Survey on the Morbidity of Obesity and Analysis of Related Factors among School Children in Shanghai, China

  • Objective : To get a better understanding of obesity and etiological factors of obesity among school children. Methods : We have conducted a questionnaire survey of 4542 (2395male and 2147female) age from 7-15 in Shanghai School District to study their 24h nutrition intakes ; we have asked all our survey participating students to take a physical exam and body mass index measurement to determine the morbidity of obesity. Common statistic method was used in analyzing the data. Results : The result shows an over all morbidity (boys and girls) of $12.3\%$ among the students we have studied. Our research shows that boys have a higher rate of morbidity of obesity $17.1\%$ vs. $7.0\%$ for girls. The morbidity for boys starts under age 10, reaches the peak at age 10 ($24.3\%$), then declines after age 11 ; as for girls the morbidity starts at under age 9, reaches the peak at 9, and then declines after 10. Our study also indicates that the daily nutrition intake of protein, fat and carbohydrate by the students have satisfied Chinese Dietary Reference Intakes (DRIs), while the daily intakes of calcium, iron, zinc, and vitamin C are below the $80\%$ of the DRIs suggested value. For students who are obese, our study shows that their average daily intakes of calories and protein have reached $90\%$ of the suggested value by the RNI while the intakes of calcium, iron, and zinc are far below standards. Furthermore, our research shows that the obese students usually intake more calories, proteins and vitamin $B_1$, but less for iron and calcium compared to the normal students. Some other factors such as the education and occupation of the parents, especially the mother, the family history of obesity and the eating habits are important factors that should be closely watched. Conclusion : The morbidity of obesity among boys is higher than girls. Obesity is closely related to the family history, the diet habits and the life styles.

Sugar Intake and Dietary Behavior of Children with and without School Lunch Program (급식과 비급식국민학교 아동의 식행동 및 설탕섭취실태 비교)

  • This study was undertaken to investigate the dietary behavior and sugar intake of korean school children. The subject were 131 school lunch program children and 149 non school lunch program children lived in Soung Nam city. Sugar intake, food intake, dietary behavior and anthropometric measurement were studied through personal interview using 24-hour recall dietary data and records. Total daily energy and nutrient intake of children were higher than RDAs except for intake of Calcium and vitamin A. The nutrients intake of school lunch program children were significantly higher than that of non school lunch program children (p<0.05). The range of daily sugar intake was 10 g to 220 g and the total daily sugar intake was $63.0{\pm}37.4\;g$. Sugar intake of school lunch program children ($51.4{\pm}25.7\;g$) was significantly lower than non school lunch program children ($73.1{\pm}42.9\;g$) (p<0.0001). The more sugar intake increase more intake of Energy, Fat and Carbohydrate. The result of stepwise multiple regression analysis using the sugar intake of children as dependent variable, energy, vitamin $B_2$, protein, niacin, calcium school lunch program, education of fathers are significant explanatory variables, explaining 44.8% of variation. Result of this study suggest that school lunch program is desirable for good dietary behavior and decreation of sugar intake of school children.

A Study on the Correlation between of Bone Mineral Density and Cold Hypersensitivity of Hands (골밀도와 수냉증과의 상관성 연구)

  • Objectives The purpose of this study was to examine the relationship between cold hypersensitivity and bone mineral density. Methods 28 outpatients who visited OO University Oriental Hospital from July 11th, 2013 to August 28th, 2013 were analyzed. Patients were subjected to thermometer, and those with thermal difference greater than 0.3 ℃ between upper arm and palm and also with more than VAS 4 of cold hypersensitivity were diagnosed with cold sensitivity. Lumba spine (L2-4) and heel bone measurement were obtained on dual energy X-ray absorptiometry (DEXA) and Quantitative Computed Tomography (QCT). Results 1. There was statistically significant correlation between T-score of DEXA and body weight (0.522), BMI (0.643), OSTA (Osteoporosis elf-assesment Tool for Asians, 0.579), BMI, T-score and Z-score of QCT. OSTA and T-score of QCT (0.521) also had statistically significance. 2. There was no statistically significant correlation between body fat, abdominal obesity rate, degree of cold hypersensitivity, upper arm and palm's temperature difference, palm temperature, height and Z-score, T-score of QCT or T-sore of DEXA. Conclusions Bone mineral density was lower in women who had cold hypersensitivity of hand and feet. But there was no statistically significant correlation between bone mineral density and degree of cold hypersensitivity, upper arm and palm’s temperature difference.

Evaluation of Waist Circumference Cut-off Values as a Marker for Fatty Liver among Japanese Workers

  • Objectives: Metabolic syndrome has received attention as a risk factor for cardiovascular disease, with particular importance attached to visceral fat accumulation, which is associated with lifestyle-related diseases and is strongly correlated with waist circumference. In this study, our aim is to propose waist circumference cut-off values that can be used as a marker for fatty liver based on a sample of workers receiving health checkups in Japan. Methods: This study was conducted in a total of 21,866 workers who underwent periodic health checkups between January 2007 and December 2007. The mean age of the subjects was 47.4 years for men (standard deviation [SD]: 8.0) and 44.7 years for women (SD: 6.9). Evaluation included abdominal ultrasound and measurement of waist circumference, body mass index, fasting blood glucose, triglycerides, high-density lipoprotein cholesterol, and blood pressure. Results: Based on receiver operating characteristic curve analysis, the optimal waist circumference cut-off values were shown as 85.0 cm (sensitivity 0.72, specificity 0.69) for men and 80.0 cm (sensitivity 0.75, specificity 0.78) for women. Conclusion: Abdominal ultrasound is the most efficient means of diagnosing fatty liver, but this examination seldom occurs because the test is not routinely performed at workers' health checkups. In people found to have a high risk of fatty liver, recommendations can be made for abdominal ultrasound based on the waist circumference cut-off values obtained in this study. That is, waist circumference can be used in high risk individuals as an effective marker for early detection of fatty liver.

A Qualitative Investigation of Dongchunghacho Jelly with Assorted Increments of Paecilomyces japonica Powder (동충하초 분말을 첨가하여 제조한 젤리의 품질특성)

  • A qualitative investigation Dongchunghacho, Jelly combined with sericultural products prepared from assorted increments(0.5, 1, 1.5 and 2%) of P. japonica powder was performed. The qualitative parameters used to assess P. japonica powder increments included, hysicochemical, textural and sensory properties. The moisture and crude protein content of the Dongchunghacho Jelly were significantly greater than the control(p<0.05). Conversely, Dongchunghacho Jelly showed a propensity towards lower crude fat content than the control group. A positive trend was observed for both mineral content and dark color with increased P. japonica powder. In contrast, texture, hardness, gumminess and chewiness of the Dongchunghacho Jelly decreased with increasing levels of P. japonica powder. Color, taste, texture and overall preference of Dongchunghacho Jelly were optimal in the 1 % P. japonica powder mixture.

Quality Characteristics of White Bread Made with Makgeolli Sourdough (막걸리 사워도우로 제조한 식빵의 품질특성)

  • Purpose: This study developed sourdough bread according to consumers' preferencesby shortening the sourdough fermentation process time through addition of makgeolli to sourdough. Methods: Fermentation time was measured by adding makgeolli and water to wheat flour. Results: Fermentation time was shorter than that of starter made with only water and flour, with 5 hours of primary fermentation and 3 hours of secondary fermentation. The optimum mixing ratio was 400 g of flour, 192 mL of water, and 48 mL of makgeolli. An increase in makgeolli sourdough content decreased crude protein and moisture contents of white bread. Crude fat and ash contents increased, and volume of bread was decreased as sourdough increased. The height of bread prepared with makgeolli sourdough was lower than that of bread without makgeolli sourdough. According to the results of the texture measurement, hardness, gumminess, and chewiness of sourdough group were lower as the addition of sourdough increased. Addition of makgeolli sourdough to white bread resulted in decreased lightness, but increased yellowness of white bread. Preparation of white bread with addition of sourdough improved taste and flavor compared to bread prepared only with wheat flour. Conclusion: The highest sensory preference was observed for white bread prepared whit 5 g of sourdough.

Effect of Red Popper on Plasma Gastrin Concentration in Normal Human Subjects (고추가 정상인의 혈장 gastrin 농도에 미치는 영향)

  • This study was conducted to investigate the effect of ingestion of rice meal with red pepper(Capsicum annuum) as a seasoning upon the plasma gastrin concentration of normal human subjects in Korea. Thirteen normal human subjects including male and female(mean age: 21 years, range: $20{\sim}24\;years$) were studied. After an overnight(about 15 hrs) fast each subject ingested a test meal and a control meal on different days. The test meal consisted of 250 g toiled rice, 250 ml radish soup containing red pepper(dried powder, 3 g), 50 g vegetables and 200 ml barley tea, corresponding to 7.0 g protein, 9.0 g fat and 82 g carbohydrate and the control meal consisted of the same amount as the test meal except that the radish soup was supplied without red pepper. The venous blood samples were drawn before and after the ingestion of meals at the following times: -30, 0, 15, 30, 45, 60, 90, and 120 min for the measurement of plasma gastrin concentration by means of radioimmunoassay. 1) Plasma gastrin concentration in response to the ingestion of control meal without red pepper increased significantly compared with the concentration in fasting state. 2) The increase of plasma gastrin concentration after the ingestion of test meal(containing red pepper) was significantly higher than that after the control meal. It is inferred from the above mentioned results that the ingestion of red pepper as a seasoning has a stimulatory influence on gastrin release in normal human subjects.

A Study on Obesity-promoting Factors for the Elementary School Chirdren

  • The purpose of this study is to examine the relationships among body composition, dietary intake, exercise, and life style in children(M=80, f=102) of the 5th and 6th grades of elementary school. Anthropometry and multifrequency bioelectrical impedance analysis were conducted to estimate body composition. Dietary intake, exercise, and life style were determined by using questionnaires. When obesity was classified greater than 120% of the ideal body weight, the prevalence rates of obesity were 31.2% for boy and 20.6% for girl. There were significant differences in body composition between nonobese(NO) and obese(OB) groups. Mean fat mass(FM) and lean body mass(LBM) were 8.6kg and 27.7kg for NO group and 16.7kg and 32.3kg for OB group, respectively. Also a significant difference was found in hydration rate(TBW/body weight)between groups( <0.01). No significant difference was found I total calorie intake and nutrient intakes between groups. No difference was found in the frequency and duration of outdoor exercise and indoor activities. Mean sleeping hours was 8-9hours for 62% of nonobese children and for 59% of obese children. However, calorie intake per body weight was significantly lower inobese children than in nonobese. The present study showed that significant differences existed in their body size and composition between NO and OB groups , while no differences existed in daily calorie intake, excercise , and life style. This may indicate that important obesity-promoting factors of early onset obesity may rely on other factors such as hereditary or environmental factors besides factors considered. The Effect of an Energy Restriction Program on the Weight Loss and the Change of Biochemical nutritional Status in Obese Women

  • This study was conducted to investigate the effect of an energy restriction program on the weight loss and changes of the biochemical nutritional status for 35 obese women. The energy restriction program took place over a 3-week period that was devided into two parts. The first part consisted of 750-800kcal diet and the second part of 800-1000kcal. Subjects were provided a low energy formula and a menu for the recommended diet. Anthropometric and biochemical measurement before and after the energy restriction program were estimated. Mean weight loss was 3.0kg, accordingly the obestiy rate was lowered from 40.2 to 34.4, BMI from 29.2 to 28.9 and fat weight from 23.3kg to 21.0kg( <0.01. <0.05). Waist circumference loss was most prominent(4.4%) compared to triceps(21% loss)and hip circumference(2.2%loss). Mean RBC count, hemoglobin and hematocrit were significantly lowered( <0.01) but they were in the normal range. Systolic blood pressure was significantly decreased from 124.1mmHg to 113.1mmHg . Mean SGOT and SGPT were lowered from 29.3u/L to 20.0u/L and from 28.7u/L to 16.6u/L, respectively. It seems that the 3 weeks of energy restriction program used in this study was effective in improving anthropometric measurements without producing deficiency of iron or other susceptible nutrients.(Korean J community Nutrition 2(5) : 695-700,1997)

Total energy intake according to the level of skeletal muscle mass in Korean adults aged 30 years and older: an analysis of the Korean National Health and Nutrition Examination Surveys (KNHANES) 2008-2011

  • BACKGROUND/OBJECTIVES: Since gain or loss of skeletal muscle mass is a gradual event and occurs due to a combination of lifestyle factors, assessment of dietary factors related to skeletal muscle is complicated. The aim of this study was to investigate the changes in total energy intake according to the level of skeletal muscle mass. SUBJECTS/METHODS: A total of 8,165 subjects ${\geq}30years$ of age from the Korean National Health and Nutrition Examination Surveys (KNHANES) 2008-2011 were included in the analysis, and multivariate-adjusted regression analyses were performed to analyze the association of the quartiles of sarcopenia index (SI) with energy intake of the study population after adjusting for age and metabolic parameters. RESULTS: The increase in SI quartile was in proportion to the gradual decrease in systemic lipids and the anthropometric measurement of fat accumulation (P < 0.001). Subjects in higher SI quartiles tended to consume more total energy and energy-producing nutrients than those in lower quartiles (P < 0.001). After age, body weight, alcohol consumption, and metabolic parameters were adjusted in the analysis, total energy intake gradually increased according to the increase in SI quartile, and the association between total energy intake and SI was more pronounced in men. However, the risk (odd ratio) of having a low SI was not affected by any single macronutrient intake. CONCLUSION: In this study, total energy intake was positively associated with SI and relative skeletal mass in both men and women. However, no significant association or a weak association was observed between any single macronutrient intake and skeletal muscle mass. The data indicated that acquiring more energy intake within the normal range of energy consumption may help to maintain skeletal muscle mass.
