다문화가정 결혼이주여성 대상 식사조사를 위한 반정량 식품섭취빈도 조사지의 개발 및 평가 (Development and evaluation of semi-quantitative food frequency questionnaire for marriage migrant women in multicultural families)

  • 본 연구는 결혼으로 한국에 이주한 다문화여성의 영양상태를 파악할 수 있는 SQ-FFQ를 개발하고 이 조사지의 신뢰도와 타당도를 검증하기 위하여 실시하였다. 본 연구에서 개발된 SQ-FFQ는 결혼이주여성 대상 선행 연구, 국민건강영양조사의 SQ-FFQ 및 국민건강영양조사 결과를 고려하여 다소비, 다빈도 식품목록과 영양소별 섭취량에 대한 기여도가 높은 급원식품들을 고려하여 41개의 식품항목으로 선정하였다. 1회 섭취 분량은 0.5인분, 1인분, 1.5인분의 3개의 구간으로 나누었고 섭취빈도는 지난 1년 동안 평균적으로 얼마나 자주 먹었는지를 10개의 구간으로 분류한 SQ-FFQ를 개발하였다. 개발한 조사지의 재현성 및 타당도를 평가하기 위하여 서울과 대전 일부 지역의 건강가정·다문화가족지원센터를 방문한 35명의 결혼이주여성을 대상으로 SQ-FFQ (2회) 및 3일 식사 기록지를 조사한 후 1, 2차 반정량 식품섭취빈도 간의 영양소 섭취량 차이 및 SQ-FFQ와 3일 식사기록 간의 영양소 섭취량의 차이를 분석하였다. 1-2주간격으로 조사한 식품섭취빈도조사지의 재현성을 검토한 결과 에너지와 다량영양소 (탄수화물, 지방, 단백질)의 섭취량 간에는 유의적인 차이가 없었고, 비타민 A, 리보플라빈, 칼슘, 칼륨의 경우 1차 조사의 평균 섭취량보다 2차 조사의 평균 섭취량이 유의적으로 약간 높았다. 1차 및 2차 조사에서 도출된 동일 영양소 섭취량 간의 상관관계를 분석한 결과, 모든 동일 영양소 섭취량 간의 상관관계는 전반적으로 높은 상관관계를 보였다 (r = 0.453-0.885). 식품섭취빈도 조사와 3일 식사기록의 결과 비교를 통한 타당도 검증 결과, 식사 기록법보다 식품섭취빈도 조사법에서 에너지, 탄수화물, 탄수화물, 비타민 C, 티아민, 엽산, 비타민B12, 칼슘, 나트륨의 섭취량이 유의적으로 높아 식품섭취빈도법이 식사기록법보다 다소 과대 추정되는 것을 알 수 있었다. 1차 식품섭취빈도조사와 식사기록법의 상관관계는 대부분의 영양소에서 유의적으로 나타났는데, 식물성 지방이 0.602로 가장 높았고, 총 섭취 에너지, 탄수화물, 지방, 티아민의 상관계수도 0.512-0.599로 다른 영양소들에 비해 상대적으로 높았다. 반면 비타민 A와 C, 칼슘, 철, 나트륨 등의 영양소에 대해서는 유의적인 상관관계를 보이지 않았다. 본 연구에서 결혼이주 다문화여성의 식생활 및 영양소 섭취실태를 파악하고자 개발된 자기기입식 SQ-FFQ는 신뢰성이 검증된 조사도구이며 에너지와 주요 영양소의 섭취 상태를 추정할 수 있는 도구로 사용하기에 적합한 것으로 사료된다. 그러나, 본 연구 결과는 최종 35명을 대상으로 분석하여 도출된 것으로 향후 대상자 수를 확대하여 적용해 볼 필요가 있다. 또한, 일부 영양소의 섭취 결과에 대한 타당성을 확보하기 위해서 조사식품 항목에 대한 조정을 고려해 보아야 할 것이며, 특히, 미량영양소의 경우 계절의 영향을 많이 받기 때문에 정성적인 조사를 병행하는 등의 보완이 필요하다. 또한 식품 및 영양소 섭취량 산출을 위한 분석 데이터베이스 구성을 위한 근거자료들이 더 확보되어야 할 것이다.

청주지역 중학생의 패스트푸드 섭취량에 대한 영양소 밀도 평가 (Nutrient Density of Fast-Food Consumed by the Middle School Students in Cheongju City)

  • The purpose of this study was to investigate fast-food consumption and it's nutrient density, and the correlation between subject's characteristics and consumption frequency of fast-foods among middle school students. The questionnaires were distributed to 150 male and 190 female students in Cheongju city. The data were analyzed using SAS and statistics used were percent of frequency, $X^2$-test, t-test, and pearson's correlation coefficient. The main reasons for using fast-food restaurants were 'good taste' and 'convenience'. The major concerns when the subjects choose fast-foods were 'taste ($65.9\%$), price ($20.3\%$) and 'nutrition'($6.0\%$). The consumption pattern of fast-foods go as follows: twice a month ($42.3\%$), once a week ($31.5\%$), and less than once a week. The number of fast food items that the subjects ate for one meal was two ($46.0\%$) or three ($33.2\%$). The most frequently chosen combination of foods for number of two choices was beefburger and cola. The mean average energy intake from fast foods for one meal was 620.7 kcal for male, 504.5 kcal for female. The energy ratio of carbohydrate : protein fat from fast foods was 49 : 14 : 43, which means fat intake is much higher than recommended level ($20\%$). Fiber was appeared to be the lowest on the nutrient density which was $17.7\%$ of the recommended level for Koreans, vitamin C was next ($22.8\%$ for male, $20.1\%$ for female). In mineral, iron was the lowest ($71.8\%$ for male, $67.1\%$ for female), and protein was over $100\%$ for both males and females. Frequency of fast food intake was positively correlated with eating frequency of the salty, the sweets, him, caffeine containing foods, instant noodles, and cookies. In conclusion, frequent consumption of fast foods can lead unbalanced nutrient intakes for middle school students, and those who consumed fast foods frequently showed undesirable food habits in their daily meal. Therefore, nutrition education for middle school students should be needed to encourage them to choose more nutritious food and have healthier dietary pattern.

영양소 섭취의 주요급원식품과 주요변이식품들을 이용한 간소화된 반정량 빈도 조사 도구의 개발 및 평가 (The Development and Evaluation of a Simple Semi-Quantitative Food Frequency Questionnaire Using the Contribution of Specific Foods to Absolute Intake and Between-Person Variation of Nutrient Consumption)

  • This study was conducted to develop a simple flood frequency questionnaire (FFQ) based on the results of contributions of specific floods to absolute intake and between-person variance in nutrients using semi-quantitative FFQ with 93 flood items. The subjects were 554 healthy adults for development of a simple FFQ, and 37 students for a validation test of a developed simple FFQ. The contribution of specific floods to 80% absolute nutrient intake was measured by assessing their percentage in total consumption of a nutrient. To assess the contributions of floods to the between-person variance in the intake of each specific nutrient, stepwise multiple regressions were performed. The number of floods necessary to account for the respective 80% of absolute intake was 11-36, depending on the nutrient, while flower floods (5-16 floods) were required for the corresponding percentage of between-person variation for all nutrients. Important floods for between-person variance include Tangsuyuk (pork) and snacks for energy and fat, fish for protein and polyunsaturated fatty acids (PUFA) and snacks for carbohydrates. Spearman correlation coefficients between 93-itemed FFQ and 63-itemed FFQ ranged from 0.91 for vitamin A to 0.99 for fat in the population data used in developing a simple FFQ. Also, the correlation coefficients between the two FFQs were 0.82-7.92 in the population for the validation test. This study suggests that useful information on dietary intake could be obtained using a simple semi-quantitative FFQ in a large-scale dietary survey in Korea.

부산지역 건강검진 수검자의 식행태와 혈청 지질과의 관계 (Relationships Between Dietary Behavior and Serum Lipids of Health Examinees at General Hospitals in Busan)

  • 본 연구는 건강검진센터에서 건강검진을 받은 수검자 209명을 대상으로 성별, 연령별 혈청지질 농도의 분포 양상을 살펴보고 한국인의 식습관 및 식품섭취유형과의 관련성을 파악하여 건강증진센터 이용자 및 심혈관계질환자들을 위한 교육의 기초 자료를 마련하고자 수행하였다. 본 연구 결과에서 조사대상자의 BMI는 여자의 경우 연령이 증가함에 따라 유의적(p<0.01)으로 높았고, 남자가 여자보다 BMI가 높았다. 혈청 지질 농도는 여자가 TC, LDL, TG가 연령이 증감함에 따라 모두 유의적(p<0.01)으로 증가하였고, 남자는 TG가 연령이 증가함에 따라 높아졌지만 유의적인 차이는 보이지 않았다. 식습관 점수는 여자가 남자보다 높았지만 유의적인 차이가 없었고, 남녀 모두 연령이 높아질수록 식습관 점수가 높게 나타났다. 식습관 전체 점수는 BMI와 유의적인(p<0.05) 음의 상관관계를, 인스턴트식품 섭취 안함은 HDL과 유의적인 양(p<0.01)의 상관관계를 보였다. 식품의 섭취빈도에서 과자류 섭취빈도는 HDL과는 유의적인(p<0.05) 음의 상관관계를, LDL과 TG와는 유의적인(p<0.05) 양의 상관관계를 보였다. 고지방 식품 섭취빈도는 HDL과는 유의적인(p<0.01) 음의 상관관계를, LDL과는 유의적인(p<0.01) 양의 상관관계가 있었다. 육류(p<0.05)와 인스턴트식품(p<0.01) 섭취빈도는 HDL과 유의적인 음의 상관관계가 있었다. 튀김류(p<0.01) 섭취빈도는 TG와 유의적인 양의 상관관계를 보였다. 본 연구에서 다양한 식품군별 섭취빈도와 혈청지질 농도와 의미있는 관계를 보인 식품군들이 나타났지만 앞으로 조사대상자를 확대시킨 이와 관련된 연구를 수행함으로써 우리나라 사람들의 식사패턴에 맞는 식사관리 지침 프로그램 지침이 마련되어야 하겠다.

수중운동 참여가 정신지체장애인의 혈액성분 및 혈관탄성에 미치는 영향 (Influence of the Aqua Exercise on Blood Component and Vascular Compliance of Mental Retardation Men)

  • 본 연구는 수중운동 참여가 정신지체장애인의 혈액성분 및 혈관탄성에 미치는 영향을 규명하기 위하여 수행되었다. 연구의 대상자는 교육 가능한 정신지체장애인 24명이었으며, 수중운동집단 12명, 통제집단 12명으로 무선배정하고 수중운동집단은 9주간 주 5회 1일 60분 실시하였다. 자료처리는 집단별 운동프로그램 참여 전, 후에 대한 평균치의 차이검증은 대응표본 t-test를 실시하였으며, 통계적 유의수준은 p<.05로 설정하여 다음과 같은 결론을 얻었다. 첫째, 수중운동 참여집단에서 체중 및 체지방률은 사후 유의한 감소를 나타냈다(p<.05). 둘째, 수중운동 참여집단에서 사후, Total-Cholesterol, Triglyceride, HDL-C, LDL-C은 유의한 감소와 증가를 나타냈다(p<.05). 셋째, 수중운동집단에서 혈관탄성은 사후, 상지 우, 좌, 하지 우, 좌는 사전과 비교하여 유의한 증가를 나타냈다(p<.05). 이상의 결과를 종합해 볼 때, 정신지체장애인을 대상으로 수중운동참여는 신체조성 및 혈액성분, 혈관탄성에 긍정적인 영향을 미치는 것으로 나타나 심혈관계질환 및 각종 합병증 발병률이 높은 정신지체장애인에게 바람직한 운동프로그램이 될 것으로 사료되며, 후속 연구에서는 정신지체장애인의 신체적 특성 및 개인의 체력수준에 알맞은 과학적이고 체계적인 운동프로그램을 모색하고 제시되어야 할 것으로 사료된다.

서울 및 근교에 거주하는 한국인의 연령별 식생활 비교 및 평가: (5) 끼니별 섭취 (Comparative Analysis and Evaluation of Dietary Intakes of Koreans by Age Groups: (5) Meal Patterns)

  • This study was performed to investigate the characteristics of meal patterns among Koreans of different age groups. The study subjects consisted of preschool children (PC), elementary school children (EC), high school students (HS), college students (CS), and adults over 30 years old living in Seoul and the surrounding areas. A dietary survey was conducted using 24-hour recall method from 1996-1998 and data were collected from 2392 subjects. The subjects over 30 years old were classified as 30∼49 years, 50∼69 years, and over 70 years. EC, HS, and CS groups consumed less than 20% of their total energy intake from breakfast. PC and EC consumed over 30% of their energy from snacks. In all age groups, the proportion of subjects who skipped a meal was highest for breakfast and lowest for dinner. Especially, 31.4% of the CS group skipped breakfast. Breakfast and snacks provided higher quality meals than the others when each meal was assessed by the index of nutritional quality (INQ). Especially, lunch of the HS group, each meal of the EC group and subjects over 70 years of age had a lower diet quality than the others did. The major snack items for PC, EC, HS, and CS groups were milk, ice cream, and snack cakes, but those for subjects over 30 years of age were fruits. The PC group obtained 35.5% of their energy from snacks, but the proportion of snack energy decreased with increasing age. PC and EC groups obtained a great proportion of calcium, vitamin B2, fat, and cholesterol intake from snacks. Snack foods provided 53.2%, 42.4%, 45.4%, and 24.6% of those nutrients, respectively, for the PC group and 54.2%, 43.5%, 37.4%, and 26.8%, respectively, for the EC group. These results suggested that each age group had specific meal patterns. Especially, breakfast and snack intake seemed to complement overall diet quality. However, more intervention is needed to improve overall dietary nutrition, e.g., efforts to decrease fat and cholesterol proportion and increase breakfast consumption. These dietary characteristics among different age groups present foundation data for the development of methods to resolve the nutritional problems of each age group.

충청지역 일부 성인 여성의 체질량지수에 따른 체중조절용 식품과 건강기능식품 섭취 실태 및 식습관과 체중 관련 인식 (Consumption of Weight-control or Health Functional Foods, Dietary Habits, and Weight Perceptions According to the Body Mass Index of Adult Women in the Chungcheong Area)

  • Objectives: This study was conducted to investigate the experience and perception among adult women regarding weight control and the consumption of weight-control foods or health functional foods based on their body mass index (BMI). Methods: The subjects were 634 adult women from the Chungcheong province, Korea, and data were collected through a self-administered questionnaire from July 2021 through September 2021. The subjects were divided into four groups based on their BMI status: underweight (< 18.5 kg/m2, 7.6%), normal weight (18.5 ~ 22.9 kg/m2, 53.3%), overweight (23 ~ 24.9 kg/m2, 19.7%), and obese (≥ 25 kg/m2, 19.4%). Results: Over the past 3 years, almost two-thirds (68.6%) of the adult women had tried weight control measures, despite the fact that a significant proportion of them were normal or underweight. More importantly, 57.6% of subjects reported the consumption of weight-control foods, with a lower proportion in the underweight (35.4%) group compared to the normal (56.2%), overweight (62.4%), and obese (65.0%) groups. The food items used for weight control were mostly salads, chicken breasts, low fat (soy) milk, slimming tea, protein shakes, low-calorie cereals, and energy/protein bars among others. In addition, one-third (31.1%) of the subjects reported the use of health functional foods containing ingredients for fat reduction. A significantly higher proportion of these was from the overweight (36.0%) and obese (38.2%) groups compared to the underweight (20.8%) and normal weight (28.1%) groups. Products containing Garcinia cambogia extract, green tea, or Cissus extract, were popular among users. Subjects who were obese had a poorer perception of their health and body. Most subjects felt the need for correct information regarding weight control, but this number was significantly more in the higher BMI groups. Conclusions: Our results suggest that the use of weight-control foods or health functional foods is popular among adult women, especially those who are obese. Thus, nutrition education courses covering facts about weight control and practice need to be developed and provided based on the BMI status of subjects.

2010 당뇨병 환자를 위한 식품교환표 개정 (Korean Food Exchange Lists for Diabetes: Revised 2010)

  • A food exchange system for diabetes is a useful tool for meal planning and nutritional education. The first edition of the Korean food exchange lists was developed in 1988 and the second edition was revised in 1995. With recent changes in the food marketplace and eating patterns of Koreans, the third edition of food exchange lists was revised in 2010 by the Korean Diabetes Association, the Korean Nutrition Society, the Korean Society of Community Nutrition, the Korean Dietetic Association and the Korean Association of Diabetes Dietetic Educators through a joint research effort. The third edition is based on nutritional recommendations for people with diabetes and focuses on adding foods to implement personalized nutrition therapy considering individual preferences in diverse dietary environment. Foods were selected based on scientific evidence including the 2007 Korea National Health and Nutrition Examination Survey data analysis and survey responses from 53 diabetes dietetic educators. While a few foods were deleted, a number of foods were added, with 313 food items in food group lists and 339 food items in the appendix. Consistent with previous editions, the third edition of the food exchange lists included six food categories (grains, meat, vegetables, fats and oils, milk, and fruits). The milk group was subdivided into whole milk group and low fat milk. The standard nutrient content in one exchange from each food group was almost the same as the previous edition. Korea Food & Drug Administration's FANTASY (Food And Nutrient daTA SYstem) database was used to obtain nutrient values for each individual food and to determine the serving size most appropriate for matching reference nutrients values by each food group. The revised food exchange lists were subjected to a public hearing by experts. The third edition of the food exchange lists will be a helpful tool for educating people with diabetes to select the kinds and amounts of foods for glycemic control, which will eventually lead to preventing complications while maintaining the pleasure of eating.

유아 교육기관의 급식 운영실태와 유아의 식사 섭취량 조사 (A Study on Kindergarten's Meal Service Program and Children's Food Intake)

  • Today, the role of kindergarten is more important as nutrition provider through snack and meal services. Desirable kinds and amounts of food items at school lunch menu were important factors of nutrients intakes in children. This study was performed to assess the kindergarten's meal service program (snack and lunch) and the consumption amount of lunch according to each dishes. Twenty four institutions that cared over one hundred children were observed at Seoul and Kyunggido area. Well-trained observers checked meal and snack serving activity and foodservice facilities by formal checklist. And using the weighing method assessed the consumption levels of food items at lunch. During 3 days, 30 children in each kindergarten were selected randomly according to age, observers measured serving and residual weight of each food. The data was compiles by performing ANOVA-test using SPSS WIN 10.0. The result were as follows: 1) Foodservice facilities in kindergarten was limited to provide qualified foodservice. $91.7\%$ of institution had kitchen, $41.7\%$ had dinning. The kitchen equipment possession rate was $12.5\%$ (oven), $30.8\%$ (heating cabinet), $58.3\%$ (refrigerator). The rate of using document about foodservice was used $83.3\%$ (menu list), $41.7\%$ (daily foodservice record), $25\%$ (standard recipe). $41.7\%$ of institution employed licensed dietitian. Only $41.7\%$ of subjects preserved meal after daily meal service. 2) Meal serving size was decided by teacher, $54.5\%$ at snack and $43\%$ at lunch and pre-divided individual portion type was $36.4\%$ at snack and $28.6\%$ at lunch. The rate of cleaning activity before meal was $72.2\%$ at snack, $90.5\%$ at lunch. And nutrition or sanitation education activity was more performed at lunch time, for examples brushing teeth activity was $12.5\%$ at snack $85.7\%$ at lunch. 3) The consumption amounts of plain cooked rice was $112.7{\pm}26.1{\cal}g$, cooked rice and cereal was $93.06{\pm}27.97{\cal}g$, curry rice was $208.35{\pm}64.84{\cal}g$ and the consumption amounts of these main dishes was significantly different by age (p < 0.001). The consumption amounts of soup was very different according to children's preference. The consumption amounts of seaweed soup was $120.18{\pm}82.13{\cal}g$, wild sesame and bean-paste soup was $40.64{\pm}23.16{\cal}g$. The consumption range of kimchis was from $6{\cal}g\;to\;13{\cal}g$, jorim (braised food) was from $3{\cal}g\;to\;25{\cal}g$, fried food (include stir fried, deep fat fried, pan fried) was from $14.5{\cal}g\;to\;22{\cal}g$, vegetable dish was from $3{\cal}g\;to\;16{\cal}g$. These consumption amount of each dishes was not reached recommended portion size of nutritionally planning menu by nutritionist.

농촌노인의 건강문제와 지각된 건강상태에 관한 연구 (A study Health problem and Perceived health status of the rural elderly)

  • Purpose: The purpose of this study is to provide a basic data that may help develop health promotion programs by identifying health problems and perceived health status of the rural elderly. Method: The subjects of the study were 366 elders recruited from 24 villages located in Mari Myun, Geochang Gun, Korea. The sample was selected using a quota sampling method. The instruments used in the study included 117 items for health problems and 4 items for perceived health status. The data were analyzed with descriptive statistics, Pearson correlation coefficient, t-test, ANOVA and scheffe test using SPSS program. Results: 1) The most prevalent health problem was ailments in 'musculoskeletal system', followed by 'fatigue', 'eyes and ears', 'genitourinary system', 'mood & temper patterns', 'cardiovascular system', 'digestive system', 'nervous system', 'respiratory system' and 'skin'. 2) Womens health problems were more prevalent than men's health problems. 3) The mean score of perceived health status was 7.68. 4) Health problems of the rural elderly were significantly correlated with demographic variables such as education (F=9.532, p=0.000), gender (t=-4.246, p=0.000), marital status (t=-3.531, p=0.000), family type (F=5.742, p=0.00l), and occupation (t=3.356, p=0.001). 5) Perceived health status of the rural elderly was significantly correlated with demographic variables such as education (F=6.408, p=0.002), gender (t=2.949, p=0.003), marital status (t=0.802, p=0.034), family type (F=4.844, p=0.003), and occupation (t=-2.485, p=0.011). 6) Health problems of the rural elderly were significantly correlated with life style pattern variables such as drinking (F=5.223, p=0.006), smoking (F=4.087, p=0.007), salty food intake (F=3.424, p=0.034), greenish yellow vegetables intake (F=6.343, p=0.002) and fat food intake (F=5.327, p=0.005). 7) Perceived health status of the rural elderly was significantly correlated with life style pattern variables such as sleeping hours (F=3.966, p=0.020) and drinking (F=7.231. p=0.001). Conclusion: The findings of this study indicate that nurses need to understand health problems and perceived health status of the rural elderly and to develop health promotion programs for them in the future in the consideration of regional and environmental elements.
