간암에서 색전술의 효과를 평가하는데 있어서 $^{99m}Tc$-MAA SPECT를 이용한 간동맥 혈류 검사의 의의 (Evaluation of Embolization Effect of Hepatocellular Carcinoma by Hepatic Arterial Flow Study with $^{99m}Tc$-MAA SPECT)

  • 본 저자들은 $^{99m}Tc$-MAA를 이용하여 정상 간조직에 대한 간암의 상대적인 미세 혈류량을 조사하고 간암 환자에서 간동맥 색전술을 시행 후 상대적인 혈류량의 변화를 검사함으로써 간동맥 색전술의 효과를 알아보고자 하였다. 여덟명의 간암 환자들을 대상으로 하였는데, 이 중 다섯 명은 조직 생검으로, 그리고 나머지 세 명은 임상적, 그리고 방사선학적으로 진단되었다. 3내지 4mCi의 $^{99m}Tc$-MAA를 2cc의 생리식염수에 혼합하여 간동맥내에 위치된 카테터(catheter)를 통하여 1분간에 걸쳐 서서히 주입한 후 바로 동적영상을 4초 간격으로 1분 동안 얻었으며, 이어서 정적 영상과 SPECT를 시행하였다. 간동맥 혈관 조영술만을 시행한 세명의 환자에서 $^{99m}Tc$-MAA를 주입하자마자 주변 간 조직에서 비해 간암내에 강한 방사능 섭취를 보였으며, 주변 간조직에 대한 간암의 방사능 섭취율은 6.5 이상이었고, 그 범위는 6.5에서 19였으며 평균은 12.5였다. 간동맥 색전술을 시행한 5명 중 4명에서 주변 간조직에 대한 간암의 방사능 섭취율이 심하게 감소하였다($0.5{\sim}1.3$). 또한 색전된 부위는 색전술 후 시행한 추적 간동맥 혈관 촬영상에서 보다 동위원소 검사에서 잘 관찰되었다. 간동맥혈관 촬영상에서 완전히 색전된 것으로 생각된 한 환자에서 동위원소 검사상 간암에 강한 방사능 섭취를 보였다(주변 간조직에 대한 간암의 방사능 섭취율 7.0). 따라서 본 저자들은 $^{99m}Tc$-MAA를 이용한 간 동맥 혈류 검사는 간암에서 색전술의 효과를 정확히 평가할 수 있는 유용한 검사법으로 이용될 수 있으리라 생각한다.

만성폐쇄성폐질환 환자의 고탄산혈증성 호흡부전 : 치료 반응 및 기도삽관의 결정인자 (COPD Patients with Hypercapnic Respiratory Failure : Response to Therapy and Determinant of Intubation)

  • 연구배경 : 만성폐쇄성폐질환 환자에서 고탄산혈증성 호흡부전에서 기계호흡 및 약물치료 등에 대한 반응과 치료경과 및 기계호흡을 결정하는 중요한 인자를 알아보고자 하였다. 방 법 : 만성폐쇄성폐질환의 고탄산혈증성 호흡부전으로 본원 호흡기내과에 입원하였던 환자를 내과치료군과 기도삽관군으로 나누어 임상적 특정 및 동맥혈가스소견 등의 검사소견을 비교하고, 호흡부전의 회복시기 및 경과, 기계호흡을 결정하는 인자, 기도삽관 시점, 그리고 기도삽관 기간 등을 비교하였다. 결 과 : 만성폐쇄성폐질환 환자에서 고탄산혈증성 호흡부전의 횟수는 132회였고, 기도삽관을 하였던 경우는 49회(37%)였고 내과적 치료에 반응을 보인 경우는 83회(63%)였다. 기도삽관군은 내과치료군에 비해 백혈구수치, BUN 및 APACHE II 점수가 높아서 질환의 중증도가 심하였고, 호흡성산혈증 및 고탄산혈증이 심하였다. pH가 7.2이하일때 기도삽관율은 87%로 매우 높았고, 기도삽관하기까지의 시간도 $2{\pm}3$시간으로 매우 짧았으며, pH 7.31-7.35일때 기도삽관율은 20%로 낮았고 기도삽관하기까지의 기간은 $97{\pm}121$시간으로 가장 길었다. 내과치료군에서 83%가 3일이내에 고탄산혈증성 호흡부전에서 회복되었다. 기계호흡군은 내원하여 24시간내에 75%에서 기도삽관이 시행되었다. 기도삽관을 결정하는 중요한 인자중 호흡성산혈증이 중요하며, 기도삽관전의 pH 평균의 범위는 7.04에서 7.24로서 pH 7.24를 기점으로 기도삽관이 결정되는 것으로 생각된다. 결 론 : 만성폐쇄성폐질환 환자의 고탄산혈증성 호흡부전에서 병의 중증도가 심하고, 호흡성 산혈증의 정도 및 고탄산혈증의 정도가 심할 때 기계호흡이 필요함을 알 수 있었고 그중 pH 7.24 이하의 산혈증이 특히 기도삽관을 결정하는 중요한 시점이라 생각되었다. 그리고 고탄산혈증성 호흡부전의 치료에 대한 반응 및 회복시기에 대한 정보가 앞으로 치료에 대한 지침이 될 것으로 생각된다.

형태학과 문자의 모양을 이용한 뉴스 비디오에서의 자동 문자 추출 (Automatic Text Extraction from News Video using Morphology and Text Shape)

  • 최근 들어 인터넷 사용의 증가와 더불어 디지털 비디오의 수요 또한 급격히 증가하고 있는 추세이다. 따라서 디지털 비디오 데이타베이스의 인덱싱을 위한 자동화된 도구가 필요하게 되었다. 디지털비디오 영상에 인위적으로 삽입되어진 문자와 배경에 자연적으로 포함되어진 배경문자 등의 문자 정보는 이러한 비디오 인덱싱을 위한 중요한 단서가 되어질 수 있다. 본 논문에서는 뉴스 비디오의 정지 영상에서 뉴스 자막과 배경 문자를 추출하기 위한 새로운 방법을 제안한다. 제안된 알고리즘은 다음과 같이 세 단계로 구성된다. 첫 번째 전처리 단계에서는 입력된 컬러 영상을 명도 영상으로 변환하고, 히스토그램 스트레칭을 적용하여 영상의 수준을 향상시킨다. 이 영상에 적응적 임계값 추출에 의한 분할 방법을 수정 적용하여 영상을 분할한다. 두 번째 단계에서는 적응적 이진화가 적용된 결과 영상에 모폴로지 연산을 적절하게 사용하여, 우선 문자 영역은 아니면서 문자로 판단되기 쉬운 양의 오류(false-positive) 요소들이 강조되어 남아있는 영상을 만든다. 또한, 변형된 이진화 결과 영상에 모폴로지 연산과 본 논문에서 제안한 기하학적 보정(Geo-corrertion) 필터링 방법을 적용하여 문자와 문자로 판단되기 쉬운 요소들이 모두 강조되어 남아있는 영상을 만든다. 이 두 영상의 차를 구함으로서 찾고자 하는 문자 요소들이 주로 남고, 문자가 아닌 문자처럼 보이는 오류 요소들은 대부분 제거된 결과 영상을 만든다. 문자로 판단되는 양의 오류 영역들을 남기는데 사용된 모폴로지 연산은 3$\times$3 크기의 구조 요소를 갖는 열림과 (열림닫힘+닫힘열림)/2 이며, 문자 및 문자와 유사한 요소들을 남기는데 사용된 연산은 (열림닫힘+닫힘열림)/2와 기하학적 보정이다. 세 번째 검증 단계에서는 전체 영상 화소수 대비 각 후보 문자 영역의 화소수 비율, 각 후보 문자 영역의 전체 화소수 대비 외곽선의 화소수 비율, 각 외곽 사각형의 폭 대 높이간의 비율 등을 고려하여 비문자로 판단되는 요소들을 제거한다. 임의의 300개의 국내 뉴스 영상을 대상으로 실험한 결과 93.6%의 문자 추출률을 얻을 수 있었다. 또한, 본 논문에서 제안한 방법으로 국외 뉴스, 영화 비디오 등의 영상에서도 좋은 추출을 보임을 확인할 수 있었다.

인간 착상 과정에 자궁내막과 배아의 역할 (The Role of the Endometrium and Embryo in Human Implantation)

  • 착상은 배아측과 모체측의 내분비적, 측분비적 및 자가분비적 인자들의 상호 작용에 의하여 조절된다. 착상의 적기는 $2{\sim}4$일로서 이 특수한 시간대 안에서 signaling, appositioning, attachment 및 invasion을 통하여 순차적으로 배아의 착상이 이루어지는데, 이는 자궁내막과 배아로부터 여러 사이토카인, 성장인자, 부착인자와 같은 다수의 생화학 인자의 생성과 분비를 포함하며 이로 인하여 수용적인 자궁내막이 형성된다. LIF, CSF-1, IL-1과 같은 사이토카인들은 착상을 이끄는 연속적인 사건에서 중요한 역할을 수행하며 integrin, L-selectin ligands, glycodelin, mucin-1, HB-EGF, pinopodes는 appositioning과 attachment에 관여한다. 배아 또한 사이토카인과 성장인자 및 LIF, CSF-1, IGF, HB-EGF에 대한 수용체들을 분비하여 능동적으로 대처한다. 자궁내막의 수용성을 평가하고 자연주기 또는 보조생식술을 이용한 임신의 예후를 예측하는데, 이와 같은 인자들이 유용한지는 앞으로 더 연구되어야 한다. 현재로서는 내막조직으로부터 채취한 integrins, pinopodes, glycodelin, LIF가, 자궁강내 세척액에서는 glycodelin과 LIF가 유망한 인자로 떠오르고 있다. 혈액내 마커로서는 VEGF, glycodelin, CSF가 약간의 연관성을 보여주고 있다. 이러한 인자들이 보조생식술 전후로 자궁내막의 기능과 임신의 예후를 평가하는 선별검사로 이용될 수 있는지를 증명하기 위해서는 향후 불임 여성과 대조군인 가임 여성을 대상으로 한 대규모의 연구가 필요할 것이다. 인간의 착상에서 이들 인자들의 기능을 충분히 이해해야 치료적 기법으로 연결되어 보조생식술에서도 더 높은 성공률을 이루어 낼 것으로 사료된다.

고선량률 강내조사선원을 이용한 근접조사선량계획전산화 개발 (Development of Dose Planning System for Brachytherapy with High Dose Rate Using Ir-192 Source)

  • 목적 : 고선량률의 Ir-192 선원을 이용한 근접조사의 모의촬영 영상을 개인컴퓨터(PC)에 입력하여 해부학적 영상에 선량분포를 구현하고 히스토그램, 선량-용적히스토그램 및 3차원 선량분포를 전산화하였다. 대상 및 방법 : 선량전산화에 이용된 선원은 원격근접조사장치(Buchler 3K, 독일)의 Co-60 대체선원으로 한국원자력 연구소와 공동으로 개발한 Ir-192이다. 선원 모양에 의존하는 선량분포의 비등방성은 선원을 미소 분할하여 구한 선량과 선원 중심에서 측방 기준점의 공중선량을 기준으로 규격화한 값을 이용하였다. 선원 주위의 조직선량은 선원 중심에서 측방으로 실측된 조직감쇠와 산란에 의한 보정계수와 에너지에 따른 공기 저지능에 대한 조직의 저지능 비로 공중-조직선량 변환계수를 적용하고 기준점에 대해 규격화한 선량률표를 검색하여 얻도록 하였다. 선량계획 전산화 과정에 모의촬영 영상입력, 선원입력과 선원의 축면 결정과 해부학적 영상을 이용한 선량분포와 점선량, 히스토그램 및 선량-용적 히스토그램을 구현하였다. 결과 : 저자들이 개발한 근접조사 선량계획시스템에는 선원모의촬영 영상을 스켄하여 비트맵 파일로 저장하고, 좌표원점과 확대율을 정해, 선원위치를 결정하고 선량분포와 선량분석 프로그램을 포함한 선량전산화를 구현하였다. 실험에 이용된 Ir-192 선원의 조직내 선량은 공중선량율과 조직에 의한 감쇠 및 산란에 의한 실험식을 이용하였다. 선원 중심에서 축상의 거리와 축에서 떨어진 거리에 따른 선량률표에서 행렬 검색하여 얻도록 하였다. 근접조사선량계획은 선원좌표 입력과 선원의 축면(principal plane)을 결정하여 선원이 포함된 평면상의 선량을 구현하였으며, 시뮬레이션 영상인 관상면과 시상면에 선량분포를 구현하였다. 선량-히스토그램에 의한 선량분포 분석은 임의의 해부학적 영상면 위에 커서가 놓인 위치의 선량 스켓치로 얻었다. 임상에 필요한 선량분석은 선원의 축에서 면의깊이를 이동하여 선량분포를 구할 수 있게 하였으며, 선량-용적 히스토그램과 3차원 선량분포를 구현하였다. 결론 : 고선량률 Ir-192를 이용하여 근접조사선량계획을 전산화하였으며, 선량분포의 분석에는 해부학적 영상의 선량분포와 선량-히스토그램, 선량-용적히스토그램을 구현하였으며, 선량분포의 면을 임의 선택할 수 있고 3차원 선량분포를 포함한 선량계획시스템을 준비하였다.

연속촬영 전자조사 문 영상을 이용한 오프라인 기반 치료 중 내부 장기 움직임 확인 시스템의 개발 (Development of an Offline Based Internal Organ Motion Verification System during Treatment Using Sequential Cine EPID Images)

  • 방사선치료 중 내부 장기의 움직임을 확인하고 이를 보정하는 것은 움직이는 종양에 정확히 방사선을 조사하는데 매우 중요한 역할을 한다. 실제 치료 중 획득한 연속촬영 전자조사 문(cine EPID) 영상을 이용해 치료 중 내부 장기 움직임을 추적하는 오프라인 기반 분석 시스템(IMVS, Internal-organ Motion Verification System using cine EPID)을 개발하였고 모형을 이용하여 개발된 시스템의 정확도와 유용성을 평가했다. IMVS는 cine EPID영상을 이용한 내부 장기 움직임 추적을 위해 내부 표지자를 이용한 유형 정합 알고리즘을 이용했다. 시스템의 성능평가를 위해 폐와 폐 종양을 묘사한 인체 모형과 이를 상하(SI, superior-inferior)방향으로 직선 운동시키는 구동 장치와 제어 프로그램을 고안했다. 모형을 4초 주기로 2 cm 직선 운동 시키면서 10 MV X선으로 3.3 fps, 6.6 fps속도로 cine EPID 영상($1,024{\times}768$ 해상도)를 획득했다. 획득된 cine EPID 영상은 IMVS를 이용하여 표적의 움직임을 추적하고 기존 외부 표지자를 이용한 비디오 영상 기반 추적시스템(RPM, Real-time Position Management, Varian, USA)으로부터 얻은 결과와 비교했다. 정량적 평가를 위해 두 시스템으로부터 움직임의 평균 주기(Peak-To-Peak), 진폭과 패턴(RMS, Root Mean Square)을 측정하여 비교했다. RPM과 IMVS로 측정한 폐 종양 모형의 움직임 주기는 각각 $3.95{\pm}0.02$ (RPM), $3.98{\pm}0.11$ (IMVS 3.3 fps), $4.005{\pm}0.001$ (IMVS 6.6 fps) 초로 실제움직임 주기인 4초와 잘 일치했다. IMVS로 획득한 모형 내부장기의 평균 움직임 진폭은 3.3 fps에서 $1.85{\pm}0.02$ cm, 6.6 fps에서 $1.94{\pm}0.02$ cm으로 실제 진폭 2 cm에 비해 각각 0.15 cm (오차 7.5%) 및 0.06 cm (오차 3%)의 차를 보였다. 움직임 신호의 일치성 평가를 위해 측정한 RMS는 0.1044 (IMVS 3.3 fps), 0.0480 (IMVS 6.6 fps)로 계획된 신호와 잘 일치 했다. cine EPID 영상을 이용하여 내부 표지자의 움직임을 추적하는 IMVS는 모형 실험에서 내부 장기의 움직임을 3% 오차 내에서 확인 가능했다. IMVS는 치료 중 내부장기 움직임을 측정하고 이를 사차원 방사선 치료계획과 비교하여 오차를 보정하는데 기여할 것으로 생각된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
