Treatment Experience with Polymyalgia Rheumatica -A report of two cases- (류마티스성 다발성 근육통의 치료 경험 -증례보고-)

  • Polymyalgia rheumatica is characterized by bilateral shoulder or pelvic girdle pain, morning stiffness of greater than 45 minutes' duration, constitutional symptoms, an elevated erythrocyte sedimentation rate and a rapid response to prednisolone (${\leq}20mg/day$). Although it is not a rare disease, many cases might be neglected and treated inappropriately in pain clinics. We describe here two cases of polymyalgia rheumatica that was neglected and treated inappropriately.

  • Ligamentum flavum hematoma (LFH) is a very rare condition of dural compression; most are observed in the mobile cervical and lumbar spine regions. A 67-year-old man who had a long level interbody fusion at L3-S1 four years ago presented with symptoms suggestive of dural compression. Magnetic resonance imaging showed a posterior semicircular mass located at the adjacent L2-L3 level. After decompression of the spinal canal and removal of the mass lesion, pathological examination of the surgical specimen revealed a hematoma within the ligamentum. The patient fully recovered to normal status after surgery. Here, we report our experience with a LFH in the adjacent segment after a long level fusion procedure and discuss the possible associated mechanisms.

Maxillary Nerve Block for Patient with Soft Palate Cancer Pain - A case report - (연구개암 환자에서의 상악신경차단 - 1예 보고 -)

  • In early stages, cancer is not usually painful. However, many patients with recurrent or metastatic cancer eventually experience pain, which becomes progressively worse. Chemotherapy, sympathetic surgery, physical therapy and nerve block can be used to control cancer pain. A 60-year old patient had severe pain of the soft palate due to squamous cell carcinoma. We successfully treated this patient with maxillary nerve block using pure alcohol by a lateral approach. Four months after maxillay nerve block, the patient is still pain free.

Sympathetic Block for Treating Primary Erythromelalgia

  • Primary erythromelalgia is a rare condition that’s characterized by erythema, an increased skin temperature and burning pain in the extremities. The pain is often very severe, and treating erythromelalgia is frustrating and difficult. We report here on the case of a 12-year old girl with primary erythromelalgia in both lower extremities. The pain was refractory to medical treatment, but a bilateral sympathetic block with lidocaine and triamcinolone resulted in relief from the pain. Our experience with this disease demonstrates that sympathetic blocks are effective in improving the symptoms and they may be attempted on erythromelalgia patients who do not respond to other treatments, including medication and epidural blocks.

  • Angiodysplasia is uncommon and frequently undiagnosed and presents a taxing surgical problem. It is usually diagnosed for unexplained gastrointestinal bleeding. Angiography gives a little information about wax and wane pattern of bleeding in the lesion. We experienced I case of angiodysplasia in G-I tract(especially colon) diagnosed in the Seoul National University Hospital. This report summarized our experience and review of east and west literature.

  • We report on our experience on utilization of collaborative tools for KISTI high energy physics group. Our main subject will be the new EVO (Enabling Virtual Organization) server that we hosted recently at KISTI. The EVO is a next generation collaborative tool from CalTech high energy physics group with several improvements from its predecessor VRVS (Virtual Room Videoconferencing System). EVO has the same baseline structure as in VRVS. The network of servers lies at the heart of EVO structure and one server called Panda communicates with local clients called Koala.

  • Exfoliative Dermatitis is a name of a group of symptoms which is characterized by red rash, itching and scale in almost whole body. The patients are suffered from hypoalbuminemia, general weakness and fever sign in addition to characteristic skin lesions. So in the treatment of this disease, adequate symptomatic treatment is important with adequate topical skin management. We experienced a 69 year old Korean male patient diagnosed as Exfoliative Dermatitis based on clinical symptoms and a medical certificate. We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Bangpungtongsung-san(剝脫性皮膚炎) and Geunsun-bang(乾癬方). After taking Oriental Medicine(the hebal medicine of internal use), clinical improvement was noticed. Based on this experience, internal use of Oriental Medicine have a good effect in Exfoliative Dermatitis.

  • Congenital cystic adenomatoid malformation is a rare, but well-known disease. It can be managed conservatively in patients without symptoms or require surgical removal when symptomatic. The surgical option of choice is en bloc resection of the affected lesion. We report an experience of life-threatening congenital cystic adenoid malformation in a low-birth-weight (1,590 g) premature neonate who was successfully treated with a lobectomy of the lung.

  • Subcutaneous emphysema is a benign condition following trauma (pneumothorax and oropharyngeal), cervical or thoracic procedures, and mediastinal infection. However, severe subcutaneous emphysema may be related to serious complications such as respiratory failure, airway compromise, and tension- related phenomena. Many alternative therapies have been tried to treat patients with this condition. We report our experience with vacuum-assisted closure therapy for treating patients with severe subcutaneous emphysema.

Optimal Reporting Stategy of an Insured -Dynamic Programming Approach-

  • We consider an insured who wishes to determine his optimal reporting strategy over a given planning horizon, when he has option of reporting of not reporting his at-fault accidents. Assuming that the premium in future period is continually adjusted by the insured's loss experience, the insured would not report every loss incurred. Rather, considering the benefits and costs of each decision, the insured may want to seek a way of optimizing his interests over the planning horizon. The situation is modeled as a dynamic programming problem. We consider an insured's discounted expected cost minimization problem, where the premium increase in future period is affected by the size of the current claim. More specifically, we examine two cases ; (1) the premium increase in the next is a linear function (a constant fraction) of the current claim size; (2) the premium increase in the next period is a concave function of the current claim size. In each case, we derive the insured's optimal reporting strategy.
