대공협착에서 발생한 회전척추동맥증후군 (Rotatory Vertebral Artery Syndrome in Foramen Magnum Stenosis)

  • Rotatory vertebral artery syndrome (RVAS) is characterized by recurrent attacks of vertigo, nystagmus, and syncope induced by compression of the vertebral artery during head rotation. A 60-year-old man with atlas vertebrae fracture presented recurrent attacks of positional vertigo. Left-beat, upbeat and count clock-wise torsional nystagmus occurred after lying down and bilateral head roll (HR) showing no latency or fatigue. Magnetic resonance imaging revealed foramen magnum stenosis (FMS) and dominancy of right vertebral artery (VA). The flow of the right VA on transcranial Doppler decreased significantly during left HR. The slower the velocity was, the more the nystagmus was aggravated. RVAS can be evoked by FMS causing compression of the VA. And the nystagmus might be aggravated according to the blood flow insufficiency.

  • Occipital dysplasias in five dogs showing various neurologic signs of unknown origin were diagnosed with clinical examination and skull radiographic assessment at this university teaching hospital. The severities of occipital dysplasia were Grade III in four dogs and Grade II in one dog. Major clinical signs were ataxia and convulsion. In skull radiographs, there were dorsal extents of the foramen magnum reached nuchal crest in all cases. Also, hydrocephalus or vertebral malformation was complicated in three cases.

Conventional Posterior Approach without Far Lateral Approach for Ventral Foramen Magnum Meningiomas

  • Objective : We present our experience of conventional posterior approach without fat lateral approach for ventral foramen magnum (FM) meningioma (FM meningioma) and tried to evaluate the approach is applicable to ventral FM meningioma. Methods : From January 1999 to March 2011, 11 patients with a ventral FM meningioma underwent a conventional posterior approach without further extension of lateral bony window. The tumor was removed through a working space between the dura and arachnoid membrane at the cervicomedullary junction with minimal retraction of medulla, spinal cord or cerebellum. Care should be taken not to violate arachnoid membrane. Results : Preoperatively, six patients were of Nurick grade 1, three were of grade 2, and two were of grade 3. Median follow-up period was 55 months (range, 20-163 months). The extent of resection was Simpson grade I in one case and Simpson grade II in remaining 10 cases. Clinical symptoms improved in eight patients and stable in three patients. There were no recurrences during the follow-up period. Postoperative morbidities included one pseudomeningocele and one transient dysphagia with dysarthria. Conclusion : Ventral FM meningiomas can be removed gross totally using a posterior approach without fat lateral approach. The arachnoid membrane can then be exploited as an anatomical barrier. However, this approach should be taken with a thorough understanding of its anatomical limitation.

Intracranial Lipoma in Medulla Oblongata

  • Intracranial lipomas are rare, and most of these tumors are found in the region of the corpus callosum, followed by cerebellopontine angle. We present a case of a intracranial lipoma in 30-year-old man. Brain computed tomography [CT] scan and magnetic resonance images [MRI] showed a mass in the medulla oblongata extending to foramen magnum. The histopathologically, diagnosis of lipoma was confirmed. Although there were several cases of cervical intraspinal lipoma extending into posterior cranial fossa, there have been no previous reports of a lipoma arising from the medulla oblongata that extended into the foramen magnum. We describe a rare case of intradural subpial lipoma in the medulla oblongata with a review of the literature.

Foramen Magnum Decompression with Adipose Tissue Grafting for Caudal Occipital Malformation Syndrome in a Maltese Dog

  • A 7-year-old intact female Maltese dog was referred to the hospital with a history of paresis in the hind limbs, left head turn, and a loss of balance that persisted for 2 weeks. Her condition was initially managed with steroids, prescribed by the referring veterinarian, but her neurological symptoms were not alleviated. Physical and neurological examinations, radiography, computed tomography, and magnetic resonance imaging were performed. Based on the findings on these examinations, caudal occipital malformation syndrome (COMS) with syringohydromyelia was diagnosed. Medical treatment was not effective in the previous trial; therefore, foramen magnum decompression, durotomy, and free autogenous adipose tissue grafting were performed. After 3 days, an improvement was observed in the clinical symptoms and was maintained for 8 months postoperatively. Based on the results, it is suggested that the decompression method with a fat graft may be considered an effective surgical treatment for the management of COMS that did not respond well to previous medical treatment.

A simple nonlinear model for estimating obturator foramen area in young bovines

  • The aim of this study was to produce a simple and inexpensive technique for estimating the obturator foramen area (OFA) from young calves based on the hypothesis that OFA can be extrapolated from simple linear measurements. Three linear measurements - dorsoventral height, craneocaudal width and total perimeter of obturator foramen - were obtained from 55 bovine hemicoxae. Different algorithms for determining OFA were then produced with a regression analysis (curve fitting) and statistical analysis software. The most simple equation was OFA ($mm^2$) = [3,150.538 + ($36.111^*CW$)] - [147,856.033/DH] (where CW = craneocaudal width and DH = dorsoventral height, both in mm), representing a good nonlinear model with a standard deviation of error for the estimate of 232.44 and a coefficient of multiple determination of 0.846. This formula may be helpful as a repeatable and easily performed estimation of the obturator foramen area in young bovines. The area of the obturator foramen magnum can thus be estimated using this regression formula.

대공부와 상위경추부 종양에 대한 수술적 치료 (Surgical Treatment of Foramen Magnum & High Cervical Spinal Cord Tumor)

  • Objective : The authors reviewed 51 patients of spinal cord tumor located at the craniovertebral junction and high cervical area to determine which factors influenced resectability, complications, and postoperative outcomes. Methods : Radiological examinations, clinical data, and operation notes were evaluated, and additional follow-up information was obtained from outpatient examinations. The mean follow-up period was 3.5 years Results : The most common neurological findings were motor deficit(72.5%) and sensory change(47.1%). There was no clinical finding that is considered as pathognomic. Meningioma(18 cases, 35.3%) was the most common tumor in this region. Total removal was achieved in 45 patients. There was no surgical mortality. Immediate postoperative motor weakness were encounted in 11 cases(21.6%) which improved in long term followup except two patients. Conclusions : The location and relationship of the tumor to surrounding struncture determine its resectability. Postoperative results were related to the preoperative neurological status and pathological findings.

Foramen Magnum Decompression with Duraplasty Using Lyoplant® for Caudal Occipital Malformation Syndrome in a Dog

  • A 3-year-old castrated male Maltese dog, weighing 4.8 kg was referred with hindlimb ataxia and right forelimb proprioceptive deficits were shown for 20 months. Chiari-like malformation and syringomyelia diagnosed through MRI at a local animal hospital and Knuckling of right forelimb and reluctance to walk were managed with steroid. The medical management was getting ineffective to manage for the symptoms one month before referred. Physical and neurological examinations, radiography, computed tomography, and magnetic resonance imaging were performed and diagnoses of caudal occipital malformation syndrome (COMS) and subsequent syringomyelia (SM) were made. Given that pharmacological treatment was previously ineffective, surgical intervention was recommended. Foramen magnum decompression with duraplasty using $Lyoplant^{(R)}$ was performed. Three days post-surgery, the dog showed improved gait and activity. After 2 months, the dog received no additional prescription medications. At the 12-month follow-up after surgery, the dog showed no clinical problems or recurrences, despite complete cessation of pharmacological treatment. In present report, we applied $Lyoplant^{(R)}$ as a dural graft has been carried out in a dog with COMS. Surgical decompression with $Lyoplant^{(R)}$ was an effective long-term (12-month) treatment for COMS without the need for any pharmacological treatment.

Acquired Chiari Malformation

  • Perioperative lumbar drainage of cerebrospinal fluid is commonly used in neurosurgical practice. However, the relationship between lumbar drainage and acquired Chiari malformation is not well established. The authors present an unusual case of paraplegia as a result of acquired Chiari malformation after lumbar drainage. Acquired Chiari malformation can induce compression of cervicomedullary junction and syrinx formation. Foramen magnum decompression is recommended for the solution of such problems.
