Two Case Reports of Facial Flushing of Women in Their Twenties Treated by Sasang Constitutional Medicine (사상방을 이용한 20대 여성 안면홍조 환자 치험 2례)

  • Objectives: The purpose of this study is to report the effect of Sasang constitutional medicine and acupuncture on facial flushing of women in their twenties. Methods: Two patients who had facial flushing were treated with Sasang constitutional medicine and acupuncture. In the first case, the patient diagnosed with Taeeumin was treated by Chungpesagan-tang and Sa-Am Liver-sedating Acupuncture. In the second case, the patient diagnosed with Soeumin was treated by Palmulgunja-tang, Sa-Am Spleen-tonifying Acupuncture and Seungyangsungi-tang which is not belong to Sasang constitutional medicine. The progress of symptoms was evaluated by NRS, frequency of facial flushing, duration time of facial flushing, amount of sweating accompanied by facial flushing. Results: After treatment, the patients' symptoms of facial flushing were improved. Conclusions: These case studies show that Sasang constitutional medicine and acupuncture are effective for the patient in their twenties who had facial flushing.

A Clinical Case Report of Taeeumin Patient Diagnosed with Systemic Contact Dermatitis and Facial Flushing (전신성 접촉피부염과 안면홍조를 호소하는 태음인 환자 치험 1례)

  • Objectives This study is to report a significant improvement in a Taeeumin patient with systemic contact dermatitis after eating urushiol chicken by herbal medicine treatment. Methods The patient had complaints of erythema, swelling, pruritus, scaly skin, and facial flushing. We treated the patient with an herbal medicine 'Galgeunhaegi-tang' for three months. We evaluated the treatment outcome of systemic contact dermatitis using the Three Item Severity(TIS) score every visit and facial flushing by Image color summarizer at the first and last visit. Results After the treatment, the severity of the patient's skin complaints lessened from moderate to mild. The Image color summarizer showed a minor decrease in the normalized red color level and a significant increase in the brightness level and facial color percentage. Conclusions The patient diagnosed with systemic contact dermatitis, treated with Galgeunhaegi-tang for three months, showed a significant improvement in skin complaints with brighter and even facial color.

A Case Report of Erythematotelangiectatic Type Rosacea and Peripheral Facial Palsy Improved by Korean Medicine Treatment with Soshioho-tang Gagambang (소시호탕가감방(小柴胡湯加減方) 등 한방 치료를 통해 호전을 보인 홍반-혈관확장형 주사피부염과 말초성 안면마비 치험례)

  • Objectives : The purpose of this study is to report a case of erythematotelangiectatic type rosacea and peripheral facial palsy improved by Korean medicine treatment with Soshioho-tang gagambang. Methods : The patient visited our clinic due to facial flushing and peripheral facial palsy symptoms. Based on Sanghallon provision, we treated this case with Soshiho-tang gagambang. The result of treatment was evaluated by Visual Analogue Scale(VAS), Dermatology Life Quality Index(DLQI) and House-Brackmann Grading System(HBGS). Results : After 12 days of taking Soshiho-tang gagambang, VAS of rosacea was decreased from 9 to 4 and DLQI was decreased from 22 to 5. After treatment, rosacea with facial flushing was improved and HBGS was reduced from 3 to 1. Conclusions : This study shows the possibility of taking Soshiho-tang gagambang for erythematotelangiectatic type rosacea and peripheral facial palsy derived from stress, fatigue and sleep disturbance according to Sanghallon provision.

  • The number of allergic contact dermatitis patients is steadily increasing but a proper reliability treatment guide is not prepared in Oriental Medicine. Also, the number of clinical cases on allergic contact dermatitis is not sufficient in Oriental medicine. As such, the facial flushing of allergic contact dermatitis which has been improved by infra-red therapy is significant in its treatment purpose. We reported on 43-year old male patient with allergic contact dermatitis. We treated him with herbal concoction, acupuncture, infra-red therapy and cupping therapy. We have received a good result in this case by using the combination of infra-red therapy. We reached a conclusion that using infra-red therapy can improve the facial flushing of patient with allergic contact dermatitis. However, further research on the effect of infra-red therapy for these symptoms is desired.

  • The climacteric is the period of passage out of the reproductive stage of life and into the nonreproductive phase. The climacteric can be accompanied by wide-ranging, uncomfortable and disruptive symptoms. Symptoms that are related to hormonal changes include abnormal bleeding, facial flushing, and urogenital discomfort. This patient was 45- year-old woman who had headache, facial flushing, thirst, fatigue, irregular menstruation, urogenital discomfort and had been diagnosed and teated climacteric. We classified this patient as Soyangin(少陽人) Yin collapsed ease(亡陰證) and prescribed Hyeongbangsabaeksan(荊防瀉白散). We experienced the good effect of Hyeongbangsabaeksan on Soyangin Mangyin-disease We report the healing process and the result of treatment in this study.

Pilot Study of a Brief WeChat Intervention in China to Increase Students' Willingness to Assist a Flushing Student to Reduce Alcohol Use

  • Objectives: This pilot study tested the effectiveness of a brief alcohol-related intervention delivered by the social media app WeChat to teach about ethanol-induced facial flushing and increase the willingness of students who see another student flushing to suggest that he or she should reduce or stop drinking. In the context of Chinese drinking culture, it is sometimes socially difficult to refuse a drink, even when experiencing physical discomfort, such as flushing. Methods: Classrooms of students in a medical university in China were randomly assigned to the intervention or control group. Students in the intervention group were invited to view 3 alcohol education lessons on WeChat during a 2-week period. A pretest and posttest before and after the 2-week period assessed changes in students' willingness to intervene if they saw someone flush while drinking. Data were collected about students' alcohol use and their ratings of the lessons. Results: Mixed-design analysis of variance yielded a significant time-by-treatment interaction effect on the variable of willingness to suggest that a flushing person stop or slow down their drinking, and the change was significant between the intervention and control groups. One-way analysis of covariance yielded a significant treatment effect at the posttest, after controlling for the pretest score. Students rated the lessons above the midpoint of the scale for being informative, interesting, and useful. Conclusions: The pilot study showed that a brief alcohol-related intervention delivered by WeChat could produce a measurable positive change in the willingness of university students to suggest that a student who flushes should stop drinking. This pilot study also suggested improvements for future lessons and evaluation design.

Superior Vena Cava Syndrome Bue to Thyroglossal Duct Cyst (갑상설관낭에 의한 상대정맥증후군 -1례 보고-)

  • This is a case report of superior vena cava syndrome due to thyroglossal duct cyst, which was surgically treated. The patient was 61 year old male who progressively complained exertional dyspnea for about 5 months and right ptosis, facial flushing, and nasal stuffiness for about 1 month before admission. The CT scan of the thorax revealed the right paratracheal cystic mass that compressed and displaced the trachea to leftward and SVC to rightward. The resection of the cystic mass was performed through the right posterolateral thoracotomy. The pathologic result was compatible with thyroglossal duct cyst. The postoperative status of the patient was uneventful.

  • Persistent facial telangiectasia, erythema and flushing are the major cosmetic problems in patients with rosacea. However various therapeutic modalities for rosacea are not effective. Five patients who had a history of rosacea which poorly controlled by oral and topical medications, were given Herb-medication and Acupuncture treatment. The size of erythema and the number of papules had been gradually decreased and finally all the symptoms were gone except telangiectasia.

Analysis of Pathogenic Factors in the Menopausal Symptoms of Middle-aged Women in Relation to Sasang Constitutional Type

  • Background: Pattern identification is a unique diagnostic method of traditional Oriental medicine that has recently been the target of questionnaire-based research. Sasang (four-types) constitutional medicine (SCM) is a practice in traditional Korean medicine that seeks to promote objectivity in diagnostics. This paper attempts to illuminate the relationship between constitutions and the pathogenic factors of pattern identification through questionnaires completed by menopausal women about their symptoms. Methods: From March to October 2012, we examined 291 women from the general population, with ages ranging from 40 to 60 years, applying the Kupperman index, the Menopause-Specific Quality of Life Questionnaire (MENQOL), pattern identification based on the Diagnosis System of Oriental Medicine (DSOM), and SCM. We then analyzed the relationship between constitutional type and pathogenic factors. Results: No significant differences were found in the scores of either the Kupperman index or MENQOL questionnaire in relation to constitutional type. However, in a statistical analysis correlating the DSOM pathogenic factor scores (PFS) with the scores of the Kupperman index and MENQOL vasomotor subscale, heat showed a significant positive correlation with SoYang type (SY) and TaeEum type (TE), but not SoEum type (SE), while insufficiency of yin and insufficiency of yang, as well as blood deficiency, showed a significant positive correlation with the TE and SE types. Conclusion: The pathogenic factors in the menopausal symptoms of middle-aged women, specifically the prominent menopausal symptom of facial flushing, differed significantly according to constitutional type.

Clinical Trial for the Heat-Rising Action of Ginseng and Cultivated Wild Ginseng to The Subject Diagnosed as Heat Pattern by Cold-Heat Patternization (한열변증을 통한 열증 대상자에 산양삼과 재배인삼의 상부 승열 작용에 대한 인체적용시험)

  • Objectives: The aim of this trial is to investigate the effect of Ginseng and Wild Cultivated Ginseng to Heat pattern subject. Methods: Eighty-nine Subjects were diagnosed as heat pattern by Cold-Heat Patternization and divided into Ginseng group (n=30), Wild Cultivated Ginseng group (n=31) and Placebo group (n=28) in their 1 st visit. In each visit, The researchers measured the subject's facial temperature using the infrared thermometer (Testo 835-T1). After that, The subjects were asked to mark the current score of flushing on the Visual Analogue Scale (VAS) and to complete the Chalder-Fatigue Scale (CFS) in each visit. The subjects took the test drug for one week and returned the remaining drug on the 2nd visit. The trial result was analyzed with one-way ANOVA using SPSS for Windows version 18. Results: 1. Systolic blood pressure was significantly lower in the Ginseng group and Wild Cultivated Ginseng group than in the control group (p=0.021). 2. There was no significant difference in facial temperature between each groups. 3. The current score of flushing showed the greatest decrease in the Ginseng group compared to the other groups but there was no significant difference (p=0.205). 4. The score of Chalder-Fatigue Scale was decreased in all groups but not statistically significant (p=0.180). Conclusions: This study showed that taking Ginseng extract and Wild Cultivated Ginseng extract do not affect to heat-rising reaction to the subjects diagnosed as heat pattern.
