This study was designed to investigate the effect of bone demineralization and tibia lead on blood lead in retired lead workers. Two hundred thirty five(126 females and 109 males) retired lead workers who worked in 4 different lead factories and 101 non-occupationally lead exposed subjects(51 females and 51 males) were recruited from March 2004 to October 2004. Bone mineral density(BMD) was measured at left calcaneous bone area by broadband ultrasound attenuation(BUA) method with QUS-2(Metra Biosystems Inc, USA). The BUA value transformed into T-score by WHO standard conversion criteria. Tibia bone lead was measured for skeletal bone lead with K-xray fluorescence(K-XRF) and blood lead was analyzed with flameless atomic spectrophotometer. Hemoglobin, hematocrit, serum calcium and iron were also analyzed. In addition, information for smoking and drinking status and basic personal data such as age, gender and lead exposure were also collected using questionnaire inquiry. Blood lead was correlated with tibia lead (r=0.664) and these two variables were negatively correlated with BMD in bivariate analysis. BMD showed significant main effect on the change of blood lead independent to tibia lead without any effect modification of age or gender; the one T-score unit decrease of mineral bone density made $0.43{\mu}g/dl$ increase of blood lead. On the other hand, tibia lead showed effect modification with gender on blood lead; the slope of tibia lead on blood lead in male was steeper than in female and crossed at around zero of tibia lead. In the multiple regression analysis of blood lead and tibia lead on BMD after adjustment of related covariates, only blood lead showed statistically significant effect on BMD. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.

This study was performed to examine the relationship between air pollution exposure and mortality in Seoul for the years of 1998∼2001. Daily counts of death were analyzed by general additive Poisson model, with adjustment for effects of seasonal trend, air temperature, humidity, and day of the week as confounders in a nonparametric approach. Daily death counts were associated with CO (current day),O$_3$ (current day), PM$_{10}$ (current day), NO$_2$ (1 day before), SO$_2$ (1 day before). Increase of 41.71 $\mu\textrm{g}$/㎥ (interquartile range) in PM$_{10}$ was associated with 1.3% (95% CI = 0.7∼1.9%) increase in the daily number of death. $O_3$ concentrations resulted in an increased risk of 1.3% for 23.86 ppb in all-aged mortality [RR = 1.013 (1.004-1.023)1. This effect was greater in children (less than 15 aged) and elderly (more than 65 aged). After ozone level exceeds 25 ppb, the dose-response relationship between mortality and ozone was almost linear. We concluded that Seoul had 1∼5% increase in mortality in association with IQR (interquartile range) in air pollutants. Daily variations in air pollution within the range currently occurring in Seoul might have an adverse effect on daily mortality. These findings also support the hypothesis that air pollution, at levels below the current ambient air quality standards of Korea, is harmful to sensitive subjects, such as children or elderly.rly.

Pollution variables were ozone, nitrogen dioxide, total suspended particles(TSP), and sulfur dioxide. Results: Daily death counts were associated with ozone(1 day before), nitrogen dioxide(1 day before), TSP(2 days before), sulfur dioxide(2 days before). The association with ozone was most statisfically significant and independent of other air pollutants. Increase of 100 ppb in ozone was associated with 0%(95% Cl= 2%-10%) increase in the daily number of death, This effect was greater in persons aged 65 and older. The relative risks of death from respiratory disease and cardiovascular disease were greater than for all-cause mortality in each pollutant. After ozone level exceeds 25 ppb, the dose-response relationship between mortality and ozone was almost linear. However, the effect of TSP, sulfur dioxide, and nitrogen dioxide on mortality might be confounded with each other. Conclusion: Daily variations in air pollution within the range currently occurring in Seoul might have an adverse effect on daily mortality.

Porcelain fused to metal crown has been used mostly over the last 50 years for restorations in dentistry. However, the patients' awareness of aesthetic aspect, biocompatibility and the problems such as an allergy to metals led to the growing interest in the 'metal free restoration'. In particular, the price of the precious metals that have been mainly used to date has risen drastically, which made them impossible to play their role as oral restorative materials anymore, and in addition, the PFM restoration has intrinsic problems of chipping and fracture. Therefore, the CAD/CAM has been drawing more attention than ever due to the popular needs for the material that is more aesthetic and stronger for restoration of the molar implant. Considerations in carrying out the full zirconia restoration are as follows: 1) strength, 2) combination work, 3) light penetrability, 4) treatment of cracks, 5) the color reproducibility of the block, 6) the abrasivity of antagonistic tooth, 7) low temperature degradation. In this presentation, the color reproducibility of the block will be discussed. One of the biggest reasons for avoiding the full zirconia restoration is that it is difficult to reproduce the natural color compared to the conventional PFM restoration. Thus, many clinicians show reluctance due to the exposure of the ugly block when the coloring on the surface is removed after occlusal adjustment. From the experience of using blocks by Zirkonzahn for more than 4 years, it is considered that these problems can be addressed to some degrees. Accordingly, how to make restorations that are well in harmony with surrounding prosthesis or natural teeth will be discussed.
Creatinine-adjustment is an important process in the urinary monitoring of the environmental exposure of children and adolescents. The purpose of this study was to investigate the concentrations of urinary creatinine and factors associated with them among Korean children and adolescents. We recruited 1,025 persons from 128 extracted schools. They were from three to 18 years old and supplied urine samples for measuring creatinine. The concentrations of urinary creatinine were 98.18 mg/dl (SD, 67.67) in arithmetic mean and 72.05 mg/dl (GSD 2.49) in geometric mean, were significantly higher among male children/adolescents than females in all age groups, and higher values appeared following increasing ages, heights and BMIs. The rates of the number who were below the lowest limit recommended by WHO (<30 mg/dl) were 25.57% among three to four year olds, 21.77% among five to six year olds, 20.0% among seven to eight year olds and 14.69% among nine to ten year olds, respectively. The rates of those above the highest limit (>300 mg/dl) were 0.0% among three to twelve year olds. The coefficient of determination R-square of the fitted regression model for urinary creatinine was 27.4% with general characteristic variables of sex, age, BMI and height. The significant variables among these were height (standardized beta = 0.372) and age (standardized beta = 0.129). Another coefficient of determination R-square was 15.3% with dietary habit variables of smoking, drinking, dining area, number of meals and snacks, and intake of milk food, cup-noodles, canned foods, popcorn, nachos, and hamburgers. In conclusion, the concentration of urinary creatinine was significantly lower in children than in adults, and was very significantly associated with the height of children. Therefore, children need the recommended concentrations for urinary creatinine, as distinguished from adults.
Background: Cigarette smoking is a well-established risk factor of pancreatic cancer (PC). Although an association between nicotine dependence phenotype, namely time to first cigarette (TTFC) after waking, and the risk of several smoking-related cancers has been reported, an association between TTFC and PC risk has not been reported. We assessed the impact of smoking behavior, particularly TTFC, on PC risk in a Japanese population. Materials and Methods: We conducted a case-control study using 341 PC and 1,705 non-cancer patients who visited Aichi Cancer Center in Nagoya, Japan. Exposure to risk factors, including smoking behavior, was assessed from the results of a self-administered questionnaire. The impact of smoking on PC risk was assessed with multivariate logistic regression analysis adjusted for potential confounders to estimate odds ratios (ORs) and 95% confidence intervals (CIs). Results: Cigarettes per day (CPD) and/or smoking duration were significantly associated with PC risk, consistent with previous studies. For TTFC and PC risk, we found only a suggestive association: compared with a TTFC of more than 60 minutes, ORs were 1.15 (95%CI, 0.65-2.04) for a TTFC of 30-60 minutes and 1.35 (95%CI, 0.85-2.15) for that of 0-30 minutes (p trend=0.139). After adjustment for CPD or smoking duration, no association was observed between TTFC and PC. Conclusions: In this study, we found no statistically significant association between TTFC and PC risk. Further studies concerning TTFC and PC risk are warranted.
Background: Numerous studies have suggested that selenium deficiency may be associated with an increased risk for several types of cancer, but few have focused on thyroid cancer. Materials and Methods: We examined the association between post-diagnostic fingernail selenium levels and differentiated thyroid cancer risk in a French Polynesian matched case-control study. Conditional logistic regression models were used to estimate odds ratios and 95% confidence intervals. Results: The median selenium concentration among controls was $0.76{\mu}g/g$. Significantly, we found no association between fingernail selenium levels and thyroid cancer risk after conditioning on year of birth and sex and additionally adjusting for date of birth (highest versus lowest quartile: odds-ratio=1.12, 95% confidence interval: 0.66-1.90; p-trend=0.30). After additional adjustment for other covariates, this association remained non-significant (p-trend=0.60). When restricting the analysis to thyroid cancer of 10 mm or more, selenium in nails was non-significantly positively linked to thyroid cancer risk (p-trend=0.09). Although no significant interaction was evidenced between iodine in nails and selenium in nails effect (p=0.70), a non-significant (p-trend =0.10) positive association between selenium and thyroid cancer risk was seen in patients with less than 3 ppm of iodine in nails. The highest fingernail selenium concentration in French Polynesia was in the Marquises Islands ($M=0.87{\mu}g/g$) and in the Tuamotu-Gambier Archipelago ($M=0.86{\mu}g/g$). Conclusions: Our results do not support, among individuals with sufficient levels of selenium, that greater long-term exposure to selenium may reduce thyroid cancer risk. Because these findings are based on post-diagnostic measures, studies with prediagnostic selenium are needed for corroboration.

The aim of this study is to evaluate the accuracy and variability of the oak pollen concentrations over the Seoul metropolitan region (SMR) simulated by the Community Multiscale Air Quality (CMAQ)-based pollen dispersion model, which is the CMAQ-pollen model integrated with the improved oak pollen emission model(PEM-oak). The PEM-oak model developed is based on hourly emission flux parameterization that includes the effects of plant-specific release, meteorological adjustment, and diurnal variations of oak pollen concentrations. A 33 day-run for oak pollen simulation was conducted by the CMAQ-pollen model with a 3 km spatial resolution for the SMR during the 2014 spring pollen season. Modeled concentrations were evaluated against the hourly measurements at three Burkard sampling sites. Temporal variations of oak concentrations were largely well represented by the model, but the quantitative difference between simulations and measurements was found to be significant in some periods. The model results also showed that large variations in oak pollen concentrations existed in time and space and high concentrations in the SMR were closely associated with the regional transport under strong wind condition. This study showed the effective application of the CMAQ-pollen modeling system to simulate oak pollen concentration in the SMR. Our results could be helpful in providing information on allergenic pollen exposure. Further efforts are needed to further understand the oak pollen release characteristics such as interannual variation of the oak pollen productivity and its spatio-temporal flowering timing.

To evaluate the neurobehavioral effects of blood lead, we performed NCTB (Neurobehavioral Core Test Battery) on 59 men who were exposed to lead occupationally. Performance on the tests was studied in relation to three ranges of blood lead $(low,\;0. Workers with high and medium blood lead concentrations showed poorer performances on digit span backward, digit symbol and Benton Visual Retention. After adjustment for age and education, high and medium group showed significantly impaired performance on Benton visual retention and slightly impaired performance on digit span backward. But it showed no different performance on tests between high and medium blood lead group.

Objectives: To clarify the association between dust from large construction sites and allergic and respiratory conditions among residents in Sejong city. Methods: We included 379 individuals older than 50 years from three areas of Sejong city; two areas near construction sites and one area for a comparison. Prevalence of self-reported allergic and respiratory conditions was compared across the three areas using logistic regression to adjust for covariates including age, gender, smoking, and educational attainment. A total of 16 individuals were interviewed to take details of the residents' experiences of construction work. Results: Those living in the nearby areas around construction sites were generally found to have the increased prevalence for all allergic and respiratory conditions. Odds Ratio(OR)s for cough and sneezing among the six symptoms and allergic rhinitis among the three diagnosed diseases reached statistical significance but only OR for cough remained significant after adjustment: adjusted OR for cough was 2.63 (95% CI, 1.07-6.46). Two relevant themes emerged from semi-structured interview were; 1) most residents from the affected area reported a considerable level of exposure to construction dust, 2) about half of them noted that construction dust influences allergic- and respiratory health. Conclusions: General pattern provides suggestive evidence that living close to construction sites may be positively associated with elevated risks of allergic and respiratory conditions. Efforts should be made to establish preventive measures and a comprehensive in-depth health impact assessment on this issue.
