning multi-organ failure demonstrates preoperative poor functional preservation of vital organs is the main factor. So early diagnosis k management for each of the failing organs & prevention of infection are mandatory of the treatment of these critically ill patients.
From the complexities and functional/operational expectations, the compositions of PC devices requires Device Drivers to deliver full performance of the Hardware. However, due to quick transition of hardwares and expectations of better performance of the system. As a rest users are exposed of unexpected failures of device drivers which will cause malfunction of the system which consequently cause damages in quantity as well as quality wise. In this paper, the method of calculation and/or forecast to unexpected failure will be suggested. It may not be the answer but can be a guide to calculation to economic damage due to device driver failure.
It happened a failure on special bolt which supported main landing gear actuator up-lock rod of 00 aircraft. Fracture was occurred at end of center drilled hole and thread machined on bolt. Metallographic, fractographic, and other characteristics of embrittlement analysis and experiments carried out on the failed bolt to find out the reason. Bolt surface was cadmium electroplated(EP) to give lubrication and provide excellent corrosion resistance. Resultly, Bolt was failed due to cadmium embrittlement occurred during baking treatment as well as center drilled hole. for the failure that are relevant to failure analysis and prevention. For their successful functional application, cadmium EP bolts require proper and adequate baking treatment after electroplating, and is complete with no center drilled hole
본 논문은 KSLV-I 상단부에 대한 고장모드 분석 결과와 시스템 수준의 비행시험 신뢰도 모델을 기술한다. 먼저, KSLV-I 상단부의 14개의 주요 기능과 비행시험 임무 프로파일을 분석하고, 기능 분석 결과와 임무 구간별 수행 기능 목록을 바탕으로 시스템 체계에 따른 상단부의 고장모드 계층 구조와 시스템 수준의 비행시험 신뢰도 모델을 구성한다.

It is shown in the present study that during the HTS (hot temperature storage) test, the metal contamination by impure elements can be highly susceptible to the void formation, leading to the open failure of the power line in the memory device. Such a functional failure associated with the metal contamination was investigated to be dominant in the early stages of the HTS test while the formation of a stress-driven void is mainly observed in the later stages. In particular, it was found that the void formed in the contaminated metal takes on a slit-like shape which has been known to be characteristic of the stress-related voiding. The impure elements leading to the metal degradation were identified to be carbon and oxygen introduced during the metal sputtering process. The experimental works show that the device reliability was significantly improved by reducing the level of such impure elements within metal. The purpose of this study was to investigate factors influencing sales of health functional foods by door-to-door saleswomen. A total of 450 women who worked as door-to-door salespersons in Seoul were enrolled. The survey questionnaire was composed of five parts as follows: general characteristics, factors related with door-to-door sales process, task education, competency required for door-to-door salespersons, and customer relationship beliefs. The survey was conducted in October 2011. Finally, data on 302 subjects were statistically analyzed using the SPSS 17.0 package program. Reason for sales of health functional foods (p<0.01), time required for sales promotion (p<0.001), difficulties during sales of health functional foods (p<0.05), satisfaction of task education (p<0.01), and failure of salesperson's memory as problems of task education (p<0.01) were significantly associated with sales of health functional foods. However, means of sales promotion, frequency and time of task education, and competency required for door-to-door salespersons were not significantly related with sales of health functional foods. Customer relationship beliefs did not show significant association with sales of health functional foods either. In conclusion, certain factors were associated with sales of health functional foods by door-to-door saleswomen. These results provide an understanding for sales of door-to-door health functional foods and provide basic information for preparation of task education for health functional food saleswomen and marketing.
Purpose: This study was to analyze adherence to self-care behaviors and identify factors affecting the adherence behaviors among inpatients with heart failure. Methods: A total 94 hospitalized inpatients from three hospitals participated in a survey. Data were collected using structured self-reported questionnaire from November 28, 2011 to March 31, 2013 and analyzed using frequency, t-test, ANOVA, Pearson's correlation coefficients and stepwise multiple regression. Results: The score of adherence to self-care behaviors among inpatients with heart failure was $26.02({\pm}8.84)$. Factors related to the adherence to self-care behaviors were living with spouse (t=-2.47, p=.019), functional state (t=2.18, p=.034), heart failure knowledge (r=-.49, p<.001), social support (r=-.35, p<.001), self-control (r=-.25, p=.016), and self-care confidence (r=-.24, p=.019). The factors affecting adherence to self-care behaviors were heart failure knowledge, self-care confidence, and social support. These factors explained 32% of the variance in adherence to self-care behaviors. Conclusion: The adherence to self-care behaviors with heart failure can be improved if heart failure knowledge, self-care confidence, and social support are improved. Therefore, developing a nursing intervention program for patient with heart failure that is considered these factors leads to improve quality of life and prevent readmission.
Prosthetic valve replacement has resulted in marked improvement in the functional status of many patients with valvular heart disease. But valve failure from various causes has necessitated re-replacement of prosthetic valve in some of these patients. This selective group of patients has many inherent problems. This report presents our experience with seven patients who received re-replacement of prosthetic valve in the Seoul National University Hospital from 1981, to 1983. The patients included one woman and six men in the range of 9 and 49 years old. As the first valve operations, there were five MVR using tissue valves, one DVR [Bjork-Shiley and Ionescu-Shiley valve] and one case of modified Bentall operation with composite graft [Ionescu-Shiley valve]. Reoperations on prosthetic valve failure were performed 17 to 54 months after the first operation [mean 34 months]. Amon8 seven patients, there were two cases of prosthetic valve endocarditis and five cases of primary tissue failure. Gross calcification of the xenograft was found in two children with Ionescu-Shiley valve. All except one had relatively successful operative results. The unsuccessful one died intraoperatively from low output syndrome.
Prosthetic reconstruction of musculoskeletal defects about the knee for tumor has many advantages, particularly the maintenance of motion and immediate functional restoration. But, prosthetic reconstruction has inherent limitations in terms of long-term durability. The authors have reported here a patient who had mechanical failure at 61 months later following use of a modular resection system to reconstruct the segmental defect of proximal tibia in osteosarcoma. In this case, another technique of extensor mechanism reconstruction using Achilles tendon allograft was attempted. Because of the concerns involving durability of tumor prosthesis, increased emphasis has to be placed on innovation in prosthetic design.
Reliability Centered Maintenance (RCM) introduces a systematic method and decision logic tree for utilizing previous operating experience focused on reliability and optimization of maintenance activities. In this paper RCM methodology is applied on safety injection system for APR-1400. Functional Failure Mode Effects and Criticality Analysis (FME&CA) are applied to evaluate the failure modes and the effect on the component, system and plant. Logic Tree Analysis (LTA) is used to determine the optimum maintenance tasks. The results show that increasing the condition based maintenance will reduce component failure and improve reliability and availability of the system. Also the extension of the surveillance test interval of Safety Injection Pumps (SIPs) would lead to an improved pump's availability, eliminate the unnecessary maintenance tasks and this will optimize maintenance activities.
