디지털 방식을 이용한 교합평가는 종래의 방식인 교합지와 occlusion foil 등에 비해 기록, 비교, 분석 등이 쉽고 간편하며 객관적 이라는 장점이 있다. 이에 본 증례보고에서는 교합평가에 디지털 기술을 활용하고자 단일 전장관 수복이 필요한 환자에서 구강스캐너(i500, Medit, Seoul, Korea)를 이용하여 매 내원시 교합을 평가하였으며, 비교를 위하여 교합지를 이용한 교합관계 확인과 디지털 교합분석장비(Dental prescale II, GC corporation, Tokyo, Japan)를 이용하여 교합관계를 평가하였다. 치료과정에서 구강스캐너를 이용한 방법과 기존의 교합지를 이용한 방법이 유사한 결과를 보여 향후 임상적으로 유용하게 사용될 수 있을 것으로 사료되었다.
PURPOSE. The purpose of this study was to investigate the misfit and screw preload at the implant abutment connection of implant supported fixed dental prosthesis with cantilever (ICFDP) manufactured using different digital manufacturing techniques and to compare the screw preload before and after cyclic loading. MATERIALS AND METHODS. Mandibular jaw model with four intra-foraminal implants was scanned using digital scanner. Stereolithography file was used to design a framework with nonengaging (NE) abutments and 10 mm cantilever distal to one terminal implant. Five frameworks were constructed using combined digital-conventional techniques (CAD-cast), and five frameworks were constructed using three-dimensional printing (3DP). Additional CAD-cast framework was constructed in a way that ensures passive fit (PF) to use as control. Scanning electron microscope (SEM) measured the implant abutment connection misfit. Sixty screws were used on the corresponding frameworks. Screws were torqued and pre-cyclic loading reverse torque value (RTV) was recorded. Frameworks were subjected to 200,000 loading cycles with a loading point 9 mm from the center of terminal implants adjacent to the cantilever and post-cyclic loading RTVs were recorded. RESULTS. Microscopic readings showed significant differences between frameworks. PF demonstrated the lowest measurements of 16.04 (2.6) ㎛ while CAD-cast demonstrated the highest measurements of 29.2 (3.1) ㎛. In all groups, RTVs were significantly lower than the applied torque. Post-cyclic loading RTV was significantly lower than pre-cyclic loading RTV in PF and 3DP frameworks. Differences in RTVs between the three manufacturing techniques were insignificant. CONCLUSION. Although CAD-cast and three-dimensionally printed (3DP) both produce frameworks with clinically acceptable misfit, 3DP might not be the technique of choice for maintaining screw's preload stability under an aggressive loading situation.
PURPOSE. This study assessed the physical and mechanical properties of interim crown materials fabricated using various digital techniques after accelerated aging. MATERIALS AND METHODS. Three groups of interim dental restorative materials (N = 20) were tested. The first group (CO) was fabricated using a conventional manual method. The second group (ML) was prepared from prefabricated resin blocks for the milling method and cut into specimen sizes using a cutting disc. The third group (3D) was additively manufactured using a digital light-processing (DLP) 3D printer. Aging acceleration treatments using toothbrushing and thermocycling simulators were applied to half of the specimens corresponding to three years of usage in the oral environment (N = 10). Surface roughness (Ra), Vickers microhardness, 3-point bending, sorption, and solubility tests were performed. A 2-way analysis of variance (ANOVA) and Fisher's multiple comparison test were used to compare the results among the groups. RESULTS. The mean surface roughness (Ra) of the resin after accelerated aging was significantly higher in the CO and ML groups than that before aging, but not in the 3D group. All groups showed reduced hardness after accelerated aging. The flexural strength values were highest in the 3D group, followed by the ML and CO groups after accelerated aging. Accelerated aging significantly reduced water sorption in the ML group. CONCLUSION. According to the tested material and 3D printer type, both 3D-printed and milled interim restoration resins showed higher flexural strength and modulus, and lower surface roughness than those prepared by the conventional method after accelerated aging.
PURPOSE. To compare the clinical outcomes of two types of implant restoration for posterior edentulous area, 3-unit bridge supported by 2 implants and 3 implant-supported splinted crowns. MATERIALS AND METHODS. The data included 127 implant-supported fixed restorations in 85 patients: 37 restorations of 3-unit bridge supported by 2 implants (2-IB), 37 restorations of 3 implant-supported splinted crowns (3-IC), and 53 single restorations (S) as controls. Peri-implantitis and mechanical complications that occurred for 14 years were analyzed by multivariable Cox regression model. Kaplan-Meier curves and the multivariable Cox regression model were used to analyze the success and survival of implants. RESULTS. Peri-implantitis occurred in 28.4% of 2-IB group, 37.8% of 3-IC group, and 28.3% of S control group with no significant difference. According to the implant position, middle implants (P2) of the 3-IC group had the highest risk of peri-implantitis. The 3-IC group showed a lower mechanical complication rate (7.2%) than the 2-IB (16.2%) and S control group (20.8%). The cumulative success rate was 52.8% in S (control) group, 62.2% in 2-IB group, and 60.4% in 3-IC group. The cumulative survival rate was 98.1% in S (control) group, 98.6% in 2-IB group, and 95.5% in 3-IC group. There was no significant difference in the success and survival rate according to the restoration type. CONCLUSION. The restoration type was not associated with the success and survival of implants. The risk of mechanical complications was reduced in 3 implant-supported splinted crowns. However, the middle implants of the 3 implant-supported splinted crowns had a higher risk of peri-implantitis.
청소년기 외상으로 인한 상악 전치부의 복잡 치관 파절은 기능적, 심미적 문제를 야기할 수 있다. 치수 노출을 동반한 치관 파절은 남아있는 치질의 양에 따라 다양한 수복 방법을 고려할 수 있다. 직접 레진 수복은 가장 일차적으로 고려되는 전통적이고 효과적인 방법이나 시간이 지남에 따라 변색되고 깨질 가능성이 높다. 전장관 수복은 성장기의 치아 이동으로 변연 부조화에 의한 재수복 가능성이 높으며 광범위한 치관 파절 시 주로 시행하는 포스트 수복은 치근 천공 및 치근 파절의 위험성을 증가시킨다. 그러나 Endocrown은 치수강 공간으로부터 유지력을 얻을 수 있는 일체형 구조로 생체역학적 측면에서 효과적인 재건이 가능하고 기능과 심미성의 회복에 유리한 이점을 제공한다. 따라서 Endocrown은 청소년기 외상으로 인한 복잡 치관 파절의 수복 방법으로 고려할 수 있다.

Background: The Discovery Elbow System (DES) utilizes a polyethylene bearing within the ulnar component. An exchange bearing requires preoperative freezing and implantation within 2 minutes of freezer removal to allow insertion. We report our outcomes and experience using this technique. Methods: This was an analysis of a two-surgeon consecutive series of DES bearing exchange. Inclusion criteria included patients in which exchange was attempted with a minimum 1-year follow-up. Clinical and radiographic review was performed 1, 2, 3, 5, 8 and 10 years postoperative. Outcome measures included range of movement, Oxford Elbow Score (OES), Mayo Elbow Performance Score (MEPS), complications and requirement for revision surgery. Results: Eleven DESs in 10 patients were included. Indications were bearing wear encountered during humeral component revision (n=5); bearing failure (n=4); and infection treated with debridement, antibiotics and implant retention (DAIR; n=2). Bearing exchange was conducted on the first attempt in 10 cases. One case required a second attempt. One patient developed infection postoperatively managed with two-stage revision. Mean follow-up of the bearing exchange DES was 3 years. No further surgery was required, with no infection recurrence in DAIR cases. Mean elbow flexion-extension and pronosupination arcs were 107°(±22°) and 140° (±26°). Mean OES was 36/48 (±12) and MEPS was 83/100 (±19). Conclusions: Our results support the use of DES bearing exchange in cases of bearing wear with well-fixed stems or acute infection. This series provides surgeons managing DES arthroplasty with management principles, successful and reproducible surgical techniques and expected clinical outcomes in performing DES polyethylene bearing exchange. 목적: 고정형 슬관절 단일 구획 치환술의 5년 추시 결과로 임상적 및 방사선적 결과, 합병증 발생 빈도를 알아보고자 하였다. 대상 및 방법: 2003년 5월부터 2011년 8월까지 고정형 슬관절 단일 구획 치환술을 시행한 25명, 26예를 대상으로 하였다. 평균 연령은 63.5세였으며 남자가 3명(3예), 여자가 22명(23예)이었으며 술 전 진단은 골관절염 23예, 골괴사 3예였다. 평균 추시기간은 평균 67개월(60-149개월)이었다. 임상적으로 술 전과 술 후의 미국슬관절학회 슬관절 점수 및 기능점수, 관절 운동 범위를 조사하였고, 방사선적으로 슬관절 기립 전후방 사진과 측면 사진, 투시기 촬영을 통하여 술 후의 정렬상태와 골용해 여부를 조사하였다. 결과: 임상적 결과로 슬관절 점수는 평균 42.0점에서 87.9점으로, 기능점수는 57.5점에서 85.0점으로 향상되었다(p<0.001). 술 전 관절 운동 범위는 평균 $132.9^{\circ}$에서 술 후 $132.5^{\circ}$로 큰 차이를 보이지 않았다. 방사선적 결과로 대퇴경골각은 술 전 평균 내반 $0.5^{\circ}$에서 술 후 외반 $2.2^{\circ}$로 측정되었다. 방사선 투과성 선(radiolucent line)이 관찰되는 경우가 2예 발견되었으며 그 중 1예는 삽입물이 안정적이었으나 다른 1예는 단일 구획 치환술 시 대퇴슬개관절의 관절염을 확인하였고, 추시상 슬관절 내측 관절선의 동통 및 슬관절의 미만성 통증이 있어 슬관절 전치환술을 권유하였다. 그 외 골용해, 감염, 술 후 강직, 탈구 등의 합병증은 없었다. 결론: 고정형 슬관절 단일 구획 치환술의 중기 추시 결과는 임상적 및 방사선적으로 만족스러운 결과를 보였다.
1급 지적장애는 일상 생활과 사회 생활의 적응이 현저하게 곤란하여 일생 동안 타인의 보호가 필요한 사람으로 보통 불량한 구강환경을 가지며 치아 우식, 치근단 염증 및 다수의 치아 결손을 보여 적절한 보철적 회복을 필요로 한다. 다수의 치아가 결손된 지적장애 환자의 경우 가철성 보철물의 안정, 유지와 관리가 어렵기 때문에 고정성 보철이 필요하며 임플란트 치료가 유일한 방법이다. 본 증례는 1급 지적장애인 45세 남성환자로 불량한 구강환경으로 인해 대부분의 치아가 상실되어 저작기능 회복을 위해 내원하였으며 임플란트 및 생체친화적이고 강한 강도의 단일 구조 지르코니아를 이용한 고정성 보철물로 최종 수복하였다. 이상의 치료과정 결과 심미적, 기능적으로 만족할 만한 결과를 얻을 수 있었기에 보고하는 바이다.
목적: 본 연구의 목적은 3본 나사 유지형 임플란트 보철물의 나사 고정 방식에 따른 치경부에 발생하는 변형율을 측정하고 비교하기 위함이다. 재료 및 방법: 2가지 종류의 임플란트(외부연결방식, 내부연결방식)를 각각 2개씩 4개 임플란트 고정체 경부에 스트레인 게이지를 부착한 후, 각 종류의 아크릴릭 레진 모형 2개를 제작하였다. CAD-CAM을 이용하여 hex 및 nonhex 지대주를 제작하였고,(EH, ENH, IH& INH 군) 나사-시멘트 유지보철 개념으로 보철물을 비귀금속으로 제작하였다. 각 지대주를 10 Ncm 토크로 고정한 후, 보철물을 합착하였다. 나사를 3가지의 다른 방법으로 30 Ncm까지 고정한 다음, 5분 후 변형율을 측정한 후 평균비교 및 군간 상호작용을 평가하였다 (α = .05). 결과: 외부연결방식 군은 내부연결방식 군에 비하여 유의하게 낮은 변형율을 나타냈으며, EH군이 ENH군보다 유의하게 낮은 변형율을 나타내었으나 (P < .05), 나사 고정 방법에 따른 차이는 없었다 (P > .05). IH군이 INH군에 비하여 유의하게 높은 변형율을 나타내었으며 IH군에서만 나사 고정 방법에 따라서도 차이가 있었다 (P < .05). 결론: 나사 유지형 임플란트 보철물에서는 외부연결방식인 경우에는 큰 영향이 없으나, 내부연결방식에서는 보다 큰 변형율이 발생하고 나사 고정 방법도 영향을 주는 것으로 나타났다.
다수 치아가 상실된 부분 무치악 환자의 전통적인 치료 방법으로 임플란트 고정성 보철물과 가철성 국소의치가 널리 이용되고 있다. 최근에는 소수의 임플란트를 보조적으로 이용하여 부가적으로 유지와 지지를 얻는 임플란트 융합 국소의치(Implant-assisted removable partial denture, IARPD)에 대한 관심이 증가하고 있다. 임플란트 융합 국소의치는 전통적인 가철성 국소의치에 비해 높은 유지력과 안정성을 가지며 심미적이다. 또한 소수의 임플란트 식립을 통해 환자에게 수술에 대한 부담을 줄여주며, 비용 절감 효과를 가진다. 본 증례의 환자는 상악에서 다수 치아를 상실한 부분 무치악 환자로 초기에 임플란트 고정성 보철물로 치료 계획하였으나 여러 요인에 의해 milled bar와 Locator attachment를 이용한 임플란트 융합 국소의치로 치료 계획을 변경하였다. 이를 통해 기능과 심미에서 만족할 만한 결과를 얻었기에 보고하는 바이다.
