이 연구의 목적은 소아청소년에서의 2급 부정교합 치료에 사용되는 기능성 악교정 장치의 효과 및 치료 시기에 따른 효과를 평가하고자 하였다. 경희대학교 소아치과에 내원한 2급 부정교합환자 30명을 대상으로 하였으며 치료에 사용된 기능성 악교정 장치에 따라 군을 나누었다. 치료 시기에 따른 비교를 위하여 경추골 성숙도 방법을 이용하여 2개의 군으로 나누어졌다. 각 실험군은 치료 전(T0)과 치료 후(T1)의 측모두부방사선사진을 비교 분석하였다. 기능성 악교정 장치 중, Twin block과 Activator는 하악골의 길이(Co-Gn)와 하안모의 길이(ANS to Me)의 유의한 증가를 보였으며, overjet과 overbite의 유의한 감소를 보였다. 반면 Fränkel appliance를 이용한 환자에서는 상하악 간의 관계 및 overjet의 유의한 개선을 보였다. 최대성장기에 기능성 악교정 장치를 사용하였을 때 최대성장기 전에 사용한 것에 비하여 유의한 하악 길이의 증가, 상악과 하악간의 관계 개선, overjet의 감소, 하악 중절치의 순측 경사의 증가를 보였다. 이 연구를 통하여 2급 부정교합을 가진 소아청소년에서 기능성 악교정 장치의 효과를 검증하였으며, 최대성장기에 치료를 시행하는 것이 더 효과적인 것을 확인하였다.

$Fr{\ddot{a}}nkel$ functional regulator have a complicated design and are of a big bulk. This makes not only the construction of appliance but also the adaption of a patient to the appliance very difficult. Therefore, orthodontic laboratory technicians should be well acquainted with the sequential fabrication procedures involved in the construction of Frankel functional regulators and construct a type most suitable for a specific patient of malocclusion. There are various types of Frankel appliances. Detailed construction procedures of FR II are described here below, which can also be applied for other types for FR appliances.

III급 부정교합은 유치열기나 혼합치열기에 이미 그 증상을 예측할 수 있을 뿐 아니라 많은 경우 그 증상의 악화를 예방할 수도 있다. III급 부정교합의 원인으로는 단순한 상하악 치아의 위치 이상에 의한 치아 치조성 혹은 기능성 III급 부정교합과 상악골과 하악골의 비정상적 성장발육에 의한 골격성 III급 부정교합으로 나누어 볼 수 있다. 특히 초기의 기능적 III급 부정교합 환자의 경우 방치되면 다양한 환경적 유전적 요소와 함께 골격성 부정교합으로 전이될 수 있고 많은 경우 악골의 비대칭적 성장에 원인이 될 수도 있다. 따라서 이러한 부정교합의 일차적 조기치료는 소아치과 치료영역에서 중요한 관심 분야라 할 수 있다. Inverted labial bow appliance는 통상적인 active plate의 장점과 activator와 같은 양악장치의 장점을 동시에 수용할 수 있는 장치로 상악치아의 개별적인 치아이동이 용이하고 동시에 기능적으로 전방 변위된 하악을 쉽게 정상위치 시킬 수 있는 장치이다. 특히 구호흡 등의 기질적 원인을 가지는 환자나 복잡한 양악장치를 착용하는데 어려움이 있는 협조도가 부족한 환자의 경우 효과적으로 사용할 수 있다. 이러한 장치를 이용하기 위해서는 골격성 III급 부정교합과의 정확한 감별진단이 선행되야 함은 물론이다. 본 증례는 기능성 III급 부정교합으로 진단된 7세 여아에서 Inverted labial bow appliance를 이용하여 전치부 반대교합을 해소한 경우로 치료결과는 상하악 전치부의 치아 치조성 이동에 의해 주로 이루어졌다.

Methods: Two Tourette's Disorder cases were managed with the Yinyang Balancing Appliance on temporomandibular joint (TMJ), combined with acupuncture. Clinical outcome measurement was based on subjective measures and clinical observations. Results: The patients showed positive changes even after the initial treatment and this effect maintained over the follow-up period. Conclusions: Although it is not clear the effect is a sustaining or temporary in its nature, a positive effect was observed and further clinical and biological research on FCST is expected.

경추부 근긴장 이상증에 대해 턱관절 자세균형 개념에 따라 구강내 균형장치를 활용한 자세훈련법 등의 치료로 긍정적인 호전을 관찰했다. 증례 보고이기는 하지만 턱관절 자세균형을 활용한 치료법에서 긍정적인 치료효과가 이어지고 있어서 앞으로 더욱 체계적인 관찰과 연구가 필요할 것으로 사료된다.

Objectives : Therapeutic effect of Balance Appliance of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance was observed in a refractory torticollis case. Methods : A post-traumatic severe torticollis case with 18 years of duration was managed by the Balance Appliance on temporomandibular joint (TMJ), combined with acupuncture and manual medicine. Results : Assessment was made by self assessment of subjective symptoms and clinical observacon. The patient reported over-90% remission and returned to ordinary daily life after 20months of therapy, which effect was reported to maintain for 9 months. Conclusions : An impressive effect was observed and further clinical and biological research on FCST is expected.
