Purpose: This study measured return of spontaneous circulation (ROSC) in relation to dispatch distance in patients with out-of-hospital cardiac arrest. Methods: Of 2,347 out-of-hospital cardiac arrest patients transported by emergency medical technicians in J Province between January 1 and December 31, 2015, those under age 18, those with reserved resuscitation, and those with traumatic cardiac arrest, leaving 855 patients in the study sample. ROSC was compared between those with dispatch distance ${\leq}4km$ (short dispatch distance, 465 patients) and those with dispatch distance >4 km (long dispatch distance, 390 patients). Results: The mean was 2.17 km in the short dispatch group and 9.87 km in the long dispatch group (p=.000). Mean distance from was 6.49 km and 13.39 km in the two groups, respectively (p=.000). ROSC differed significantly between the short and long dispatch distance groups (7.1% for short dispatch distance, 3.6% for long dispatch distance, p=.025). The length of time from to cardiopulmonary resuscitation also differed significantly between the short and long dispatch distance groups (8.77 minutes and 14.63 minutes, respectively, p=.000). Conclusion: ROSC was lower in areas of long dispatch distance compared to those of short dispatch distance. We expect this was most likely due to differences in response time by age and dispatch distance to the scene of cardiac arrest. However, no significant differences were found between the groups in the factors affecting ROSC.

The estimated on-site accident rate in Forestry is relatively high. According to statistics of the accident, in the recent 5 years, from 2014 to 2018, forestry accidents have resulted in 98% of injuries and 87% of fatalities. Especially, there are significant geographical constraints to access to the scene in case of an accident. Even though the capacity of first aid capacity is notably emphasized its importance to minimize the scale of damages, the relevant employees have been educated only basic first aid, which is not considered circumstances or geographic limitations, by Occupation Safety and Health Acts. Therefore, the purpose of this study is to derive a direction for a forest emergency service system to increase forestry workers' survival and prevent secondary injury through securing 'Golden Time.' This study conducts analyzing relevant laws and regulations in domestic and international settings as well as looking at several concerned accident cases. The outcome of analysis presents an issue regarding the implementation of onsite first aid in forestry and existing risk factors depending on the working process. Finally, we suggest two ways to improve the forest emergency service that are 1) an appropriate curriculum and kit for forest first aid; and 2) a system for emergency transfer through sharing information between National Fire Agency (NFA) and emergency medical service center, and emergency and rescue mission using helicopter from NFA and Korea Forest Service.

본 연구는 구급대원의 스트레스대응과 현장충격스트레스가 신체적 증상에 어떤 영향을 주는지를 증명하고자 하였다. 소방공무원 970명을 대상으로 설문 조사를 하였고, 자료수집은 2007년 1월부터 12월까지 실시하였으며, 자료분석은 SPSS 14.0과 AMOS 7.0 통계패키지를 사용하였다. 스트레스 대응이 신체적 증상에 미치는 영향을 분석한 결과, 스트레스 대응 1(p<.001), 4(p<.01), 2(p<.05) 순서로 신체적 증상 1에 유의한 영향을 주는 것으로 나타났다. 스트레스 대응 1(${\beta}$=.302), 4(${\beta}$=121)이 높을수록, 스트레스 대응 2(${\beta}$=-.070)이 낮을수록 신체적 증상1이 높아지는 것으로 나타났으며, 스트레스 대응이 신체적 증상 1을 설명하는 설명력의 정도는 16.5%이었다. 스트레스 대응 1(p<.001), 3(p<.001), 4(p<.001) 순서로 신체적 증상 2에 유의한 영향을 주는 것으로 나타났다. 스트레스 대응 1(${\beta}$=.261), 3(${\beta}$=129), 4(${\beta}$=129)가 높을수록 신체적 증상2가 높아지는 것으로 나타났으며, 스트레스 대응이 신체적 증상2를 설명하는 설명력의 정도는 16.3%이었다. 스트레스 대응 1(p<.001), 4(p<.001), 2(p<.05) 순서로 신체적 증상3에 유의한 영향을 주는 것으로 나타났다. 스트레스 대응 1(${\beta}$=.238), 4(${\beta}$=156)이 높을수록, 스트레스 대응 2(${\beta}$=-.081)이 낮을수록 신체적 증상 3이 높아지는 것으로 나타났으며, 스트레스 대응이 신체적 증상 3을 설명하는 설명력의 정도는 13.1%이었다.
Purpose : The purpose of this study was to describe the current status of 119 EMT to violence experiences in Korea. Method : The number of 119 EMT in this study 2522. Self-administering questionnaire data were collected from April 27 to March 8 in 2009. Collected data were analyzed using SPSS 14.0 program. Real number, percentage, mean and standard deviation were calculated. Pearson correlation coefficient was analyzed. Result : 119 EMTs experienced 95.32%-verbal insults, 61.54%-violence aggression, 60.51%- potential violence, 38.70%-physical assault. Within 30 days from the most recent period of violence experienced verbal insults and violence aggression-51.07%, potential violence and physical assault-48.41%. The patients(50.59%) and the patients and patient's carers(38.26%) proved to be violent offender. The main reason of violent act was drug or alcohol addiction(56.15%). Recent exposure to violence was 34.82% by 7 point reward of stress level. 5-point scale showed the average of experiences of violence. Response average of emotional reactions was 2.66 points and that of physical reaction was 2.18 points. Social reaction was 2.06 points. Conclusion : This study was based on the violence experience of 119 EMT. It is necessary to prevent the violence from emergency scene. So we proposed the preventive measure against violence as well as critical incident stress debriefing(CISD). Policy for CISD and stress management will be implemented in the near future in Korea.
This article is a study on the activity of rescue workers for designing simulation training scenarios assuming chemical accidents. On the basis of the complexity of the indoor scene in the case of chemical accidents and terrorism, we designed a 12-step simulation training scenario for two teams to analyze the improvement in firefighters' capabilities. On the basis of activity measurement in the simulation scenario, step 2 of training had the most drops in the maximum heart rate, as follow: N1, from 163 bpm to 153 bpm; N2, from 186 bpm to 151 bpm; N3, from 168 bpm to 162 bpm; and N4, from 166 bpm to 152 bpm. In terms of intensity level in the allowable activity time, it was found that in step 2 both N1 and N2 reduced from Level 5 to Level 3, N3 remained at Level 4, N4 reduced from Level 4 to Level 3, and the maximum allowable activity time increased.

Previous studies have found that firefighters have a tenfold higher prevalence of Parkinson's disease (PD) compare to the general population. Firefighters are constantly exposed to various occupational hazards including toxic chemicals of fire residue and the toxic chemicals can effects development and progression of PD. Nevertheless, there were no studies about the association between exposure to chemical byproducts of combustion and the development of PD among firefighters. Thus the aim of this study is to look into existing researches regarding the effect of chemical byproducts of combustion on the development of PD. An extensive literature search was conducted to identify harmful chemical components of smoke and fire residue, using the PubMed database during November of 2016. We searched for relevant articles by combining several keywords that contained "Parkinson's disease" and each of the different toxic chemicals, yielding a total of 1401 articles. After applying the selection criteria, 12 articles were chosen. Chemical substances reported to have a harmful effect on PD, in at least one article, were carbon monoxide, toluene, manganese and lead. Carbon monoxide and metal substances including manganese and lead were found to be associated with an increased PD risk in more than two articles. There was a heightened risk of PD in firefighters due to exposure of chemical byproducts of combustion including carbon monoxide, toluene, manganese and lead. However, to the best of our knowledge, to support this result we need more systematic epidemiological studies about these risk factors of PD among firefighters. In addition, further studies for the effects of prolonged exposure to toxic fire residue on the development and progression of PD in firefighters are needed.

To perform the engagement level simulation between the underwater vehicle model and the surface model those are constituted with various systems/ sub-systems, we implemented four different federates as a federation according to the IEEE 1516 HLA (High Level Architecture) protocol that is the international standard in the distributed simulation. Those are CFCS (Command and Fire Control System) federate, motion federate, external entities (torpedos, countermeasure and surfaceship) federate, and visualization federate that interacts with OSG (Open Scene Graph)-based visualization rendering module. In this paper, we present the detailed method about the model constitution for discrete event simulation in the distributed environment. For the sake of this purpose, we introduce the DEVS (Discrete Event System Specification)-HLA-based modeling method of the CFCS federate that reflects not only the interations between models, but also commands from user and tactics manager that is separated from the model. The CFCS federate makes decisions in various missions such as the normal diving, the barrier misision, the target motion analysis, the torpedo launch, and the torpedo evasion. In the perspective of DEVS modeling, the CFCS federate is the coupled model that has the tactical data process model, command model and fire control model as an atomic model. The message passing and time synchronization with other three federates are settled by the $m\ddot{a}k$ RTI (Runtime Infrastructure) that supports IEEE 1516. In this paper, we provides the detailed modeling method of the complicated model that has hierarchical relationship such as the CFCS system in the submarine and that satisfies both of DEVS modeling method for the discrete event simulation and HLA modeling method for the distributed simulation.
This study is to analyze the characteristics of the yellow insulation ring type of the CSST used for tubing when it is artificially deteriorated and damaged by burning. The CSST for tubing consists of a tube, protective coating, nut, yellow insulation ring, packing, and socket. In addition, it is thought that a yellow insulation ring and rubber packing were used to connect the tube and socket in order to improve the airtightness and insulation performance. The result of the verification of the data acquired from the tests in the 95% confidence interval shows that the Anderson-Darling (AD) and P value were analyzed to be 0.945 and 0.015, respectively. This confirms that the test data of the CSST for tubing is reliable. The analysis of the arithmetic mean of the insulation resistance of a CSST showed that the CSST damaged by burning by a torch, and the one damaged by electrical burning, was $16.7k{\Omega}$ (the greatest relatively) and $208{\Omega}$ (the lowest), respectively, while it was $1.72k{\Omega}$ in the case of a normal product. Therefore, the analysis result of the insulation resistance of the CSST collected from the scene of a fire can be utilized to examine the cause of damage by burning. In addition, it was found that when the maximum current of 97 A was applied to the CSST for about 5 s using a Primary Current Injection Test System (PCITS) the protective film and insulation ring of the CSST has no difference from that of a normal product. However, a part of the metal tube was melted.

Objectives: Rapid Intervention Crews (RIC) are indispensable for rescuing firefighters who are lost or trapped. They are included in Disaster Scene Standard Operating Procedures. This study was conducted to examine the use of RIC at emergency scenes in South Korea. Methods: We conducted a nationwide survey of 4,913 firefighters in South Korea. The firefighters' experiences of use of RICs were assessed by asking, "How often are RICs available at fires?" Results: A full 40.8% of firefighters answered that they 'never' have RICs available at fires. Another 8.4% responded that they 'always' have RICs available, which is much lower than the rate among US firefighters (19.9%). RICs are available more often for firefighters in the capital area than for firefighters outside the capital area (13.4% vs. 5.1%, p<0.001). The larger the jurisdiction served, the more likely the firefighters were to have RICs available (p<0.001). Conclusions: These results imply that firefighters in South Korea are at risk. It is necessary to recruit personnel to improve the use of RICs and to explicitly include RICs in SOPs, training, and business plans.

The purpose of this study is to determine the rapid saving of life through the drones when natural disasters such as earthquake and fire occur. Drones are equipped with riders, temperature, hazardous gas sensors and wireless cameras are registered with the application server for monitoring the disaster site and real-time monitoring functions are performed to identify the situation on site before rescuing personnel are active. When monitoring finds a person to save, the application server provides real-time image information for effective life-saving.
