Objective : To identify the risk factors for postmenopausal osteoporosis in Korea Materials and methods : Bone mineral density (BMD) at the lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry in 808 apparently normal postmenopausal Korean women. Questions about life style, demographic parameters, medical history and social habits etc. were asked on these women; 2ll women with normal bone mineral density, and 597 women with osteopenia or osteoporosis. Results 'Age of >50 years, low body mass index (BMI; <18.5 kg/m2), long duration of menopause(>10 years), and previous history of fracture were associated with increased prevalence of osteopenia or osteoporosis. Women without the outside activity also showed a higher frequency of low bone mass, Risk for osteopenia or osteoporosis was low in women with high BMI (>23 kg/m2) and women with job. The prevalence of low bone mass appeared to be independent of the following parameters: socioeconomic status, familial history, smoking, drinking, exercise, previous use of oral contraceptive, coffee or milk consumption, and degree of sunlight exposure. Conclusion 'Age, BMI, duration of menopause, previous history of fracture and degree of outside activity are the risk factors for postmenopausal osteoporosis in Korea.
In this work we simulate explicitly the dynamic fracture propagation in reinforced concrete beams. In particular, adopting cohesive theories of fracture with the direct simulation of fracture and fragmentation, we represent the concrete matrix, the steel re-bars and the interface between the two materials explicitly. Therefore the crack nucleation within the concrete matrix, through and along the re-bars, the deterioration of the concrete-steel interface are modeled explicitly. The numerical simulations are validated against experiments of three-point-bend beams loaded dynamically under various strain rates. By extracting the crack-tip positions and the crack mouth opening displacement history, a two-stage crack propagation, marked by the attainment of the peak load, is observed. The first stage corresponds to the stable crack advance, the second one, the unstable collapse of the beam.
A 6-year-old male mongrel dog with a 7-month history of ulnar-radial nonunion fracture was treated with implantation of recombinant human bone morphogenetic protein-2 (rhBMP-2). The dog had received surgical correction three times prior to the admission but radiography of the affected limb revealed a typical figure of nonunion fracture. Glossly, the fractured ends were sclerotic and the area between the ends was filled with fibrous tissue. After debridement the shaft was fixed by an 10-hole plate. rhBMP-2 at a total dose of 256 micrograms was implanted with a synthetic carrier into the 10-mm defect formed by the debridement. Callus formation responding to rhBMP-2 was radiographically observed at 4 weeks after implantation and the defect bridged both fracture ends by 8 weeks after implantation. The plate was removed at 12 months after implantation. Any complications were not observed for 5 months after removal of the plate.
Violent or severe persistent coughing is rarely associated with rib fracture. We report a rare case of a cough-induced rib fracture in a patient without any traumatic history or any other underlying disease. A 32-year-old female presented to the emergency department complaining of having had right-sided pleuritic chest pain for 5 days. She had a background of an 8-week coughing illness. A posteroanterior view of the chest radiograph showed no definite fractured line. Chest computed tomography revealed a subtle break cortical line of the 7th rib in the right-sided chest wall. Early identification of a cough-induced fracture of the rib by using computed tomography may avoid unnecessary further work-ups including laboratory examination and may lead to appropriate discharge instructions including rest and reassurance. The clinical presentations and radiologic findings of rib fractures caused by coughing are presented along with a review of the literature.
Isolated stress fracture of the first rib is rare, and repeated muscular pulling and fatigue of bone is thought to be responsible for this fracture. The diagnosis can be made by taking a thorough history and performing chest roentgenography, computed tomography or magnetic resonance imaging. Conservative treatment generally cures this condition. We report here on a case of exercise-induced isolated stress fracture of the first rib in a non-athlete college student and we review the related articles.
Failure modes result in fracture or tearing, which may cause deterioration of resistance and reduction of inelastic deformation capacity. The potential failure modes for Special Concentrically Braced Frames (SCBFs) include fracture or tearing of the brace, net section fracture of the brace or gusset plate, fracture of the gusset plate welds, shear fracture of the bolts, block shear, excessive bolt bearing deformation, and buckling of the gusset plate. HSS tubular braces are commonly used in SCBFs, and net section fracture of the tubular brace may also occur through the brace net section at the end of the slot cut into the tube to slip over the gusset plate. This failure mode is categorized as a tension failure mode, and may cause dramatic loss of resistance and brittle behavior. Net section reinforcement is required according to AISC design specifications (AISC 2001). In this paper, the need to reinforce the net section area was discussed. Initially, the results of the net section fracture tests done by the University of California in Berkeley were presented with the modeling of these tests using FE models. To investigate the possibility of net section fracture in an actual frame, the slot end hole model was adapted to the frame FE model, and alternate near-fault histories were applied with tension-dominated cycles, since previous analyses showed that loading history was the most critical factor in net section fracture. The need for this reinforcement (cover plate) and the tension-dominated near-fault history were investigated.
Purpose: This study was undertaken in order to identify the characteristics of patients diagnosed with occult an hip fracture after hip trauma. Methods: We retrospectively reviewed the medical records and radiology reports of all patients who underwent hip skeletal computed tomography (CT) for suspected hip fractures but had normal initial X-rays after hip trauma between August 2006 and January 2012. The variables evaluated included age, gender, body mass index (BMI), accident mechanism, previous fracture, independence, late presentation, ability to bear weight, pain on passive rotation, tenderness of the groin area, diagnosis and treatment. Patients were divided into two groups, with hip fracture (occult hip fracture group) and without hip fracture (no fracture group) to evaluate the characteristics associated with an occult hip fracture. Results: The patients, a total of 139, had a mean age of 58.3 years and included 72 male patients(51.8%). The occult hip fracture group included 43 patients(30.9%). Of those 43, 21 patients(48.8%) had intertrochanteric or trochanteric fractures, 8 patients(18.6%) had femur neck fractures and 14 patients(32.6%) had acetabular fractures. Of the 43, 15 patients(34.9%) needed operative treatment. Age was higher in the occult hip fracture group than it was in the no fracture group($64.4{\pm}19.1$ years vs. $55.5{\pm}23.6$ years, p=0.021). A previous fracture was associated with the presence of a new fracture (p=0.014; OR=3.971, 95% CI=1.314-11.997). Conclusion: Further evaluation of patients who are older or have history of fractures is prudent, even though the initial X-rays are normal.
Since the propagation of a short fatigue crack is directly related to the large crack which causes the fracture of bulk specimen, the detailed study on the propagation of the short crack is essential to prevent the fatigue fracture. However, a number of recent studies have demonstrated that the short crack can grow at a low applied stress level which are predicted from the threshold condition of large crack. In present study, the threshold condition for the propagation of short fatigue crack is examined with respect to the micro-structure and cyclic loading history. Specimens employed in this study were decarburized eutectoid steels which have various decarburized ferrite volume fraction. Rotating bending fatigue test was carried out on these specimens with the special emphasis on the "critical non-propagating crack length" It is found that the reduction of the endurance limit of their particular micro-structures can be due to the increase of the length of critical non-propagating crack, and the quantitative relationship between the threshold stress ${\sigma}_{wo}$ and the critical non-propagating crack length $L_c$ can be written as ${\sigma}_{wo}{^m}{\cdot}L_c=C$ where m,C is constant. Further experiments were carried out on cyclic loading history on the length of critical non-propagating crack. It shown that the length of critical non-propagating crack is closely related to cyclic loading history.
There has been no report of bilateral pedicle stress fractures involving two vertebrae. The authors describe a unique case of spondylolisthesis accompanying a bilateral pedicle stress fracture involving two vertebrae. De novo development of spondylolisthesis at the L5-S1 vertebrae accompanying a bilateral pedicle stress fracture at L4 and L5 was observed in a 70-year-old woman. The patient's medical history was unremarkable and she did not have any predisposing factors except severe osteoporosis. Interbody fusion with bone cement augmented screw fixation was performed. Surgical treatment resulted in good pain management and improved functional recovery.
Sacral insufficiency fractures are usually known to develop in elderly patients with osteoporosis without definite trauma history. It is difficult to diagnose the sacral insufficiency fracture at an early stage because lower lumbar diseases. concurrently or not, may also be presented with similar symptoms and signs. We report a rare case of sacral insufficiency fracture who was not diagnosed initially but, instead, showed progressively worsening of clinical symptoms and radiological findings after decompression surgery for upper level lumbar stenosis.
