호스피스 간호 관련 국내 연구 동향 (Trends in Research on Hospice Care in Korea)

  • 목적: 본 연구는 우리나라 호스피스 간호 연구 동향을 알아보기 위해 시도되었다. 방법: 본 연구의 대상은 1985년부터 2009년까지 24년 동안 발표된 국내 호스피스 간호 관련 논문 총 183편으로 간호학위논문 62편과 비학위논문으로서 학술지에 게재된 논문 121편이었다. 본 분석대상 논문은 한국교육학술정보원에서 제공하는 학술정보서비스(http://www.riss.kr)를 이용하여 호스피스(완화) 간호, 말기(암)환자의 핵심어를 조합하여 검색하였다. 검색결과 호스피스 관련 국내 학위논문과 대한간호학회지, 아동간호학회지, 정신간호학회지, 성인간호학회지, 병원간호사회지, 행정간호학회지, 종양간호학회지, 한국 호스피스 완화의료학회지, 호스피스 학술지, 호스피스 논집 등에 게재된 논문들을 대상으로 하였고, 학위논문 중 학술지에 게재된 것은 학술지를 분석대상으로 하였다. 단, 종설과 인터넷으로 원문수집이 불가능한 논문은 제외하였다. 결과: 호스피스 간호 관련 논문 수가 급증하였고, 연구설계 유형은 양적연구가 질적연구보다 월등하게 많았다. 분석대상 논문에서 도출된 주요어는 5개 영역에서 총 74개가 도출되었고, 총 225개의 주요어를 사용하였다. 자료수집시 윤리적 고려는 서면동의가 많았고, IRB를 통과한 연구가 극소수이며, 다학제간 연구 지원금은 점차 증가하였다. 결론: 결론적으로 말기암환자의 간호문제나 현상을 연구하기 위해서는 조사연구보다는 질적연구 방법의 접근이 많이 필요하고, 연구개념도 삶의 질과 더불어 죽음의 질에 대한 연구가 필요하며, 중재연구에 있어서는 말기암환자의 가장 큰 문제로 대두되고 있는 통증완화, 기타 주요 신체간호, 그리고 보완대체요법에 관한 간호연구가 더 필요하다고 본다. 또한 간호학 연구를 넘어 다학제적 팀 연구가 더 많이 수행되어야 하며, 마지막으로 아동호스피스에 관한 연구가 매우 미비한데, 이에 관한 연구가 더 시행되어야함을 제안한다.

간호장교의 복지정책을 위한 조사연구 - 여성복지와 군복지 일반을 중심으로 - (A Study On The Welfare Policies For Nurse Officers)

  • It is clear that the women's participation in social activity is dramatically increased in civilian field as well as military profession. Because of the characteristics of military nursing service, there are many difficulties. Frequent move, medical insurance, baby care. and the education are typical forms of difficulties for nurse officers in the military. The purpose of this research is to contribute to the perfect execution of nursing service. guarantee the active attitude for research and improve the combat strength by solving those private and economic problems. The method of research was analysis of question survey and the review of related literatures. The subject of research was 300 active nurse officers who work at the Army Hospitals. The survey was conducted 14 days from Oct 15 to Oct 28. The collected data was processed by computer using SPSS(Statistical Package of Social Science). Frequencies and percentages were used to examine the demographic characteristics of subject, and T-test was also used in the case of necessity. The result showed as follows; As the general characteristics of subject group; 28.7 years of average age, 73.6 months of service period, 63.4% of married. In regard of specialty; General 57.7%. Intensive Care 12.7% and Psychiatries 8.8%. The dissatisfactory factors about military welfare system were the difficulties in children education (36.9%). disadvantages on the civil medical insurance system (27.3%), and little chance of self education and development (21.5%). The problems in performing their duties were shown as dwelling instability due to frequent move (67.7%), and bring up children (14.2%). The reasons for resigning their job were shown as the instability of living status (64.2%), bring up children (18.8%) and dissatisfaction to the service (11.2%). The residential status was shown that military offered houses (45.2%), rental houses (29.3%) and own houses(14.64). The average numbers of moving residencies were; 3-4 time(34.6%), less than 2 times(33.1%), and 5-7 times(21.5%). Higher than 94.7% of the subject group spent more than 50.000 Won. In regard of education, they wanted to attend graduate school with their own expense(26.2%), computer science(20%) and Office Job Training(20%). The ways of taking care of children were mother-in-low(49.6%), mother(14.6%), and others(25%). The average expenditure per month for children were 20-30 hundred Won(44.2%), 10-20 hundred Won(25%) and 30-40 hundred Won(22.3%). The places of children care selected were public or occupational care center(56.2%), religious organization(20.8%), and other center managed by social organizations(10.4%). The result of survey for general welfare of nurse officers are as follows; By and large they seem to be satisfied with their job. however. there are some dissatisfactory factors. They are children care facilties, promotion. income. welfare facilities. disadvantage in medical insurance and civil hospitals. house purchase. unfair chances in specialty training. influence on promotion by educational status. and insufficient role for their children and husbands. As conclusion. the recommendations for improving nursing service are as follows; 1. Children care center managed by occupation 2. Dormitory system for children by military personnel 3. Equal opportunities in education according to ability 4. Reasonable moving price according to the distance and scope of family and extra allowance

Quantity over Quality? Perception of Designating Long-Term Care Hospitals as Providers of Hospice and Palliative Care

  • 목적: 호스피스 완화의료에의 접근성 향상을 위해 요양병원을 호스피스 전문기관으로 지정하는 개정안이 최근 시행되었다. 호스피스 전문기관의 양적 증대도 중요하지만, 이러한 개정에 따른 서비스의 질적 저하 역시 충분히 고려되어야 한다. 이러한 관점에서 본 연구는 1,001명의 암환자, 1,006명의 가족 간병인, 928명의 의사 및 1,005명의 일반인이 해당 개정안이 가진 이점과 비용에 대해 어떻게 인식하고 있는지 확인하였다. 방법: 2016년 7월부터 10월까지 다기관 단면조사를 시행하였다. 연구 참여자들은 전문가 인터뷰 및 선행연구에서 추출된 본 개정안의 이점과 비용에 대해 각각 얼마나 동의 혹은 동의하지 않는지 응답하였다. 분석에는 카이제곱 분석, 단변량 및 다변량 로지스틱 회귀분석을 활용하였다. 결과: 참조집단인 일반인 집단과 비교했을 때, 의사 집단은 요양병원이 양질의 호스피스 완화의료 서비스를 제공하기 위한 시설 및 인력이 충분하지 않다는 점에 더 동의하였으나, 요양병원에서 과한 진료비를 청구할 것이라는 점에는 더 동의하지 않았다. 가족 간병인의 경우 일반인에 비해 요양병원에서 서비스를 제공한다면 접근성이 좋아질 것이라는 점에 더 동의했으나, 호스피스 정신이 훼손될 수 있음과 가족들이 환자 돌봄에 신경 쓰지 않을 것을 더 우려하였다. 일반인과 비교했을 때, 암환자 역시 마찬가지로 호스피스 정신이 훼손될 수 있음을 더 우려하였으며, 서비스의 질이 좋아질 것이라는 점에 대해서는 더 동의하지 않았다. 결론: 본 연구를 통해 호스피스 완화의료의 잠재적인 서비스 이용자 및 제공자가 해당 개정의 이점뿐만 아니라 비용 역시 인식하고 있음을 확인하였다. 본 연구 결과 및 유럽 국가들의 사례를 통해 개정안이 실제로 현장에서 실행되기 전에 호스피스 전문기관으로서 요양병원이 새롭게 갖추어야 할 요건 및 방향성에 대해 제언하였다.

한국과 미국 교육부 장관 인사청문회 비교 - 이명박 정부와 오바마 정부의 사례를 중심으로 - (A Comparative Study on Confirmation Hearings for Secretary of Education in South Korea and the United State - Focus Cases on Administrations of Myungbak Lee and Barack Obama -)

  • 본 연구는 시기적으로 인접한 이명박 정부와 오바마 정부의 첫 교육부 장관 인사청문회 전후 과정과 내용을 비교 분석하여, 양국 교육부 장관 후보자 인선 과정 및 인사청문회의 절차적 내용적 특징을 밝히려 하였다. 그 결과 후보자 인선 과정에서 한국은 청와대 비서실 주관으로 1주일 이내에 후보자 개인 신상 등을 조사하는 간단한 검증에 그친 반면 미국은 백악관 인사실을 중심으로 여러 기관이 협력하여 2~3개월에 걸쳐 철저하게 검증하였다. 또, 한국은 초선 및 비례대표 의원이 다수인 국회 교육위원회에서 약 1주일에 걸쳐 청문회를 준비하였는데, 제출 자료의 완비, 인사청문회의 시행, 인사청문경과보고서의 채택 여부가 후보자 임명 조건이 되지 않았다. 반면 미국은 재선 이상의 연방 상원의원들로 구성된 보건 교육 노동 연금 위원회(Committee on Health, Education, Labor and Pensions)에서 약 1개월 정도 준비하였는데, 제출 자료의 사전 완비, 소관 상임위원회의 인준 결정이 후보자 임명의 필수 요건이었다. 인사청문회에서 한국은 여당위원이 정책 질의나 해명 기회 제공용 질문이 많았고, 야당의원은 도덕성 검증에 주력하였다. 특히 청문위원들은 각기 가지고 있는 이념적 성향에 따라 청문을 주도하였다. 반면 미국은 도덕성 질문은 없었고 여야를 막론하고 후보자의 경력과 전문성, 국가적 교육 이슈나 교육 정책에 대한 후보자의 견해, 주에 대한 연방정부의 지원 문제 등에 대해 질문하고 충분한 답변 기회를 주었다. 이상의 연구 결과를 토대로 후보자 인선 과정의 내실화와 전담 인력 배정, 청문회 자료 제출 의무화와 준비 기간 연장, 후보자 답변 시간 보장, 주제별 분리 실시, 개별의원의 자율성 보장 등을 내용으로 하는 정책적 시사점을 제시하였다.

병원 간호사의 선호근무시간대에 관한 연구 (A Study on Hoslital Nurses' Preferred Duty Shift and Duty Hours)

  • The duty shifts of hospital nurses not only affect nurses' physical and mental health but also present various personnel management problems which often result in high turnover rates. In this context a study was carried out from October to November 1995 for a period of two months to find out the status of hospital nurses' duty shift patterns, and preferred duty hours and fixed duty shifts. The study population was 867 RNs working in five general hospitals located in Seoul and its vicinity. The questionnaire developed by the writer was used for data collection. The response rate was 85.9 percent or 745 returns. The SAS program was used for data analysis with the computation of frequencies, percentages and Chi square test. The findings of the study are as follows: 1. General characteristics of the study population: 56 percent of respondents was (25 years group and 76.5 percent were "single": the predominant proportion of respondents was junior nursing college graduates(92.2%) and have less than 5 years nursing experience in hospitals(65.5%). For their future working plan in nursing profession, nearly 50% responded as uncertain The reasons given for their career plan was predominantly 'personal growth and development' rather than financial reasons. 2. The interval for rotations of duty stations was found to be mostly irregular(56.4%) while others reported as weekly(16.1%), monthly(12.9%), and fixed terms(4.6%). 3. The main problems related to duty shifts particularly the evening and night duty nurses reported were "not enough time for the family, " "afraid of security problems after the work when returning home late at night." and "lack of leisure time". "problems in physical and physiological adjustment." "problems in family life." "lack of time for interactions with fellow nurses" etc. 4. The forty percent of respondents reported to have '1-2 times' of duty shift rotations while all others reported that '0 time'. '2-3 times'. 'more than 3 times' etc. which suggest the irregularity in duty shift rotations. 5. The majority(62.8%) of study population found to favor the rotating system of duty stations. The reasons for favoring the rotation system were: the opportunity for "learning new things and personal development." "better human relations are possible. "better understanding in various duty stations." "changes in monotonous routine job" etc. The proportion of those disfavor the rotating 'system was 34.7 percent. giving the reasons of"it impedes development of specialization." "poor job performances." "stress factors" etc. Furthermore. respondents made the following comments in relation to the rotation of duty stations: the nurses should be given the opportunity to participate in the. decision making process: personal interest and aptitudes should be considered: regular intervals for the rotations or it should be planned in advance. etc. 6. For the future career plan. the older. married group with longer nursing experiences appeared to think the nursing as their lifetime career more likely than the younger. single group with shorter nursing experiences ($x^2=61.19.{\;}p=.000;{\;}x^2=41.55.{\;}p=.000$). The reason given for their future career plan regardless of length of future service, was predominantly "personal growth and development" rather than financial reasons. For further analysis, the group those with the shorter career plan appeared to claim "financial reasons" for their future career more readily than the group who consider the nursing job as their lifetime career$(x^2$= 11.73, p=.003) did. This finding suggests the need for careful .considerations in personnel management of nursing administration particularly when dealing with the nurses' career development. The majority of respondents preferred the fixed day shift. However, further analysis of those preferred evening shift by age and civil status, "< 25 years group"(15.1%) and "single group"(13.2) were more likely to favor the fixed evening shift than > 25 years(6.4%) and married(4.8%)groups. This differences were statistically significant ($x^2=14.54, {\;}p=.000;{\;}x^2=8.75, {\;}p=.003$). 7. A great majority of respondents(86.9% or n=647) found to prefer the day shifts. When the four different types of duty shifts(Types A. B. C, D) were presented, 55.0 percent of total respondents preferred the A type or the existing one followed by D type(22.7%). B type(12.4%) and C type(8.2%). 8. When the condition of monetary incentives for the evening(20% of salary) and night shifts(40% of. salary) of the existing duty type was presented. again the day shift appeared to be the most preferred one although the rate was slightly lower(66.4% against 86.9%). In the case of evening shift, with the same incentive, the preference rates for evening and night shifts increased from 11.0 to 22.4 percent and from 0.5 to 3.0 percent respectively. When the age variable was controlled. < 25 yrs group showed higher rates(31.6%. 4.8%) than those of > 25 yrs group(15.5%. 1.3%) respectively preferring the evening and night shifts(p=.000). The civil status also seemed to operate on the preferences of the duty shifts as the single group showed lower rate(69.0%) for day duty against 83. 6% of the married group. and higher rates for evening and night duties(27.2%. 15.1%) respectively against those of the married group(3.8%. 1.8%) while a higher proportion of the married group(83. 6%) preferred the day duties than the single group(69.0%). These differences were found to be statistically all significant(p=.001). 9. The findings on preferences of three different types of fixed duty hours namely, B, C. and D(with additional monetary incentives) are as follows in order of preference: B type(12hrs a day, 3days a wk): day shift(64.1%), evening shift(26.1%). night shift(6.5%) C type(12hrs a day. 4days a wk) : evening shift(49.2%). day shift(32.8%), night shift(11.5%) D type(10hrs a day. 4days a wk): showed the similar trend as B type. The findings of higher preferences on the evening and night duties when the incentives are given. as shown above, suggest the need for the introductions of different patterns of duty hours and incentive measures in order to overcome the difficulties in rostering the nursing duties. However, the interpretation of the above data, particularly the C type, needs cautions as the total number of respondents is very small(n=61). It requires further in-depth study. In conclusion. it seemed to suggest that the patterns of nurses duty hours and shifts in the most hospitals in the country have neither been tried for different duty types nor been flexible. The stereotype rostering system of three shifts and insensitiveness for personal life aspect of nurses seemed to be prevailing. This study seems to support that irregular and frequent rotations of duty shifts may be contributing factors for most nurses' maladjustment problems in physical and mental health. personal and family life which eventually may result in high turnover rates. In order to overcome the increasing problems in personnel management of hospital nurses particularly in rostering of evening and night duty shifts, which may related to eventual high turnover rates, the findings of this study strongly suggest the need for an introduction of new rostering systems including fixed duties and appropriate incentive measures for evenings and nights which the most nurses want to avoid, In considering the nursing care of inpatients is the round-the clock business. the practice of the nursing duty shift system is inevitable. In this context, based on the findings of this study. the following are recommended: 1. The further in-depth studies on duty shifts and hours need to be undertaken for the development of appropriate and effective rostering systems for hospital nurses. 2. An introduction of appropriate incentive measures for evening and night duty shifts along with organizational considerations such as the trials for preferred duty time bands, duty hours, and fixed duty shifts should be considered if good quality of care for the patients be maintained for the round the clock. This may require an initiation of systematic research and development activities in the field of hospital nursing administration as a part of permanent system in the hospital. 3. Planned and regular intervals, orientation and training, and professional and personal growth should be considered for the rotation of different duty stations or units. 4. In considering the higher degree of preferences in the duty type of "10hours a day, 4days a week" shown in this study, it would be worthwhile to undertake the R&D type studies in large hospital settings.

치과위생사의 이직요인에 대한 조사연구 (Dental Hygienists' Turnover Intention and its Related Factors)

  • 유능한 치과위생사의 이직을 예방하여 구강보건 인력관리에 효율성을 증가시키고, 양질의 구강보건서비스를 제공하는 데 기초자료로 활용하고자 문헌과 자료를 조사하고 각 지역의 구강진료기관에 종사하는 치과위생사를 대상으로 2004년 9월부터 12월까지 약 4개월 동안 치과위생사의 이직에 실질적으로 영향을 미치는 요인을 파악하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 치과위생사의 이직 경험에 대하여 살펴본 결과, 이직경험이 있는 경우는 39.7%로 나타났고, 이직의 횟수는 1회가 28.2%, 2회가 8.0%, 3회가 2.9% 순으로 나타났다. 이직의 원인은 근무조건이 66.7%로 가장 많았고, 다음으로 큰 기관으로의 이동 36.2%, 급여수준 21.7%, 치과의사와의 관계 11.6%, 출퇴근거리 11.6% 순으로 나타났다. 2. 이직 희망 여부에 대하여 살펴본 결과, 희망하고 있는 경우가 82.8%로 나타났으며, 원인으로는 근무조건이 44.4%로 가장 많았고, 다음으로 급여수준 33.3%, 출퇴근거리 18.1%, 결혼 13.2%, 건강 및 여가선용 11.8%, 출퇴근 시간 10.4% 순으로 나타났다. 3. 이직 희망기관으로는 보건(지)소가 38.5%로 가장 많았고, 치과위생사로서 취업을 희망하는 기한에 대해서는 경제적으로 안정될 때까지가 50.0%로 가장 많았고, 다음으로 정년 때까지가 34.5%로 나타났다. 본인소득이 가정경제에 미치는 책임정도에 대해서는 일부 책임이 있다고 응답한 경우가 47.7%로 가장 많았고, 책임이 없다고 응답한 경우가 31.6%로 나타났다. 4. 향후 치과위생사를 그만 둘 계획이 있는지에 대하여 살펴본 결과, 있다고 응답한 경우가 61.5%로 나타났고, 원인으로는 결혼이 29.0%로 가장 많았고, 다음으로 근무조건 27.1%, 출산 22.4%, 건강 및 가사 18.7%, 급여수준 15.9%, 학업과 여가선용 15.0% 순으로 나타났다.

Association of Selected Medical Conditions With Breast Cancer Risk in Korea

  • Objectives: To estimate the effect of medical conditions in the population of Korea on breast cancer risk in a case-control study. Methods: The cases were 3242 women with incident, histologically confirmed breast cancer in two major hospitals interviewed between 2001 and 2007. The controls were 1818 women each admitted to either of those two hospitals for a variety of non-neoplastic conditions. Information on each disease was obtained from a standardized questionnaire by trained personnel. Odds ratios (ORs) for each disease were derived from multiple logistic regression adjusted for age, age of menarche, pregnancy, age of first pregnancy, and family history of breast cancer. Results: Among all of the incident breast cancer patients, pre-existing diabetes (OR, 1.33; 95% confidence interval [CI], 0.99 to 1.78), hypertension (OR, 1.46; 95% CI, 1.18 to 1.83), thyroid diseases (OR, 1.26; 95% CI, 1.00 to 1.58), and ovarian diseases (OR, 1.70; 95% CI, 1.23 to 2.35) were associated with an increased risk of breast cancer when other factors were adjusted for. In a stratified analysis by menopausal status, pre-existing hypertension (pre-menopause OR, 0.80; 95% CI, 0.48 to 1.34 vs. post-menopause OR, 1.87; 95% CI, 1.44 to 2.43; p-heterogeneity <0.01) and ovarian disease (pre-menopause OR, 4.20; 95% CI, 1.91 to 9.24 vs. post-menopause OR, 1.39; 95% CI, 1.02 to 1.91; p-heterogeneity 0.01) showed significantly different risks of breast cancer. Conclusions: Our results suggest the possibility that medical conditions such as hypertension affect breast cancer development, and that this can differ by menopausal status. Our study also indicates a possible correlation between ovarian diseases and breast cancer risk.

한국 소아치과의 현재와 전망 - 치아우식증관리 분야를 중심으로 - (PRESENT SITUATION AND PROSPECT OF PEDIATRIC DENTISTRY IN KOREA - FOCUSED ON MANAGEMENT OF DENTAL CARIES -)

  • General status of pediatric dentistry in Korea is to conduct vigorous academic activities and specialized medical care centering the Korean Association of Pediatric Dentistry (KAPD) that has about 1,000 pediatric dentists as members, pediatric dentistry departments of 11 Colleges of Dentistry, numbers of pediatric dentistry training institutions and private clinics specialized in children. From 1996, the accredited pediatric dentists were produced by the KAPD and from 2008, the state began to produce the accredited pediatric dentists. Since then, doctors with expertise in pediatric care had opened private clinics in addition to the university hospitals, it became the basis of a momentum to deepen the specialty of pediatric dentistry. The Dentistry community of Korea is going through rapid and profound changes recently, and the underlying reasons for such changes can be classified largely into a few categories: (1) Decreasing population and structural changes in population (2) Increase in numbers of dentists, (3) Changes in the pattern of dental diseases and (4) Changes in medical environment. In Korea, the children population in the age range of 0 ~ 14 years old had been decreased by 2 million in 2010 compared to that of 2000 due to reduction of birth rate. The current population of children in the age range of 0 ~ 4 years old in 2010 takes up 16.2% of the total population, but it is estimated that such percentage would decrease to 8.0% by 2050. Such percentage is largely behind the estimated mean global population of 19.6% by 2050. On the other hand, the number of dentists had been largely increased from 18,000 in 2000 to 25,000 in 2010. And it is estimated that the number will be increased to 41,000 by 2030. In addition, the specialized personnel of Pediatric dentistry had been shown as increased by 2.5 times during past 10 years. For the changes in the pattern of dental diseases, including dental caries, each df rate of 5 years old children and 12 years old children had been decreased by 21.9% and 16.7% respectively in 2010 compared to 2000. Each df Index also had been decreased by 2.5 teeth and 1.2 teeth respectively. The medical expenditure of Korea is less than that of OECD and more specifically, the expenditure from the National Health Plan is less than OECD but the expenditure covered by households is larger than OECD. These facts indicate that it is considered as requiring the coverage of the national health plan to be reinforced more in the future and as such reinforcement needs continuous promotion. In medical examination pattern of Pediatric dentistry, the preventive and corrective treatment were increased whereas the restorative treatment was decreased. It is considered that such change is caused from decrease of dental caries from activation of the prevention project at national level. For the restorative treatment, the restorations in use of dental amalgam, pre-existing gold crowning and endodontic treatment had been decreased in their proportion while the restorative treatment in use of composite resin had been increased. It is considered that such changes is caused by the change of demands from patients and family or guardians as they desired more aesthetic improvement along with socio-economic growth of Korean society. Due to such changes in dentistry, the pediatric dentistry in Korea also attempts to have changes in the patterns of medical examination as follows; It tends to implement early stage treatment through early diagnosis utilizing various diagnostic tools such as FOTI or QLF. The early stage dental caries so called white spot had been included in the subjects for dental care or management and in order to do so, the medical care guidelines essentially accompanied with remineralization treatment as well as minimally invasive treatment is being generalized gradually. Also, centering the Pediatric dentists, the importance of caries risk assessment is being recognized, in addition that the management of dental caries is being changed from surgical approach to internal medicinal approach. Recently, efforts began to emerge in order to increase the target patients to be managed by dentists and to expand the application scope of Pediatric dentistry along with through such changes. The interest and activities of Pediatric dentists which had been limited to the medical examination room so far, is now being expanded externally, as they put efforts for participating in the preventive policy making process of the community or the state, and to support the political theories. And also opinions are being collected into the direction that the future- oriented strategic political tasks shall be selected and researches as well as presentations on the theoretical rationale of such tasks at the association level.
