A Case Report of Polyarteritis Nodosa Treated with Korean Medical Treatment (결절성 다발 동맥염 환자의 한방치료 치험 1례)

  • Objectives : The purpose of this study is to report the one clinical experience of korean medical treatment on polyarteritis nodosa(PAN). Methods : We treated a 46-year old man patient with acupuncture, pharmacopuncture, Daehwangchoseok-tang, Chijadaehwangsi-tang. We evaluated the improvement of patients by Visual Analogue Scale(VAS) and photography. Results : After the treatments, symptoms of skin lesion including pus, swelling, subcutaneous nodule and bleeding were improved and VAS of pain decreased from 10 to 1. Conclusions : This study shows that the korean medical treatment can improving symptoms of PAN.

  • Tarsal tunnel syndrome caused by ganglion as space occupying lesion is unusual and known that excellent result can be expected from surgical treatment carried out soon after onset of the condition. The object of the current study is to report our experience of tarsal tunnel syndrome caused by ganglion of flexor digitorum longus tendon sheath treated with operative management in a 39 year-old man with a review of the literature.

  • Tarsal tunnel syndrome caused by space occupying lesion is unusual but it rarely occur by ganglion which is presented below the flexor retinaculum due to compression neuropathy of posterior tibial nerve and its branches. The object of the current study is to report our experience of surgical treatment about tarsal tunnel syndrome caused by the ganglion of flexor digitorum longus tendon sheath and multifocal myxoid degeneration of medial plantar nerve with a review of the literatures.

Re-replacement of prosthetic heart valves: report of 7 cases (인공심장판막의 재치환수술: 7례 보)

  • Prosthetic valve replacement has resulted in marked improvement in the functional status of many patients with valvular heart disease. But valve failure from various causes has necessitated re-replacement of prosthetic valve in some of these patients. This selective group of patients has many inherent problems. This report presents our experience with seven patients who received re-replacement of prosthetic valve in the Seoul National University Hospital from 1981, to 1983. The patients included one woman and six men in the range of 9 and 49 years old. As the first valve operations, there were five MVR using tissue valves, one DVR [Bjork-Shiley and Ionescu-Shiley valve] and one case of modified Bentall operation with composite graft [Ionescu-Shiley valve]. Reoperations on prosthetic valve failure were performed 17 to 54 months after the first operation [mean 34 months]. Amon8 seven patients, there were two cases of prosthetic valve endocarditis and five cases of primary tissue failure. Gross calcification of the xenograft was found in two children with Ionescu-Shiley valve. All except one had relatively successful operative results. The unsuccessful one died intraoperatively from low output syndrome.

  • Actinomycosis is a chronic suppurative granulomatous disease due to Actinomyces israelii characterized by multiple abscess and sinus tract formation with dense fibrous scarring. This disease, especially thoracic infection, is very rare in Korea so we are not famiIliar with to make diagnosis and treatment. Otherwise the unspecificity of the clinical symptoms and the lack of adequate examination recedure (as anaerobic fungus culture) are the causes of misdiagnosis. Thoracic actinomycosis is very similar to chronic infectious disease of the lung and chest or thoracic neoplasm. Recently we experienced a case of thoracic actinomycosis (bronchopulmonary) which had been confused with chronic lung abscess and pathologically confirmed as broncho-pulmonary actinomycosis. The purpose of this report is to review our experience more thorouly to enhance consideration of Artinomycosis.

  • Gabapentin is an oral antiepileptic agent with an unknown mechanism of action. There have been many proposed uses for gabapentin, including neuropathic pain, reflex sympathetic dystrophy, postherpetic neuralgia, midscapular pain secondary to radiation myelopathy and migraine prophylaxis. This report presents patients who were treated with gabapentin when other pharmacologic interventions failed to relieve neuropathic pain 3 patients with neuropathic pain were included among these cases. All patients were started on 200 mg gabapentin. The maximum dose required for pain relief was between 800 mg and 2400 mg. Gabapentin may be a useful adjunct for treating neuropathic pain with minimum of side effects.

  • Granulocytic sarcoma (chloroma) is a rare, solid tumor of myelogenous stem cells, shows usually in patients with acute myelogenous leukemia and less commonly in patients with chronic myelogenous leukemia or myeloproliferative disorders. We report here a patient presenting acute paraparesis due to spinal epidural granulocytic sarcoma causing spinal cord compression in acute myelogenous leukemia with a brief review of literature.

  • Stellate ganglion block is most commonly used in pain clinic because it is an easy procedure and it has broad indications reported that Angina pectoris, tachyarrhythmia and long QT syndrome (LQTS) are indicated. LQTS is a disorder of the abnormalities of cardiac sympathetic innervation and of myocardial repolarization. LQTS is characterized by marked prolongation of the QT interval, often manifestating as syncope, seizures, or sudden death due to polymorphic ventricular tachyarrhythmia known as torsades de pointes. Treatment of symptomatic patients usually begin with beta blocker. The elective treatment of LQTS patients unresponsive to beta blocker is the left cardiac sympathetic denervation. We report a case of LQTS patient who had received stellate ganglion block.

  • Lipomas are the commonest soft tissue tumor. However, those arsing in the hand are infrequent. Lipomas in the hand that exhibit a size of more than 5 cm call giant lipoma, these are very rare only case reports and small series of this entity have been described. We could experience a case about giant lipoma of the hand which cannot easily contact, we report a case including review of literatures.

  • Mucoepidermoid cancer of the bronchus in childhood is extremely rare. These tumors generally produce symptoms of airway obstruction, often misdiagnosed as unresolved tuberculosis. Distant metastasis is an uncommon finding of this malignancy, therefore complete surgical resection is treatment of choice. The prognosis of these tumors correlates with the histologic grade of the tumor. We report our clinical experience of mucoepideromoid cancer in a 10-year-old child who visited our hospital with symptoms of recurrent cough and fever. The patients underwent successful removal of tumor by bilobectomy via thoracotomy after bronchoscopic biopsy.
