Objective : Posture induced common peroneal nerve (CPN) palsy is usually produced during the prolonged squatting or habitual leg crossing while seated, especially in Asian culture and is manifested by the onset of foot drop. Because of its similarity to discogenic foot drop, patients may be diagnosed with a lumbar disc disorder, and in some patients, surgeons may perform unnecessary examinations and even spine surgery. The purpose of our study is to establish the clinical characteristics and diagnostic assessment of posture induced CPN palsy. Methods : From June 2008 to June 2012, a retrospective study was performed on 26 patients diagnosed with peroneal nerve palsy in neurophysiologic study among patients experiencing foot drop after maintaining a certain posture for a long time. Results : The inducing postures were squatting (14 patients), sitting cross-legged (6 patients), lying down (4 patients), walking and driving. The mean prolonged neural injury time was 124.2 minutes. The most common clinical presentation was foot drop and the most affected sensory area was dorsum of the foot with tingling sensation (14 patients), numbness (8 patients), and burning sensation (4 patients). The clinical improvement began after a mean 6 weeks, which is not related to neural injury times. Electrophysiology evaluation was performed after 2 weeks later and showed delayed CPN nerve conduction study (NCS) in 24 patients and deep peroneal nerve in 2 patients. Conclusion : We suggest that an awareness of these clinical characteristics and diagnostic assessment methods may help clinicians make a diagnosis of posture induced CPN palsy and preclude unnecessary studies or inappropriate treatment in foot drop patients.
Objective : The development of magnetic resonance neurography(MRN) has made it possible to produce highresolution images of peripheral nerves themselves, as well as associated intraneural and extraneural lesions. We evaluated the clinical application and utility of high-resolution MRN techniques for the diagnosis and treatment of a variety of peripheral nerve disorder(PND)s. Material and Method : MRN images were obtained using T1-weighted spin echo, T2-weighted fast spin echo with fat suppression, and short tau inversion recovery(STIR) fast spin-echo pulse sequences. Fifteen patients were studied, three with brachial plexus tumors, five with chronic entrapment syndromes, and seven with traumatic peripheral lesions. Ten patients underwent surgery. Results : In MRN with STIR sequences of axial and coronal imagings, signals of the peripheral nerves with various lesions were detected as fairly bright signals and were discerned from signals of the uninvolved nerves. Increased signal with proximal swelling and distal flattening of the median nerve were seen in all patients of carpal tunnel syndrome. Among the eight patients with brachial plexus injury or tumors, T2-weighted MRN showed increased signal intensity in involved roots in five, enhanced mass lesions in three, and traumatic pseudomeningocele in three. Other associated MRI findings were adjacent bony signal change, neuroma, root adhesion and denervated muscle atophy with signal change. Conclusion : MRN with high-resolution imaging can be useful in the preoperative evaluation and surgical planning in patients with peripheral nerve lesions.
Meloxicam-loaded microspheres were prepared with poly(D,L-lactic acid)(PLA) by a solvent-emulsion evaporation method. The morphology, particle size, drug loading capacity, drug entrapment efficiency (EE) and release patterns of drug were investigated in vitro. Various batches of micro spheres with different size and drug content were obtained by changing the ratio of meloxicam to $PLA^{\circ}{\AE}s$ with different molecular weight, PLA concentration in the dispersed phase and stirring rate. Meloxicam crystals on microsphere surface, which were released rapidly and could act as a loading dose, were observed with increasing drug content. The release rate was increased with increase in drug contents and decrease in the molecular weight of PLA. Microspheres prepared with smaller molecular weight produced faster drug release rate. The release rate of meloxicam for long-acting injectable delivery system in vitro, which would aid in predicting in vivo release profile, could be controlled by properly optimizing various factors affecting characteristics of microspheres. Blood concentration-time profile of meloxicam after intramuscular injection of meloxicam-loaded microspheres in rabbits showed possibility of long term application of this system in clinical settings.
Chiefly, painful heel syndrome is observed at old age. Many possible reasons were bursitis, plantar fasciitis, calcaneal periostitis around the calcaneus and achilles tendon. But the exact cause was not clearly identified due to complexity of subcalcaneal pain mechanism. The incidence of calcaneal spur and overweight were significant in painful heel syndrome. Our purpose of this study was analysis of underlyng cause and correlation about bony spur and overweight in painful heel syndrome. The author used incidence of heel spur on painful heel syndrome and body mass index to evaluate overweight. The material is 55 cases of painful heel syndrome patients and 60 cases of control group. Bony spur was one of the cause of painful heel syndrome. Body weight and calcaneal spur was developing factors on painful heel syndrome. And the result was as follows. 1. Spur formation incidence is 35 cases (63.6 %) in painful heel syndrome, 8 cases (13.3%) in normal control group. So, patient's group is significantly high (p<0.01). 2. Body mass index is 26.48 in painful heel syndromes, 21.75 in normal control groups. Overweight above index 27 is 22 cases (40%) in painful heel syndromes, 3 cases (5%) m normal control groups. So, patient's group is significantly high (p<0.01). 3. In painful heel syndrome, tenderness site is 46 cases (83.6%) in medial calcaneal tuberosity, 4 cases (7.3%) in central calcaneal tuberosity, 1 cases (1.8%) in both site. 4. Underlying causes of painful heel syndrome is 19 cases (34.5%) in plantar fasciitis, 16 cases (29.1%) in calcaneal periostitis, 11 cases (20%) in bursitis, 4 cases (7.3%) in tendinitis, 2 cases (3.6%) in entrapment neuropathy.
Carpal Tunnel Syndrome(CTS) is a common entrapment neuropathy of the median nerve at the wrist. An Electrophysiologic study has been widely used for the diagnosis of carpal tunnel syndrome. The subjects of this study were 48 cases (88 hands) with clinically suspected carpal tunnel syndrome who underwent electrodiagnostic examination from Jan 1, 2001 to Sep 30, 2001, The results were as follows: 1. Among 48 persons with a clinically suspected carpal tunnel syndrome, 40 patients were female 83.33$\%$ and the patients who are above 60 years old were 37.50$\%$. 2. Electrodiagnostic results were 22 cases (45.84$\%$) with bilateral carpal tunnel syndrome and 10 cases (20.83$\%$) with normal. 3. Physical findings consisted of tingling sensation in 48.86$\%$ of the involved hands, positive Phalen's Sign in 20.46$\%$ of them, thenar atrophy in 15.91$\%$ of them, and weakness in 14.77$\%$ of them. 4. Electrophysiologic studies showed a decreased sensory conduction velocity in 20 cases (22.73$\%$) of total hands, a prolonged latency in 3 cases (3.41$\%$) of them, abnormal sensory and motor fiber in 33 cases (37.50$\%$) of them, and normal in 27 cases (30.68$\%$) of them. Considering above results, we had better make a diagnosis precisely the patients with clinically suspected carpal tunnel syndrome through subjective symtoms, physical examinations, and electrophysiologic studies.
Process of screw-pumping system (SPS) was optimized for mass production of encapsulated bifidus. SPS entrapment device was composed of feeding component, with optimized nozzle size and length of 18G (0.91 cm) and 4 mm, respectively, screw pump, and 37-multi-nozzle. Screw component had five wing turns [radius (r)=26 to 15 mm] from top to bottom of axis at 78-degree angle from middle of the screw, and two wings were positioned at screw edge to push materials toward nozzle. For nozzle component, 37 nozzles were attached to 20-mm round plate. Air compressor was attached to SPS to increase productivity of encapsulated bifidus. This system could be operated with highly viscous (more than 300 cp) materials, and productivity was higher than $1128\;{\pm}\;30\;beads/min$. Viability of encapsulated bifidus was $5.45\;{\times}\;10^8\;cfu$/bead, which is superior to that of encapsulated bifidus produced by other methods ($2.51{\times}10^8\;cfu$/bead). Average diameter of produced beads was $2.048\;{\pm}\;0.003\;mm$. Survival rate of SPS-produced encapsulated bifidus was 90% for Simulator of the Human Intestinal Microbial Ecosystem test and 88% in fermented milk (for 14 days). These results show SPS is effective for use in development of economical system for mass production of viable encapsulated bifidus.
글루코스 옥시다아(GOx)제 고정화 바이오센서를 두 가지 방법으로 제조 하였다. 첫 번째 방법은 폴리(maleic anhydride) 그래프트 탄소나노튜브(PMAn-g-MWCNT) 전극에 감마선 조사법으로 제조 된 Au 나노입자를 물리적으로 흡착시킨 후, GOx을 고정화 시켜 바이오센서를 제조한 경우이고, 다른 하나는 PMAn-g-MWCNT 전극에서 Au 이온을 전기화학적으로 환원시켜 Au 나노입자를 코팅 시키고, 그 위에 GOx을 고정화 시켜 바이오센서를 제조 한 경우이두. 제조된 바이오센서에 대해 효율 평가를 수행 하였는데, 물리적 흡착법으로 제조된 전극의 경우 검출 범위는 $30\;{\mu}M\sim100\;{\mu}M$이었으며, 검출한계는 $15\;{\mu}M$이었다. 또한 ascorbic acid와 uric acid에 대한 검출한계는 7.6%이었다. 물리적으로 Au 나노입자가 흡착된 전극의 경우가 글루코스 측정에 매우 우수한 전극임을 확인 하였다.
This essay offers a historically based critique of the idea of "alternative modernities" that has acquired popularity in scholarly discussions over the last two decades. While significant in challenging Euro/American-centered conceptualizations of modernity, the idea of "alternative modernities" (or its twin, "multiple modernities") is open to criticism in the sense in which it has acquired currency in academic and political circles. The historical experience of Asian societies suggests that the search for "alternatives" long has been a feature of responses to the challenges of Euromodernity. But whereas "alternative" was conceived earlier in systemic terms, in its most recent version since the 1980s cultural difference has become its most important marker. Adding the adjective "alternative" to modernity has important counter-hegemonic cultural implications, calling for a new understanding of modernity. It also obscures in its fetishization of difference the entrapment of most of the "alternatives" claimed--products of the reconfigurations of global power--within the hegemonic spatial, temporal and developmentalist limits of the modernity they aspire to transcend. Culturally conceived notions of alternatives ignore the common structural context of a globalized capitalism which generates but also sets limits to difference. The seeming obsession with cultural difference, a defining feature of contemporary global modernity, distracts attention from urgent structural questions of social inequality and political injustice that have been globalized with the globalization of the regime of neoliberal capitalism. Interestingly, "the cultural turn" in the problematic of modernity since the 1980s has accompanied this turn in the global political economy during the same period. To be convincing in their claims to "alterity", arguments for "alternative modernities" need to re-articulate issues of cultural difference to their structural context of global capitalism. The goal of the discussion is to work out the implications of these political issues for "revisioning" the history and historiography of modernity.
Since vitamin $D_3$ is an important regulator of osteoblastic differentiation, a presently-established vitamin $D_3$-entrapped calcium phosphate film (VCPF) was evaluated for hard tissue engineering. The entrapped vitamin $D_3$ more rapidly induced bone nodule formation. To characterize the cellular events leading to regulations including faster differentiation, signal transduction pathways were investigated in osteoblastic MG63 cells at a molecular level. Major signaling pathways for MG63 cell proliferation including phosphatidylinositol-3-kinase, extracellular signal-regulated kinase, c-Jun N-terminal kinase and focal adhesion kinase pathways were markedly down-regulated when cells were cultured on calcium phosphate film (CPF) and VCPF. This agreed with our earlier observations of the immediate delay in proliferation of MG63 cells upon culture on CPF and VCPF. On the other hand, the p38 mitogen-activated protein kinase (p38 MAPK) and protein kinase A (PKA) pathways were significantly up-regulated on both CPF and VCPF. CPF alone could simulate differential behaviors of MG63 cells even in the absence of osteogenic stimulation and entrapment of vitamin $D_3$ within CPF further amplified the signal pathways, resulting in continued promotion of MG63 cell differentiation. Interplay of p38 MAPK and PKA signaling pathways likely is a significant event for the promotion of differentiation and mineralization of MG63 cells.

Reliable strength assessment of the Liquefied Natural Gas (LNG) cargo containment system under the sloshing impact load is very difficult task due to the complexity of the physics involved in, both in terms of the hydrodynamics and structural mechanics. Out of all those complexities, the proper selection of the design sloshing load which is applied to the structural model of the LNG cargo containment system, is one of the most challenging one due to its inherent randomness as well as the statistical analysis which is tightly linked to the design sloshing load selection. In this study, the response based strength assessment procedure of LNG cargo containment system has been developed and proposed as an alternative design methodology. Sloshing pressure time history, measured from the model test, is decomposed into wavelet basis function targeting the minimization of the number of the basis function together with the maximization of the numerical efficiency. Then the response of the structure is obtained using the finite element method under each wavelet basis function of different scale. Finally, the response of the structure under entire sloshing impact time history is rapidly calculated by synthesizing the structural response under wavelet basis function. Through this analysis, more realistic response of the system under sloshing impact pressure can be obtained without missing the details of pressure time history such as rising pattern, oscillation due to air entrapment and decay pattern and so on. The strength assessment of the cargo containment system is then performed based on the statistical analysis of the stress peaks selected out of the obtained stress time history.
