  • Fault tree analysis (FTA) is a widely used methodology for reliability analysis. The method is useful in that it suggests a very comprehensive way of describing the hierarchical relations of causes of faults and the corresponding results. However it is difficult to get appropriate fault trees for given products or systems without very profound knowledge and experience. This work aims to develop a methodology of fault tree construction using the results of function deployment for machine parts, which provides an objective way of preparing fault trees. The failure modes are defined to each function network generated by the function deployment method and the fault tree with respect to each viewpoint is constructed by arranging the failure modes. The fault tree is finally obtained by synthesizing the fault trees with respect to each viewpoint. The example of fault tree construction is also shown.

Laparoscopic Surgery for Intussusception in Children (소아 장중첩증에 대한 복강경 수술의 경험)

  • Intussusception is common cause of intestinal obstruction in children. Most of intussusceptions can be treated with non-operative reduction using air or barium. However, about 10% patients need operative treatment due to failure of reduction, peritonitis, and recurrence after reduction. We introduce our experience of laparoscopic surgery for intussusception. From April 2010 to March 2013, we reviewed 57 children who diagnosed intussusception. Twelve patients underwent an operation. The cause of operation was 7 of failure of air reduction and 5 of recurrence after air reduction. Median age was 21.5 months (range: 5.0~57.7 months) and 11 children (91.7%) underwent successful laparoscopic reduction. Median operating time was 50 minutes (range: 30~20 minutes) and median hospital days was 4.5 days (range: 3~8 days). One patient had a leading point as a heterotopic pancreas and underwent bowel resection through conversion. There was neither intra-operative nor postoperative complication. Laparoscopic reduction for intussusception can bring an excellent cosmetic effect with high success rate.

  • When implementing a new information system, the success has been infrequent even though implementing a new IS can bring about various benefits to organizations. Employee resistance has been identified as a critical reason for such failure of implementing and operating a new IS. The vital role of orgazniational support with employee personality traits (e.g., tolerance for ambiguity and openness to expeirence) to reduce employee resistance has been missing when explaining to the failure of impleneting a new IS. The purpose of this study was to test a research model regarding employee resistnace, investigating the relationship bewteen organizational support and employee personality traits in that how organizational support influences employee resistance and also how employee personality traits (e.g., tolerance for ambiguity and openness to expeirence) interact with organizational support to decrease employee resistance the most. The results of this study supported our hypotheses and theoretical and practical implications are discussed.

  • In this study, software products developed in the course of testing, software managers in the process of testing software test and test tools for effective learning effects perspective has been studied using the NHPP software. The finite failure nonhomogeneous Poisson process models presented and the life distribution applied exponential and log shaped type hazard function. Software error detection techniques known in advance, but influencing factors for considering the errors found automatically and learning factors, by prior experience, to find precisely the error factor setting up the testing manager are presented comparing the problem. As a result, the learning factor is greater than autonomous errors-detected factor that is generally efficient model could be confirmed. This paper, a failure data analysis of applying using time between failures and parameter estimation using maximum likelihood estimation method, after the efficiency of the data through trend analysis model selection were efficient using the mean square error and coefficient of determination.

Clinical Experience of Multiple Valve Replacement (심장 다판치환술의 임상적 고찰)

  • From April 1982 to December, 1988, multiple valve replacement was performed in 49 patients. Mitral and aortic valve replacement were done in 42 patients, 4 underwent mitral and tricuspid valve replacement and 3 patients underwent triple valve replacement. Of the valve implanted, 50 were Duromedics, 21 St. Jude, 13 Bjork-Shiley, 9 Carpentier-Edwards, 6 Ionescu-Shiley, and 2 Medtronic. The hospital mortality rate was 28.5 % [14 patients] and the late mortality rate was 6.1 % [3 patients], the mortality rate was high in early operative period but decreased with time. [20% at 1986, 18.2 % at 1987, 9.5% at 1988] The causes of death were low cardiac output in 8, congestive heart failure in 2, multiple organ failure in 1, LV rupture in 1, intracerebral hemorrhage in 1 and sudden death in l. The actuarial survival rate excluding operative death was 77% at 7 years.

Surgical Experience on St. Jude Medical Valve Replacement (St. Jude Medical 기계판의 임상적 연구)

  • St. Jude Medical cardiac valve replacement was performed in 90 consecutive patients from Jan. 1984 to Dec. 1987. 54 had mitral, 12 had aortic and 24 had multiple valve replacement. Follow up extended for 1 to 47 months[mean 17.1 month] with a cumulative period of 1351 months. The overall actuarial survival rate at 4 years was 87.1% and overall hospital mortality was 6.7%. The linearlized incidence of thrombotic obstruction, thromboembolism, valve failure was 0.8% / pt. yr. each. The lower incidence of valve related mortality or morbidity was statistically significant. The performance of the St. Jude Medical mechanical valve was excellent compared to other substitute valves and has low incidence of thromboembolism or valve failure.

  • In general, circuit board assemblies experience various mechanical loadings during assembly and in actual use. The repeated cyclic bending can cause electrical failures due to circuit board cracks, solder interconnects cracks, and the component cracks. In this paper, we report on the failure characteristics of semiconductor chips under the repeated cyclic bending. We first describe a new 4-point bending tester, which is developed according to JEDEC standard No. 22B113. The performance of the tester is then estimated through actual experiments. Test results reveal that the cracks first occur on the outer balls around 20,000 cycles and gradually propagate to the inner balls where cracks are found around 70,000 cycles.

Clinical Experiences of St. Jude Medical Cardiac Valve Replacement (St. Jude Medical valve의 임상적 고찰)

  • 50 months experience with St-Jude Medical Cardiac Valve Prosthesis The St. Jude Medical valve has become our mechanical valvular prosthesis of choice because of favorable hemodynamic results that associated with marked clinical improvement and low incidence of thromboembolism. The data for this study was collected from April 1986 to May 1990, four years period. There were total of 110 patients[female 53, male 58] in this series with 22 isolated aortic valve, 66 isolated mitral valve, 20 double valve, 2 tricuspid valve replacement. The mean follow up time was 23 months. Postoperatively, 77% of cases were in New York Heart Association[NYHA] functional class I, and mild and moderate symptoms[NYHA II ] were present in 20% and there were very few patients remaining in higher functional classifications. In postoperative echocardiographic study showed marked improved cardiac function. The overall early mortality was 5.4% and was higher after double[13.3%] and mitral valve replacement[5.6%] and the late mortality was one case after mitral valve replacement due to endocarditis. The cause of death in early mortality was attributed to heart failure, acute renal failure, sepsis, etc.

  • This paper describes the experience gained from the treatments for prevention of blade failure occurred in the low-pressure turbine. Some cracks due to high cycle fatigue were found at the blades in low-pressure turbines after long time operation. Such failure was mainly caused by the resonance of the blade with the vane passing frequency excitation. If a natural frequency of the blade exists near the excitation frequency, a resonant vibration can occur and leads to a large amount of stress which may cause fatigue failures in turbine blades. To avoid the resonance of the blade, some modifications have been performed and full-scaled mockup testing has been done to confirm the verification for modification. Test result shows that enlarging the span cover is very useful to change the natural frequency of the grouped blades effectively.
