  • This study evaluates the seismic performance of reinforced concrete (RC) flat plate structures relation to the gravity shear ratio. For this purpose, 3 and 7 story framed buildings were designed for gravity loads only. Subsequently, a nonlinear static pushover analysis and a nonlinear time history analysis for the prototype buildings were carried out. In the nonlinear analysis, newly propose analytical slab-column joint model was utilized to capture punching shear failure and fracture mechanism in the analysis. The analytical results showed that seismic performance of RC flat plate frame is strongly influenced by the gravity shear ratio. In particularly, in the RC flat plate frame with a large gravity shear ratio the lateral strength and maximum drift capacity decreased significantly.

  • The objective of this study is to report an accidental detection of colorectal cancer (CRC) metastasis to spine during conservative treatment for lumbar disc herniation. We treated a 65-year-old female who was diagnosed with lumbar disc herniation on September 2019 by acupuncture, pharmacopuncture, cupping treatment, chuna manual therapy, herbal medicine treatment, medicine treatment and physical therapy. After that we analyzed medical record from December 12, 2019 to February 11, 2020. The patient was diagnosed with CRC and received tumor resection in 2014. After 2 times of chemotherapy, she arbitrarily interrupted the treatment. Since she stated that CRC treatment was terminated, we had difficulty in finding connection between symptom and CRC. During the treatment period, compression fracture at L3 body was found, which was caused by CRC metastasis. Rigorous question, appropriate radiological and clinical tests are required to patients who have history of malignant tumor.

  • During winter, the CWR (continuous welded rail) may be broken when a temperature drop below the neutral level changes the axial force, causing tensile fracture and creating a rail gap. The passage of a train on a rail with an open gap may lead to very costly derailments. In this paper, the use of a track-and-train-coupled model whose rail has an open gap is proposed for dynamic interaction analysis. Linear track and train systems were coupled in this study by a nonlinear Herzian contact spring, and the complete system matrices of the total track-train system were constructed. Moreover, the interaction phenomenon considering the presence of an open gap in the rail was toughly defined by assigning the irregularity functions between the two sides of the gap. Time history analysis, which has an iteration scheme such as the Newmark-$\beta$ method (based on the Modified Newton-Raphson methods), was conducted to solve the nonlinear equation. .Finally, numerical studies were conducted to assess the effect of the various parameters of the system when applied to various speeds, open-gap sizes, and support stiffnesses of the rail.

  • Conventional methods for assessing remaining life of critical high temperature components in fossil power plants rely on nondestructive inspection practices and accompanying life analysis based on fracture mechanics By using these conventional methods. It has been difficult to perform uninterrupted in-service inspection for life prediction. Thus, efforts have been made for developing on-line remaining life monitoring systems employing information on the shape of structures, operating variables and material properties. In thus study, a software for on-line life monitoring system which performs real-time life evaluation of a high temperature system headers was developed. The software is capable of evaluating creep and fatigue life usage from the real-time stress data calculated by using temperatures/stress transfer Green functions derived in advance for the specific headers. The major benefits of the developed software life in determining future operating schedule, inspection interval, and replacement plan by monitoring real-time life usage based on prior operating history.

  • Alloplastic implants have been used to repair orbital wall fractures in most cases. Orbital hemorrhage is a rare complication of these implants and has been reported rarely in Korea. The purpose of this article is to report a late complication case focusing on their etiology and management. A 20-year-old male patient underwent open reduction with Medpor (porous polyethylene) insertion for bilateral orbital floor fractures. The initial symptom occurred with proptosis in the right side as well as vertical dystopia, which had started 4 days earlier, 8 months after surgery. Any trauma history after the surgery was not present. We performed an exploration and removal of hematoma with Medpor titanium meshed alloplastic implant. A case of delayed orbital hematoma following alloplastic implant insertion was identified. It occurred within the pseudocapsule of the implant. One week after surgery, overall symptoms improved successfully, and no complications were reported during the 11-month follow-up period. Although rare, orbital hemorrhage is a potential complication of alloplastic orbital floor implants, which may present many years after surgery. As in the case presented, delayed hematoma should be included in the differential diagnosis of late proptosis or orbital dystopia.

  • Background: To compare the sensory change and recovery of infraorbital area associated with zygomaticomaxillary and orbital floor fractures and their recoveries and investigate the factors that affect them. Methods: We retrospectively reviewed 652 patients diagnosed with zygomaticomaxillary (n= 430) or orbital floor (n= 222) fractures in a single center between January 2016 and January 2021. Patient data, including age, sex, medical history, injury mechanism, Knight and North classification (in zygomaticomaxillary fracture cases), injury indication for surgery (in orbital floor cases), combined injury, sensory change, and recovery period, were reviewed. The chi-square test was used for statistical analysis. Results: Orbital floor fractures occurred more frequently in younger patients than zygomaticomaxillary fractures (p< 0.001). High-energy injuries were more likely to be associated with zygomaticomaxillary fractures (p< 0.001), whereas low-energy injuries were more likely to be associated with orbital floor fractures (p< 0.001). The sensory changes associated with orbital floor and zygomaticomaxillary fractures were not significantly different (p= 0.773). Sensory recovery was more rapid and better after orbital floor than after zygomaticomaxillary fractures; however, the difference was not significantly different. Additionally, the low-energy group showed a higher incidence of sensory changes than the high-energy group, but the difference was not statistically significant (p= 0.512). Permanent sensory changes were more frequent in the high-energy group, the difference was statistically significant (p= 0.043). Conclusion: The study found no significant difference in the incidence of sensory changes associated with orbital floor and zygomaticomaxillary fractures. In case of orbital floor fractures and high-energy injuries, the risk of permanent sensory impairment should be considered.

  • A 5-month-old female domestic short-haired cat presented with a history of seizure episodes for two months following an animal bite injury to the head. There were no remarkable findings on physical and neurological examination or blood analysis. Computed tomography revealed a fracture of the left parietal bone with an inward displacement of the bone fragment while magnetic resonance imaging revealed an enlarged temporal horn of the left lateral ventricle and a pseudomeningocele compressing the adjacent cerebral parenchyma. Subsequently, cerebrospinal fluid analysis results were normal. The patient was diagnosed with traumatic brain injury (TBI), with subsequent post-traumatic hydrocephalus (PTH) and pseudomeningocele. Despite treatment with phenobarbital and levetiracetam, seizures were not sufficiently controlled. Craniectomy for bone fragment removal and duraplasty were performed after a week. The patient then returned to normal condition with no further seizure activity. On repeated MRI two months after discharge, the hydrocephalus of the lateral ventricle and pseudomeningocele were enlarged; however, the patient maintained a good clinical status without any neurological signs. To the best of our knowledge, PTH and intracranial pseudomeningoceles have not yet been reported in cats. PTH and pseudomeningocele are among the complications of TBI and may not have any significant relevance with the clinical signs in this case. Thus, to broaden our knowledge about PTH and pseudomeningocele in cats, we describe serial changes in the clinical findings of this cat over the treatment period.

  • CWR (Continuous Welded Rail) may be broken when a temperature drop below the neutral temperature changes in axial force, causing tensile fracture and rail gap, in winter. Rail-breaks may lead to the damage of the rail and wheel by dynamic load, and the reduction of running safety if not detected before the passage of a train. In this study, the track and train coupled model with open gap for dynamic interaction analysis, is proposed. Linear track and train systems is coupled by the nonlinear Herzian contact spring and the complete system matrices of total track-train system is constructed. And the interaction phenomenon considering open gap, was defined by assigning the irregularity functions between the two sides of a gap. • Purpose: The purpose of this study was to review primary traumatic anterior shoulder dislocation in the 40 years and older to determine the incidence of recurrence, rotator cuff injury and associated fractures. Materials and Methods: From May, 1991 to October, 1998, seventeen patients were available to participate in the study. The average age was 55 years(range, 40 to 75 years). Follow-up averaged 27 months(range, 12 to 48 months). There were 12 men and 5 women. 10 right and 7 left shoulders were involved. Any participant with a history of shoulder pain and limitation of motion was excluded from the study. Each shoulder was evaluated with UCLA shoulder functional assessment. Results: The overall results were excellent in 9 patients(52.9%), good in 3 patients(17 .6%), fair in 3 patients(17.6%), and poor in 2 patients(11.7%). Surgical treatment of the associated rotator cuff and greater tuberosity fracture injuries resulted in 5 patients with excellent(55.6%), 3 patients with good(33.3%) and I patient with poor(11.1 %) and Nonsurgical treatment of the associated injuries resulted in 2 patients with excellent(33.3%), 3 patients with fair(50%), 1 patient with poor(16.7%)(P=0.025). Conclusion : Our findings on small number of patients suggest that the diagnosis of a rotator cuff and other pathologies should be approached aggressively by an MRI, CT arthrogram if significant pain and weakness are still present and early surgical repair of rotator cuff and greater tuberosity can lead to restoration of shoulder function and better outcomes in selected patients.
