  • An esophageal leiomyoma is the most common benign tumor of the esophagus mainly occurred in intramural portion. Occasionaly, it is difficult to discriminate esophageal malignancy from large leiomyoma. Although F-18 FDG PET has been used for differentiating malignant from benign disease, false-positive cases have been reported. Recently, uterine leiomyoma has been reported to have relatively high F-18 FDG uptake in some patients but little is known about how an esophageal leiomyoma might be showed on F-18 FDG PET. We report a case of esophageal leiomyoma that showed high FDG uptake on PET images.

  • Esophageal leiomyoma is a very rare disease. We present a patient who underwent enucleation of esophageal leiomyoma through a left thoracotomy. The patient was suffered from substernal pain and chest discomfort for 4 months.The esophagogram revealed irregular ovoid smooth filling defect in just proximal portion of G-E junction with the normal mucosal folds. Chest CT demonstrated well-defined, polypoid tumor mass on the anterolateral wall of the distal esophagus. Esophagoscopy revealed normal intact mucosal patterns with swollen hard protruded tumor mass lesion from the just proximal portion of G-E junction. In June, 1993, patient underwent enucleation of esophageal leiomyoma through the left thoracotomy. A horseshoe and spiral shaped, whitish firm tumor mass was noted on the distal esophagus, and the tumor mass was enucleated by blunt dissection carefully. The esophageal leiomyoma was confirmed with histopathological examination. The postoperative course was smooth and uneventful.

  • Though leiomyoma is the most common benign esophageal tumor, it is rare in the upper third of esophagus. Recently, we experienced two cases of esophageal leiomyoma in the upper third of esophagus. The patient were a 53 year old female and a 40 year old male. On esophagoscopy, masses were found at 20\ulcorner23 cm, 22\ulcorner26 cm from upper incisor with normal overlying mucosa. Enucleation was done through a right posterolateral thoracotomy without complications.

  • We experienced a case of esophageal leiomyoma combined with achalasia that is very rare. Patient had suffered from severe dysphagia and postprandial vomiting and diagnosis was accomplished by esophagography, esophagoscopy, chest CT, and esophageal motility test. The operative treatment was done through left lateral thoracotomy by enucleation of the submucosal tumor and esophagomyotomy. By histopathological findings, the diagnosis of leiomyoma was confirmed and LES biopsy revealed absence of the ganglion cells of myenteric and Auerbach`s plexus. Symptoms of the patient were completely relieved and postoperative course was uneventful.

Thoracoscopic Enucleation of Esophageal Leiomyoma (흉강경을 이용한 식도 평활근종의 절제)

  • Leiomyoma is the most common benign tumor of the esophagus,and surgical enucleation is the treatment of choice. Recently we successfully performed thoracoscopic enucleation of large esophageal leiomyoma without complication in one patient. The 46 years old male patient complained epigastric discomfort and showed a submucosal mass in lower esophagus under the endoscopic ultrasonography . During operation minimal perforation occurred, it was closed with clipping without conversion to an open procedure.The tumor size was 8cm x 3cm x 1.5cm respectively. There were less post-operative pain,minimal wound size, and early recovery time.Patient was satisfactory these outcome. These result suggest that esophageal enucleation was performed more large size benign tumor and esophageal perforation during operation was treated thoracoscopically.

  • A 59-year old woman visited us for incidentally detected posterior mediastinal mass. Preoperative esophagography, esophagoscopy, esophageal ultrasound and computed tomography showed a esophageal submucosal tumor. With the diagonsis of esophageal leiomyoma, the patient underwent right side video-assisted thoracoscopic surgery (VATS): The mediastinal pleura and the esophageal muscle layers were longitudinally opened and the tumor was enucleated. Esophagography performed at 6th postoperative day revealed no esophageal mucosal bulging or leakage. The patient was discharged reveiving a soft diet on the 7th postoperative day.

  • Intramural leiomyoma is the most common benign esophageal tumor, but it is rare compared with carcinoma. Although the most common symptom is dysphagia, this tumor may not cause symptoms before attaining large size. Radiographic and endoscopic findings allow an accurate diagnosis to be made before operation in most patients. The treatment of choice is enucleation without mucosal encroachment. Recently, we experienced two cases of esophageal leiomyoma on the mid portion of esophagus and enucleation of tumors were done by blunt dissection with caution. In one case, postoperative chylothorax was complicated, but cured with conservative treatment. The tissue diagnosis was confirmed with pathological finding.

  • Though leiomyoma is the most common tumor of esophagus, it accounts for only 1% of all esophageal tumors. Most of the leiomyomas are intramural type originating from the muscularis propria and only l% of them is intraluminal pedunculated type originating from muscularis mucosae. Recently, a 30-year-old male was admitted to our hospital because of dysphagia. Radiologic examination showed that intraluminal tumor 5cm in diameter was found at the cervical esophagus. Endoscopic examination showed that the tumor was covered with normal mucosa. The patient underwent surgical excision through the left cervical approach. After full, longitudinal esophagotomy, the intraluminal pedunculated tumor was successfully enucleated. Esophageal leiomyoma was confirmed histopathologically. Postoperative course was uneventful and the patient was relieved from dysphagia.

  • A 56 years old woman referred to our hospital with dysphagia and epigastric soreness. Gastroendoscopy revealed huge submucosal tumor with ulceration extending from distal esophagus to lesser curvature of stomach. Subsequent computed tomography(CT) demonstrated soft tissue mass encircling distal esophagus, and 18F-FDG PET/CT demonstrated intense $^{18}F-FDG$ accumulation in it. Finally this case was diagnosed as esophageal leiomyoma based on pathologic evaluation of the surgical specimen.

  • We experienced a case of upper esophageal leiomyoma successfully excised by thoracoscopic surgery. A 29-year-old male was presented with retrosternal discomfort and mild dysphagia and an esophagogram revealed smooth fElling defect In the upper third of the intrathor cic esophagus, and esophagoscopy showed a submucosal tumor without mucosal infiltration. Chest CT and MRI were performed to confirm size, character and location of the esophageal mass, the absence of infiltration of surrounding structures, and to define mediastinal Iymphadenopathy. The tumor was excised by thoracoscopic surgery and it was diagnosed as leiomyoma (4$\times$2xlcm in size). The postoperative course was uneventful.
