뇌졸중 환자의 가정간호중재 프로토콜 개발 (Study on the Development of Home Care Nursing Intervention Protocol for Stroke Patients)

  • Stroke patient needs rehabilitation after receiving an acute treatment in a hospital. When stroke patient gets involved in an early discharge program, home care nurse plays a pivotal role to make them to gain a full strength and to come back to his/her prior life before he/she is sick. In spite of the importance of home care nursing intervention protocol for home care nurses to perform home care nursing autonomously, home care nursing intervention protocol for stroke patient is rarely developed. Therefore this study was conducted to develop home care nursing protocol that is applicable for stroke patients in home care nursing area. 41 home care nursing charts for stroke patients registered in home care nursing agencies from December 1st 1994 to August 31st 1999 at Y hospitals in Seoul and Won-Ju city were analyzed. 44 home care nurses who were having over three years' experience on stroke patients were participated in this study as a user validity validation group. The results of this study are as follows. 1. 28 nursing diagnoses were selected on the basis of evaluation of nursing diagnoses of stroke patients presented in a previous literature and case studies on home care nursing. 2. 17 nursing diagnoses were classified through the frequency analysis of home care nursing charts for 41 stroke patients who had received home care nursing. The order of sequence was like these: impaired skin integrity, risk for infection, nutritional deficit, impaired physical mobility, constipation, knowledge deficit, ineffective airway clearance, anxiety in family members, risk for aspiration, self care deficit, altered urinary elimination, ineffective individual coping, social isolation, risk for injury, self-esteem disturbance, impaired verbal communication, fatigue of family caregiver. 3. Based on validation on expert and user validities, 44 nursing interventions which were above ICV=.80 were chosen. 4. Nursing intervention protocols which showed above ICV=.90 were developed and were like these; pressure ulcer care, position change, preventive care for circulatory dysfunction, tube care : catheter, vital sign monitor, constipation/impaction management, artificial airway management, suction of airway secretion, environmental management : safety, and fall prevention.

Prenatal nursing intervention studies published in Korean nursing journals: a scoping review

  • The purpose of this study was to describe prenatal nursing intervention studies on pregnant women and their families published in Korean nursing journals to identify research trends and to analyze the characteristics of intervention studies. This scoping review was conducted using Arksey and O'Malley's framework. We identified a research question and searched six domestic electronic databases for relevant articles. Forty-five references that met the inclusion and exclusion criteria were finally selected. We extracted the data using an analytic framework, and then collated and summarized the characteristics of the intervention studies. The most frequently used research designs were non-randomized controlled trials (91.1%), and only a few studies applied a specific theoretical framework (24.4%). The participants were mainly pregnant women only (64.4%) during the third trimester (35.6%) of pregnancy. Prenatal education was the most common type of intervention (48.9%), followed by complementary therapy (37.8%) and psychosocial support programs (13.3%). The most commonly used outcome variables were drawn from the psychological domain (44.5%), although distinct types of outcome variables-especially from the psychological and physical domains-were used to measure the effectiveness of different types of prenatal interventions. This review suggests that further prenatal nursing intervention studies in Korea should expand the study participants to include pregnant women's family members, high-risk and vulnerable groups, and women throughout entire pregnancy. Furthermore, it is necessary to develop integrative prenatal nursing interventions that promote family support and participation by facilitating partnerships among women, families, and nurses before, during, and after pregnancy.

집단적 지지 간호중재 프로그램이 뇌졸중 환자 가족 수발자의 부담감에 미치는 효과 (The Effects of a Support Group Intervention on the Burden of Primary Family Caregivers of Stroke Patients)

  • Purpose: The purpose of this study was to develop and evaluate the effects of a support group intervention on the burden of primary family caregivers of stroke patients. Method: A nonequivalent control group pretest-posttest design was used. The subjects were 36 primary family caregivers of stroke patients [experimental(N=18) and control(N=18) groups] in a neurosurgery ward of a university hospital. The experimental group members participated in six sessions of a support group intervention for two weeks and the degree of their caregiving burden was evaluated. Data was analyzed by Chi-square tests, t-tests, and paired t-tests using SPSS 10.0. Result: The experimental group had a significantly lower total burden score (t=2.06l, p= .047)and sub-scales of emotional(t=-3.319, p= .002), time-dependent(t=-2.045, p= .049) and developmental(t=-2.656, p= .012) burden scores than the control group, while no significant differences were found in physical, social or financial burden scores between the two groups. Within the experimental group, there was a significant decrease in physical(t=2.507, p= .023), emotional(t=4.754, p= .000), social(t=2.932, p= .009), time- dependent(t=5.015, p= .000) and developmental(t=7.541, p= .000) burden scores but not the financial burden score. Conclusion: The results suggest that a support group intervention can be utilized as an effective nursing program to reduce the burden of primary family caregivers of stroke patients.

만성환아의 가족에 관한 국내 연구논문 분석 (An Analysis of Nursing Research on the Family with chronfcally ill children in Borea)

  • The purpose of study was : 1) to analyze the trend of research on the family with chronically ill children in Korea, 2) to suggest direction for future study on the family with chronically ill children, and contributing to the use of intervention in family nursing practice. Research studies on the family with chronically ill children were selected from the Korean Nusre, the Korean Nurses' Academic Society Journal, and from dissertations, which were conducted between 1975 and 1995. The total numbers of the studies were 35. These studies were analyzed for 1)time of publication or presentation, 2)research design, 3)characteristics of subjects, 4) type of chronic disease, 5)main concepts, 6)measurement tool, 7) the sis for a degree or nondegree, 8) result of correlational studies. The findings of the analysis were as follows : 1) The numbers of studies on the family with chronically ill children have increas rapidly the early 1990's. In research design, the numbers of survey research studies were the highest. Especially, the most frequently research design was the correlational survey. There were 19 correlational studies(25.7%) during the early 1990's. 2) The subjects in 16 studies(45.7%) were mother of chronically ill children and, in 8 studies (22.9% ) were their parents. 3) In most types of chronic diseases, there were 14 hematooncologic disease(32.6%) and 14 hadicapped children (32.6% ). 4) Frequently used research concepts were stress, degree of coping or way of coping, social support, parents' support, family functioning, intensity of family and family adaptation. 5) Acceding to the results of correlational studies, the more family stress was higher the more degree of coping, family functioning, intensity of family and degree of family adaption was lower. The more degree of social support was higher the more stress was lower and degree of coping, family functioning and intensity of family was higher. The more family functioning was higher the more intensity of family and family adaptation was higher. 6) 24 researches on the family with chronically ill children were done for a thesis for a degree and 11 were nondegree research studies. The following suggestions are made based on the above findings : 1) The pattern of these studies related to the family with chronically ill children in domain of Nursing need to be compared with trend in other domains. 2) More replicated research on the family with chronically ill children is needed to develop family nursing intervention and prove the effect of that and more qualitative research on the family with chronically ill children is needed to comprehensive indepth the family with chronically ill children. 3) Further research on the family with chronically ill children is needed to verify subjects and type of chronic disease, develop applicable measurement tools in Korea and identify relation between other concepts. 4) Family nursing researchers should make an effort to apply research result in various clinical settings and community settings, and try to carry out not only team research with clinical nurse but also other multidisciplinary researcher related to the family.

입원 환아 가족의 가족 적응 모형검증: 중국 가족 대상 (Verification of Model on Family Adaptation in Families of Hospitalized Children: Focused on Parents of Hospitalized Children in China)

  • Purpose: The purpose of this study was to identify the effect of family stressors, family strains, general family values, family values for hospitalized children and family hardiness on family adaptation based upon McCubbin's family resilience model for families of hospitalized children in China. Methods: A survey using a self-report questionnaire was conducted with 330 hospitalized children's parents. Six instruments were used in this model. The data were analyzed using SPSS WIN 19.0 and AMOS 19.0 program. Results: Family strain had a significant negative direct effect, general family values and family hardiness had a significant positive direct effect, and family values for hospitalized children had a significant positive direct effect and indirect effect on family adaptation of hospitalized children. Conclusion: In this study McCubbin's resilience model and resiliency factors were identified. According to the finding of this study nursing interventions to reduce family stress, enhance family hardiness and consolidate general family values and family values for hospitalized children can help to increase family adaptation in families of hospitalized children. Therefore, the significance of this study is that the results can be used to develop family nursing intervention programs based on family resilience to strengthen family adaptation in families of hospitalized children.

정형외과 간호단위에 적용되는 간호중재 분석 (Analysis of Nursing Interventions used in Orthopedic Nursing Unit)

  • Purpose: This study was done to identify the Domains, Classes, labels and nursing activities of nursing interventions used with 117 patients who were admitted to orthopedic nursing units. Method: Data were collected in January and February, 2004 using a computerized nursing process program that contained nursing diagnosis-outcome-intervention (NNN) linkages. The program was developed by the researcher. Frequencies and percentages were used in the analysis. Results: Sixty-five nursing intervention labels were identified. The Domains of the nursing interventions showed higher percentages for 'physiological: basic' (75.9%), 'physiological: complex' (12.9%), 'behavioral'(7.8%) 'family'(1.3%), and 'safety'(1.1%). The Classes of nursing interventions showed higher percentages for 'activity and exercise management'(30.8%), 'physical comfort promotion'(19.3%), 'immobility management'(14.5%), 'drug management'(8.1%), and 'coping assistance'(5.6%). Nursing intervention labels showed higher percentages for 'pain management'(14.7%), 'body mechanics promotion'(8.0%), 'exercise therapy : ambulation'(7.2%), 'splinting'(5.4%), and 'positioning'(5.1%). In the comparison of numbers between performed nursing activities and nursing activities of NIC according to nursing intervention label, the mean of combined rate was 52.3%. Conclusion: These findings will help in building of a standardized language for orthopedic nursing units and enhance the quality of nursing care.

Research trends over 10 years (2010-2021) in infant and toddler rearing behavior by family caregivers in South Korea: text network and topic modeling

  • Purpose: This study analyzed research trends in infant and toddler rearing behavior among family caregivers over a 10-year period (2010-2021). Methods: Text network analysis and topic modeling were employed on data collected from relevant papers, following the extraction and refinement of semantic morphemes. A semantic-centered network was constructed by extracting words from 2,613 English-language abstracts. Data analysis was performed using NetMiner 4.5.0. Results: Frequency analysis, degree centrality, and eigenvector centrality all revealed the terms ''scale," ''program," and ''education" among the top 10 keywords associated with infant and toddler rearing behaviors among family caregivers. The keywords extracted from the analysis were divided into two clusters through cohesion analysis. Additionally, they were classified into two topic groups using topic modeling: "program and evaluation" (64.37%) and "caregivers' role and competency in child development" (35.63%). Conclusion: The roles and competencies of family caregivers are essential for the development of infants and toddlers. Intervention programs and evaluations are necessary to improve rearing behaviors. Future research should determine the role of nurses in supporting family caregivers. Additionally, it should facilitate the development of nursing strategies and intervention programs to promote positive rearing practices.

뇌졸중 환자를 돌보는 가족의 가족기능 영향요인 (Influencing Factors on Family Functioning of Caregivers in Families with Stroke Patients)

  • Purpose: The purpose of this study was to identify the main factors influencing family functioning of caregivers in families with stroke. Method: A Convenient sample of 173 primary family caregivers who take care of a stroke patient at an Oriental medicine hospital in Jeonbuk. Interviews were done with a standardized questionnaire including family functioning by nurses. Results: In Pearson's correlation analysis, the influencing factors related to family functioning were ADL(p=.017), level of paralysis(p=.019) as stressors, Quality of relation(p=.000) as situational variables, and family caregivers' burden(p=.000). Stepwise multiple regression analysis showed 29.9% of the variance family functioning was significantly accounted for by the quality of relationship between stroke patient and caregiver(26.8%), and caregiver burden(3.1%). Conclusions: Findings indicate that families of stroke patients need family-focused nursing intervention as supported care to improve the relationship between patient and primary caregiver and relieve caregiver burden by culturally tailoring to Korean.

여고생의 문제행동양상과 문제행동 영향요인 (Factors related to Problem Behaviors in High School Girls)

  • Purpose: The purpose of this study was to investigate the relationships between problem behavior, school stressor and family strength in high-school girls. Methods: Data were collected from 200 high school girls in 2008. Data were analyzed with SPSS/WIN 18.0. Results: There was a positive correlation between problem behavior and school stressor. There was a negative correlation between problem behavior and family strength. The predictors of problem behavior were school stressor, family strength, and monthly family income and these factors explained 43.0% of problem behavior. Conclusion: The findings of this study suggest the necessity of developing nursing intervention programs for enhancing high-school girls' skills in interpersonal relationship and communication to decrease school stressor and increase family strength.

뇌졸중 환자 가족돌봄제공자의 영성과 삶의 질 모델: 경로분석 (Spirituality and Quality of Life Model of Family Caregivers Caring for Patients with Stroke: Path Analysis)

  • Purpose: This study was to test a structural model of spirituality and the quality of life of stroke survivors' caregivers in order to provide guidelines for the development of intervention and strategies to improve their quality of life. Methods: Data were collected from 133 family caregivers of stroke patients who were hospitalized in C university hospital located in Seoul. Data collection using survey questionnaires was done from May, 2013 to February, 2014. Results: Fitness of the hypothetical model was appropriate. Physical component of quality of life of family caregivers is directly affected by two variables (51.5%), burden and depression. Mental component of quality of life of family caregivers is directly affected by three variables (77.6%), depression, burden, and functional dependence of patients. Depression as well as burden were directly affected by spirituality and functional dependence of patients respectively. Thus, spirituality directly affected depression and burden and indirectly affected the quality of life of family caregivers. Conclusion: Therefore, spiritual intervention to improve the stroke caregivers' quality of life might be necessary to support and strengthen their spirituality as a mediating variable that can contribute to decreasing their depression and burden.
