This study was performed to determine the effects of anastomosis on the internal diameter and wall thickness of jugular vein. Tro shepherd dogs were used for this experiment. In dog 1, xylazine(2 mg/kg) and ketamine(5.5 mg/kg) were injected intramuscularly for induction followed by enflurane(3%) anesthesia. In dog 2, acepromazine(0.03 mg/kg) and ketamine(5 mg/kg) were injected intravenously for induction followed by enflurane(3%) anesthesia. The dogs were heparinized(1 mg/kg) for the prevention of thrombosis. After jugular vein was exposed by incision of left cervical area, two Johns Hopkins bulldog clamps were clamped. Jugular vein was cut between two clamps, and it was reanastomosed using 5-0 silk. Ultrasonography was done along the jugular vein on both sides of each dogs 21 days after anastomosis surgery. The internal diameter and circumference of the vein in the anastomosis area were markedly reduced with thickening of the vein wall, but no thrombi were observed.
Intravenous anesthesia was compared with inhalation anesthesia in 20 patients of oral and maxillofacial surgery. The patients were randomly assigned to two treatment groups so that 20 patients were injected ketamine and propofol and 20 patients were administered enflurane. The hemodynamic responses of patients and recovery profile of the two groups were compared. Intravenous anesthesia group awoke significantly faster than inhalation anesthesia after operation. Time to full recovery in intravenous anesthesia group was significantly shorter than that of inhalation anesthesia group. We conclude that intravenous anesthesia is a practical technique for oral and maxillofacial surgery patients and intravenous anesthesia may be more preferable because of the significant shortness of recovery time.

Objectives : Interruption and subsequent restoration of blood flow into the kidney result in renal injury. As an approach to preventing the renal injury, we determined the optimal conditions and the underlying mechanisms by which supernatant of hot water extract of ground Triticum aestivum L. (extract) attenuated ischemia/reperfusion (I/R) injury. Methods : One hour after administration of the extract (400 mg/kg) by intraperitoneal injection, renal I/R injury was generated by clamping the left renal artery in rats after surgical removal of the right kidney, followed by reperfusion. The maximal difference between the vehicle-treated and the extract-treated group under ketamine/xylazine or enflurane anesthetization was assessed at varying periods of ischemia (30-45 min) and reperfusion (3-48 hr), based on the renal function assessed with serum creatinine levels, tissue injury with hematoxylin/eosin staining, and apoptosis with terminal deoxynucleotidyltransferase-mediated dUTP nick-end labeling staining. Results : Enflurane anesthetization with 40 min of ischemia and 24 hr of reperfusion was identified to be the optimal condition, under which condition serum creatinine levels and tubular damage in the extract-treated group were significantly reduced compared with those in the vehicle-treated group ($1.3{\pm}0.2$ versus $2.7{\pm}0.3$ mg/dL, P < 0.01, and average score $1.8{\pm}0.1$ versus $3.5{\pm}0.3$, P < 0.01, respectively). These beneficial effects were mediated by inhibition of apoptotic cascades through attenuation of renal tissue malondialdehyde levels, Bax/Bcl-2 ratio and caspase-3 levels. Conclusions : The extract conferred renal protection against ischemia/reperfusion injury in rats by scavenging reactive oxygen species and consequently blocking apoptotic cascades, plausibly augmented by enflurane protection.
Intravenous anesthesia was compared with inhalation anesthesia in 20 patients of oral and maxillofacial surgery. The respond of patients consciousness and general recovery condition of the two groups were compared. Intravenous anesthesia group were awake significantly faster without complications such as nause, vomiting, and agitation after operation than inhalation anesthesia group. Full recovery time of intravenous anesthesia group was significantly 3 times less than inhalation anesthesia group. The authors conclude that intravenous anesthesia is a practical technique for oral and maxillofacial surgery patients undergoing and may be preferable to intravenous anesthesia because of the significantly short of recovery time without complications.
Chloral hydrate단독 경구투여는 소아치과 환자의 진정에 가장 널리 사용되고 있는 방법중의 하나지만 실패하는 경우도 드물지 않게 보게된다. Chloral hydrate의 처음 투여용량이 실패할 경우 추가 용량을 투여하기보다는 다른 약물의 투여가 바람직하며, 이는 안전성을 그대로 유지하면서 보다 효과적인 진정에 도달할 수 있는 장점이 있다. 본 실험의 목적은 chloral hydrate의 수면효과가 나타나지 않는 소아환자에게 치료를 연기하지 않고 즉시 안전하게 사용할 수 있는 방법을 재발하기 위한 노력의 일환으로 아산화질소-산소와 enflurane 혼합가스를 이용한 초기 수면유도법의 효과를 검증하기 위함이었다. 의식하 진정을 이용한 치과치료가 예정된 건강한 환자에게 kg당 70mg의 chloral hydrate를 경구투여하고 조용한 방에서 60분이 경과한 다음, 환자가 수면상태에 이른 경우를 진정효과의 성공으로 평가하고 계획된 치료를 수행하였다(I군). 수면에 이르지 않아 실패로 평가된 환자는 50%의 아산화질소와 2vol%의 enflurane이 혼합된 가스를 약 $1\sim2$분간 투여하여 초기 수면유도를 시행한 후 예정된 치료를 수행하였고(III군), 다음 내원에는 chloral hydrate와 hydroxyzine(2mg/kg)을 병용투여 하였다(III군). 대조군과 실험군 각각 35명씩 총 70명 (남자 44명, 여자 : 26명)의 환자가 실험대상으로 사용되었으며 이들의 평균 연령은 $35.9{\pm}8.9$개월 체중은 $14.4{\pm}2.2kg$이었다. 치료과정을 10단계로 나누어 환자의 생징후(말초 동맥혈 산소 포화도, 심박수, 수축기 및 이완기 혈압)를 측정하였으며, 행동양상은 video촬영하여 역시 10단계로 나누어 1명의 평가자가 OSU behavior rating scale과 Automated Counting System을 사용하여 평가하였다. 생징후의 경우 각 군 모두 정상범위 내에서 안정된 양상을 보였으나, 개구기 삽입이나 국소마취 단계에서 발생하는 자극에 대한 말초 동맥혈 산소포화도와 심박수의 반응정도가 I군에 비해 II군에서 낮게 나타났다(p<0.05). 임상적으로 바람직한 행동양상(Q : Quiet)의 비율이 III군에 비해 II군에서 높게 나타났으며(p<0.05), II군과 I군간에는 유의한 차이를 보이지 않았다 (p>0.05). 본 실험에서 시도한 두 가지 진정요법이 비교적 높은 임상적 치료 성공률(II군 : 97.14%, III군 : 88.57%)을 보여 만족할 만한 결과를 나타낸 것으로 평가되었다.
It is one of difficulties to control children who show definitely negative behavior in dental clinic. In such a case, the pharmacologic management has been used to provide quality care, minimize the extremes of disruptive behavior, promote a positive psychologic response to treatment and patient welfare and safety. Deep sedation can be defined as a controlled, pharmacologically-induced state of depressed consciousness from which the patient is not easily aroused which may be accompanied by a partial loss of protective reflexes. In this retrospective report, the sedation records of 200 pediatric dental patients of ASA Class I & II who were not successfully treated under conscious sedation were used for analysis. Most frequently used regimen of deep sedation was the co-medication of midazolam(0.3mg/kg), enflurane(1.0-2.0 vol%) and 50-70% $N_2O_2$. The average age and weight of the patients was 4.6 yr (S.D: 2.72) and 18.7kg(S.D: 6.35) respectively. The average operative time was 52 minutes and midazolam (0.1-0.2cc) was additionally administered intranasally to prolong the operative time as needed. The episodes of untoward side effects were reported during and/or after the procedure in 58 patients. Serious adverse reactions such as cyanosis or laryngospasm were even reported in 7 patients but without mortality. Deep sedation is a very effective way of completing the dental treatments for those who failed to respond well to the conscious sedation. This technique has many practical advantages over general anesthesia case but the demands for the rigid monitoring criteria limit its use in general practice setting. The continuous efforts to improve the safety of the medication and the technique are required for the benefits of the patients and parent.
Purpose: Takotsubo cardiomyopathy is a relatively uncommon type of stress-induced cardiomyopathy characterized by transient left ventricular regional wall motion abnormalities. Emotional and physical stresses play a key role in this type of cardiomyopathy in postmenopausal women. The current hypothesis is that the syndrome represents a form of catecholamine surge due to stress or epinephrine-mediated acute myocardial stunning. Methods: A 44-year-old woman had suffered premature ventricular contraction following a cardiogenic shock during a breast augmentation surgery under enflurane anesthesia and tumescent solution infiltration. She was treated with cardiopulmonary resuscitation at a local clinic. Then she was brought to the Emergency Department of the authors' hospital. Results: The woman's echocardiogram showed an ejection fraction of 20~25% with associated basal hyperkinesis and left ventricular apical ballooning. The patient was admitted to the ICU and required inotropic support for two weeks. The patient's condition dramatically improved, and her ejection fraction returned to 70%. Conclusion: It is believed that there were multiple triggering factors of the onset of Takotsubo cardiomyopathy in the woman's social and family history, including infiltration of a large volume of the tumescent solution and VPCs induced by enflurane anesthesia without premedication. The importance of careful history-taking, careful pre-operative consultation on psychological suffering especially for breast surgery, premedication before surgery, patient reassurance, and post-operative psychosocial and emotional assistance was again seen in this case.
A 4 years old male mixed breed dog, which weighed 25.5 kg, referred to Veterinary Teaching Hospital at College of Veterinary Medicine in Chonnam National University, because of chronic hindleg lameness. Click sound by flexion and extension of stifle joint, positive reaction by cranial drawer movement, fat pad sign and tibial position in cranial drawer position on the lateral radiographic view, medial patellar luxation (grade II), and lameness score 2.5 at standing and 2 at walking in right hindleg were showed. Under general anesthesia with enflurane, after medial arthrotomy, it was performed to remove remnants of cranial cruciate ligament and torn medial menisci and joint closed. In modified retinacular imbrication technique, one lateral fabellar/tuberosity suture, one medial fabellar/tuberosity suture, and one imbrication suture adjacent to the patella were placed. At 9 days after operation, lameness score 0 was observed and general condition was excellent.
IV Deep sedation is useful for Waardenburg syndrome Child undergoing extensive dental treatment. We experienced a case of dental treatment under W deep sedation in a 3 years old boy with Waardenburg syndrome. He was treated on an outpateint basis. He was diagnosed hypothyroidism and received thyroid hormone. Premedication was done using ketamine and atropine IM. Induction using $N_2O$-enflurane mask ventilation was uneventful and intravenous cathetering was carried out. IV deep sedation was maintained with midazolam-$N_2O$ combination. After monitoring the patient for 2 hours and confirming his recovery, he was discharged from the day care unit. In summary, we report this successful IV Deep sedation management of Waardenburg syndrome child during dental treatment in as out patient.
