말똥가리의 상완골 골절에 있어서 외부고정술 일례 (A Case of External Fixation for Humeral Fracture in a Common Buzzard (Buteo buteo))

  • Authors introduce a case of successful orthopedic surgery for humeral fracture of Common buzzard (Buteo buteo). The bird, juvenile male, was rescued due to illegal gun shot in Yeoncheon-Gun, Kyonggi-Do. The external skeletal fixation (ESF) with intramedullary pinning (Tie-in method) using Imex clamps was chosen for this fracture case. The common buzzard was anesthetized with hand made face mask and Isoflurane. In particular, the rescued bird was needed rehabilitating procedures, such as flight training or physical exercise for their survivals in wild habitats. The injured buzzard is successfully recovered without any damage to ligaments and nerves after 6 weeks.

개의 전배측 고관절 탈구에서 변형된 외골격 고정 적용 5례 (Modified External Skeletal Fixation for Craniodorsal Coxofemoral Luxation in 5 Dogs)

  • Luxation of the hip is the most common luxation seen in small animals. Luxations are classified by the direction in which the femoral head lies in relation to the acetabulum and craniodorsal luxations are the most common type. Recommendations for treatment are based on the presence of preexisting disease, the type and duration of luxation and severity of concurrent injuries. In this study modified external skeletal fixation was used for craniodorsal coxofemoral luxation in 5 dogs. Luxated head Joint was reduced and fixator pins were applied into the greater trochanter, the middle point of iliac body and the lateral surface of sacroiliac joint in a closed fashion. Reduction was successful in 4 of the 5 operated limbs. Complications noted in this study included pin tract drainage, pin loosening and secondary fracture at the pin insertion site.

고라니에서 발생한 손허리뼈 개방 복합골절의 원형 외부골격고정법 적용 (Management of an Open Comminuted Fracture of the Metacarpus using Circular External Skeletal Fixation in a Korean Water Deer (Hydropotes inermis argyopus))

  • 고라니가 교통사고 후 구조되었다. 임상검사와 방사선검사에서 손허리뼈에 심한 연부조직손상을 동반한 3형의 개방골절로 진단되었다. 골절은 4개의 원형 외부고정 링을 이용하여 비개방정복으로 비침습적으로 정복 후 고정하였다. 술 후 고라니는 성공적으로 자연으로 복귀되었다. 고라니에서 원형 외부골격고정술을 이용하여 심한 감염성 상처와 개방골절을 치료 할 수 있었다. 원형 외부골격고정술은 고라니에서 부작용 없이 사용할 수 있을 것으로 사료된다.

Application of epoxy putty external skeletal fixator for stabilization of tarsocrural arthrodesis in small dogs and cats

  • Two small-breed dogs and two cats, with an average body weight of 4.88 kg (range: 4.3-5.5 kg), suffered hindlimb lameness due to luxation with or without fractures of the tarsocrural joint. These patients underwent tarsocrural arthrodesis with epoxy putty external skeletal fixator. The animals' skins were incised minimally, and the articular cartilage of the tarsocrural joint was removed, followed by autogenous cancellous bone grafting. Epoxy putty and positive Centerface®, pins with diameters 1.2 mm and 2.0 mm, were used for connecting bar and as a full pin fixation, respectively. All the patients regained the ability to bear weight on the affected limb within 3-7 days and resumed a normal gait within 9-15 weeks. The external skeletal fixator frame was removed within 13-17 weeks without major complications. Tarsocrural arthrodesis using epoxy putty external skeletal fixator resulted in excellent outcomes without severe postoperative complications in this study. Epoxy putty external skeletal fixator can be a valuable surgical option for tarsocrural arthrodesis in patients weighing less than 5.5 kg.

하악골 수직골절단술시 MINI-HOFFMANN SETS을 이용한 하악 과두의 중심위의 보존 (CONSERVING THE CENTRIC RELATION POSITION OF CONDYLAR HEAD WITH MINI-HOFFMANN SETS IN VERTICAL RAMUS OSTEOTOMY)

  • 악안면 기형에 대한 활발한 치료가 행해지고 있는 가운데 악교정술 후에 발생하는 회귀현상(Relapse)에 대해서는 아직도 많은 연구가 진행되고 있다. 특히 하악골 전돌증으로 후방이동술을 시술받은 환자의 경우에 회귀의 원인으로 Schendel, Epker, Lake, Worms, Ive, Poulton과 Ware등 많은 학자들이 하악과두의 부적합한 위치를 강조하였다. 하악골 시상골절단술시에는 이미 Leonard(1976), Zecha 등 (1978)이 상악의 선부자에 acrylic과 wire 또는 retainer 등을 이용하여 하악근심 골편의 보존을 시도하였다. 이에 저자는 하악골 전돌증의 후방 이동량이 큰 경우나 심한 안면 기형이 있는 19명의 환자에 대해 하악지 수직골절단술을 시술한 경우에 주로 정형외과에서 사용하는 External Skeletal Pin Fixation인 Mini-Hoffmann Sets을 이용하여 하악과두의 중심교합위 보존에 도움을 주었기에 보고하는 바이다.

Factors Affecting the Healing of Radial Fractures Using Acrylic Pin External Fixation in Small Breed Dogs

  • Healing of 48 cases of radial fractures in small breed dogs treated with acrylic pin external fixation was evaluated retrospectively to investigate the factors affecting the healing of radial fractures. The mean age of the subjects was 15.02 months, the mean body weight was 3.48 kg, and the mean maximum length of the radius (MLR) was 90.77 mm. External skeletal fixation frames type 1a, 2a, and 3 were used; among them type 2a was used most frequently (75%). The mean time to clinical union (TCU) was 67.17 days, and the success rate was 95.8%. Age, bodyweight, MLR, and occurrence of complications increased TCU significantly. Among complications, pin loosening, swelling of the operated forelimb tissue for more than 2 weeks, and coexistence of two or more complications increased TCU significantly (p < 0.05). There was no relationship between location of the fracture and TCU. The results of this study may be useful to predict the prognosis of radial fractures in small dogs.

한우 송아지의 정강뼈 성장판 골절에서의 교차핀과 임시 외부 골격 고정장치의 이용 (Use of Cross Pins and Temporal External Skeletal Fixator for Stabilization of a Tibial Physeal Fracture in a Korean Native Calf)

  • 한달 된 한우 수송아지가 오른쪽 뒷다리 체중부하를 못하는 문제를 가지고 전북동물의료센터에 내원하였다. 방사선검사와 CT 검사 상에서 Salter-Harris III형의 먼쪽 정강뼈 성장판 골절로 진단하였다. 개방골절정복 후 교차핀을 이용하여 고정한 후 임시 외부 골격 고정장치를 이용하여 5주 동안 추가적인 지지를 실시하였다. 수술 5주 후 방사선사진 검사에서 골절된 뼈의 유합이 관찰 되었다. 수술2년 후 과도한 움직임 뒤에 미약한 체중부하 파행이 관찰 되었지만, 거의 정상 수준으로 보행이 가능하였다. 송아지에서 이와 같은 증례는 매우 드물고 교차핀과 추가적인 임시 외부 골격 고정장치를 이용한 골절 정복법은 먼쪽 정강뼈 골절 치료에 유용하게 사용될 거라고 생각 된다.

Long Bone Fractures in Raptors: 28 cases (2004-2007)

  • Medical records from the Veterinary Medical Teaching Hospital of the University of Missouri-Columbia from 2004 to 2007 were available for 28 raptors that underwent long bone fracture repair. There were 14 owls, 10 hawks, 2 vultures, 1 eagle, and 1 falcon. Mean body weight was 780 g (ranged from 150 to 1400 g) for 14 owls; 650 g (ranged from 150 to 1270 g) for 10 hawks; 1760 g (ranged from 1520 to 2000 g) for 2 vultures; 5000 g for 1 eagle; and 130 g for 1 falcon. Of all 28 fracture cases, 11 cases (39%) and 1 case (3%) were related to hit-by-car and shooting respectively. Physical examination revealed dehydration in 18 raptors (64%) and lethargy in 12 raptors (42%). Forty one long bone fractures were included in 28 cases. The radiographs revealed 13 ulnar fractures (32%), 12 humeral fractures (30%), 10 radial fractures (25%), 4 tibiotarsal fractures (9%), 1 femoral fracture (2%), and 1 fibular fracture (2%). External skeletal fixation using polymethylmethacrylate (PMMA) combined with intramedullary fixation was used in 19 long bone fractures (46%). Intramedullary fixation using intramedullary Kirschner pin was used in 16 long bone fractures (39%). No surgical treatment was performed in 6 long bone fractures (15%). This study reported that many of raptors presented dehydration and lethargy when admitted for treatment. Therefore, proper hydration and nutrition are critical pre-surgical requirements. In addition, combination of internal fixation and external skeletal fixation using PMMA might be better option to treat raptors with comminuted fracture that results from mostly trauma of hit-by-car.

개에서 외부고정법을 이용한 양측성 하악골절의 치유 일례 (A Case of External Skeletal Fixation for Treatment of Bilateral Mandibular Fracture in a Dog)

  • A 5-year-old, male mixed breed dog was presented to Gyeongsang National University Animal Hospital for bilateral mandibular open fracture by bite. The dog had loss of the teeth and injury of the oral mucosa. Radio-graphic and blood examinations were conducted. Fracture lines were observed between first premolar and second premolar, but temporomandibular luxation was not observed on radiographs. In serum analysis, elevated levels of CPK and LDH were found. The dog was anesthetized and prepared for aseptic surgery. Acrylic external skeletal fixator with Kirschiner wire was applied. Pin loosening and malalignment of the right fracture line was observed at 7 weeks after surgery. We applied Kirschner-Ehmer fixator and half pins during the second surgical procedure. At 4 weeks after a second surgical procedure, the fixator and pins were removed from mandible. The fractures were healed with no complications.
