정상인의 오름 경사로 보행 시 경사각에 따른 하지 관절의 삼차원적 동작 분석 (The 3-D Motion Analysis of Kinematic Variety on Lower Extremity during Ramp Ascent at Different Inclinations)

  • The aim of this study was to investigate the kinematics of young adults during ascent ramp climbing at different inclinations. Twenty-three subjects ascended a four step at four different inclinations(level, $8^{\circ},\;16^{\circ},\;24^{\circ}$). The 3-D kinematics was analysed by a camera-based falcon system. Groups difference was tested with one -way ANOVA and SNK test. The different kinematic patterns of ramp ascent were analysed and compared to level walking patterns. The kinematics of ramp walking could be clearly distinguished from the kinematics of level walking. In sagittal plane, Ankle joint was more dorsiflexed at initial contact and Max. dorsiflex. during stance phase with $16^{\circ},\;24^{\circ}$ inclination and more plantarflexed at toe off and Max. plantarflex. during swing phase with $24^{\circ}$(p<.001). Knee joint was more flexed at initial contact with $16^{\circ},\;24^{\circ}$ inclination(p<.001). Hip joint was more flexed at initial contact and Max. flex. during swing phase with $16^{\circ},\;24^{\circ}$ inclination and at toe off with $24^{\circ}$(p<.001) and was more extended at Max. ext. during stance phase with $24^{\circ}$(p<.05). In frontal plane, ankle joint was more everted at Max. eversion. during stance phase with $16^{\circ},\;24^{\circ}$ inclination(p<.001). Knee joint was more increased at Max. varus. during stance phase with $16^{\circ},\;24^{\circ}$ inclination(p<.001). Hip joint was not differentiated with different inclinations. In horizontal plane, all joints were not differentiated with different inclinations. Conclusionally, In ascent ramp walking, the different gait pattern generally occurred at over $16^{\circ}$ on the ascending ramp in sagittal and frontal plane. These results suggest that there is a certain inclination angle or angular range where subjects do switch between a level walking and a ascent ramp walking gait pattern. This shows their motor control strategy between level and ascent ramp walking. Further studies are necessary to confirm and detect the ascent ramp gait patterns.

Twenty-three subjects descended four steps at four different inclinations (level, $-8^{\circ}$, $-16^{\circ}$, $-24^{\circ}$). The 3-D kinematics were measured by a camera-based Falcon System. The data were analyzed using one-way ANOVA and the Student-Newman-Keuls test. The kinematics of descent ramp walking could be clearly distinguished from the kinematics of level walking. On a sagittal plane, the ankle joint was more plantar flexed at initial contact with $-16^{\circ}/-24^{\circ}$ inclination, was decreased in the toe off position with all inclinations (p<.001),and was decreased at maximum plantar flexion during the swing phase (p<.001). The knee joint was more flexed at initial contact with the $-24^{\circ}$ inclination (p<.001), was more flexed in the toe off position with all inclinations (p<.001), and was more flexed at minimum flexion during stance phase and at maximum flexion during swing phase with $-16^{\circ}$, $-24^{\circ}$ inclination (p<.001). The hip joint was more flexed in the toe off position with $-16^{\circ}$, $-24^{\circ}$ inclination and was deceased at maximum extension during stance phase with $-16^{\circ}$, $-24^{\circ}$ inclination (p<.05). In the frontal plane, the ankle joint was more everted at maximum eversion during stance phase with $-16^{\circ}/-24^{\circ}$ inclination (p<.01) and was decreased at maximum inversion during swing phase with $-16^{\circ}$, $-24^{\circ}$ inclination (p<.01). The knee joint was more increased at maximum varus during stance phase with $-16^{\circ}/-24^{\circ}$ inclination (p<.001). The hip joint was deceased at maximum adduction during stance phase with $-24^{\circ}$ inclination (p<.05). In a horizontal plane, only the knee joint was increased at maximum internal rotation during stance phase with $-24^{\circ}$ inclination (p<.05). In descent ramp walking, the different gait patterns occurred at an inclination of over $16^{\circ}$ on the descending ramp in the sagittal and frontal planes. These results suggest that there is a certain inclination angle or angular range where subjects do switch between level walking and descent ramp walking gait patterns.

몸통 굽힘이 어깨위팔리듬과 어깨뼈의 움직임 및 근육 활성도에 미치는 영향 (Effect of Trunk Flexion on Muscle Activity, Motion of Scapular and Scapulohumeral Rhythm)

  • The purpose of this study was to determine the effect of trunk posture on muscle activity and motion of scapular and scapulohumeral rhythm. Thirty-one healthy subjects performed right-arm abduction and adduction along the frontal plane while standing in both upright and flexed posture of trunk. Scapular upward rotation, anterior tilting and internal rotation ware recorded using a motion analysis system and muscle activity of upper trapezius, lower trapezius and serratus anterior ware recorded using surface electromyography during abduction and adduction in both trunk postures. then, scapulohumeral rhythm was calculated. Scapulohumeral rhythm and scapular posterior tilting in flexed posture was significantly decreased than in upright posture. Also, muscle activity of lower trapezius in flexed posture was significantly increased and serratus anterior was significantly decreased than in general posture. The result of this study revealed that flexed posture of trunk altered the muscle activity and kinematic of scapular. Measurement of trunk posture should be included the evaluation of dysfunction and disorder of shoulder girdle since rehabilitation of trunk posture is important to restore of upper limbs function.

2D Single-legged Dynamic Knee Valgus assessments Methods: Evaluating Risk Factor for Internal Derangement of the Knee; Literature Review

  • Objective: The objective of this study is to evaluate various research that have examined dynamic knee valgus and to pinpoint a straightforward, clinically practical 2D assessment method for dynamic knee valgus that is user-friendly. Design: A literature review Methods: This literature review was conducted in Pubmed, MEDLINE® and Google Scholar with the following key words: Knee valgus angle, Knee valgus evaluation, Knee valgus assessment, Dynamic knee valgus. After removing duplicate studies, 53 articles were initially chosen using this method, with 17 studies ultimately meeting the selection criteria. Results: Based on the comprehensive review of various studies, the Single Leg Squat (SLS) was identified as the most popular test method, followed by the Single Leg Landing (SLL) as the next most common test method. The Frontal Plane Projection Angle (FPPA) method was the most representative method for measuring dynamic knee valgus (DKV) during these tests. SLS was found in a total of 10 studies, while SLL was found in 7 studies. Conclusions: The most commonly proposed test for assessing DKV is measuring the SLS using the FPPA method. However, when applied to individuals without knee pathology, the discriminative power of this method may be limited. This suggests the need for further research to explore alternative methods for assessing DKV in this population.

The effects of increased unilateral and bilateral calcaneal eversion on pelvic and trunk alignment in standing position

  • Objective: Generally, it is known that there is a correlation between excessive calcaneus eversion and a patient with low back pain and it also affects pelvic alignment. However, there are not enough studies that show calcaneal eversion having an effect on the alignment of the trunk. Design: Cross-sectional study. Methods: A 3-dimensional motion analysis system was used to assess the lower limbs, pelvic alignment, and trunk alignment with increased unilateral and bilateral calcaneal eversion in twenty-one subjects. All subjects were asked to maintain a static posture for seven seconds on a wedge three times per posture for measurement and analysis purposes. The wedge used in the process was a lateral wedge with a 10-degree tilt to the lateral direction. To unify all of the subjects' foot position, the front and inner side of the wedge were marked. The height of the tilted wedge's inner side and flat wedge were balanced equally in order to be able to maintain the lateral part of the foot to the same height when producing an increased calcaneal eversion. Results: Comparing the changes in trunk and pelvic alignment in accordance to calcaneal eversion for each posture, there was a significant different in the X and Y-axis for each posture, but not in the Z-axis (p<0.05). Thus, it can be confirmed that calcaneal eversion in the sagittal plane and frontal plane may have and effect on the pelvis and the trunk. Conclusions: Postures with increased bilateral and unilateral calcaneal eversion has an effect on pelvic alignment, but does not cause any changes in trunk alignment.

골프 퍼팅 스윙시 성공과 실패에 따른 운동역학적 분석 (Biomechanics analysis by success and failure during golf putting swing)

  • 본 연구의 목적은 남자 대학 골프선수들을 대상으로 지면반력의 성분들이 골프 퍼팅의 정확성에 어떤 영향을 미치는지 알아보고 바른 기술을 습득할 수 있돌고 기초자료를 제시하며, 일선지도자나 코치들에게는 합리적이고 과학적인 훈련으로 지도할 수 있도록 하고, 특히 골프라는 운동에 있어 퍼팅의 중요도를 인식시키는 데 그 목적이 있으며, 대학 골프 선수 10명에게 골프 퍼팅 스윙을 수행하게 하여 지면반력 시스템을 통해 각 운동역학적 변인들을 분석한 결과 다음과 같은 결론을 얻었다. 국면별 전후방향의 지면반력 성분은 성공시 왼발이 실패시 왼발보다 전 국면에서 높게 나타났고, 오른발은 AD, BS, FS에서 실패시 오른발이 높게 나타났으며, IP시 성공시 오른발이 높게 나타났다. 국면별 좌우방향의 지면반력 성분은 성공시 왼발이 IP, FS에서 높게 나타났으며, AD, BS에서 실패시 왼발이 높게 나타났고, 오른발은 IP에서 실패시 오른발이 높게 나타났으며, AD, BS, FS시 성공시 오른발이 높게 나타났다. 국면별 수직방향의 지면반력 성분은 성공시 왼발이 AD, BS, FS에서 높게 나타났으며, IP시 실패시 왼발이 높게 나타났고, 오른발은 IP에서 실패시 오른발이 높게 나타났으며, AD, BS, FS에서 높게 나타났으며, IP시 실패시 왼발이 높게 나타났고, 오른발은 IP에서 실패시 오른발이 높게 나타났으며, AD, BS, FS시 성공시 오른발이 높게 나타났다. 국면별 회전력의 지면반력 성분은 성공시 왼발과 오른발의 전 국면에서 높게 나타났다. 위의 결과에 대한 결론을 요약해 볼 때 첫째, 전후방향의 힘은 무분별한 힘의 변화보다는 안정된 자세에서 리드미컬한 힘의 변화가 나타나는 동작이 필요하고, 둘째 안정된 자세에서 힘의 좌우 이동 폭이 큰 동작보다는 작은 동작이 퍼팅을 잘 할 수 있는 것으로 생각되었으며, 셋째 공의 진행 방향 즉 클럽의 진행 방향으로 체중이 이동하는 것이 바람직한 동작이며, 넷째, 완벽한 진자운동에 가까운 퍼팅이 바람직한 동작이라 생각되었다.

노인의 계단과 경사로 오르기 동안 하지의 운동학적 분석 (Kinematic Analysis of Lower Extremities during Stairs and Ramp Climbing with Older Adults)

  • 본 연구는 노인의 계단과 경사로 오르기와 평지보행 동안 하지의 운동학적 변화를 분석하는 것이었다. 건강한 노인 14명(남:10, 녀:4)이 실험에 참여하였으며 측정도구는 삼차원동작분석기인 Vicon system(Vicon, Oxford Metrics, England)을 사용하였다. 보행주기 각 시점에서 하지 관절의 각도 변화를 보행 조건(평지, 계단, 경사로)에 따라 알아보기 위해 일요인 반복측정 분산분석(one-way repeated measure of ANOVA)을 사용하였다. 전후면상에서 골반은 평지 보행에서 보행 주기 동안 전방굴곡이 나타났으며 계단과 경사로 오르기에서 전방굴곡이 증가하였다. 고관절과 슬관절는 모두 평지보행과 비교해 계단 오르기에서 굴곡이 더 증가하였다. 족관절은 평지보행과 비교해 경사로 오르기에서 굴곡이 더 증가하였다. 좌우면상에서 보행 주기 동안 평지보행과 비교해 계단 오르기에서 골반이 크게 올라갔으며 고관절도 계단 오르기에서 내전과 외전의 변화가 크게 나타났다. 또한 슬관절도 계단 오르기에서 내반이 증가하였고 족관절 역시 계단 오르기에서 내번이 증가하였다. 수평면상에서 보행 주기 동안 골반은 평지 보행과 비교해 계단과 경사로 오르기에서는 내회전이 대체로 증가하였고 고관절은 계단과 경사로 오르기에서 내회전이 증가하였으나 통계적으로 유의한 차이는 없었다. 슬관절은 계단 오르기에서 대체적으로 내회전이 증가하였고 족관절은 계단 오르기에서 대체적으로 외회전이 증가하였다. 이러한 결과는 노인의 계단과 경사로 오르기는 평지 보행과 비교해 하지의 운동학적 보행 변수들을 크게 변화시키는 것을 알 수 있었다.

Osteokinematic analysis during shoulder abduction using the C-arm

  • Objective: Despite reliable evidence of abnormal scapular motions increases, there is not yet sufficient evidence of abnormal humeral translations. This study aims to analyze the motion of the humeral head toward the scapula when the shoulder is actively abducted using the C-arm. Design: A case report. Methods: The participant was a healthy man without any limitation and pain during shoulder movement. The participant's shoulder was abducted; this movement in the frontal plane was measured using a C-arm (anterior-posterior view) and was analyzed with computer-aided design. The starting posture was $15^{\circ}$, and as the participant abducted his shoulder measurements were taken and analyzed at $30^{\circ}$, $60^{\circ}$, $90^{\circ}$, $120^{\circ}$, $150^{\circ}$, and ending at $165^{\circ}$. A line was drawn perpendicularly to the line connecting the humeral head axis to the glenoid, and another line was drawn perpendiculary to the line connecting the scapular axis to the glenoid. The distance between the two lines measured is defined as the e value. Results: At the starting posture ($15^{\circ}$), the central axis of the humeral head was located 1.92 mm inferior to the central axis of the scapula. The humeral head was superiorly translated from the starting posture to $120^{\circ}$, and then, showed an inferior translation to the ending posture ($165^{\circ}$). Conclusions: The results of this study showed that the humeral head moved upward from the starting posture ($15^{\circ}$) up to $120^{\circ}$ indicating, superior translation, and it moved downward when the posture was past $120^{\circ}$, indicating inferior translation.

단신 : 페달링 시 정량적인 동적 피팅을 위한 실시간 평가 시스템 (Technical-note : Real-time Evaluation System for Quantitative Dynamic Fitting during Pedaling)

  • In this study, a real-time evaluation system for quantitative dynamic fitting during pedaling was developed. The system is consisted of LED markers, a digital camera connected to a computer and a marker detecting program. LED markers are attached to hip, knee, ankle joint and fifth metatarsal in the sagittal plane. Playstation3 eye which is selected as a main digital camera in this paper has many merits for using motion capture, such as high FPS (Frame per second) about 180FPS, $320{\times}240$ resolution, and low-cost with easy to use. The maker detecting program was made by using Labview2010 with Vision builder. The program was made up of three parts, image acquisition & processing, marker detection & joint angle calculation, and output section. The digital camera's image was acquired in 95FPS, and the program was set-up to measure the lower-joint angle in real-time, providing the user as a graph, and allowing to save it as a test file. The system was verified by pedalling at three saddle heights (knee angle: 25, 35, $45^{\circ}$) and three cadences (30, 60, 90 rpm) at each saddle heights by using Holmes method, a method of measuring lower limbs angle, to determine the saddle height. The result has shown low average error and strong correlation of the system, respectively, $1.18{\pm}0.44^{\circ}$, $0.99{\pm}0.01^{\circ}$. There was little error due to the changes in the saddle height but absolute error occurred by cadence. Considering the average error is approximately $1^{\circ}$, it is a suitable system for quantitative dynamic fitting evaluation. It is necessary to decrease error by using two digital camera with frontal and sagittal plane in future study.
