Purpose: To explore the relationship between auxiliary lymph node metastasis and clinical features, and to identify the factors that affect metastasis occurrence. Methods: A total of 164 cases of primary breast cancer were selected to investigate features such as age, concomitant chronic disease and pathologic diagnosis. Immunohistochemistry was used to detect the expression of the estrogen receptor (ER) and CerbB-2. Logistic regression was employed to analyze the factors that affect the incidence of lymph node metastases. Results: The incidence of lymph node metastases was 46.3% among elderly patients with breast cancer. Based on logistic regression, chronic disease, scale of tumor, age, and ER expression affected the occurrence of lymph node metastases; the ORs were 3.05, 2.18, 0.34, and 3.83, respectively. Between different pathologic diagnoses and the risk factors, the OR scores were 12.7 and 8.02, respectively, for aggressive ductal carcinoma and aggressive lobular carcinoma auxiliary lymph node metastases. Conclusion: The incidence of lymph node metastases is affected by chronic disease, scale of tumor, age, ER expression and pathologic diagnosis.
Background: Korea shows rapid population aging and increase in healthcare service use and expenditure. Also, this would be accelerated because of the baby boomers who will be 65 years old and more in 2020. Chronic disease is another reason that increases the use of healthcare service and expenditure of the middle- and old-aged households. Catastrophic health expenditure (CHE) is the index which can indicate the households' burden of health spending. Despite the importance, there are few studies on CHE of middle- and old-aged households and especially no panel study yet. This is the reason that this study is carried out. Methods: This study used 3-year data from the Korea Welfare Panel Study conducted from 2009 to 2011. We defined CHE if a household's health expenditure is equal or greater than the threshold value if income remaining after subsistence needs has been met. We used 4 different threshold values which are 10%, 20%, 30%, and 40%. In order to look at the households which experienced CHE, we conducted panel logit analysis after correspondence analysis and conditional transition probability analysis. Results: This study showed three notable results. First, there has been a difference among age groups, which implies that the older people are, the more easily they can experience CHE. Second, the households with no private insurance are shown to have a higher CHE occurrence rate. Lastly, there has been a significant difference among the kinds of chronic diseases. The households which have cancer, cerebrovascular disease, and heart disease have a higher CHE occurrence rate. However, the households with diabetes have no significant effects to CHE occurrence. Also, hypertension has a negative effect to the occurrence. Conclusion: With the results, it can be implied that elderly people with chronic disease are more needed in medical coverage and healthcare. Also, private insurance can play its role in protecting households from CHE. Therefore, it needs to conduct studies on CHE especially about different age groups, private insurance, and chronic disease.
Purpose: The purpose of this study was to investigate the occurrence of first onset of constipation during the rehabilitation stage and risk factors for constipation in patients with cerebral vascular disease (CVD). Methods: Participants for this retrospective study were 214 CVD patients admitted to rehabilitation units. First onset of constipation and factors influencing constipation such as dependency level were recorded for 14 days during the rehabilitation stage. Survival analysis with Cox proportional hazard model and descriptive statistics were conducted. Results: Age, patients' severity, types of diet, and dependency level were different between constipation and non-constipation groups. In survival analysis, 99% of participants developed constipation within 14 days after admission to rehabilitation units. Median constipation occurrence time was 7.4 days. Patients who were immobile and had tube feedings were more likely to develop constipation 4.07 times (95% CI: 1.018~16.301, p=.047) and 2.09 times (95% CI: 1.001~4.377, p=.050) respectively compared to patients who were independent and had a regular diet, respectively. Conclusion: Most CVD patients experienced constipation within 2 weeks after entering the rehabilitation stage. Constipation was linked to types of diet and dependency level. These factors associated with constipation should be considered when caring for patients in rehabilitation.

Yellowing disease of palms caused by phytoplasma is spreading in the Arabian Gulf region. Surveys were conducted to determine the occurrence of the disease. Electron and fluorescence microscopy, and polymerase chain reaction (PCR) techniques were used to detect the phytoplasma associated with the yellowing disease of ornamental palm Washingtonia sp. grown in Kuwait. An accumulation of phytoplasmal DNA was observed by fluorescence microscopy in phloem tissues of diseased palms. Electron microscopy showed that phytoplasma cells were primarily confined to the phloemsieve elements of tissue samples collected from infected mature palms in the field. The pathogen was identified on the basis of molecular analysis using universal and specific nested primers in PCR amplifications. Prokaryotic 16S rDNA gene was detected in amplified PCR products. Nested PCR resulted in DNA amplification of 1.2 kbp fragment. This is the first report of a phytoplasmal rDNA gene identified from the putative causal pathogen of yellows in ornamental palms in the Arabian Gulf region.
Severe spotted lesions were observed on tulip plants grown in Asan, Yongin, and Icheon areas in Korea during a disease survey in 2000 and 2001. Diseased plants with severe symptoms were blighted and rotted at the late stage of disease development. A total of 15 isolates was obtained from the infected plant parts of tulip. All the isolates were identified as Botrytis tulipae based on their morphological and cultural characteristics. Three isolates of B. tulipae were tested for their pathogenicity to tulip by artificial inoculation. Spotted lesions similar to those observed in the fields were induced on tulip leaves. This is the first report of tulip fore disease caused by B. tulipae in Korea.
Dispersal of citrus bacterial canker caused by Xanthomonas axonopodis pv. citri on Unshiu orange was investigated in naturally infested nursery plot at Seogwipo in Jeju island, Korea. Based on phage detection, over 2% of the bacterial pathogen over-wintered in canker lesions and started to multiply in late May. However, symptoms were first observed 1 month after the phage detection. The disease dispersed non-directionally to nearby plants possibly because of indirect dissemination of the bacterium by rain splashes. The disease increased from late June to late August and decreased thereafter. Population of phage increased constantly, however, disease occurrence somewhat fluctuated due to environmental factors. Disease incidence and severity were correlated with rainfall with wind that occurred 14-32 days earlier from late May to late August.
This is a case report on the occurrence of inclusion body hepatitis (IBH) and infectious bursal disease (IBD) among the broilers in a local farm located in Wanju, Jeollabukdo. Mostly IBH could be caused by adenovirus if the bird's immune system was first weakened by exposure to immunosupressive agents such as infectious bursal disease virus (IBDV) and chicken anemia virus (CIAV). However IBH primary occurred before IBD in this case. And recent work has demonstrated that virulent adenovirus alone can produce the disease.
The genus Pseudoperonospora, an obligate biotrophic group of Oomycota, causes the most destructive foliar downy mildew disease on many economically important crops and wild plants. A previously unreported disease by Pseudoperonospora was found on oriental pickling melon (Cucumis melo var. conomon) in Korea, which is a minor crop cultivated in the temperate climate zone of East Asia, including China, Korea, and Japan. Based on molecular phylogenetic and morphological analyses, the causal agent was identified as Pseudoperonospora cubensis, and its pathogenicity has been proven. Importantly, two phylogenetic clades of P. cubensis, harboring probably two distinct species, were detected within the same plots, suggesting simultaneous coexistence of the two clades. This is the first report of P. cubensis causing downy mildew on oriental pickling melon in Korea, and the confirmation of presence of two phylogenetic clades of this pathogen in Korea. Given the high incidence of P. cubensis and high susceptibility of oriental pickling melon to this disease, phytosanitary measures, including rapid diagnosis and effective control management, are urgently required.

In May 2020, we surveyed disease occurrence on vegetables grown in Seosan area, Korea. During the disease survey, white rot symptoms were observed in Korean wild chive (Allium monanthum) plants growing in fields. The symptoms occurred mainly in the seed bulb-producing fields of the crop. The above ground parts of the diseased plants displayed premature yellowing and dying of older leaves and stunting of the plants. The bulbs and roots of the diseased plants turned black and rotted. The disease occurred in a range of 1-60% in four of the eight fields surveyed. Three isolates of Sclerotium sp. were obtained from the bulb lesions of diseased plants. All isolates were identified as Stromatinia cepivora based on the morphological characteristics and phylogenetic analysis. Pathogenicity of the isolates on Korean wild chive was confirmed by artificial inoculation test. The lesions induced by the inoculation test were similar to those observed in the investigated fields. This is the first report of S. cepivora causing white rot in Korean wild chive.

Mass mortality in commercially cultured Amur catfish (Silurus asotus), showing symptoms of dermal ulcerations, occurred on a private farm in Mar 2019 in Korea. β-hemolytic bacteria were isolated from the ulcers and kidneys of the fish and identified as Aeromonas veronii. The isolate was resistant to tetracycline and possessed cytotoxic heat-labile enterotoxin (aerolysin/hemolysin). We investigated the genetic determinants associated with tetracycline resistance, and the isolate has been confirmed to simultaneously possess tetA and tetE genes. This is the first report on the occurrence of tetracycline-resistant A. veronii infection related to mass mortality in commercially cultured Amur catfish in Korea.
