Hematopoietic stem cell transplantation in children with acute leukemia: similar outcomes in recipients of umbilical cord blood versus marrow or peripheral blood stem cells from related or unrelated donors

  • Purpose: This study compared outcomes in children with acute leukemia who underwent transplantations with umbilical cord blood (UCB), bone marrow, or peripheral blood stem cells from a human leukocyte antigen (HLA)-matched related donor (MRD) or an unrelated donor (URD). Methods: This retrospective study included consecutive acute leukemia patients who underwent their first allogeneic hematopoietic stem cell transplantation (HSCT) at Samsung Medical Center between 2005 and 2010. Patients received stem cells from MRD (n=33), URD (n=46), or UCB (n=41). Results: Neutrophil and platelet recovery were significantly longer after HSCT with UCB than with MRD or URD ($p$ <0.01 for both). In multivariate analysis using the MRD group as a reference, the URD group had a significantly higher risk of grade III to IV acute graft-versus-host disease (GVHD; relative risk [RR], 15.2; 95% confidence interval [CI], 1.2 to 186.2; $p$=0.03) and extensive chronic GVHD (RR, 6.9; 95% CI, 1.9 to 25.2; $p$ <0.01). For all 3 donor types, 5-year event-free survival (EFS) and overall survival were similar. Extensive chronic GVHD was associated with fewer relapses (RR, 0.1; 95% CI, 0.04 to 0.6; $p$ <0.01). Multivariate analysis showed that lower EFS was associated with advanced disease at transplantation (RR, 3.2; 95% CI, 1.3 to 7.8; $p$ <0.01) and total body irradiation (RR, 2.1; 95% CI, 1.0 to 4.3; $p$=0.04). Conclusion: Survival after UCB transplantation was similar to survival after MRD and URD transplantation. For patients lacking an HLA matched donor, the use of UCB is a suitable alternative.

Outcome of allogeneic hematopoietic stem cell transplantation for childhood acute lymphoblastic leukemia in second complete remission: a single institution study

  • Purpose: The survival rate for childhood acute lymphoblastic leukemia (ALL) has improved significantly. However, overall prognosis for the 20 to 25% of patients who relapse is poor, and allogeneic hematopoietic stem cell transplantation (HSCT) offers the best chance for cure. In this study, we identified significant prognostic variables by analyzing the outcomes of allogeneic HSCT in ALL patients in second complete remission (CR). Methods: Fifty-three ALL patients (42 men, 79%) who received HSCT in second CR from August 1991 to February 2009 were included (26 sibling donor HSCTs, 49%; 42 bone marrow transplantations, 79%). Study endpoints included cumulative incidence of acute and chronic graft-versus-host disease (GVHD), relapse, 1-year transplant-related mortality (TRM), disease-free survival (DFS), and overall survival (OS). Results: Cumulative incidences of acute GVHD (grade 2 or above) and chronic GVHD were 45.3% and 28.5%, respectively. The estimated 5-year DFS and OS for the cohort was $45.2{\pm}6.8%$ and $48.3{\pm}7%$, respectively. Only donor type, i.e., sibling versus unrelated, showed significant correlation with DFS in multivariate analysis ($p$=0.010). The rates of relapse and 1 year TRM were $28.9{\pm}6.4%$ and $26.4{\pm}6.1%$, respectively, and unrelated donor HSCT ($p$=0.002) and HLA mismatch ($p$=0.022) were significantly correlated with increased TRM in univariate analysis. Conclusion: In this single institution study spanning more than 17 years, sibling donor HSCT was the only factor predicting a favorable result in multivariate analysis, possibly due to increased TRM resulting from unrelated donor HSCT.

진구성 상완 신경총 마비에 대한 유리박근이식술 (Gracilis Muscle Transplantation in Neglected Brachial Plexus Palsy)

  • Complete denervation after severe brachial plexus injury make significant muscle atrophy with loss of proper function. It is much helpful to reconstruct the essential function of the elbow flexion movement in patient with total loss of elbow flexion motion after brachial plexus lesion which was not recovered with nerve surgery or long term conservative treatment from onset. In whole arm type brachial plexus injury, if there were no response to neurotization or neglected from injury, the volume of the denervated muscle is significantely reduced month by month. About 18 months most of the muscle fibers change to fibrous tissues and markedly atrophied irreversibly, further waiting is no more meaningful from that period. Authors performed 14 cases of functioning gracilis muscle transfer from 1981 to 1995 with microneurovascular technique, neuromusculocutaneous free flaps were performed for reconstruction of lost elbow flexion function. Average follow-up period was 5 years and 6 months. We used couple of intercostal nerves as a recipient nerve which were anastomosed to muscular nerve from obturator nerve in all cases. Recipient vessels were three deep brachial artery and eleven brachial artery which were anastomosed to medial femoral circumflex artery with end to end or end to side fashion. Average resting length of the transplanted gracilis were 24 cm. We can get average 54 degree flexion range of elbow with fair muscle power from flail elbow. There were one case of muscle necrosis with lately developed thrombosis of microvascular anastomosed site which comes from insufficient recipient arterial condition, 3 cases of partial marginal necrosis of distal skin of the transplanted part which were not significant problem with spontaneously solved with time goes by gracilis muscle has constant neurovascular pattern with relatively easy harvesting donor with minimal donor morbidity. Especially it has similar length and shape with biceps brachii muscle of upper arm and longer nerve pedicle which can neurorrhaphy with intercostal nerve without nerve graft if sufficient mobilization of the nerves from both sides of gracilis and intercostal region. Authors can propose gracilis muscle transplantation with intercostal nerves neurotization is helpful method with minimal donor morbidity for neglected brachial plexus palsy patients.

박근의 원위혈관경을 이용한 도서형 근피판술 (Musculocutaneus Island Flap Based on the Distal Vascular Pedicle of Gracilis Muscle)

  • The gracilis that is frequently used as a donor of free muscle trasfer is appropriate in the muscular shape and vascular position. This muscle is belonged to the second type of muscle group by the classification of the pattern of muscular nutrient vessel. The adductor branch or first perforating branch of deep femoral artery which supplies the proximal 1/3 of this muscle is a dominant one and this is used for the microscopic anastomosis of muscle or musculocutaneous flap. The minor vascular pedicles which enter the distal 1/3 of this of this muscle are branches of the superficial femoral artery and it is 0.5mm in diameter, 2cm in length with two venae comitantes. These minor pedicles supplies distal half of the gracilis muscle. This island musculocutaneous flap using distal vascular pedicle can be used to cover the defect of soft tissue around the distal femoral supra-condylar area, knee joint and proximal tibial condyle area which cause limitation of motion of knee joint, or in the cases that usual skin graft is impossible. The important operative procedure is as follows; The dissection is carried proximally and distally and the entire gracilis muscle including proximal and distal pedicle is completely dissected. After temporary blocking of the proximal vascular pedicle, the adequate muscle perfusion by the distal pedicle is identified and it is rotated to the recipient site around knee joint. The advantages of this procedure are simple, no need of microscopic vascular anastomoses and no significant functional loss of donor site. Especially in the cases of poor condition of the recipient vessel, this procedure can be used effectively. From 1991 to 1996, we performed 4 cases; complete survival of flap in 3 cases and partial survival of flap with partial necrosis in 1 case. This procedure is though to be useful in the small sized soft tissue defect of distal femoral supra-condylar area, knee joint and proximal tibial condylar area, especially in the defect of anterior aspect which expected to cause limitation of motion of knee joint due to scar contracture. But the problems of this procedure are the diameter of distal vascular pedicle is small and the location of distal vascular pedicle is not constant. To reduce the failure rate, identify the muscular perfusion of distal vascular pedicle after blocking the proximal pedicle, or strategic delay will be helpful.

근단 변위 판막술을 이용한 매복 상악 중절치의 맹출유도 (ERUPTION GUIDANCE OF IMPACTED MAXILLARY CENTRAL INCISOR WITH APICALLY POSITIONED FLAP)

  • 상악중절치의 매복은 임상적으로 흔히 볼 수 있다. 대부분의 경우 중절치는 협측에 매복되어 있다. 이러한 협측매복은 치료가 무척 어렵다. 협측매복시 치은퇴축과 부착치은의 상실,치주낭이 발견된다. 따라서 성공적인 치료를 위해서는 외과적 술식을 통해서 부착치은의 소실을 최소화해야 할 것이다. 매복치의 위치와 부착치은의 양에 따라 수술방법으로 간단한 치은절제술(gingivectomy)외에도 필요하면 근단변위판막술(apically positioned flap), 측방변위판막술(laterally positioned flap), 유리치은 이식술(free gingival graft) 등과 같은 여러 판막술과 치아의 생리적인 맹출기전을 복제한 폐쇄 맹출법 (closed eruption technique)을 고려 할 수 있다. 하지만 치은절제술은 근단변위판막술에 비하여 부착치은의 양이 적어 많은 양의 부착치은 이 필요한 경우에는 근단변위판막술이 적응증이며 매복치아가 nasal spine근처에 있을 때는 폐쇄 맹출법을 선택해야 할 것이다. 본 증례에서 두 증례에서는 근단변위 판막술을 한 증례에서는 폐쇄 맹출법을 사용한 바 다음과 같은 결론을 얻었다. 폐쇄 맹출법이 더 심미적이었으나 부착치은의 양은 작았다. 이에 상악 중절치의 매복의 경우에서 올바른 임상적, 방사선학적인 검사를 통해 적응증에 맞는 외과적 수술로써 외과적 견인후 합병증을 최소화할 수 있었다.

폴리카복실레이트 분자 구조에 따른 시멘트페이스트의 분산 및 흡착 특성 연구 (The Dispersibility and Adsorption Behaviour of Cement Paste with Molecular Structures of Polycarboxylates)

  • 분자 구조 중 $\pi$ 결합을 갖고 있는 카복시산과 폴리에틸렌글리콜 메틸에테르 메타크릴레이트(PMEM)를 자유 라디칼 반응에 의해 그라프트 공중합된 폴리카복실레이트(PC)계 고유동화제를 합성했고, FT-IR, $^{13}C-NMR$, 그리고 GPC를 활용하여 합성된 PC의 화학 구조 및 분자량을 조사했다. 4종의 카복시산(메타크릴산, 아크릴산, 무수말레인산, 그리고 이타콘산)과 [카복시산]/[PMEM]의 몰비를 변수로 시멘트페이스트에 적용한 결과, [카복시산]/[PMEM]의 몰비가 높을수록 시멘트 입자 상에 흡착량과 시멘트페이스트 유동성은 증가했다. 흡착량은 메타크릴산을 적용한 PC가 가장 높았고, 유동성은 아크릴산을 구조에 적용했을 때 가장 우수했다. 그러나 무수말레인산과 이타콘산을 주쇄로 사용한 경우 흡착 및 유동 특성이 좋지 않았다.

경부식도암에 대한 수술적 치료 (Surgical Treatment for Cervical Esophageal Cancer)

  • 배경: 경부식도암은 흉부식도암에 비해 발생 빈도가 상대적으로 낮고 방사선치료나 항암 약물치료의 역할이 흉부식도암에서보다 상대적으로 높다. 따라서 경부식도암에 대한 수술적 치료의 역할이 흉부식도암에 비해 상대적으로 제한적이다. 저자들은 경부식도암에 대한 수술적 치료의 결과를 알아보기 위해 본 연구를 시행하였다. 대상 및 방법: 1989년 1월부터 2002년 12월까지 경부식도암(경흉부식도 암 포함 )으로 수술을 시행 받은 환자 43명의 결과를 후향적으로 분석하였다. 모든 환자에서 추적이 가능하였으며, 최종 추적일은 2004년 2월 28일이었다. 결과: 대상 환자의 평균 나이는 60세$(43{\sim}77)$였고, 남녀 비는 40:3이었다. 조직학적으로 편평상피세포암이 42명, 악성 흑색종이 1명이었다. 식도 재건 방법은 gastric pull-up 32명, 유리 공장 이식 7명, colon interposition 4명이었다. 전체 43명 중 31명(72%)의 환자에서 합병증이 발생하였고, 수술 사망자는 7명(16%)이었다. 수술 후 최종 병기는 I 3명, IIa 14 명, IIb 1명, III 19명, IVa 6명이었다. 수술 사망자를 제외한 36 명 중 16명(44%)에서 종양이 재발하였고, 수술 후 3년, 5년 생존율은 29.3% 와 20.9%이었다. 결론: 경부식도암에 대한 수술은 수술 사망률, 합병증 발생률, 재발률이 높고, 장기 생존율이 낮지만, 방사선치료나 항암약물치료 만으로는 연하곤란의 완전한 해소 및 종양의 완치가 어려우므로 수술이 포함되는 다방법 병합치료가 필요하다고 생각한다.

표층상완축상피판이식술과 전층망상식피술의 적용 예 (Recognition of Complication of Superficial Brachial Axial Pattern Flap in a Dog)

  • 13개월령, 체중 30 kg의 암컷 진도개가 왼쪽 전완부의 혈관염과 혈관 주위염으로 인한 심한 피부 결손을 주증상으로 내원하였다. 환자에 왼쪽 표층 상완 축상 피판술이 시행 되었다. 수술 일주일 후 평가에서, 피판은 1/3만이 성공적으로 이식이 이루어졌다. 따라서 결손 된 2/3부위는 전층 망상 식피술로써 재수술을 시행 하였다. 2차 수술 10일 후 결손부가 완전 복구되었다. 이식된 피판의 평가에서도 스스로 혈액 공급이 이루어지고 있었다. 이러한 피판 이식술은 피판의 혈관화가 이루어질 때까지 적절한 혈액 순환의 연속성이 중요하다. 피판 이식술은 다른 이식판보다 두껍기 때문에 이식에 따른 치유 진행 속도가 늦고, 꼬임이나 기저의 장력에 약하다. 따라서, 표층 상완 피판 이식술을 시행할 때, 반드시 피판의 길이와 두께 고려되어야 할 것으로 생각된다.

낭종성 병변을 동반한 상완골의 섬유성 골이형성증에서 steroid의 효과 - 증례 보고 - (The effect of steroid for fibrous dysplasia of the humerus combined with multiple cystic lesion)

  • 섬유성 골이형성증은 골의 발육 이상으로 해면골이 섬유성 조직으로 대치되어 나타나는 양성 병변으로 대퇴골, 경골, 두개골, 늑골, 상완골 등에 호발하며 동통, 진행하는 병변, 병적 골절 등을 일으키기 쉬워 여러 가지 치료법이 시도되어 왔으나 그 중 골 소파 및 골 이식술이 현재까지 널리 시행되고 있다. 본 교실에서는 17세 여자 환자로 우측 상완골 전장에 걸친 다발성 골 낭종을 동반한 섬유성 골이형성증 환자에 steroid 주입법을 약 3개월 간격으로 총 2차례 시행하였고 처음 주입 후 약 11개월 후에 시행한 추시 단순 방사선 촬영상 병변의 크기가 감소하였고, 골음영 및 피질골의 두께가 증가하였으며, 자기공명영상촬영 검사상 병변의 크기가 상당히 감소하였고, T2 강조 영상에서 정상 골수의 신호강도와 비슷하게 변화 하였으며, 처음 주입 후 약 2년 8개월 후의 외래 추시상 통증 호소하지 않고 무리하지 않는 한도내에서 큰 불편 없이 지내고 있어 섬유성 골이형성증의 호전 소견을 보였기에 문헌고찰과 함께 보고하는 바이다.

불유합의 위험 인자를 가진 족관절 병변에 시행한 족관절 유합술로서 Ilizarov 외고정 기구 장착술 및 자가골 이식술의 유용성 (Efficacy of Ilizarov External Fixation and Autologous Bone Grafting for Performing Arthrodesis on the Ankle Arthropathy with Risk Factors)

  • Purpose: The aim of this study is to evaluate if the Ilizarov external fixation procedure with axial compression can help to obtain fusion across the ankle joint in patients with a high risk of nonunion. Materials and Methods: From January 2006 to December 2015, the study reviewed 17 patients who underwent ankle arthropathy with a high risk of nonunion and who underwent ankle fusion using the Ilizarov external fixator with axial compression and auto bone grafting. After the lateral surface of the ankle joint was exposed through a lateral trans-fibular approach, massive removal of the articular cartilage and excision of any loose or avascular bone were done. With the cortical bone harvested from the pelvis as corticocancellous bone blocks, we inserted the two cortical blocks longitudinally into the anterior and posterior part of the free ankle space from lateral to medial to make the rectangular chamber to fill the cancellous bones. After the Ilizarov external fixator was equipped, we tightened the frame by 5 mm to compress the bone graft space. We accessed the American Orthopaedic Foot and Ankle Society (AOFAS) AnkleHindfoot score both preoperatively and postoperatively. Results: The average age at the time of operation was 63.4 years (range, 47~78 years). The mean frame time was 17.4 weeks (range, 15~23 weeks). The average follow-up period was 3.7 years (range, 2~6 years). Osseous fusion was obtained in 15 patients (88.2%). There were two stable pseudarthroses among the rheumatoid arthritis patients, and we continued their follow-up. The mean AOFAS AnkleHindfoot score improved from 48.5 to 73.7 points. Conclusion: Ankle arthrodesis using the Ilizarov external fixation with axial compression and auto bone grafting on the ankle arthropathy that had difficult conditions to achieve union is considered one of the useful methods with a correspondingly low incidence of complications.
