Experience for The Decontamination & Decommissioning of The Core Assembly of KRR-2 Research Reactor (연구용 원자로 2호기의 로심 집합체 제염$\cdot$해체 경험)

  • The research reactor (TRIGA Mark-III(KRR-2)) was constructed and had been operated in 1972. In 1999 the radioisotope process units had stopped its operation due to normal operation of HANARO. In 2003 the core assembly was decommissioned by D&D program. The contact exposure rate on the core assembly and the rotary specimen rack are from 300mSv/h to 700mSv/h. This report describes the decontaminationing procedures, the health physics programs, and the waste management.

A Case of Laryngeal Saccular Cyst (후두에 발생한 소낭낭포 1례)

  • Laryngeal saccular cyst may develop when obstruction of the orifice of the saccule occurs with resultant dilatation of saccule. It is classified into anterior saccular cyst and lateral saccular cyst. When saccular cyst occurs in infancy, it usually may cause respiratory distress, inspiratory stridor, cyanosis and dysphagia. In adults, it may cause hoarseness, dyspnea, dysphagia, pain and neck mass. The laryngeal saccular cyst may be treated by its removal with endoscopic or external approach, depending on the classification of saccular cysts, its size and individual patient factor. Recently, we have experienced a case of laryngeal saccular cyst so we report our clinical experience with brief review of literature.

  • The transgender is who has persistent discomfort with his of her sex or sense of inappropriateness in the gender role of that sex. Many transgenders have been undergone trans-sex operations after 1980's. But there were not so much experience of voice surgery for transgenders. Vocal pitch can be elevated by various surgical techniques. Type IV thryoplasty, which one of the cricothyroid approximation methods, increases tension to the vocal folds, thus increasing fundamental frequency and upper pitch range. We treated a transgender (MTF, Male to female) using this technique. Acoustic and aerodynamic measures were obtained both pre- and post-operatively. Clinically significant improvement was achieved in both study; increasing of vocal pitch, widening of pitch range, and improvement of vocal efficiency.

  • The purpose of this ethnography is to describe and analyze the process of becoming a wild geese mother. Thirteen mothers from New York, California, and Texas of U.S.A. were interviewed with an unstructured questionnaire. There are several different patterns of the process; 'the short term', 're-departure after returning to Korea', 'a step to initiate their family's immigration', 'sudden and unintentional'. In the first stage of the family living rearrangement, the main issues are about the wider free world, proper time for leaving and proper period for staying, where to live and my husband's independent living. The wild geese mothers perform the concrete tasks about the everyday living for the new mother-child family in the foreign country. They report their subjective interpretation for their new life.

  • The surgical indications for the treatment of gastroesophageal reflux disease (GERD) in patients with esophageal motility disorders have been debated. We report a case of antireflux surgery performed in a patient with absent esophageal motility as categorized by the Chicago classification (2011). A 54-year-old man underwent laparoscopic Toupet fundoplication due to apparent GERD and desire to discontinue all medications. After surgery, his subjective symptoms improved. Furthermore, objective findings including manometry and 24-hour pH-metry also improved. In our experience, antireflux surgery can improve GERD symptoms patients, even with absent esophageal motility.

Entirely Laparoscopic Gastrectomy and Colectomy for Remnant Gastric Cancer with Gastric Outlet Obstruction and Transverse Colon Invasion

  • It is well known that gastrectomy with curative intent is the best way to improve outcomes of patients with remnant gastric cancer. Recently, several investigators reported their experiences with laparoscopic gastrectomy of remnant gastric cancer. We report the case of an 83-year-old female patient who was diagnosed with remnant gastric cancer with obstruction. She underwent an entirely laparoscopic distal gastrectomy with colectomy because of direct invasion of the transverse colon. The operation time was 200 minutes. There were no postoperative complications. The pathologic stage was T4b (transverse colon) N0M0. Our experience suggests that laparoscopic surgery could be an effective method to improve the surgical outcomes of remnant gastric cancer patients.

  • Bone-anchored auricular prosthesis have been proved that was another apporach to the treatment of the defective external ear. A 19years old woman with hemifacial microsomia was treated with osseointegrated implants for the support of craniofacial prosthesis. 3 implants were placed in temporal region and craniofacial prosthesis was retained with telescopic magnet retention system. The literatures and surgical technique are reviewed and our experience is presented.

  • The osteotomy for rhinoplasty is a useful method to make the nasal bony pyramid get narrow, correct the deviated nose and prevent the open roof deformity after hump nose resection. The osteotomy for rhinoplasty is divided medial osteotomy, lateral osteotomy and transverse osteotomy. If the osteotomy is well done, it produces very effective and esthetic results. However, the osteotomy has problems that precise operation is often impossible for the difficulty of the access and that the possibility of the complication is very high. We report our clinical experience about the osteotomy for rhinoplasty.

Failure of Intravenous Sedation due to Significant Hypertension -A Case Report- (혈압 상승으로 인한 의식하 진정 요법의 실패 -증례보고-)

  • Sedation is often indicated for the relief of anxiety for outpatient oral surgery. In combination with local anesthesia, it is safe and effective method of treatment. However, it is not always effective in allowing the physician to complete the planned oral surgery procedure. On occasion, a procedure is left unfinished due to patient combativeness and discomfort and hypertension in spite of increase in sedative doses. Episodic increases in blood pressure were most commonly caused by light anesthesia or sedation and by the patient's experience of pain during treatment. Female patient was 42 years old. blood pressure is 150/90 mmHg. Extraction and implant surgery was done under IV sedation. During seadtion, her blood pressure was increased (200/100 mmHg). Surgery was stopped. She was done monitoring blood pressure. The blood pressure was decreased to 130/90 mmHg. Sedation was failed due to significant hypertension. Blood pressure is seldom increased during sedation but we should evaluate the patient's medical history and know guideline for hypertension crisis.

Successful ankle replantation in two cases with different presentations

  • We report our experience of treating two patients with ankle amputation with different presentations. The first case was a clean-cut sharp amputation. The second case was an avulsion injury following a motor vehicle accident in a patient who arrived 8 hours after the injury. Replantation was successful in both cases. In avulsion injuries, a secondary operation for wound coverage is required at a later stage. With good strategy and a support team, encouraging limb survival outcomes are possible post-replantation.
