치유개념을 적용한 치유단지의 계획특성 연구 - 국내 조성되어 운영되고 있는 주요 단지를 중심으로 - (A Study on the Planning Characteristics of a Healing Complex applying the Concept of Healing - Focusing on major complexes that have been constructed and operated in Korea -)

  • There are more and more citizens suffering from severe fatigue, and they wish to escape from it and spend their leisure time for healing. As a result, buildings and complexes are being constructed nationwide with healing as their theme. Particularly, they tend to build facilities with concepts like a spa, beauty, healing, meditation, nature, or forest healing. The purpose of this study is to examine the concept of healing environment and the nationwide tendencies of building facilities with healing as their theme and also investigate the planning characteristics of complexes and architecture with three representative complexes as examples. Complexes intended for healing have immersion into nature being freed from one's routine as their concept. When planning the flow of human traffic within the complexes, they try to obtain the autonomy of choice as well as the diversity of space and experiential factors in order to provide opportunities for experiencing nature. In the complexes selected for a case study here, they have planned the factors of physical environment that are associated with one another based on architectural education programs using red clay, programs specializing poetry, and healing programs using food. Typically, this is centered around outdoor experiential space, indoor meditation and education space, or fitness space. Also, it is characterized by the planning of physical environment and the complex operation of programs. Particularly, public space is divided into communal space, resting space, and health and treatment space, and health/resting space is mainly intended for health and exercise, for example, fitness, spas, or jjimjilbang (Korean dry saunas). Also, it is characterized by the planning of pitched roofs harmonized with nature and also facade planning that can positively adopt the factors of natural environment.

림프부종의 물리치료적 접근과 관리 : 전문가 견해 (Physical Therapy Approach and Management for Lymphedema : Expert Opinion)

  • Background : Lymphedema is a progressive disorder characterized by the impairment of lymph flow from tissues to the blood circulation system. This occurs as a result of damage to the lymphatic system. Complex decongestive therapy (CDT) is a multimodal, conservative therapeutic approach that is used for the management of lymphedema. CDT consists of a combination of compression therapy, manual lymphatic drainage, exercise, and skin care. Purpose : This study aimed to provide a review of available physical therapy interventions as well as general care guidelines for patients with lymphedema. Methods : The recommendations and guidelines for physical therapy management, medical management, and general information were reviewed from the following sources: 1) The American Physical Therapy Association, 2) The Norton School of Lymphatic Therapy, and 3) The International Society of Lymphology. This review contains general information, including the medical management and the importance of physical therapy in lymphedema. Physical therapy management should be based on an assessment of the patients' presenting impairments, including based on inclusion or exclusion of physical therapy interventions. This review also outlines a step-by-step approach that starts with disease diagnosis and progression all the way through to rehabilitation as an outpatient. Conclusion : Depending on the patients' journey to recovery and the requirement for rehabilitation, physical therapy interventions should focus on the patients' needs including pain, appearance, physical function and general rehabilitation. We hope that this review will provide information on evidence-based physical therapy and general care to patients with lymphedema.

중년여성의 화병과 상지관절동통에 관한 문화기술지 (An Ethnographic Study on Middle aged Women's Hwa-Byung with Upper Limb Arthropathy)

  • Background: Hwa-Byung is known as a specific Korean cultural syndrome which corresponds to DSM-IV(MMPI). Some Korean women who have experienced heart aching anger(Hwa-Byung) complain physical pains as well as psychological problems. As for these physical pains, upper limb arthropathy(e.g. golf elbow pains, tennis elbow pains, or shoulder-joint pains) have not been paid attentions. In spite of not having done her excessive physical endeavor or exercise, some Korean middle aged women complain those arthropathy pains. And they go round orthopedics, pain clinics, and oriental medicine clinics. Purpose: This study was practiced in Severance Oriental Medicine Clinic. The oriental medicine doctor had a question about the major origin of upper limb pains which were not caused by excessive physical endeavor. To answer the question, this study has been practiced. In the process of interview with some those women, the major cause of those syndromes has been revealed as Hwa-Byung. The purpose of this study is to discover the meanings of the women's life who have been experienced Hwa-Byung with upper limb arthropathy. Results: These upper limb arthropathy can be explained by meridian theory. Shoulder-joint pain and golf elbow with Hwabyung can be explained by Heart meridian of hand-shoyin and the points of these pains are in the flow of this meridian. Tennis elbow with Hwabyung can be explained by Small intestine meridian of hand-taiyang and this point is in the flow of this meridian. The results of interview with 9 middle aged women was analyzed and interpreted according to Spradley's method of ethnography. The analysis revealed three core cultural themes : 1) There are certainly external cause to provoke Hwa-Byung. A patricentric family system, husband's playing around with another woman, cruel treatment by husband's family, or financial failure may be present anteriorly. 2) The chief existing condition is the discord between husband and wife. The stoppage of mutual communication, lacking in understanding, unfeeling, heartless, or unsympathetic is an major phenomenon of married life. So the important factor is not the sexual relations or problems but the discords of communication. 3) The feeling of anger, the sense of nihility, the desire to escape, and the effort to forgiveness coexist together. At a glance, they seemed to be a process. For example, anger seemed to present at first, then nihility seemed at second, then escape seemed at third, and then forgiveness seemed at last. But at point of visiting clinic to be treated, they are mixed up together as if in a jumble.

동맥관 개존증을 보이는 개에서의 이중 결찰술 실시 3예 (Application of double ligation in 3 dogs with patent ductus arteriosus)

  • Three dogs with suspected patent ductus arteriosus were referred to Veterinary Medical Teaching Hospital of College of Veterinary Medicine, Konkuk University because of cough, shortness of breath, exercise intolerance, and syncope, Continuous murmur and thrill were detected in physical examination. Left ventricular enlargement, cardiomegaly, bulged main pulmonary artey were found in radiography. Tall R wave, wide P wave, atrial fibrillation, and turbulent flow were observed in electrocardiogram and echocardiogram. Thoracotomy was performed at left fourth intercostal space under isoflurane anesthesia. Patent ductus arteriosus was double ligated with 1-0 silk. The median (mean ${\pm}$ SD) diameter of patents was $8.0{\pm}2.0mm$. The median operation time was $36{\pm}6.6min$. On examination right after surgery, continuous murmur, thrill, atrial fibrillation, and turbulent flow were disappeared. On 30 days after surgery, clinical signs, left ventricular enlargement, and, bulged main pulmonary artery were disappeared. VHS 12.5 and 13.5, R wave 3.3 and 3.0 mV, and P wave 0.05 and 0.05 sec were decreased to 10.0 and 10.5, 1.8 and 2.0 mV, and 0.04 and 0.04 sec respectively in case 1 and 2. Ratio of aorta and main pulmonary artery in diameter was changed 1 : 1.3 and 1 : 1.6 into 1 : 1.1 and 1 : 1 respectively in case 1 and 2. Mild tear developed during dissection in case 3 and hemorrhage was controlled by vascular Devakey forceps. However, the dog died. On 12 months after surgery, patients (case 1 and case 2) have not showed abnormal signs.

거시경제변수를 반영한 실물옵션 시나리오별 공공임대주택리츠 분양전환 가치 분석 (Analysis of Value for Ownership Conversion in the Public Rental Housing REITs According to Real Option Scenarios Reflecting Macroeconomic Variables)

  • 공공임대주택리츠는 임대기간이 만료되면 임차인이 우선 분양전환 할 수 있는 권리를 가지고 있다. 10년 공공임대주택의 분양전환 시점은 임대의무기간 후 공고되지만 공공주택 특별법의 개정으로 임대의무기간의 1/2만 지나면 임대인과 임차인의 합의하에 분양전환 할 수 있게 되었다. 이를 통해 자가 마련을 위한 수요는 많지만 임대사업자는 수익확보가 되지 않는다는 이유로 조기 분양전환에 소극적인 태도를 보이고 있으며, 분양전환가 산정에 있어 임대인과 임차인의 합의점 도출에 어려움이 있다. 또한 공공임대주택이 '리츠' 방식으로 공급되면서 조기분양전환 수익성에 관련된 이해관계자가 증가하여 임대주택의 공공성을 저해하고 공기업과 정부의 불신이 발생할 우려도 있다. 따라서 공공임대주택리츠의 수익성을 확보하고 공익적 목적을 동시에 실현할 수 있는 조기분양전환 적정시점 분석이 필요하다. 본 연구에서는 임차인이 행사할 수 있는 분양전환권을 옵션의 특성으로 본다. 분석 대상인 '공공임대주택리츠'의 특성이 거시경제변수의 영향을 받을 것으로 고려되는 바 거시경제 상황의 변화에 따른 시나리오별 실물옵션 가치를 분석했다. DCF(Discountes Cash Flow) 모형에 의해 조기분양전환 적정시점 확보가 가능하다. 따라서 변수의 변동성을 예측하여 조기분양전환 시점을 고려해야 할 것으로 생각한다.

치료 후 기류제한이 호전된 만성폐쇄성폐질환 환자에서 폐용적의 변화 (Change of Lung Volumes in Chronic Obstructive Pulmonary Disease Patients with Improvement of Airflow Limitation after Treatment)

  • 배 경 : 만성폐쇄성폐질환 환자의 운동능력 및 호흡곤란은 폐용적에 크게 영향을 받는다. 그러나 만성폐쇄성폐질환에서 기류의 변화와 이와 관련된 폐용적 변화에 대한 연구는 많지 않다. 본 연구는 기류제한이 호전된 만성폐쇄성폐질환 환자에서 기류제한의 호전과 폐용적 변화의 상관관계를 알아보고자 하였다. 방 법 : 폐용적의 변화를 평가하기위해 중등도 이상의 만성 폐쇄성폐질환 환자를 대상으로 추적 검사에서 $FEV_1$이 기저치에 비해 12.3% 이상 증가한 17명의 환자를 대상으로 기저치와 추적 폐활량 및 폐용적을 비교 분석하였다. 결 과 : 평균 연령은 66세 였고, 76%가 남자였다. 기저치 $FEV_1$은 평균 0.98 L (예측치의 44.2%), $FEV_1$/FVC은 47.5%, 기저치 평균 RV은 4.65 L (예측치의 241.5%) 였다. 추적 폐기능검사는 평균 80일이 지나서 시행하였다. 추적 폐기능 검사의 평균값의 변화는 $FEV_1$ 270 ml, FVC 390 ml 정도 증가한데 비하여 전폐용적은 690 ml 그리고 잔기량은 1,040 ml 감소하였고, 이들 사이에 통계적으로 유의한 상관관계는 관찰되지 않았다. 결 론 : 만성폐쇄성폐질환 환자에서 추적시 폐용적을 포함한 폐기능검사가 치료반응 판정에 도움을 줄 수 있을 것으로 사료된다.

  • 저자들은 내원 1개월 전부터 시작된 잦은 마른기침, 운동 시 호흡곤란, 8 kg의 급속한 체중 감소가 주 증상인 8세 남아에서 특발성 간질성 폐렴으로 진단하고, 경구용 프레드니손으로 치료한 후 1년 이상 추적 관찰하여 현재 학교에서 가벼운 체육활동을 할 수 있을 정도의 임상적 호전을 보인 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

  • 배뇨 후 실신은 신경 매개성 반사에 의한 실신으로 사태실신의 한 종류이며 건강한 남성에서 아침에 기립 배뇨시에 잘 일어난다. 실신의 원인은 다양하기 때문에 배뇨후 실신의 진단을 위해 정확한 병력과 심전도, 운동부하검사, 심초음파, 기립경사검사, 뇌파, 뇌 자기공명영상, 요류 동태 검사 등이 사용된다. 배뇨후 실신의 기전은 부교감 신경계의 반사 자극으로 서맥과 말초혈관 확장이 일어나고 Valsalva 효과에 의해 심실로 정맥환류가 감소하게 되어서 대뇌 혈류감소가 일어나 실신하게 된다. 실신시 뇌파는 대뇌 혈류감소에 의해 고진폭의 서파와 평탄화 소견이 나타나게 되고 심한 허혈시에는 허혈성 경련을 일으킬 수도 있다. 저자들은 9세 남아로 기립배뇨시 실신을 보인 후 저혈압을 보이고 특징적인 뇌파소견을 보인 환아를 경험하였기에 보고하는 바이다.

  • Asthma is a prevalent and serious health problem in Korea. Recently, the Korean Asthma Guideline has been updated by The Korean Academy of Tuberculosis and Respiratory Diseases (KATRD) in an effort to improve the clinical management of asthma. This guideline focuses on adult patients with asthma and aims to deliver up to date scientific evidence and recommendations to general physicians for the management of asthma. For this purpose, this guideline was updated following systematic review and meta-analysis of recent studies and adapting some points of international guidelines (Global Initiative for Asthma [GINA] report 2014, National Asthma Education and Prevention Program [NAEPP] 2007, British Thoracic Society [BTS/SIGN] asthma guideline 2012, and Canadian asthma guideline 2012). Updated issues include recommendations derived using the population, intervention, comparison, and outcomes (PICO) model, which produced 20 clinical questions on the management of asthma. It also covers a new definition of asthma, the importance of confirming various airflow limitations with spirometry, the epidemiology and the diagnostic flow of asthma in Korea, the importance and evidence for inhaled corticosteroids (ICS) and ICS/formoterol as a single maintenance and acute therapy in the stepwise management of asthma, assessment of severity of asthma and management of exacerbation, and an action plan to cope with exacerbation. This guideline includes clinical assessments, and treatment of asthma-chronic obstructive pulmonary disease overlap syndrome, management of asthma in specific conditions including severe asthma, elderly asthma, cough variant asthma, exercise-induced bronchial contraction, etc. The revised Korean Asthma Guideline is expected to be a useful resource in the management of asthma.

잠수함 수동소나 시뮬레이터 알고리즘 (An Algorithm for Submarine Passive Sonar Simulator)

  • 잠수함 소나 운용 요원의 능력을 향상시키기 위한 실제 해상 훈련은 많은 비용과 제약사항이 따른다. 소나 시뮬레이터는 이러한 문제점을 해결하고, 실제와 유사한 전장 환경을 모의함으로써 소나 운용 요원의 능력과 훈련 성과를 극대화시킨다. 본 연구에서는 수동 소나 시뮬레이터의 알고리즘을 제시하였으며, 알고리즘은 기동모듈, 소음원모듈, 소음 전달 모듈의 3가지 모듈로 나뉘었다. 기동모듈은 3차원 좌표계를 이용하여 함정의 기동을 구현하였으며, 시간 간격은 함정의 변침률에 따라 설정하였다. 소음원 모듈은 표적 소음, 해양 배경 소음, 자체소음으로 구성하였다. 표적 소음은 주파수 특성에 따라 비변조 협대역, 변조 협대역, 비변조 광대역, 변조 광대역 신호로 구분하였으며, 톤수와 속력에 의존하는 함정 방사소음 준위를 적용하였다. 해양 배경 소음은 음향도파관 효과가 고려된 바람 소음과 그외 배경 소음으로 모의하였으며, 자체소음은 유체소음과 소나돔 삽입 손실로 모의하였다. 소음 전달 모듈은 음선 기반의 모델을 이용하였으며, 기동 모듈의 각 시간에서 고유음선의 진폭, 위상, 시간지연을 합산하여 주파수 영역에서 표적 소음에 곱하였다. 최종적으로 시나리오에 따른 모의 결과 실제 해양에서 발생하는 소음과 유사한 경향을 확인할 수 있었다.
