근로자 노출평가제도 내 위험성평가 방법론의 적용 (The Adoption of Risk Assessment Methodology in Exposure Assessment)

  • Objectives: Exposure Assessment for workplace hazards where the exposure level is below occupational exposure limits(OELs) has been performed without considering either the degrees of risk or exposure levels and has failed to lead to intervention in many cases. The objective of this study was to suggest and test an application framework for risk assessment methodology under the current exposure assessment system in Korea. Materials: First, we investigated the exposure assessment systems in Korea and other countries. To adopt some risk assessment techniques, we also analyzed risk assessment systems and compared them to exposure assessment systems. A few suggestions were made. We held a public hearing during an industrial hygiene conference and took surveys using a questionnaire. Results: The first suggestion was to implement the risk assessment and exposure assessment through a "one-stop" system. In that case, one expected question would be who has been doing the jobs so far. In most cases, industrial hygiene consulting services or laboratories have been performing exposure assessment for business owners. Business owners are required to perform risk assessment. As two different groups of people will be required to implement two things in a one-stop system, they need to share information. As an information vehicle to share information, commonly filed survey checklists were suggested. The second suggestion was to categorize exposure level into four groups instead of the current binary divisions based on OELs. In the risk assessment system, exposure level is divided into four groups utilizing the cut-points of 10%, 50%, and 100% of OELs. The same schema can be adopted in the exposure assessment system and different levels of requirements can be assigned for each group. The third suggestion was regarding the regulation system. To provide the suggestions some thrust toward being implemented in the field, changes should be made in the legal system. Two different types of new exposure assessment result reporting forms were suggested. Some investigations such as an ergonomic survey are officially accepted as risk assessment under the current legal system. A few items were suggested to be included in the exposure assessment result reporting to be accepted as risk assessment. A pilot study in two small factories was performed and pointed out the strengths and weakness of our suggestions. Conclusions: Discussions and studies on the improvement of the exposure assessment system have been held for decades and no tangible changes have yet been made. We hope this result can help realize healthy lives for workers in Korea.

노출 모델의 화평법 적용성: ECETOC TRA와 Stoffenmanager Tier 1 노출 모델을 활용한 벤젠의 작업자 노출 평가 (Occupational Exposure Assessment for Benzene Using Exposure Models (ECETOC TRA and Stoffenmanager) and Applicability Evaluation of Exposure Models in K-REACH)

  • Objectives: The objectives of this study are to estimate the inhalation exposure level of benzene for workers using Tier 1 exposure models ECETOC TRA (European Center for Ecotoxicology and Toxicology of Chemicals Target Risk Assessment) and Stoffenmanager, and to investigate their reliability for exposure assessment in K-REACH. Methods: Two exposure scenarios, 'manufacture of benzene' and 'use as solvents,' were developed for assessment of workers' exposure to benzene. The Process Category (PROC) for ECETOC TRA was collected from the European Chemical Agency (ECHA) registration dossier, and the Activity for Stoffenmanager was converted from PROC using translation of exposure models (TREXMO). The information related to exposure, such as working duration, Respiratory Protective Equipment (RPE), Local Exhaust Ventilation (LEV), and Risk Management Measure (RMM) were classified into high, medium, and low exposure conditions. The risk was determined by the ratio of the estimated exposure and occupational exposure limits of benzene. Results: Under high exposure conditions, the worker exposure level calculated from all PROCs and Activities exceeded the risk level, with the exception of PROC 1 and Activity 1. In the medium exposure condition, PROC 8a, 8b, and 9 and Activity 3, 7, and 8 all exceeded the risk, whereas in the low condition, all PROCs and Activities were determined to be safe. As a result, action corresponding with the low exposure condition is required to reduce the risk of exposure among workers in workplaces where benzene is manufactured or used as a solvent. In addition, the predicted exposure levels derived from the exposure models were lower than measured levels. The exposure levels estimated from Stoffenmanager were more conservative than those from ECETOC TRA. Conclusions: This study demonstrates the feasibility of exposure models for exposure assessment through the example of occupational inhalation exposure assessment for benzene. For more active utilization of exposure models in K-REACH, the exact application of collected information and accurate interpretation of obtained results are necessary.

불산 누출사고에 따른 지역사회 구성원들의 노출평가 (Exposure Assessment on Sub-Populations of the Local Community following a Hydrofluoric Acid Accident)

  • Objectives: The purpose of this study was to propose an indirect exposure assessment method using a questionnaire survey at a time when direct exposure assessment would be impossible after a chemical accident. Methods: About two weeks after an accident, a questionnaire survey was performed with 1,264 persons from the local community. Variables related to exposure were extracted from the survey contents, weighted and then graded for comparison with subjective symptoms in order to evaluate the extent of exposure. Survey items suitable for reflecting the previous exposure level during the accident were extracted, weighted and divided into quartile ranges. Subjective symptoms showed an increasing tendency with higher exposure level when compared with final exposure level (p<0.01). Results: For the relationship between the final exposure grade and subjective symptoms, as the exposure grade was increasing the rates complaining of symptom also showed an increasing tendency. However, when adjusted for demographic characteristics, there was a tendency for the eye irritation symptom to appear higher in women, and respiratory organ irritation appeared higher in smokers. Conclusions: When the problem of recall bias is considered, this study may not have completely unraveled exposure and the characteristics of the participants can affect subjective symptoms. Nevertheless, the exposure rating method of using a questionnaire showed a significant relationship with symptom level. It can be deemed that assessment of past exposure may be successfully evaluated by questionnaire in cases such as chemical accidents.

베트남 참전 제대 군인의 범주화된 에이전트 오렌지 개인 폭로량과 혈청 다이옥신 측정치와의 상관성에 관한 연구 (A Study on the Correlation between Categorization of the Individual Exposure Levels to Agent Orange and Serum Dioxin Levels Among the Korean Vietnam Veterans)

  • Objectives : In an epidemiologic study on the health impact of Agent Orange exposure, the valid estimation of exposure level is the most important step. Based on recent studies, we examined the correlation between exposure levels categorized by personal exposure estimates and serum 2,3,7,8-tetrachlorodibenzo-p-dioxin (2,3,7,8-TCDD, Dioxin), exploring the possibility of utilizing the exposure level as a surrogate for the estimate of exposure to agent orange. Methods : During the study period (Jan 1996-Feb 1996), blood specimens of 745 subjects taken randomly among 1,329 persons and kept frozen, were analyzed for 2,3,7,8-TCDO and six other dioxin congeners. The serum dioxin and congeners were measured in 1998 by CDC, adjusted for serum lipids. We categorized the total exposure scores into five groups based on Agent Orange exposure data collected by interview and military records. Pearson and Spearman's correlation coefficients & multiple regression analysis were used to identify the relationship of the exposure level categorized with serum concentration of 2,3,7,8-TCDD, and six other dioxin congeners. Results : Dioxin and the other congeners, except 1,2,4,6,7,8-HpCDD, showed significant correlations to exposure categories (p<0.005): 2,3,7,8-TCDD and OCDD showed positive correlations, whereas the other congeners did negative. The values of 2,3,7,8-TCDD differed according to exposure category and proportionally increased from the low exposure group to the high, a dose-response relationship, even after other possible confounding variables were adjusted for. In multiple regression analysis, age$(\beta=0.033)$, dioxin$(\beta=0.433)$, 1,2,3,7,8-PeCDD$(\beta=-0.998)$, 1,2,3,4,7,8-HxCDD$(\beta=-0.773)$, 1,2,3,6,7,8-HxCDD$(\beta=0.255)$, 1,2,3,7,8,9-HxCDD$(\beta=-3.468)$, 1,2,3,4,6,7,8-HpCDD$(\beta=0.109)$ we re found to be significantly related to the total exposure score(p<0.005). Conclusion : This study demonstrated that the use of such categorizations as a surrogate measure of agent orange exposure in identifying exposure degrees in a health impact study is valid.

석면방직업 근로자의 석면노출 실태와 과거농도 추정에 관한 연구 (A Study on Exposure Among Asbestos Textile Workers and Estimation of their Historical Exposures)

  • From July 8 to September 2 1994, asbestos exposure level among asbestos textile workers was surveyed. Six plants out of plants in Korea were selected for this study. In addition to the exposure level, the relationship between the level of exposure and some factors affecting exposure were studied. Also, using historical data of asbestos concentrations in asbestos textile plants plus current data, trend of asbestos exposure level could be introduced. Historical exposure level was estimated on the basis of these data. The main results of this study are follows. 1. Average concentration of all six plants surveyed was 1.54 f/cc, and range of those concentrations was 0.03 - 11.58 f/cc. The minimum average concentration was 0.32 f/cc and the maximum was 8.04 f/cc which is four times higher than the Korean standard. A wide difference of exposure level among the workers of different plants was observed. In three plants, the half of all the plants surveyed, their average concentrations exceeded the Korean standard, and those in all the plants exceeded the ACGIH TLV. 2. Among total 56 samples, 22 samples(39%) were in excess of the Korean standard, and 53 samples(95%) were above the ACGIH TLV. Among 32 personal samples, 15 samples(47%) exceeded the Korean standard, and 30 samples(94%) exceeded the ACGIH TLV. Among 24 area samples excluding a few samples collected in office area, seven samples exceeded the Korean standard, and 23 samples( 96%) exceeded the ACGIH TLV. 3. Distributions of concentrations were observed by processes. In weaving, the highest, average concentration was 4.29 f/cc, and range was 2.61 - 11.58 f/cc. In spinning, average concentration was 2.22 f/cc, and range was 0.41 - 8.93 f/cc. In carding, average concentration was 1.98 f/cc, and range was 0.23 - 10.93 f/cc, In twisting, average concentration was 1.65 f/cc, and range was 0.21 - 9.83 f/cc. In mixing, the lowest, average concentration was 0.48 f/cc, and range was 0.22 - 1.20 f/cc. 4. All the samples from basic processes of asbestos textile plants were above the ACGIH TLV. Nineteen samples(45%) out of all these 42 samples exceeded Korean standard. Fourteen samples(58%) of total 24 personal samples, and five samples(28%) of total 18 area samples exceeded the Korean standard. Considering processes, all the samples in weaving process exceeded the Korean standard and 50 did 54% of those in spinning, 40% in carding, and 27% in twisting. 5. Trend of decreasing asbestos concentrations in asbestos textile plants was observed by time. 6. Asbestos concentrations in asbestos textile plant in 1975 were estimated to be 11.0 - 92.4 f/cc.

Current Exposure Status to Cadmium through Multimedia Pathway in Korea

  • The human exposure level of cadmium can be varied by environmental level of cadmium and individual life style. This study was conducted to estimate current exposure of cadmium using up-to date in Korea. The exposure estimates were intended to representative of the general adult group with 60kg. This study quantified human exposure level of cadmium through food, air and soil except water because domestic all data was not detected. (omitted)

여수국가산업단지 석유화학산업 근로자들의 벤젠 노출 실태와 관리대책 (Status of benzene exposure and suggested countermeasures for petrochemical workers in the Yeosu Industrial complexs)

  • Status of benzene exposure was evaluated upon petrochemical workers in the Yeosu Industrial Complex, based on questionnaire responses by workers, review of previous work environment assessment reports between 1996 and 2004, in addition to short-term exposure measurements for unit tasks in the field. Questionnaire results showed that workers in the field were suffering concern on and symptoms of respiratory diseases, cancers or other illnesses of unknown causes. In particular, workers were highly worried about the risk of exposure to chemical hazards including benzene, while conducting specific tasks (e.g. sampling, draining, gauging) among normal operations during which equipments are opened and contents are exposedin the air, as well as periodic turnaround (TA) task. However, the review of previous work environment assessment reports found out that short-term exposure assessment data for unit tasks accounted for less than 1% of total data, and most of them were on 8-hr average exposure level. It also turned out that benzene was not detected in 83% of total samples, suggesting routine but pointless sampling has been repeated. Short-term exposure level was assessed focusing on tasks with high level of complaints on the exposure risk, based on the questionnaire responses. As predicted, a maximum of 741 ppm benzene exposure was reached depending on types and conditions of operations involved. In conclusion, these findings suggest that the evaluation and management on the short-term high exposure tasks including turnaround are crucial in reducing benzene exposure and preventing haematopoietic cancer in workers in the petrochemical industry.

다매체 노출을 고려한 Perchloroethylene의 인체위해성평가연구 (Human Risk Assessment of Perchloroethylene Considering Multi-media Exposure)

  • Objectives: Perchloroethylene (PCE) is a volatile chemical widely used as a solvent in the dry-cleaning and textile processing industries. It was evaluated as Group 2 "probably carcinogenic to humans" by the Integrated Risk Information System (IRIS) of the United State Environmental Protection Agency (U.S. EPA) in 2012. In order to provide a scientific basis for establishing risk management measures for chemicals on the national priority substances list, aggregate risk assessment was conducted for PCE, included in the top-10 substances. Methods: We conducted the investigation and monitoring of PCE exposure (e.g., exposure scenario, detection levels, and exposure factors, etc.) and assessed its multi-media (e.g., outdoor air, indoor air, and ground water) exposure risk with a deterministic and probabilistic approach. Results: In human risk assessment (HRA), the level of human exposure was higher in the younger age group. The exposure level through inhalation at home was the highest among the exposure routes. Outdoor air or uptake of drinking water represented less than 1% of total contributions to PCE exposure. These findings suggested that the level of risk was negligible since the Hazard Index (HI) induced by HRA was below one among all age groups, with a maximum HI value of 0.17 when reasonable maximum exposure was applied. Conclusion: In conclusion, it was suggested that despite low exposure risk, further studies are needed considering main sources, including occupational exposure.

EQC 모델을 이용한 벤조일 퍼록사이드의 다매체 환경거동 예측 (Estimation of Multimedia Environmental Distribution for Benzoyl peroxide Using EQC Model)

  • 벤조일 퍼록사이드는 수서생물에 대해 매우 높은 독성을 나타냄에도 불구하고 환경 중 잔류 농도 및 노출영향에 대해 연구가 이루어지지 않아 OECD에서 추천하고 있는 대표적인 다매체 환경거동 모델인 EQC 모델을 이용하여 본 물질에 대한 환경중의 농도를 예측하고 위해성평가 및 화학물질의 관리를 수행하기 위한 기초자료로 활용코자 하였다. 평형, 정상상태에서 100,000 kg의 벤조일 퍼록사이드가 환경내로 유입된 상태를 나타내는 Level I과 평형, 정상상태 이류와 분해현상이 있고 일정한 속도 1,000 kg/h로 유입되었을 경우를 나타내는 Level II에서 벤조일 퍼록사이드는 주로 토양(68.3%)과 물(28.7%)로 배출되는 것으로 예측되었다. 비평형, 정상상태, 이류와 분해현상이 있고 다매체 이동을 하는 시스템에서 벤조일 퍼록사이드가 대기, 물, 토양, 침전물의 각각의 4개 매체에 연속적으로 1,000 kg/h로 유입될 경우인 Level III에서는 주로 토양(99.9%)으로 배출되었고 전체 잔류시간은 3.4년으로 예측되어 벤조일 퍼록사이드가 환경 중에 잔류성이 있는 물질로 평가되었다.

저농도 유기용제 노출 사업장 근로자의 심혈관질환 위험인자 변화 추적(2008~2012) (Five Year's Follow-up of the Cardiovascular Disease Risk Factors among the Low Level Organic Solvent Exposure Workers)

  • Purpose: The objective of the present study was to evaluate the effects of occupational exposure to low dose organic solvents on the prevalence of cardiovascular risk factors. Methods: Study design was retrospective cohort study subjected on 191 exposures and 118 controls working in a petrochemical manufacture company. The eight indicators related to CVD risk were followed up for five years from 2008 to 2012. The risk level was compared during the follow up years and subject's characteristics, and the change of risk level were analyzed with repeated measures ANOVA and multiple logistic regression analysis. Results: At the start year 2008, the rate over cutoff value (ROCV) of BS (p<.001) and mean systolic BP (p=.017) were higher in organic solvent exposure group and the others showed no difference. And by the subject's characteristics, odds ratio of the ROCV of BS were higher in organic solvent exposure group and work shift group as 2.51 and 3.07. Comparing the results in 2012 to those of 2008, cardiovascular disease risk in organic solvent exposure group was about 1.5 times higher than that of in the control group. Conclusion: Gradual increase in the CVD risk was identified in organic solvent exposure group. However, the risk might be influenced by shift work and bad behaviors rather than organic solvent exposure.
