Maxillary Sinus Augmentation Using Macroporous Biphasic Calcium Phosphate ($MBCP^{TM}$) : Three Case Report With Histologic Evaluation (합성골 이식재인 Macroporous biphasic calcium phosphate를 이용한 사람의 상악동 거상술-증례 보고)

  • Background Several bone grafting materials have been used in sinus augmentation procedures. Macroporous Biphasic Calcium Phosphate($MBCP^{TM}$) consists of the mixture of 60% HA and 40% ${\beta}-TCP$. Therefore, it can provide good scaffold for the new bone to grow owing to HA, in the other hand, it can have bioactivity for bone remodeling owing to ${\beta}-TCP$. The purpose of this study was to evaluate bone formation following maxillary sinus augmentation using $MBCP^{TM}$ by means of histologic analysis. Material and Method $MBCP^{TM}$ was placed as a primary bone substitute for maxillary sinus augmentation. Three patients were selected after evalaution of their medical dental examination. $MBCP^{TM}$ only, $MBCP^{TM}$ combined with Irradicated cancellous bone and $MBCP^{TM}$ combined with autogenous bone were used for each patient. After about eight months, bone biopsies were harvested for histologic evaluation and fixtures installed. Results Eight months after surgery we observed new vital bone surrounding $MBCP^{TM}$ particle and the amount of new bone was about 30% even though there were discrepancies between specimens. This case report documents that $MBCP^{TM}$ when used as a grafting material for sinus floor augmentation whether combined other bone graft material or not, may lead to the predictable results for dental implants on posterior maxillary area with insufficient vertical height for fixture installation.

The comparative study - the regenerative effect depends on size of bone graft material in bone loss site around dental implant (임플란트 주위 골 결손부의 이식재 크기에 따른 재생 효과 비교 연구)

  • Purpose: The purpose of this study is to investigate on the regenerative capacity by using different size of graft materials around bony defect around implant. Material and Methods: Dental implant fixtures(Bio-TIS, Korea) were placed into the tibia of 8 rabbits. After placement of implant, artificial defects were created for each group, and the size of bone graft materials were used according to each designated group. 4 weeks after surgery, 8 rabbits were sacrificed. The histologic and histomorphometrical study were done for comparison of the regenerative capacity using $80-90{\mu}m$ and $200{\sim}1000{\mu}m$ size of grafting materials of OCS-$B^{(R)}$. Result: Matured bone formation was significantly increased more in Group E1($80-90{\mu}m$) than in Group E2($200{\sim}1000{\mu}m$). Group E1($80-90{\mu}m$) showed more significant augmentation in marginal length of graft material per unit area than Group E2($200{\sim}1000{\mu}m$). Group E1($80-90{\mu}m$) showed more interspace in graft material than Group E2($200{\sim}1000{\mu}m$). Control group showed no new bone formation around and inside of implanted fixture. Conclusion: Small grafting material size has great influence on bone regeneration.

Maxillary sinus floor augmentation with anorganic bovine bone : Histologic evaluation in humans (Anorganic bovine bone을 이용한 상악동저 거상술의 조직학적 평가)

  • Purpose: The aim of this report is to investigate the efficacy of anorganic bovine bone xenograft(Bio-$Oss^{(R)}$) at maxillary sinus floor augmentation. Materials and methods: Two male patients who missed maxillary posterior teeth were included. They were performed maxillary sinus floor augmentation using anorganic bovine bone xenograft(Bio-$Oss^{(R)}$). After 10 or 13 months, the regenerated tissues were harvested using trephine drills with 2 or 4mm diameter and non-decalcified specimens were made. The specimens were examined histologically and histomorphometrically to investigate graft resorption and new bone formation. Results: Newly formed bone was in contact with Bio-$Oss^{(R)}$ particles directly without any gap between the bone and the particles. The proportions of newly formed bone were $23.4{\sim}25.3%$ in patient 1(Pt.1) and 28.8% in patient 2(Pt.2). And the proportions of remained Bio-$Oss^{(R)}$ were $29.7{\sim}30.2%$ in Pt.1 and 29.2% in Pt.2. The fixtures installed at augmented area showed good stability and the augmented bone height was maintained well. Conclusion: Anorganic bovine bone xenograft(Bio-$Oss^{(R)}$) has high osteoconductivity and helps new bone formation, so that it can be used in maxillary sinus floor augmentation.

The effect of implant shape and bone preparation on primary stability

  • Purpose: The purpose of this study was to evaluate the effects of implant shape and bone preparation on the primary stability of the implants using resonance frequency analysis. Methods: Sixty bovine rib blocks were used for soft and hard bone models. Each rib block received two types of dental implant fixtures; a straight-screw type and tapered-screw type. Final drilling was done at three different depths for each implant type; 1 mm under-preparation, standard preparation, and 1 mm over-preparation. Immediately after fixture insertion, the implant stability quotient (ISQ) was measured for each implant. Results: Regardless of the bone type, the ISQ values of the straight-screw type and tapered-screw type implants were not significantly different (P>0.05). Depth of bone preparation had no significant effect on the ISQ value of straight-screw type implants (P>0.05). For the tapered-screw type implants, under-preparation significantly increased the ISQ value (P<0.05), whereas overpreparation significantly decreased the ISQ value (P<0.05). Conclusions: Within the limitations of this study, it is concluded that bone density seemed to have a prevailing effect over implant shape on primary stability. The primary stability of the tapered-screw type implants might be enhanced by delicate surgical techniques.

Influence of the implant abutment types and the dynamic loading on initial screw loosening

  • PURPOSE. This study examined the effects of the abutment types and dynamic loading on the stability of implant prostheses with three types of implant abutments prepared using different fabrication methods by measuring removal torque both before and after dynamic loading. MATERIALS AND METHODS. Three groups of abutments were produced using different types of fabrication methods; stock abutment, gold cast abutment, and CAD/CAM custom abutment. A customized jig was fabricated to apply the load at $30^{\circ}$ to the long axis. The implant fixtures were fixed to the jig, and connected to the abutments with a 30 Ncm tightening torque. A sine curved dynamic load was applied for $10^5$ cycles between 25 and 250 N at 14 Hz. Removal torque before loading and after loading were evaluated. The SPSS was used for statistical analysis of the results. A Kruskal-Wallis test was performed to compare screw loosening between the abutment systems. A Wilcoxon signed-rank test was performed to compare screw loosening between before and after loading in each group (${\alpha}$=0.05). RESULTS. Removal torque value before loading and after loading was the highest in stock abutment, which was then followed by gold cast abutment and CAD/CAM custom abutment, but there were no significant differences. CONCLUSION. The abutment types did not have a significant influence on short term screw loosening. On the other hand, after $10^5$ cycles dynamic loading, CAD/CAM custom abutment affected the initial screw loosening, but stock abutment and gold cast abutment did not.

  • Since Chrysler Motor Co. had experienced the digital development system in the beginning of 1990's, most of leading automobile companies are trying to apply a digital information system for their own business process reengineering based upon concurrent engineering system from product planning phase. This is called as virtual DMU(Digital Mock-Up) system instead of the traditional PMU(Physical Mock-Up) system. By using the virtual prototype, all of the design requirements and system specifications can be checked, changed and optimized more quickly and more efficiently. This paper consists of five chapters for the DMU information system. In the 1$^{st}$ chapter, the principle of digital design system is suggested by using four basic modules such as product design module, process design module, manufacturing system design module and central control module. The basic scheme of DMU is introduced with the benefits of application in the chapter 2. In the chapter 3, a digital design process of new car development is explained with the detailed DMU design and design review processes. In the chapter 4, the practical DMU manufacturing techniques and applications are introduced as CAD/CAM analyses, DPA(Digital Pre-Assembly)reviews for development, production, operation and maintenance phases, digital tolerance analyses and digital factory analyses for assembling line simulation, automated robot welding processes, production jig & fixtures and painting process simulation. Finally, the activities of digital design support; CAS-styling, CAE-engineering and CAT-testing are summarized for design optimization in the chapter 5. As today's automobile manufactures and related business organizations are struggling to compete in the global marketplace, they are concentrating on efficient use of DMU information system to reduce the new car development cost, to have shorten the delivery schedule and to improve product design quality. To meet the demand of those automobile industries on digital information systems, the CALS(Computer aided Acquisition and Logistics Support) and EC(Electronic Commerce)initiative has been focused as a dominant philosophy in defense & commercial industries, specially automobile industries.s.

Numerical study on buckling of steel web plates with openings

  • Cellular and castellated steel beams are used to obtain higher stiffness and bending capacity using the same weight of steel. In addition, the beam openings may be used as a pass for different mechanical fixtures such as ducts and pipes. The aim of this study is to investigate the effect of different parameters on both elastic and inelastic critical buckling stresses of steel web plates with openings. These parameters are plate aspect ratio; opening shape (circular or rectangular); end distance to the first opening; opening spacing; opening size; plate slenderness ratio; steel grade; and initial web imperfection. The web/flange interaction has been simplified by web edge restraints representing simply supported boundary conditions. A numerical parametric study has been performed through linear and nonlinear finite element (FE) models, where the FE results have been verified against both experimental and numerical results in the literature. The web plates are subject to in-plane linearly varying compression with different loading patterns, ranging from uniform compression to pure bending. A buckling stress modification factor (${\beta}$-factor) has been introduced as a ratio of buckling stress of web plate with openings to buckling stress of the corresponding solid web plate. The variation of ${\beta}$-factor against the aforementioned parameters has been reported. Furthermore, the critical plate slenderness ratio separating elastic buckling and yielding has been identified and discussed for two steel grades of DIN-17100, namely: ST-37/2 and ST-52/3. The FE results revealed that the minimum ${\beta}$-factor is 0.9 for web plates under uniform compression and 0.7 for those under both compression and tension.

  • Statement of Problem: To conduct a successful function of implant prosthesis in oral cavity for a long time, it is important that not only structure materials must have the biocompatibility, but also the prosthesis must be designed for the stress, which is occurred in occlusion, to scatter adequately within the limitation of alveolar bone around implant and bio-capacity of load support. Now implant which is used in clinical part has a very various shapes, recently the fixture that has tapered form of internal connection is often selected. However the stress analysis of fixtures still requires more studies. Purpose: The purpose of this study is to stress analysis of the implant prosthesis according to the different implant systems using finite element method. Material and methods: This study we make the finite element models that three type implant fixture ; $Br{\aa}nemark$, Camlog, Frialit-2 were placed in the area of mandibular first premolar and prosthesis fabricated, which we compared with stress distribution using the finite element analysis under two loading condition. Conclusion: The conclusions were as follows: 1. In all implant system, oblique loading of maximum Von mises stress of implant, alveolar bone and crown is higher than vertical loading of those. 2. Regardless of loading conditions and the type of system. cortical bone which contacts with implant fixture top area has high stress, and cancellous bone has a little stress. 3. Under the vertical loading, maximum Von mises stress of $Br{\aa}nemark$ system with external connection type and tapered form is lower than Camlog and Frialit-2 system with internal connection type and tapered form, but under oblique loading Camlog and Frialit-2 system is lower than $Br{\aa}nemark$ system.

A STUDY ON THE BONE FORMATION OF OPEN TYPE AND CLOSED TYPE IMPLANTS (개방형과 폐쇄형 임플랜트 매식후 주위골 형성에 관한 실험적 연구)

  • A two-stage procedure is ideal for getting a successful osseointegration. But if a one-stage procedure can achieve a similar osseointegration, the one-stage procedure has several advantages. The purpose of this study was to observe the initial bone formation and bone remodeling of open type (nonsubmerged) and closed type (submerged) titanium implants. Eight ITI hollow-screws and eight Branemark fixtures were divided into two groups (submerged and nonsubmerged) and were installed on the lower jaws of four mongrel dogs. The animals were sacrificed three months later and bone sections with implants were processed for light microscopic and fluorescent microscopic observation. The results were as follows : 1 There was no significant difference in bone-to-implant contact between submerged and nonsubmerged implants. 2. Smooth surface titanium implants showed more bone-to-implant contact than that of titanium plasma coated implants histologically. 3. Under fluorescent microscopy, the active bone remodeling and new bone formation were observed in the interface zone. 4. Under fluorescent microscopy, submerged and nonsubmerged implants had no difference in bone remodeling pattern, and intramembranous bone formation was more prominent. 5. The connective tissue fibers orienting perpendicularly toward implant surface were oberved in the neck of implants.

  • In this paper, we have studied to minimize the lifetime and fault occurrence of LED fixtures, which have a short lifetime and frequent faults. In the current LED chip, the lifetime is semi-permanent, but compared to the lifetime of the LED chip, Drivers do not last long. In recent years, low-priced LED illuminators such as those from China have entered the market, and many consumers are perceiving LED illuminators. In order to solve these problems, we designed an LED illuminator of 25W class by using AC Direct IC, which has a longer lifetime than the conventional driver, by removing the electrolytic capacitor in the LED driving driver which is the cause of the failure.
