A Case of Plummer-Vinson Syndrome (Plummer-Vinson 증후군 1례)

  • Plummer-Vinson syndrome is a clinical entity characterized by dysphagia, iron deficiency anemia, cheilosis, glossitis, and cervical esophageal web, especially in middle aged women. Recently, the authors experienced a case of Plummer-Vinson syndrome. A 53-year-old female was admitted due to intermittent solid food dysphagia for 18 months. She had a 2 years history of iron deficiency anemia. On admission glossitis, fissures at the angle of the mouth, spoon nails, and iron deficiency anemia were noted. Esophagogram and esophagoscopic examination revealed thin walled concentric web at upper esophagus. Esophageal web was succefully teared by endoscopic balloon dilatation with subseguant improvement of dysphagia. Skin manifestations as well as anemia were markedly improved after oral iron replacement therapy.

The Analysis of Normal Values on the Ocular Protrusion and Height of Palpebral Fissure on Women University Students in Korean (한국 여자 대학생들의 검열 폭과 안구돌출의 정상 값 분석)

  • The purpose of this study was undertaken to determine the analysis of normal values of height of palpebral fissure and Hertel exophthalmometry in Korean woman university students. These measurements were taken from 118 health subjects aged 18-21 years in Korea. In 18-21 years, the mean values of height of palpebral fissure were $6.967{\pm}1.599mm$, $7.172{\pm}1.502mm$, $8.015{\pm}1.44mm$ and $7.1{\pm}1.224mm$, respectively. However, In 18-21 years, the mean values of protrusion were $12.50{\pm}2.33mm$, $12.343{\pm}2.634mm$, $12.773{\pm}1.974mm$ and $13.036{\pm}2.23mm$, respectively. As a results, according to increase the aging, the palpebral fissure height and ocular protrusion was more increase. But in 21 ages the palpebral fissure was some decrease. Also in 19 ages the protrusion was some decrease. On the other hand, the right and left eye have not statistically significant.

  • Kabuki(Niikawa-Kuroki) syndrome was first reported by Niikawa et al(1981). The faces of the patients are similar to the make-up of traditional Japanese Kabuki actors: long palpebral fissures, an ectropium of the lateral third of the eyelids, and arching eyebrows with sparse lateral halves. Craniofacial findings include a depressed nasal tip, short nasal septum, large and prominent ears, and micrognathia. Other main features area mild to moderate mental deficiency, short stature, skeletal and dermatoglyphic abnormalities, including prominent finger tip pads. Oral anomalies are common in KS(over 60%) and include abnormal dentition, widely spaced teeth, cleft palate or lip, high vault of palate, hypodontia, conical incisors, screw driver-shaped incisors and ectopic upper 6-year molars. The increased occurrence of cleft lip and palate or the development of a high vault of palate has been described by a number of authors. This condition is believed to be common in Japan, but has been reported from other parts of the world. The objective of this presentation is to report a case of this syndrome in six-year-old girl, with characteristic findings.

The Strength and Fracture Behavior characteristics of Irradiated Zr-2.5Nb CANDU Pressure Tube Materials (Zr-2.5Nb 중수로 압력관의 조사후 강도 및 파괴거동 특성)

  • The tensile and fracture toughness tests have been conducted to investigate the degradations of mechanical properties induced mainly by neutron irradiations in Zr-2.5Nb CANDU pressure tube materials operated in Wolsung Unit-1. the tests were performed at room, 150, 200, 250, 300 $\^{C}$ for the irradiated and unirradiated specimens in hot cell. The specimens were directly machined from the tube retaining original curvature using specially designed electric discharge machine(EDM). From the tensile tests of the irradiated specimens, it was found that tensile strength was increased and total elongation was decreased compared to those of the unirradiated ones. The active voltages in the fracture toughness tests for the irradiated showed the discontinuous abrupt increases caused by crack jumping in lower temperature. In the crack resistance curves we found the stable crack growth in the unirradiated, whereas the unstable and three crack growth stages in the irradiated specimens due to the accumulated irradiation defects. The various fracture characteristic values in the irradiated are remarkably lower than those of the unirradiated. Through the fractography, we found in the irradiated that smaller dimple and shorter fissures than the unirradiated, and that the fractured surface had three regions that were flat, transition and slant/shear area. These can explain the difference in the crack growth characteristic values of the irradiated and the unirradiated ones.

Correction of Problematic Hypospadias with Dartos Fascia-Reinforced Flap and Slanted Incision of Fistula

  • Background Hypospadias is the most common congenital malformation of the male urinary tract, wherein the urethral opening is located proximal to the normal site. Tubularized incised-plate urethroplasty reconstruction, and its efficacy, remains controversial due to the high recurrence rate. This study aimed to evaluate the results of dartos fascia-reinforced flap surgery in hypospadias patients with previous operative complications. Methods Nine patients (mean age, 12 years) who had previous one- or two-stage repair with TIP urethroplasty and suffered from urethrocutaneous fistulas or fissures as complications, underwent dartos fascia-reinforced flap surgery in our clinic between January 2010 and December 2012. The mean postoperative follow-up period was 6 months. Results Among the 9 patients, 7 had complete correction. In all the patients, the maximum uroflow angle was > $45^{\circ}$. No patient complained of an unnatural urinating position. The patients and parents gave very high satisfaction scores (4.7 points) with regard to the appearance of the outer genitalia. Conclusion The dartos fascia-reinforced flap could be a useful and reliable option for complications of hypospadias repair or fissure with the prepuce preserved, given that histological aspects vary and that the possibility of recurrence in the glanular region is high.

Pathological studies on exudative epidermitis in experimentally infected pigs I. Macroscopical and histopathological observations (실험적 돼지 삼출성 표피염에 관한 병리학적 연구 I. 육안적 및 병리조직학적 관찰)

  • To elucidate morphologic lesion of porcine exudative epidermitis which is occurred sporadically in Korea, Staphylococcus hyicus subsp. hyicus isolated from the naturally affected pigs was inoculated to suckling pigs. The infected piglets were observed grossly and histopathologically. Although affected piglets were taking acute, subacute, or chronic course, some piglets suffered from chronic disease showed poor prognosis and marked growth depression. Affected peglets had erythematous skin on the face, ear, and abdomen and these localized lesions appear as brownish spots of exudative epidermitis and fromed crust in the early stage. But, after this stage, the skin were covered by viscous greasy exudate and formed blackish brown crust and appeared fissures and hypertrophy. Grossly, there has been hemorrhage with the removal of crust-like materials of epidermis and edematous subcutis. The superficial lymph nodes were edematous and swollen or congested and hemorrhagic. Some piglets had swollen ureters, cysts in the renal cortex, or polyarthritis. A few cases had mild edematous swelling of kidney, intestinal catarrh and congestion of brain. Microscopically, skin lesions had detachment of keralinized layer and parakeratosis of epidermis, hydropic degeneration of epidermal cell, and retrogressive degeneration of hair root sheath. Dermis had edema, and infiltration of neutrophils and mononuclear cells. As the disease was proceeded, there was marked perivasculitis with lots of mononuclear inflammatory cells. More chronic lesions formed granuloma-like bodies(nodules) due to more mononuclear, perivascular inflammatory cell infiltration and proliferation of fibroblast. Lots of plasma cells and eosinophils were also present in dermis. Epidermis was hyperplastic by proliferation of basal cells stratum germinativum and epidermal pegs often extended into the dermis. In secondary infection, lots of neutrophils could be seen in epidermis and derms. Kidney had neutrophilic infiltration, necrotic and cystic glomeruli, and dilation of renal tubules and ureters. Purulent arthritis was sometimes observed in joints. Three days old mice administrated Staphylococcus hyicus subsp hyicus subcutaneously before had focal congestion and hemorrhage, necrosis, and subcutaneous edema of the skin. This observation was also seen in the study of mice administrated exfoliatin toxin of Staphylococcus which evoked human staphylococcal scalded skin syndrome.

Oxygen Isotope Study of Mulgeum, Yangseong, Maeri and Kimhae Iron Ore Deposits in Gyeongnam Province, Korea

  • Mulgeum, Yangseong, Maeri and Kimhae iron ore deposits in Gyeongnam Province are hydrothermal skarn type magnetite ore deposits in propylitized andesitic rock near the contact with Cretaceous Masanite. Symmetrical zoned skarns are commonly developed around the magnetite veins. The skarn zones away from the vein are quartz-garnet skarn, epidote skarn and epidote-orthoclase skarn. Oxygen isotope analyses of coexisting minerals from andesitic rock, Masanite and major skarn zones, and of magnetite, hematite and quartz were conducted to provide the information on the formation temperature, the origin and the evolution of the hydrothermal solution forming the iron ore deposits. Becoming more distant from the ore vein, temperatures of skarn zones represent the decreasing tendency, but most ${\delta}^{18}O$ and ${\delta}^{18}O_{H2O}$ values of skarn minerals represent no variation trend, and also the values are relatively low. Judging from all the isotopic data from the ore deposits, the major source of hydrothermal solution altering the skarn zones and precipitating the ore bodies was magmatic water derived from the deep seated Masanite. This high temperature hydrothermal solution rising through the fissures of propylitized andesitic rock was mixed with some meteoric water, and occurred the extensive isotopic exchange with the propylitized andesitic rock, and formed the skarns. During these processes, the temperature and ${\delta}^{18}O_{H2O}$ value of hydrothermal solution were lowered gradually. At the main stage of iron ore precipitation, because all the alteration was already finished, the new rising hydrothermal solution formed only the magnetite ore without oxygen isotopic exchange with the wall rock.

Size Effect of Specimen and Aggregate on Fracture Characteristics of Cemented Sand (경화 모래의 파괴 특성에 대한 시료 및 입자의 크기 영향)

  • In the past it has been often observed that the shear stresses at failure are much smaller than the shear strength obtained from traditional laboratory tests and conventional analysis technique is inadequate in stiff soil, such as cemented sand. Many researchers have brought attention to the fact that the presence of flaws i.e. fissures, cracks, joints have a great effect on the strength and overall stress-strain behavior of such materials. They have thought that fracture mechanics may appropriately be adopted as a good tool for analysis of these materials. However, the use of fracture mechanics concept especially for cemented sands is faced with difficulties in obtaining relevant parameters, because fracture parameters and predictions are highly dependent on the material constituents and the size of specimens as well as the size of particles. This paper addresses the effects of sizes which include specimen and aggregate on fracture properties of cemented sand. The results of laboratory tests show that the sizes of specimens and particle have a great effect on the fracture properties such as nominal strength of cemented sand.

Structural Characteristics of Low Molecular Weight Laminarin Prepared by Ionizing Irradiation (이온화 방사선 조사에 의해 얻어진 저분자 laminarin의 분자구조 특성)

  • Recently, it has been reported that low molecular weight laminarin had the enhanced biological activities. In this study, molecular structure of low molecular weight laminarin prepared by ionizing irradiation was studied. Low molecular weight laminarin samples of 13.5, 8.5, 7, and 6 kDa were obtained from 15 kDa laminarin by irradiation. From gel permeation chromatography data, low molecular weight laminarin was shown to have low polydispersity. To define the changes of functional groups in laminarin with different molecular weights, Fourier-transform infrared analysis was carried out. There was found no significant changes of functional groups in low molecular weight laminarin, except the increase of carbonyl group. The granular fissures from scanning electron microscopy showed the breakage of glycosidic bond in low molecular weight laminarin. These results could be utilized for the investigation of the enhanced biological activities of low molecular weight polysaccharides including laminarin.

Evaluation of Cooling Process for Marine Shaft Forging Products (선박용 축류 단조품 냉각공정 평가)

  • This study was performed to solve the quality problems of forging propeller shaft components in the marine diesel engines during the final cooling process and provide reasonable guidelines to increase the production of forging products. Residual hydrogen existing on the inside of forging products begins to diffuse and accumulates at the pores, micro-fissures, and grain boundaries as the temperature of forging products begins to decrease and reaches a critical temperature range, and finally transforming into internal defects. These defects were easily found near the surface of products after milling the surface of forging products. In this work, four types of forging products (shaft flange, shaft journal, thrust shaft, and propeller shaft) were chosen to evaluate the temperature history of products during the cooling process, employing non-linear numerical analyses with the ANSYS program. The times elapsed to reach 250 ℃ after cooling were approximately 9 ~ 23 hours for each forging product. These times can be used as cooling process guidelines on the quality and productivity of products after heat treatment.
