Objectives : While there are many studies about treatments of facial palsy, no study has been performed on general population of Korea, especially concerning about comparison between western medicine and oriental medicine. This study aimed to investigate magnitude of health visits and treatment patterns for Korean patients with facial palsy through the computerized database of Health Insurance Review and Assessment Service(HIRAS). Methods : According to the HIRAS database over 5 years' period from 2004 to 2008, the medical records of patients with facial palsy as a main diagnosis were extracted. Inclusion criteria of facial palsy are Bell's palsy(G510), Geniculate ganglionitis(G511), Melkersson's syndrome(G512), Other disorders of facial nerve(G518), Disorder of facial nerve, unspecified(G519) in western medicine. And Paralytic face(G016), Deviated eye and mouth(J01), The other facial palsy(J013) were included in oriental medicine. We compared the claim number of western medical care with that of oriental medicine treatment by year and month. Results : The total claim number of facial palsy was increasing on both western medicine and oriental medicine from 2004 to 2008. In western medicine, the claim number of Bell's palsy(G510) is the most. In oriental medicine the inpatients claim number of Deviated eye and mouth(J01) is the most, while outpatients claim number of the other facial palsy(J013) is the most. Conclusions : Medical database of HIRAS provided comprehensive and vast information on epidemiologic characteristics and treatment, which can be more reliable data to expect medical demand for facial palsy in condition that accurate diagnosis and standardized treatment is delivered in clinical settings.
Objective: The purpose of this study was to determine how gaze angle affects muscle activity and kinematic variables during treadmill walking and to offer scientific information for effective and safe treadmill training environment. Method: Ten male subjects who have no musculoskeletal disorder were recruited. Eight pairs of surface electrodes were attached to the right side of the body to monitor the upper trapezius (UT), rectus abdominis (RA), erector spinae (ES), rectus femoris (RF), bicep femoris (BF), tibialis anterior (TA), medialis gastrocnemius (MG), and lateral gastrocnemius (LG). Two digital camcorders were used to obtain 3-D kinematics of the lower extremity. Each subject walked on a treadmill with a TV monitor at three different heights (eye level; EL, 20% above eye level; AE, 20% below eye level; BE) at speed of 5.0 km/h. For each trial being analyzed, five critical instants and four phases were identified from the video recording. For each dependent variable, one-way ANOVA with repeated measures was used to determine whether there were significant differences among three different conditions (p<.05). When a significant difference was found, post hoc analyses were performed using the contrast procedure. Results: This study found that average and peak IEMG values for EL were generally smaller than the corresponding values for AE and BE but the differences were not statically significant. There were also no significant changes in kinematic variables among three different gaze angles. Conclusion: Based on the results of this study, gaze angle does not affect muscle activity and kinematic variables during treadmill walking. However, it is interesting to note that walking with BE may increase the muscle activity of the trapezius and the lower extremity. Moreover, it may hinder proper dorsiflexion during landing phase. Thus, it seems to reasonable to suggest that inappropriate gaze angle should be avoided in treadmill walking. It is obvious that increased walking speed may cause a significant changes in biomechanical parameters used in this study. It is recommended that future studies be conducted which are similar to the present investigation but using different walking speed.

Cat eye syndrome (CES) is a very rare chromosomal syndrome characterized by various malformations such as anal atresia, preauricular malformation, coloboma of the iris, and congenial heart and renal defects. This genetic disorder is caused by partial duplication of chromosome 22, mostly as a result of a supernumerary isodicentric marker chromosome idic(22)(q11.2). Various congenital abnormalities and extreme phenotypic variability in CES patients have been reported, which have made prenatal diagnosis of CES difficult. We report the first case diagnosed with CES prenatally by multiplex ligation-dependent probe amplification in a woman who was referred to our hospital, for a fetus presenting with heart anomaly.
Background: Eye movement desensitization and reprocessing (EMDR) therapy has been reported to be very efficacious for treating post-traumatic stress disorder (PTSD) and other anxiety-related conditions. However, a review of the literature reveals the sparse use of this therapy in the field of pediatric dentistry. This study aimed to evaluate anxiety trends in pediatric dental patients during local anesthesia and extraction with and without EMDR therapy. Methods: Children in the age range of 8-12 years who required dental extractions were assigned randomly into two groups: an EMDR group (group 1) and a routine behavior management therapy group (group 2; receiving more traditional interventions such as tender love and care behavioral modeling, and distraction). Anxiety scores were recorded at four levels using the visual facial anxiety scale (VFAS) preoperatively, after therapy, after the administration of local anesthesia (LA), and after extraction. Results: Reduced anxiety was observed after the delivery of EMDR therapy, after LA administration, and post-extraction in the EMDR group compared to pre-operative anxiety scores of anxiety (P < 0.001; unpaired Student's t and Mann-Whitney U tests). In the control group, mild reductions in anxiety after routine behavior management therapy were observed, accompanied by spikes in anxiety levels after LA and extractions. Conclusion: EMDR therapy was found to be valuable for reducing anxiety among pediatric dental patients during tooth extraction procedures.
틱의 치료에 있어서 양방적으로는 일과성 틱 장애가 아닌 만성 틱장애, 뚜렛 장애의 경우에는 약물치료가 대개 시행된다. 주로 사용되는 약물은 도파민 수용체 길항제이거나 이에 반응하지 않는 경우 클로니딘, 구안파신 등의 성인의 고혈압 치료에 쓰이는 약물을 쓰기도 하고, 또는 항우울제를 쓰기도 한다고 한다. 본 증례의 환자는 처음 양방정신과 방문 후 상기 처방 등을 투여받았으나 부작용(자꾸 정신이 몽롱해지고, 잠이와서 일상생활이 불가능해지는 문제)으로 한방치료를 원한 경우였다. 처음 내원했을 때의 상태는 불그스름한 얼굴, 심한 현맥, 근긴장 및 계속해서 틱증상을 보이는 양상 등을 보였고 운동틱의 정도가 매우 심하여 간주근(肝主筋)의 원칙에 의하여 간양화풍(肝陽化風)의 기전으로 환자의 상태를 변증할 수 있었기에 억간산(抑肝散)을 투여하기로 하였고 아울러 복진상(腹診上) 좌측제부동계항진(左側臍部動悸亢進)이 보여 간열(肝熱)로 신경(神經)이 흥분하여 위장애(胃障碍)가 나타나서 생기는 위내정수(胃內停水)의 소견을 보였기 때문에 진피(陳皮), 반하(半夏)를 가(加)하여 투여하였다. 아울러 YGTSS를 시행한 바 환자가 내원 초기(2003년 6월 9일) 에 시행한 YGTSS의 점수는 30점이었고, 어느 정도 증상의 호전을 보여 폐약(閉藥)을 원했던 시기(2002년 7월 24일)의 YGTSS에서는 11점으로 증상의 상당한 호전을 보였으며 약을 중지한 이후에도 지속적으로 증상의 호전이 되었다 하면서 다시 내원한 2003년 9월 22일에는 약 20분동안의 상담시간동안 거의 틱증상의 발현을 볼 수 없었다. 2003년 10월 20일 마지막으로 시행한 YGTSS의 점수는 3점으로 거의 완치에 가까운 결과를 보여주고 있다. 이는 억간산(抑肝散) 가(加) 진피(陳皮) 반하(半夏)의 처방을 투여함으로서 환자의 간열(肝熱)을 지속적으로 내려주었고, 아울러 상담을 통하여 원만한 가족관계를 유지할 수 있도록 도와주어 환자에게 자꾸 간기(肝氣)의 항진(亢進)을 야기시키는 원인인 가정불화를 어느정도 완화시켜 주었기 때문이라고 사료된다. 지금까지 만성 틱장애나 뚜렛 장애에 대하여는 양 한방적으로 뚜렷한 효과를 보이는 증례를 많이 발견할 수 없는 현실에서 본 증례는 상기 질환에 대한 한방치료의 가능성을 보여주는 예라고 판단되어 보고하는 바이다.
Objectives : The aim of the present study was to examine whether anxiety and depression of children and mothers were associated with the children's medically unexplained somatic symptoms in attention-deficit/hyperactivity disorder(ADHD). Methods : 83 clinic-referred boys with ADHD and 52 boys without ADHD were included in this study. The frequency of the medically unexplained somatic symptoms, such as general-ache, headache, nausea, eye problems, skin problems, abdominal pain, vomiting was evaluated using the somatic symptom domain of the Child Behavior Checklist(CBCL). Children's anxiety and depression were evaluated using Kovacs Children's Depression Inventory(CDI) and Spielberger's State-Trait Anxiety Inventory(STAI) for children. Maternal anxiety and depression were measured by Spielberger's State-Trait Anxiety Inventory(STAI) and Beck's Depression Inventory(BDI). Stepwise linear regression analysis was used to examine the hypothesis. Results : Score of maternal state anxiety affects the T score of the somatic symptom domain in CBCL significantly(adjusted $R^2$=0.057 ; p=0.026). Conclusion : The frequency of medically unexplained somatic symptom of children perceived by mothers was associated with the high level of maternal state anxiety in ADHD. Frequent maternal report of the children's somatic symptom may be a warrant for the evaluation and management of the maternal state anxiety in ADHD.
Objectives: The purpose of this study was to identify the needs of bio-signal devices for the diagnosis, assessment, and analysis of neurocognitive disorder in Korean medicine (KM) hospitals and clinics. Methods: A questionnaire was developed to survey the current status of medical device use, and diagnosis and interventions for patients with cognitive disorders in KM hospitals and clinics. November 11~December 2, 2019, 114 responses (71.9% completed) were collected by internet-based questionnaires from the members of the Korean society of Oriental Neuropsychiatry. Results: The clinical requests were in the descending order of hematology analyzer, ultrasound imaging system, and electroencephalography among the 15 most commonly used devices of which research would support for their clinical usability. The biosignal-based devices showed the highest research demands for patients with mild cognitive impairment rather than more severe stages of cognitive impairment. Prevention rather than diagnosis, or several treatment regimens was the strongest clinical area of the KM for patients with neurodegenerative cognitive impairment. Many responded that five to 10 minutes of test duration and 20,000 won to 30,000 won of cost would be appropriated for a new device to be developed. Conclusions: There were strong demands for the development of bio-signal devices for neurocognitive disorders among the KM doctors. Specifically, it showed high needs for the technology that can be used in the prevention area of cognitive disorders. Additionally, new medical devices to assess cognitive functions and to obtain KM pattern-related information were the high needs.
Objectives: This study examined the case of a 69-year-old man with a history of stroke and Alzheimer's disease who had been diagnosed with probable-rapid eye movement sleep behavior disorder (probable-RBD). Methods: The patient was treated with herbal medicine (Ukgan-san, Ukgansangayonggolmoryeo-tang, and powdered Gamisoyo-san extract), Western medicine (clonazepam, antiplatelet, psychotropic agents, antihypertensive drugs, and others), and acupuncture. Their effects were evaluated by the frequency and severity of sleep-related behavioral symptoms. Results: After treatment, the observed frequency and severity of sleep-related behavior decreased. Conclusion: The results suggest that using traditional Korean medicine with clonazepam can be effective in the treatment of patients with probable-RBD.
목적: 본 연구의 목적은 신체화 평가 척도를 개발하고자 하는 데 있다. 방법: 일차로 성인 정상인 109명을 대상으로 신체화 평가 척도 예비설문을 시행하여 40개 문항을 추출하였다. 2차로 이 문항들이 포함된 설문을 정상인 215명을 대상으로 실시하였다. 결과: 이 자료들을 요인분석한 결과 5개의 하위요인 즉 심장호흡기 및 신경계 반응, 신체적 과민반응, 위장관계 반응, 일반적 신체반응, 비뇨생식기 눈 및 근육계 반응이 추출되었다. 상기 척도의 신뢰도는 정상인 62명에게 2주 간격으로 검사-재검사를 시행, 분석한 결과 5개 하위척도 점수와 척도 전체 점수 간의 상관계수가 .86~.94로 모두 유의한 상관성올 보였다. 한편 내적 일치도는 5 개 하위척도의 Cronbach's ${\alpha}$가 .72~.92, 척도 전체의 Cronbach's ${\alpha}$가 .95이었다. 공존타당도는 global assessment of recent stress (GARS) scale, perceived stress questionnaire (PSQ), 스트레스반응척도 총점, symptom checklist-90-R(SCL-90-R)의 신체화 척도를 비롯한 하위척도와 전체지표 각각의 총점과 본 척도의 5개 하위척도의 점수 및 척도 전체점수 간의 상관성을 각각 비교한 결과 모두 유의하게 높은 것으로 나타났다. 변별타당도는 환자군 242명(불안장애 71 명, 우울장애 73명, 신체형장애 47명, 정신신체장애 51명)과 정상군 215명 간에 척도의 총점과 하위척도 점수를 비교한 결과 5개 하위척도 점수와 총점에서 각각 유의한 차이를 보였다. 각 장애군이 정상군보다 척도 전체점수가 유의하게 더 높았으나, 우울장애군만이 하위척도 점수 및 척도 전체점수 모두에서 정 정상군보다 유의하게 높게 나타났다. 또한 신체적 과민반응에서는 우울장애군만이 정상군보다 점수가 유의하게 더 높고 일반적 신체반응에서는 우울장애 및 신체형장애군만이 정상군 보다 유의하게 더 높았다. 대상자들에서 여자가 남자보다 척도 전체 점수가 유의하게 더 높았다. 결론: 이상의 결과들은 신체화 평가 척도가 신뢰도 및 타당도가 모두 유의한 수준으로 스트레스 및 신체화에 관련된 분야의 연구에 효과적으로 이용될 수 있는 도구임을 시사해 준다 또한 우울장애 및 신체형장애군이 불안장애 및 정신신체장애군에 비해 더 광범위한 신체화 경향을 보임을 시사해 준다.

Cornelia de Lange 증후군은 여러 신체 기관의 선천적 기형과 성장지연, 신경 발달학적 이상을 동반하는 질환이다. 어떠한 생화학적 혹은 염색체 표지자도 아직까지 Cornelia de Lange 증후군에 특이적인 소견은 발견되지 못하였고 진단은 임상적인 관찰에 전적으로 의존하게 되어있다. 따라서 임상가의 경험이 본 질환의 발견에 중요한 만큼 저자들은 정신지체와 자폐장애를 보이는 Cornelia de Lange 증후군 사례를 보고하는 바이다. 본 증례의 환아는 6세 4개월의 여환으로 걷기 등의 운동발달이 늦었다고 하며 현재까지도 한단어도 말하지 못하는 심각한 언어지체를 보였다. 또한 눈 맞춤이 빈약하고 타인과 정서적 교류가 되지 않는 등 사회적 상호작용의 저하와 상동행동을 보였으며 따라서 정신지체와 자폐장애로 진단 할 수 있었다. 환아는 저신장을 보였고 눈썹이 짙고 속눈썹도 길고 짙었으며 치아 간격이 넓고 코 끝이 들려 있었으며 입주위에 수염이 나 있었다. 낮은 헤어라인을 보였고 몸통과 사지에 다모증을 보였으며 손가락은 짧은 편이었다. 방광 요관 역류와 잦은 상기도 감염과 폐렴의 과거력이 있었고 4차례 열성경련이 있었다. 본 증례의 경우 특징적인 얼굴 모습과 성장발달지연, 정신지체, 행동양상, 동반된 내과적 질환의 병력 등으로 Cornelia de Lange 증후군으로 진단할 수 있었다.
