Dosimetric advantages and clinical outcomes of simultaneous integrated boost intensity-modulated radiotherapy for anal squamous cell carcinoma

  • Purpose: The purpose of this study was to explore the dosimetric difference between simultaneous integrated boost intensity-modulated radiotherapy (SIB-IMRT) and three-dimensional conformal radiotherapy (3DCRT), and the clinical outcomes of anal squamous cell carcinoma (ASCC) chemoradiotherapy featuring SIB-IMRT. Materials and Methods: This study included ten patients with ASCC who underwent chemoradiotherapy using SIB-IMRT with 5-fluorouracil and mitomycin C. SIB-IMRT delivered 54 Gy to each primary tumor plus metastatic lymph nodes and 45 Gy to regional lymph nodes, in 30 fractions. Four patients received additional boosts to the primary tumors and metastatic lymph nodes; the median total dose was 54 Gy (range, 54 to 60 Gy). We additionally created 3DCRT plans following the Radiation Therapy Oncology Group 9811 protocol to allow dosimetric comparisons with SIB-IMRT. Locoregional control, overall survival, and toxicity were calculated for the clinical outcome evaluation. Results: Compared to 3DCRT, SIB-IMRT significantly reduced doses to the external genitalia, bladder, and intestine, delivering the doses to target and elective nodal region. At a median follow-up time of 46 months, 3-year locoregional control and overall survival rates were 88.9% and 100%, respectively. Acute toxicities were treated conservatively. All patients completed radiotherapy with brief interruptions (range, 0 to 2 days). No patient experienced ${\geq}grade$ 3 late toxicity during the follow-up period. Conclusion: The dosimetric advantages of SIB-IMRT appeared to reduce the toxicity of chemoradiotherapy for ASCC achieving high locoregional control in the extended period.

Personal Values and their Relationships with Public Library Use (개인의 가치관과 공공도서관 이용간의 관계성 분석)

  • The purpose of this study was to investigate the relationship between public library use and the individual's personal value system. The personal value system was measured by Schwartz's (1992) Theory of Basic Human Values. The theory includes ten universal values, which can be categorized into four higher-order values: namely, self-transcendence vs. self-enhancement; and openness to change vs. conservation. The data were collected on the web from 1,000 Korean adults who were 18 years old or above using a quota sampling method with respect to the distributions of gender, age, and geographic locations of the general Korean adult population. The collected data were analyzed using multi-dimensional scaling method, t-tests, and multiple correlations. The results showed that the public library is more likely to be used by the individuals who value "openness to change" (representing self-direction and stimulation), and "self-enhancement"(representing achievement, power, hedonism/sensuous pleasure) than those who value "conservation" (representing safety, stability of society, conformity to rules, tradition) and "self-transcendence" (representing interest in other's well-being, and tolerance, welfare of humanity and community). The findings suggest that public library uses reflect the characteristics of major library services, such as reading and self-learning, which are very autonomous and individualistic in their nature. The findings are valuable in understanding the nature of the functions performed by the current Korean public libraries that are reflected in the personal values of Korean adult library users.

Mechanism of Metronidazole Resistance Regulated by the fdxA Gene in Helicobacter pylori. (헬리코박터 파일로리에서 fdxA 유전자에 의한 메트로니다졸 내성 조절 기전 연구)

  • Resistance to metronidazole in Helicobacter pylori results from inactivation of rdxA and frxA, the chromosomal genes for a nitroreductase that normally converts metronidazole from prodrug to bactericidal agent. Two types of metronidazole susceptible strains had been found distinguishable by their apparent levels of frxA expression. Most common in the populations we had studied were strains that required only rdxA inactivation to become resistant to moderate levels of metronidazole(type I strains). The second strain type required inactivation of both frxA and rdxA to become resistance to metronidazole(type II strains): this was linked to a relatively high level of frxA gene transcription in the type II strains. The fdxA gene regulated fdxA as well as rdxA gene. Thus, to study the function of fdxA as a regulatory gene we constructed a null mutant of fdxA in H. pylori genome and identified over-and under-expressed proteins by fdxA using two-dimensional(2-D) electrophoresis and MALDI-TOP-MS. There were four over-expressed proteins in fdxA mutant; nifU-like protein(HP0221), frxA(HP0642), nonheme ferritin(HP0653), and hypothetical protein(HP0902). Three under-expressed proteins were also identified in fdxA mutant, including 5'-methylthioadenosine/S-adenosylhomocysteine nucleosidase (HP0089), (3R)-hydroxymyristoyl ACP dehydratase(HP1376), and thioredoxin(HP1458).

Optimization on the Preparation Conditions of Instant Rice Gruel Using Paecilomyces japonica mycelia (동충하초(Paecilomyces japonica)균사체를 이용한 즉석죽 제조조건의 최적화)

  • Cordyceps is very famous of its board biological activities and vital function in traditional medicines. Fourdimensional response surface methodology was used for monitering of instant rice gruels. The organolepitc color of instant rice gruel prepared using mushroom and glutinuous rice showed the maximum score in 38.52% rate of glutinous rice 321.46 mL content of added water and 1.30 g content of P. japonica mycelia. The organoleptic taste of instant rice gruel prepared using mushroom and glutinous rice showed the maximum score in 64.86% rate of glutinous rice 270.66 mL content of added water and 2.32 g content of P. japonica mycelia. The organoleptic flavor of instant rice gruel prepared using mushroom and glutinous rice was maximized in 36.75% rate of glutinous rice, 323.40 mL content of added water and 1.42 g content of P. japonica mycelia. The organoleptic mouth-feel of instant rice gruel prepared using mushroom and glutinous rice was maximized in 65.33% rate of glutinous rice 275.37 mL content of added water and 2.41 g content of P. japonica mycelia. The organoleptic overall palatability of instant rice gruel prepared using mushroom and glutinous rice showed the maximum score in 40.94% rate of glutinous rice 352.79 mL content of added water and 1.27 g content of P. japonica mycelia.

Monitoring on Extraction Yields and Functional Properties of Onion (Allium cepa) Extracts by Using Response Surface Methodology (반응표면분석을 이용한 양파의 추출효율 및 기능성에 대한 모니터링)

  • Extraction characteristics of onion (Allium cepa) and the functional properties of corresponding extracts were monitored by the response surface methodology (RSM). The maximum extraction yield of 44.02% was obtained under the specific extraction conditions, such as microwave power of 143.13 W, ethanol concentration of 61.77% and extraction time of 3.39 min. The maximum electron donating ability (EDA) was found as 45.85% at the conditions of 143.55 W microwave power, 27.52% ethanol concentration and 4.86 min extraction time. The maximum inhibitory effect on tyrosinase was 65.40% at 132.29 W, 36.47% and 7.62 min. The maximum superoxide dismutase (SOD)-like activity was 69.84% under the extraction conditions of 99.41 W, 8.47% and 7.13 min. Based on superimposition of four dimensional RSM with respect to extraction yield, electron donating ability and SOD-like activity obtained under the various extraction conditions, the optimum ranges of extraction conditions were found to be microwave power of 0$\sim$120 W, ethanol concentration of 70$\sim$80% and extraction time of 2$\sim$10 min.

The Effects of Socioeconomic Deprivation on Public Library Book Circulation: A Community-level Study (지역사회 사회경제적 박탈이 공공도서관 대출 책수에 미치는 영향)

  • This study analyzes the effects of community-level socioeconomic deprivations (SED) on public libraries' book circulation in the Seoul metropolitan area. The study design draws upon the theory of local information landscapes, which explains the relationship between community characteristics and information behavior. Using four-year (2015-2018) open government and public library circulation data, we constructed a socioeconomic deprivation index by adjusting a multi-dimensional deprivation index and generated other variables. Multi-level robust linear regression models were used to examine the relationship between SED and public library circulation. In addition, we tested the moderating effects of the library collection size and the number of libraries per unit area, respectively, on library circulation. The results show that there is a significant negative relationship between socioeconomic deprivation and library circulation rate. Also, we found that the size of the library collection negatively moderates the effects of SED in areas with a large number of books, and the number of libraries per unit area was positvely related to the library book circulation, not moderating the effects of SED. These findings suggest that public libraries and policymakers should consider community characteristics in designing strategic plans for public libraries.

Risk Analysis and Selection of the Main Factors in Fishing Vessel Accidents Through a Risk Matrix (위험도 매트릭스를 이용한 어선의 사고 위험도 분석과 사고 주요 요인 도출에 관한 연구)

  • Though, fishing vessel accidents account for 70 % of all maritime accidents in Korean waters, most research has focused on identifying causes and developing mitigation policies in an attempt to reduce this rate. However, predicting and evaluating accident risk needs to be done before the implementation of such reduction measures. For this reasons, we havve performed a risk analysis to calculate the risk of accidents and propose a risk criteria matrix with 4 quadrants, within one of which forecasted risk is plotted for the relative comparison of risks. For this research, we considered 9 types of fishing vessel accidents as reported by Korea Maritime Safety Tribunal (KMST). Given that no risk evaluation criteria have been established in Korea, we established a two-dimensional frequency-consequence grid consisting of four quadrants into which paired frequency and consequence for each type of accident are presented. With the simple structure of the evaluation model, one can easily verify the effect of frequency and consequence on the resulting risk within each quadrant. Consequently, these risk evaluation results will help a decision maker employ more realistic risk mitigation measures for accident types situated in different quadrants. As an application of the risk evaluation matrix, accident types were further analyzed using accident causes including human error (factor) and appropriate risk reduction options may be established by comparing the relative frequency and consequence of each accident cause.

Extra-phase Image Generation for Its Potential Use in Dose Evaluation for a Broad Range of Respiratory Motion

  • Background: Four-dimensional computed tomographic (4DCT) images are increasingly used in clinic with the growing need to account for the respiratory motion of the patient during radiation treatment. One of the reason s that makes the dose evaluation using 4DCT inaccurate is a change of the patient respiration during the treatment session, i.e., intrafractional uncertainty. Especially, when the amplitude of the patient respiration is greater than the respiration range during the 4DCT acquisition, such an organ motion from the larger respiration is difficult to be represented with the 4DCT. In this paper, the method to generate images expecting the organ motion from a respiration with extended amplitude was proposed and examined. Materials and Methods: We propose a method to generate extra-phase images from a given set of the 4DCT images using deformable image registration (DIR) and linear extrapolation. Deformation vector fields (DVF) are calculated from the given set of images, then extrapolated according to respiratory surrogate. The extra-phase images are generated by applying the extrapolated DVFs to the existing 4DCT images. The proposed method was tested with the 4DCT of a physical 4D phantom. Results and Discussion: The tumor position in the generated extra-phase image was in a good agreement with that in the gold-standard image which is separately acquired, using the same 4DCT machine, with a larger range of respiration. It was also found that we can generate the best quality extra-phase image by using the maximum inhalation phase (T0) and maximum exhalation phase (T50) images for extrapolation. Conclusion: In the present study, a method to construct extra-phase images that represent expanded respiratory motion of the patient has been proposed and tested. The movement of organs from a larger respiration amplitude can be predicted by the proposed method. We believe the method may be utilized for realistic simulation of radiation therapy.

A Study of Hydraulic Characteristics in Front of the Seawall under the Coexistence of Wave and Wind (파랑과 바람 공존장에서의 호안 전면 수리특성 검토)

  • In this study, a two-dimensional hydraulic model test was conducted to examine the hydraulic phenomena that occur around the seawall when wave and wind coexist. Based on recent seawall repair and reinforcement examples, the experimental section was constructed under the condition of installing wave dissipation blocks on the safety surface of four different representative seawalls. Water level fluctuation, reflection, overtopping and wave pressure characteristics according to external force change were reviewed. It was confirmed that the top concrete shape of the seawall is the most important factor of the hydraulic characteristics that appear in front of the seawall, and the tendency is more pronounced when wind acts. Even in the case of vertical type seawall, when wind of 3 m/s~5 m/s occurs, the amount of overtopping increases to about 5%~12%. In the case of wave pressure, it was confirmed from the experimental results that the value increased from about 1.5 to 2.2 times in front of the top of concrete block. In addition, it was confirmed that when the shape of the seawall was different, the range of change in the hydraulic characteristics appeared larger. Therefore, when designing a seawall of a new shape, a more detailed review of the hydraulic characteristics should be accompanied based on these experimental results.

Imaging Diagnosis of Dilated Cardiomyopathy in a Maltese Dog

  • A 6-year-old, spayed female, Maltese dog with tachypnea and dry cough was presented to Gyeongsang National University Veterinary Medical Teaching hospital. On physical examination, its respiration rate was 132 per minute. Decreased partial pressure of oxygen, partial pressure of carbon dioxide, and hyperlactatemia were found on arterial blood gas analysis. Its diastolic blood pressure was 80 mmHg. Auscultation revealed arrhythmia. Electrocardiogram revealed P pulmonale, P mitrale, and ventricular premature complexes. Thoracic radiographs revealed mild enlargement of both atrium and moderate enlargement of the left ventricular. There was also a moderate alveolar pattern in the right and caudal part of the left cranial lung lobe. Two-dimensional echocardiography showed enlargement of generalized four chambers without remarkable findings of valvular degeneration. M-mode echocardiography showed decreased left ventricular fractional shortening and enlarged left ventricular internal diameter at both end-systolic and end-diastolic. Color-flow Doppler imaging revealed eccentric turbulent flow starting below the left ventricular outflow tract and extending into the left atrium during systole. Spectral Doppler recordings revealed a high velocity flow through the mitral, tricuspid, aorta, and pulmonic regurgitation. Restrictive transmitral flow revealed high E-wave velocity, short E-wave deceleration time, and reduced A-wave velocity. There was also low ejection velocity thorough left ventricular out tract flow. Based on echocardiographic examination, dilated cardiomyopathy was the tentative diagnosis. The dog was medicated with inotropes, angiotensin converting enzyme inhibitor, and diuretics. At the 10-day following-up, the dog died suddenly. This report describes echocardiographic diagnosis and prognosis of dilated cardiomyopathy rarely reported in small breed dogs.
