치과위생사의 직무스트레스와 식행동 및 식습관에 관한 연구 (A Study on Job Stress and Eating Behavior and Habits of Dental Hygienists)

  • 본 연구는 서울 및 서울 근교지역에서 치위생 업무를 수행하는 치과위생사 273명을 대상으로 직무스트레스 점수, 식행동 점수, 근무환경, 식습관, 스트레스 관련 요인을 조사하여 치과위생사의 건강증진을 위한 기초 자료로 쓰고자 하며, 그 결과를 요약하면 다음과 같다. 1. 조사대상자의 연령은 25세 이하 46.5%, 26~30세 38.1%로 나타났으며, 교육수준은 전문대 졸업이 71.4%로 가장 많았다. 근무형태는 치과위생사 7명 이상 치과의원이 33.0%, 치과병원 28.2%로 나타났고, 치과위생사 총 경력은 1~3년 38.1%, 3~5년 18.7%, 월 급여는 151~200만원이 46.9%로 가장 많게 나타났다. 2. 식습관과 관련된 요인에서 아침식사를 거르는 주된 원인으로 60.4%가 '시간이 없어서'로 응답하였고, 식습관 중 개선하기 원하는 것은 33.0%가 과식을 꼽았으며, 과식 혹은 폭식을 하는 경우는 51.6%가 스트레스 때문이라고 응답하였다. 3. 본인 스트레스 자각 정도는 15.8%가 스트레스를 '매우 많이', 43.2%가 '많이 느낀다'고 응답하였고, 스트레스 원인으로 39.6%가 업무량을, 동료관계가 28.2%로 나타났다. 스트레스 해결방법은 39.9%가친구들과 수다로 가장 많이 해결하고 있으며, 스트레스 시 선호하는 맛은 매운맛 54.2%로 가장 높았고, 스트레스 후 식사 섭취량의 증가는 12.1%가 평소보다 매우 많이, 49.8%가 평소보다 많이 먹는 것으로 조사되었다. 4. 직무스트레스가 높은 상위 그룹에서 본인 스트레스 자각 정도가 높게 나타났다(p<0.001). 5. 직무스트레스 점수가 높은 군에서 신경예민(p<0.01), 허리 통증(p<0.001), 눈의 피로(p<0.01), 두통(p<0.01), 소화불량(p<0.01)을 경험하고 있어, 직무스트레스가 건강에 영향을 끼치고 있음이 드러났다. 6. 직무스트레스와 식행동의 관계에서는 직무스트레스가 높을수록 식행동이 불량한 것(p<0.01)으로 조사 되었고, 사후분석(LSD)을 실시한 결과 Middle rank>Middle rank>High rank 순으로 직무스트레스가 식습관 정도에 대해 차이(p<0.05)가 있는 것을 알 수 있다. 7. 치과위생사 3명 이하 치과의원에서 직무스트레스 점수가 높았고(p<0.05), 식행동 점수도 낮게 나타났으며, 주 4일 야간진료를 하는 그룹에서 직무스트레스가 높고, 식행동 점수도 낮게 나타났다(p<0.05). 근무 중 휴식 시간이 없는 경우 직무스트레스 점수가 높게 나타났고(p<0.001), 식행동 점수도 낮게 드러났으며(p<0.01), 스트레스 자각을 매우 많이 느끼는 경우 직무스트레스 점수가 가장 높게 나타났으며(p<0.001), 식행동 점수도 가장 불량하게 조사되었다(p<0.05). 직무스트레스가 식행동 및 식습관뿐만 아니라 건강상태에도 불량하게 영향을 끼치는 것으로 확인 되었기에 각 사업장에서는 치과위생사의 건강증진을 위해 다양한 방안을 모색할 필요가 있다.

역사문화경관 이미지가 치유특질에 미치는 영향 - 경주시 양동마을을 중심으로 - (Effects of Historical and Cultural Landscape Images on Healing Characteristics - Around the Yangdong Village in Gyeongju -)

  • 집중력회복 이론(ART)에 의하면 인간이 치유특질을 가진 경관을 경험하면 의도적인 집중력으로 유발된 정신적 피로감이 치유될 수 있다. ART에서는 다양한 경관이 치유특질을 가질 수 있다고 인정하고, 경관의 네 가지 치유특질은 지각강도가 클수록 치유효과가 강해진다고 설명하였다. 지금까지 치유경관에 대한 연구는 주로 자연경관과 도시경관을 대상으로 연구되어 왔다. 본 연구의 연구목적은 다음과 같다. 첫째, RCS를 이용하여 양동마을 경관을 대상으로 치유특질을 평가하고, 치유특질이 대상지경관의 방문 선호도에 미치는 영향을 조사하고자 한다. 둘째, 양동마을경관을 대상으로 경관이미지를 평가하여, 경관이미지가 경관의 치유특질에 미치는 영향을 조사하는 것이다. 본 연구의 대상지는 경주시 양동마을이며, 127명의 대학생이 설문조사에 참여하였다. 그리고 경관 이미지를 분석하기 위한 SD 16항목, 치유특질을 평가하기 위한 Laumann (2001)의 RCS 22항목을 측정도구로 사용하였다. 연구결과는 다음과 같이 요약한다. 첫째, SD법을 통해 양동마을의 경관 이미지를 분석하였으며, 양동마을 경관 이미지에 대한 요인분석 결과는 장소성 요인과 특이성 요인으로 도출되었다. 둘째, RCS의 신뢰성과 타당성을 입증하고 있으며, 요인분석 결과 세 가지 요인이 추출되었다. 셋째, 경관 이미지가 RCS의 세 가지 치유특질에 긍정적인 영향이 있는 것으로 나타났다. 넷째, 도출된 RCS의 세 가지 치유특질이 방문선호도에 긍정적인 영향을 미치는 것으로 나타났으며, 상대적인 방문선호도에 미치는 영향력의 순서는 매력·적합, 탈출감, 공간감으로 분석되었다. 앞으로의 연구에서는 RCS의 신뢰성을 재검증하기 위한 연구가 필요하다. 또한, 다양한 경관유형을 대상으로 경관 이미지를 분석하여 치유특질에 미치는 영향을 구체적으로 파악하는 것도 필요하다고 판단된다.

국제항공운송 승무원의 일상적 노무제공지 (The Place Where the Cabin or Flight Crew of International Air Carrier Habitually Carries Out his/her Work - CJEU, 2017. 9. 14., C-168/16, C-169/16 - Sandra Nogueira and Others v. Crewlink Ltd Miguel José Moreno Osacar v. Ryanair)

  • 국제항공운송에 종사하는 승무원은 업무의 성격상 여러 나라에서 근로를 제공하게 된다. 국제사법에 의하면 일상적 노무제공지는 국제근로계약의 준거법 결정(제28조 제2항), 국제재판관할의 결정(제28조 제3항, 제4항)에서 중요한 역할을 수행한다. 일상적 노무제공지는 국제재판관할과 준거법의 결정을 위하여 EU에서 제창된 개념으로, 국제항공법에서 항공안전을 확보할 목적으로 승무원의 피로관리를 위해 도입된 개념인 모기지(母基地)와 입법목적이 서로 달라, 일상적 노무제공지와 모기지는 같은 개념이라고 볼 수 없다. 승무원의 일상적 노무제공지를 결정하기 위해서는 (i) 승무원이 근무를 시작하고 종료하는 곳, (ii) 승무원이 업무를 수행하는 항공기가 주로 주기(駐機)된 곳, (iii) 사용자가 전달한 지시사항을 알게 되고 근무활동을 조직하는 곳, (iv) 근로계약상 승무원이 거주할 의무가 있는 곳, (v) 사용자가 제공하고 승무원이 사용할 수 있는 사무실이 있는 곳, (vi) 승무원이 업무에 부적합하거나 징계사유가 발생할 때 참석할 의무가 있는 곳 등을 종합적으로 고려하여야 한다. 그런데 위에서 열거한 사항은 모두 모기지 소재지와 동일하므로, 승무원의 일상적 노무제공지를 결정할 때 모기지를 가장 중요한 요소로 고려하는 것이 타당하다. 이와 달리 기적국을 일상적 노무제공지로 보아서는 안 된다. 대상사안에서 CJEU는 국제항공운송에 종사하는 객실승무원의 일상적 노무제공지에 관하여 최초로 판단기준을 제시하였는바, 이는 우리나라 국제사법의 모범이 되었던 브뤼셀 규정의 해석이므로, 국제항공운송 승무원에 관하여 국제사법상 일상적 노무제공지의 개념을 이해하는데 많은 도움이 될 것으로 보인다.

유방암 수술환자의 디스트레스 및 연관인자 : 단면연구 (Distress and Associated Factors in Patients with Breast Cancer Surgery : A Cross-Sectional Study)

  • 연구목적 유방암 환자가 치료 초기에 경험하는 디스트레스는 유방암 생존기까지 지속되는 만성 디스트레스의 위험요소로 알려져 있다. 본 연구는 유방암 첫 치료로써 수술을 받은 환자를 대상으로 디스트레스 온도계(distress thermometer, DT)를 이용하여 디스트레스의 정도와 이와 연관된 인자를 조사하는 것이다. 방 법 2013년 4월 1일부터 2017년 3월 31일 사이에 경북대학교병원에서 유방암 첫 치료로써 수술을 받고 일주일 이내에 있는 환자를 대상으로 하였다. DT로 디스트레스의 정도를 측정하였고 한국판 세계보건기구 삶의 질척도 단축형(Korean version of the World Health Oganization Quality of Life Scale Abbreviated Version, WHOQOL-BREF)으로 삶의 질을 측정하였다. 그 외 기본 인구학적 자료, 암 및 암 치료 관련 자료를 수집하였다. DT 점수 4점 이상인 대상자를 디스트레스 군으로 분류하였다. 디스트레군과 연관된 인자를 알기 위하여 디스트레스군과 비-디스트레스 군 사이의 변수 차이를 단변수 분석(univariable analysis) 하였다. 로지스틱 회귀분석을 시행하여 디스트레스 군을 예측할 수 있는 인자를 조사하였다. 결 과 대상자는 총 307명이었다. 이 중에서 DT에 응답하지 않은 39명, WHOQOL-BREF에서 6개 항목 이상 답을 하지 않아서 채점을 할 수 없었던 4명을 제외한 264명이 분석 대상자였다. 65.5%(n=173)가 디스트레스 군으로 분류되었다. 단변수 분석(univariable analysis)에서 디스트레스 군이 비-디스트레스 군과 비교하여 더 젊었으며(p=0.045), 배우자와 동거하지 않는 경우가 더 많았다(p=0.032). 삶의 질 영역에서 디스트레스 군이 전반적 삶의 질(p=0.009), 전반적 건강상태(p=0.005), 신체건강영역 p<0.001), 심리건강영역(p=0.002)에서 더 나쁜 삶의 질을 보였다. 로지스틱 회귀분석에서 60세 이상의 환자에 비하여 40-49세의 환자가 디스트레스에 이환 될 확률이 약 3배 높았고[OR=2.992, 95% confidence intervals (CI) 1.241-7.215], WHOQOL-BREF영역 중 신체건강영역이 디스트레스를 예측할 수 있는 인자로 나타났다(OR=0.777, 95% CI 0.692-0.873). 결 론 유방암 수술 치료 초기단계에서부터 상당한 수의 환자가 유의한 수준의 디스트레스를 경험하고 있다. 특히 생산성이 왕성한 삶의 주기에 있는 40대의 환자가 60대 이상의 환자에 비하여 수술 후 일주일 이내에 겪는 심리적 고통이 심할 가능성이 높다. 이러한 환자들에게서 수술 초기부터 신체적 삶의 질과 연관된 영역(예 : 통증, 불면, 피로 등)에 대해 평가하고 개입하는 것이 디스트레스 수준을 경감시킬 가능성이 있다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
