황련아교탕(黃連阿膠湯)으로 호전된 3례의 병인(病因) 분석 : 분노와 수면장애에 대한 『상한론(傷寒論)』 기록의 이해 (Analyzing etiologic factors of Cases : Understanding Sanghanlun record of anger and sleeping disorder)

  • Objective : We analyzed 3 clinical cases to estimate the possibility of shanghanlun as a completed clinical text which has its own diagnostic system. Methods : We collected 3 cases treated by huang lian e jiao tang conducted by a practiced shanghanlun doctor and defined the conceptional meaning of each provisions by analyzing the application of them in each case. Results : Shanghanlun provisions dualized with Je Gang(提綱) and Jo Mun(條文) is the record of preceding factors which describe the phenomenon initiating the diseases. Conclusions : Rudimentarily a factor can bring about many outcomes, so if shanghanlun is structured record of the factors which can make a human sick from many diseases, it can be used as the completed clinical text only with those 113 prescriptions. Factors that shanghanlun recorded seem somewhat different from western conventional medicine because the field of each factors are very varied from biological phenomenon to psychological phenomenon. However it is not that weird because psychosomatic medicine in western conventional medicine also point out the factors come from mental, society and behavior.

Etiology and Patterns of Maxillofacial Fractures in 518 patients in Korea

  • Purpose: Different patterns in the causes of maxillofacial injury are thought to correlate with socioeconomic status and regional environment. This study investigated maxillofacial fractures in order to analyze maxillofacial trauma characteristics and the relationship between the causes and injury patterns in Korea. Material and methods: A total of 518 patients with maxillofacial fractures who were treated at the Seoul National University Boramae Hospital between 1996 and 2004 were retrospectively analyzed. Data were obtained from the patients' medical records and radiographs. The male to female ratio in the patient group was 2.78:1, and the mean age was 32.3 years. Results: Midfacial fractures were the most common location of injury (46.1%). The most common etiologic factor was an activity associated with daily life (42.6%) including falls, stumbling, and collisions. The second most common cause was assault (32.4%), followed by traffic accidents (13.7%). In the case of midfacial fractures and mandibular fractures, assault was the most common etiologic factor, whereas in the case of alveolar bone fractures, activities associated with daily life were the most common cause. With regard to age groups, assault was the most common cause for patients between 10 and 39 years old and an activity associated with daily life was the most common cause in those under 10 years and over 40 years. Conclusions: This study concluded that activities associated with daily life and assault causes a large proportion of Korean maxillofacial injuries and that preventive measures should be implemented in order to minimize these risks.

치주질환시의 혈생화학적연구 (Blood Chemistry in Periodontal Disease)

  • The causes of periodontal disease have been descried as malocclusion, trauma from occlusion, local irritants and poor oral hygien. The systemic factor has also been considered as a etiologic factor of periodontal disease. On the other aspect systemic condition could be affected by periodontal disease. For the study of relationships between periodontal disease and systemic condition, twenty patients with periodontal disease and twenty persons with no periodontal involvement were evaluated for inorganic elements and organic materials in the blood. The results of the blood analysis of the two groups were as follows. : Phosphorus and alkline-phoshatase in the group with periodontal disease showed slightly increased phenomenon compared to the control group, and on the other hand calcium and calcium-phosphorus ratio decreased phenomenon. But there is no any significant alteration in the content of each element between the two groups.

  • Conclusions for the relationship between a conceptional model of stress theory and its examination based on the Oriental medicine; 1. It was considered that a general conception of stress is included into the meaning of Ki in the Oriental medicine. 2. The response-based model to stress could be comparable to a conception of Ki-pathogens interchange In the Onental medicine. 3. The stimulus-based model may be explained as a modern conception of immoderation feeling related with an internal etiological factor, specially injury of seven emotions, among three groups of etiological factors for disease. 4. The feedback conception based on the interaction model could explain the principal of reversible emotion therapy in the nine Ki. 5. In the Oriental medicine, a study to clarify a emotional etiologic factor and its pathophysiologlcal mechanism has been continued for long time before establishment of stress theory.

하악골 골절 환자들의 악관절 상태 평가: 일차보고 관절내시경적 검사, 조직형태학적 및 관절활액 분석 (ANALYSIS OF TMJ STATUS IN THE PATIENTS WITH MANDIBULAR FRACTURES: PRELIMINARY STUDY ARTHROSCOPIC EXAMINATION, HISTOMORPHOLOGY AND JOINT FLUID ANALYSIS)

  • The purpose of this study is to validate the potential etiologic factors for temporomandibular disorder(TMD). TMJ arthroscopic examination was performed in upper joint compartment of 32 joints from 20 patients with mandibular fractures. Synovial fluid was collected from the upper joint space during pumping manipulation with normal saline. Cytologic smearing and histomorphologic exam of synovial fluid were performed in 15 joints. Prostaglandin $E_2(PGE_2)$ concentration was measured in 11 joints. Leukotriene $B_4(LTB_4)$ concentration was measured in 8 joints. There were several arthroscopic variables such as ecchymosis, fibrillation, and adhesion. Histomorphologic exam showed a variety of findings such as bloody smears, cellular cluster, degenerated cells and cartilage, undifferentiated crystal. Mean $PGE_2$ concentrations were 316.5 pg/ml. Mean LTB4 concentrations were 45.9pg/ml. This study demonstrated a variety of findings on inflammatory and degenerative changes of TMJ. Because acute trauma such as mandibular fracture is a major etiologic factor in cartilage degradation and biochemical and intraarticular pathology, clinicians must identify and address TMJ signs and symptoms during follow-up periods in the long term.

일란성 쌍생아들에서 관찰된 과잉치 (Supernumerary Teeth in Monozygotic Twins)

  • 과잉치의 발생 원인 및 빈도, 치료 등 과잉치에 대한 많은 선학의 연구가 있었으나, 현재에 있어서도 과잉치의 발생 원인은 여전히 명확하지 않은 상태이다. 전체 인구에서 0.15%~1.0%의 발병률을 보이는 과잉치의 병인론으로는 계통발생학적 돌연변이(atavism), 치배분열론(dichotomy theory), 치아판의 국소적인 증식 그리고 유전 및 환경적 요인들의 조합(unified etiologic explanation) 등이 있다. 동일한 수정란으로부터 생긴 쌍생아인 일란성 쌍생아는 유전학적으로 동일한 개체이다. 이들에게서 나타난 과잉치에 대한 보고는 과잉치의 병인론에 있어 유전적인 요인이 강하게 작용함을 뒷받침할 수 있다는 면에서 의미를 가진다. 본 연구에서는 경북대학교 치과병원 소아치과 외래를 내원한 3쌍의 일란성 쌍생아에서 나타난 과잉치를 관찰하였으며, 이는 유전적인 원인에 의한 과잉치의 발생을 뒷받침함에 의미가 있어 보고하는 바이다.

Expression of Helicobacter pylori urease in plants to use as an edible vaccine

  • Helicobacter pylori is the etiologic agent of human gastritis and peptic ulceration and produces urease as the major protein component on its surface. H. pylori urease is known to serve as a potent immunogen as well as major virulence factor. In order to express the recombinant urease in tobacco plants, a DNA fragment containing the minimal H. pylori urease gene cluster was subcloned into a plant expression vector. The recombinant vector was transformed to tobacco plants. The integration of the recombinant plasmids into tobacco chromosomal genome was verified by genomic PCR. Expression to mRNA was confirmed by Northern blot analysis, and expression to recombinant urease protein was observed by Western blot analysis. These results showed that the recombinant urease can be produced in tobacco plants and will be tested for immune response to use as an edible vaccine.

Polyclonal Antibody Against the Active Recombinant Helicobacter pylori Urease Expressed in Escherichia coli

  • Helicobacter pylori is the etiologic agent of human gastritis and peptic ulceration and produces urease as the major protein component on its surface. H. pylori urease is known to serve as a major virulence factor and in a potent immunogen. In order to express the recombinant urease at a higher level, a DNA fragment containing the minimal H. pylori urease gene cluster was subcloned into a high copy-number vector. The recombinant H. pylori urease expressed in an E. coli strain that was grown in a rich medium supplemented with added nickel was purified to near homogeneity by using DEAE-Sepharose, Superdex HR200, and Mono-Q (FPLC) columns and the purified enzyme possessed the specific activity of 1255 U/mg. Polyclonal antibodies raised against the purified recombinant H. pylori urease were shown to be very specific when subjected to Western blot analysis, in which crude extracts from the H. pylori ATCC strain and the recombinant E. coli strains expressing various bacterial ureases were exnmined for cross-reactivity.

  • Ludwig's angina is an aggressive infection that bilaterally involves the submandibular, sublingual, and submental fascial spaces. Dental infection is the major etiologic factor in the pathogenesis. The treatment of Ludwig's angina involves management of the airway, administration of appropriate antibiotics, and surgical intervention when needed. Airway obstruction due to edema of the mouth floor remains the most life-threatening problem of Ludwig's angina. We report a case of Ludwig's angina with review of literatures.

지속적인 치통으로 나타난 상악동 악성 림프종 1예

  • Many teeth have been mistakenly extracted or endodontically treated because of an incorrect diagnosis of orofacial pain including toothache, A case report of persistent toothache originating from a malignant lymphoma of left maxilary sinus is presented. Root canal therapy and extraction of left upper quadrant teeth from canine to second molar did not resolve the chief complaint. The patient was referred to a neurologist and was diagnosed with a malignant lymphoma, a rare lesion of the maxillary sinus. The case stresses the importance of malignant neoplasms of maxillary sinus as a potential etiologic factor in the differential diagnosis of orofacial pain.
