A Study on the Muscle Activity During Asymmetric Load Handling (허리의 비틀림 각도에 따른 근육 활동 분석)

  • Low back pain has been known as the most frequent musuculoskeletal disorders in modern industrial society and cost by low back pain is increasing mon and more. The asymmetric lifting has been identified as a major risk factor of low back pain. In this study, the muscle activity and muscle exertion level during asymmetric load handling (without trunk flexion) was estimated. The results of normalized MVC measurement were decreased about 16%, 24%, 34% respectively as the asymmetric angle was $30^{\circ}$, $60^{\circ}$, $90^{\circ}$. From the results of EMG measurement contralateral muscles were more active than ipsilateral muscles. RMSEMG values of right erector spinae muscles were decreased as the work posture went to 90$^{\circ}$ and those of left erector spinae muscles were increased until the asymmetric angle was 40$^{\circ}$ but decreased continually over 40$^{\circ}$. And for seven of subjects, activities of left and right latissimus dorsi muscles were maintained constantly, while for remainer, those were irregular.

A Study for Maximal Force Exertion on Upward Slopes (상승면상의 최대 발휘근력에 대한 연구)

  • Many studies have been done to determine the magnitudes of force a man can exert on the objects. However, very little attention has been paid to those In postures on slopes. This paper was aimed to evalute how human work postures would affect the push/pull force and to grasp the relationships between the rectified EMG(REMG) measured at the trunk muscles and force exerted on upward slopes. Two subjects participated in the experiment. The results showed no lineal relationship between the REMG and exerted force at handle. But as the slope and handle height increased, exerted force and muscular stress on erector spinae or rectus abdominis were generally increased. It was notified that since ANOVA did not detect any statistical significance in REMG variation due to dominant muscles, careful application and interpretation of the REMG should be required in analyzing maximal force exertions.

  • TAPVC represents 1 to 4% of all congenital cardiac defects. Generally severe and refractory cardiac failure develops in the majority of patients in the early infancy. In a small minority of patient, they are relatively asymptomatic in infancy, but symptoms will develop of necessity In the later life. Our three cases had dyspnea on exertion and cyanosis and had a history of frequent respiratory infections. All 3 cases had the typical showman configuration on roentgenograms of the chest. Current surgical therapy has greatly altered the unfavorable course of these patients after institution of extra-corporeal circulation. We have experienced three cases from June, 1973 to May, 1983, the patients were undergone complete repair with extracorporeal circulation at the Department of Thoracic and Cardiovascular Surgery in the Yonsei University. The ages ranged from 12 years to 14 years, all cases were supracardiac type of TAPVC. Only one case died due to bleeding from anastomosis site between LA and pulmonary venous trunk. In the remaining two patients with complete corrections of TAPVC, there was excellent relief of symptoms such as dyspnea and cyanosis. Their postoperative course have been good during follow-up.

Multiple Pulmonary Arteriovenous Fistula combined with Cyanosis Report of one Case (청색증을 동반한 다발성 폐동정맥루 치험 1례)

  • Pulmonary arteriovenous fistula is a rare congenital vascular malformation in the lung, resulting from erroneous capillary development, with incomplete formation or disintegration of the vascular septa that would normally divide the primitive connection between the venous and arterial plexus. The pathogenesis of its symptom is that unoxygenated and desaturated arterial blood enter into the pulmonary venous system directly. Recently we have experienced a case of multiple pulmonary arteriovenous fistula in a 15 year old male patient, who presented the symptom of cyanosis and dyspnea on exertion. The operation revealed well circumscribed and multilobulated aneurysmal lesion in left lower lobe with its subpleural and posterolateral basal location, and another aneurysmal lesion in inferior lingular segment of left lung. There was no abnormal connection between the fistula and systemic circulation. The left lower lobectomy was performed along with local extirpation of the inferior lingular segment of left lung. Both lesions showed angiomatous dilatation of the various sized vessels embedded in the parenchyma microscopically. Postoperative clinical course disclosed much improvement in symptoms and in the value of blood gas analysis. The patient was discharged without any complication.

  • We have experienced a case of left side chylothorax following the operation of PDA and lung cancer. A 26-years old woman visited to Inje medical college Pusan Paik hospital with frequent upper respiratory infection and dyspnea on Exertion. The result of the series of studies, she has been PDA and lung cancer, and then simple ligation of PDA and left pneumonectomy was performed. At 6days after operation, chylothorax was detected. The chylothorax was treated with conservative treatment only thoracentesis, NP0 and IV hyperalimentation

Coronary Arteriovenous Fistula (관상동정맥루 -수술치험 1례-)

  • Congenital coronary arteriovenous fistula is relatively uncommon, but with widespread use of cardiac catheterization, angiography and selective coronary arteriography are being recognized with increasing frequency. Recently we experienced one case of congenital coronary artery fistula which originated from the right coronary artery. The case was 25 year-old-male, who complained of dyspnea on exertion and continuous murmur was heard, and diagnosed as right coronary artery fistula by the cardiac catheterization and aortography. On the operation field, the right coronary artery was markedly dilated from aorta to the middle segment at acute margin of the right ventricle, which the hen-egg sized aneurysm was noticed. The dilated coronary ostium and fistular site were obliterated with several horizontal mattress sutures. And the dilated tortuous right coronary artery with aneurysm was excised. Postoperative course was uneventful and discharged without problem.

Surgical Correction of the Stenosis of Descending Thoracic Aorta in Takayasu's Arteritis (Takayasu 동맥염에 의한 하행흉부대동맥 협착의 수술치험 -2례 보고-)

  • Takayasu`s arteritis is one of chronic inflammatory disease characteristically involving the aorta and it`s major branches. We experienced two surgical cases of Takayasu`s arteritis associated with the stenosis of the descending thoracic aorta. One case was 15 year-old girl and she was admitted because of dyspnea on exertion for 12 months. Aortogram showed the stenosis of the descending thoracic aorta from just below left subclavian artery to the 9th thoracic vetebra. The other case was 10 year-old girl and she was admitted because of URI and hypertension. Aortogram showed narrowing of right innominate artery, but developed collateral circulation, and the stenosis of the descending thoracic aorta near the 9th thoracic vertebra. In each case, bypass graft from the ascending aorta to the abdominal aorta just above the inferior mesenteric artery was performed with satisfactory result.

  • The maintenance of normal blood glucose levels at rest and during exercise is critical. The maintenance of blood glucose homeostasis depends on the coordination and integration of several physiological systems, including the sympathetic nervous system and the endocrine system. During prolonged exercise increased demand for glucose by contracting muscle causes to increase glucose uptake to working skeletal muscle. Increase in glucose uptake by working skeletal muscle during prolonged exercise is due to an increase in the translocation of insulin and contraction sensitive glucose transporter-4 (GLUT4) proteins to the plasma membrane. However, normal blood glucose level can be maintained by the augmentation of glucose production and release through the stimulation of liver glycogen breakdown, and the stimulation of the synthesis of glucose from other substances, and by the mobilization of other fuels that may serve as alternatives. Both feedback and feedforward mechanisms allow glycemia to be controlled during exercise. This review focuses on factors that control blood glucose homeostasis during prolonged exercise.

An Extra-adrenal Pheochromocytoma Presenting with Spontaneous Intracerebral Hematoma

  • We report a 18-year-old man, who has been taking antihypertensive medication for 1month in a local clinic, presented with a sudden onset headache followed by left blindness. He experienced palpitation and chest discomfort during physical exertion since 2years before admission, but unfortunately has been ignored. Brain CT showed intracerebral hemorrhage in the left temporoparietal area, but cerebral angiogram and magnetic resonance image revealed no vascular anomaly. He was managed conservatively, and headache and visual loss were improved over time. Subsequently, on the evaluation of hypertension, he was diagnosed as having extra-adrenal pheochromocytoma on left paraaortic area from the results of endocrinological evaluations, abdominal CT scan, and $^{131}I$-MIBG scintigraphy.

  • It is widely acknowledged that Achilles tendinopathy and resultant degeneration of the Achilles tendon could be either due to vigorous physical exertion or due to inflammation of the tendon associated with systemic disease. The overuse injuries are generally multifactorial in origin and are caused by repetitive strain of the affected tendon till the tendon can no longer endure the tensile stress. Various alignment and biomechanical faults are claimed to play a causative role. Only 2% of patients complaining of Achilles tendon pain are caused by systemic disease. However, to ensure the right approach to treatment, it is necessary to rule out inflammatory tendinitis caused by systemic diseases such as rheumatoid arthritis, gout, and seronegative spondyloarthrosis.
