FMEA Measures for Service Failure Management (서비스 실패 관리를 위한 FMEA 이용 방안)

  • Purpose: This study identifies preventive measures for VOC management by analyzing the causes and effects of factors that contribute to high risk service failure using FMEA on KORAIL VOC data. Methods: Two research methods were used. First, a Risk Priority Number (RPN) was assigned to each KORAIL VOC based on Failure Mode and Effect Analysis (FMEA). Second, multiple regression analysis was run with RPN factors that include severity, occurrence, and detection as the independent variables and customer dissatisfaction as the dependent variable. Results: Multiple regression analysis showed that RPN factors including severity, occurrence, and detection had significantly positive relationship with customer dissatisfaction. Based on these results, an FMEA was performed on VOC categories with high RPN for railroad stations including platform, ticketing, ticket verification, parking, and escalator, and VOC categories with high RPN for trains including entrance doors, cafes, air quality, announcement, and ticket verification. Conclusion: This study has practical implications to service failure management. A priority order using FMEA was established for the list of customer dissatisfactions that should be addressed to actively manage service failure, and strategies for tackling this priority list are offered.

Reaction Wheel Momentum Dumping with Magnetic Torquer Failure (자기토커 고장시 반작용휠 모멘텀 덤핑)

  • High precision pointing satellite uses the reaction wheels for the attitude control and their momentum dumping is performed by the three magnetic torquers. In this paper, the effects of one magnetic torquer's failure on the momentum dumping will be reviewed. When the satellite on the high inclination angle orbit holds LVLH (Local Vertical Local Horizontal) attitude, pitch axis magnetic torquer failure causes the momentum dumping failure. But in case of other torquer's failure, momentum dumping is still possible with degraded dumping performance. When pitch axis magnetic torquer fails, momentum dumping is possible by changing the satellite attitude. This paper propose the satellite attitude change to make the momentum dumping possible when pitch axis magnetic torquer fails. In addition, if torquer arrangement is modified, momentum dumping is always possible regardless of any torquer's failure.

Hydriding Failure Analysis Based on PIE Data

  • Recently failures of nuclear fuel rods in Korean nuclear power plants were reported and their failure causes have been investigated by using PIE techniques. Destructive and physico-chemical examinations reveal that the clad hydriding phenomena had caused the rod failures primarily and secondarily in each case. In this study, the basic mechanisms of the primary and the secondary hydriding failures are reviewed, PIE data such as cladding inner and outer surface oxide thickness and the restructuring of the fuel pellets are analyzed, and they are compared with the predicted behaviors by a fuel performance code. In addition, post-defected fuel behaviors are reviewed and qualitatively analyzed. The results strongly support that the hydriding processes, primary and secondary, played critical roles in the respective fuel rods failures and the secondary hydriding failure can take place even in the fuel rod with low linear heat generation rate.

  • The mechanism of failure of lubricated surfaces at high sliding speeds was Investigated. Experiments were performed with the ball-on-flat and cylinder-on-flat geometries, using lubricants of four different chemical reactivities. Surface failure was found to not be predictable using the ratlo, $\lambda$, of fluid film thickness to composite surface roughness except when chemically inert lubricants are used. Even then the influence of temperature rise on fluid film thickness does not adequately explain the low load carrying capacity of lubricants at high sliding speeds. which causes surface failure. The protective layers on sliding surfaces that form by chemical reaction with the lubricant were found to reduce the surface roughentrig and Increase the load carrying capacity of surfaces to values of $\lambda$ as low as 0.03. Neither the surface toughening nor the formation of the protective layers have been incorporated Into failure models for lubricated systems.

  • Recently, the trauma patients have been markedly increasing due to the vast increase of traffic accident, industrial disaster, incidental accident and violence. The authors have analysed of 22 patients of thoracic injuries combined with abdominal injuries and summarized as follows. The ratio of male to female was 3.4:1 and their age distribution was from 5 years to 68 years and mean age was 34.4 years. The etiologies of injury were traffic accident, stab wound, fall down and violence. Associated injuries were fractures, bowel perforation, kidney rupture, head injury, liver laceration, spleen rupture and so forth. The modes of treatment were closed thoracostomy, repair of diaphragm, ruptured bowel repair, explo-thoracotomy, splenectomy, hepatic lobectomy in this order of frequency. The postoperative complications were atelectasis, wound infection, pneumonia, empyema, acute renal failure, respiratory failure and bleeding. The mortality rate was 13.6% [3/22 and the causes of death were respiratory failure 1 case, acute renal failure 1 case and hypovolemic shock 1 case.

  • Since the behavior of structural members subjected to blast load shows different responses, the effect of impulse as well as peak load should be considered in the damage analysis. The threshold on P-I diagram that causes specific damage level divides the diagram into the failure zone and the non-failure zones. In this study, numerical analysis is performed based on single-degree-of-freedom (SDOF) techniques to generate rational P-I diagram considering material non-linearity and dual failure modes (flexure and direct shear) of RC beams. From the comparison with existing test results it is concluded that proposed numerical method is good to derive failure mode of RC beam under blast load.

  • Power transmission lines have been playing a key role as the mainstay of national industry. When a power failure occurs, it can have severe effects on national security as well as national industry and economy. In this paper, we consider an insulator failure, which is one of the main causes of such a power failure. In spite of its importance, however, a shortage of manpower in the insulator maintenance field is getting more serious due to working environments with a high voltage and a high place. For this reason, a new active maintenance technique using a robot system is required to prevent such an insulator failure. In this paper, a new robot mechanism for insulator cleaning and inspection was developed. We confirmed its effectiveness through experiments.

  • Toluene is an aromatic hydrocarbon found in glues, cements, and solvents. It is known to be toxic to the nervous system, hematopoietic system, and causes acid-base and electrolyte disorders. Acute respiratory failure with hypokalemia and rhabdomyolysis with acute renal failure should be considered as potential events in protracted glue sniffing. We reported the case of 26-year-old woman was admitted to the emergency department with the development of respiratory failure and altered mentality due to hypokalemia after chronic glue sniffing. She was weaned from the ventilator 3 days later after potassium and sodium bicarbonate replacement and was discharged without respiratory symptoms and other complications.
