THREE-DIMENSIONAL FINI6E ELEMENT ANALYSIS OF THE ENDOSSEOUS IMPLANT DESIGNS (삼차원 유한요소 해석에 의한 골내 임프란트의 구조에 관한 연구)

  • The stress distribution generated in the surrounding bone was calculated and compared for various geometry of the dental implants by means of the finite element methods. The models were designed to represent the screw type endosseous implants(varing the size, shape, direction of the screw thread and the angle of the body) with supporting bone and the cylinder type endosseous implants(varing the lower portion-Round type, tapered type) with supporting bone. Static mean bite forces were applied 100N vertically and 25N horizontally on the center of the implant and three dimensional finite analysis was undertaken using software ANSYS 5.1 Version. The result demonstrated that different implant shape leads to significant variations in stress distribution in the bone. In the case of variation of the screw size, direction and shape the implant model with normally directional and triangular screw implied lower stress than with upper directional or lower directional and quadrangular screw but among models a different screw size, within a variation of 0.2mm there was no meaningful difference in maximum stress. In the case of variation of angle of body the straight implied lower stress than the tapered. As a result of analysis of cylinder type, the implants with larger radius of curvature of the round form and larger diameter of the tapered form implied lower stress.

Three Dimensional Optimum Design of Endosseous Implant in Dentistry by Multilevel Response Surface Optimization (다단계 반응표면법을 이용한 치과용 임플란트의 3차원 형상최적설계)

  • In this paper, an optimum design problem for endosseous implant in dentistry is studied to find best implant design. An optimum design problem is formulated to reduce stresses arising at the cortical as well as cancellous bones, in which sufficient design parameters are chosen for design definition that encompasses major implants in popular use. Optimization at once (OAO) with the large number of design variables, however, causes too costly solution or even failure to converge. A concept of multilevel optimization (MLO) is employed to this end, which is to group the design variables of similar nature, solve the sub-problem of smaller size for each group in sequence, and this is iterated until convergence. Each sub-problem is solved based on the response surface method (RSM) due to its efficiency for small sized problem.

  • Purpose: Placement of endosseous implants in the atrophic maxilla is often restricted because of the lack of supporting bone. In this article, augmentation of the maxillary sinus floor with deproteinized bovine bone to enable insertion of endosseous implants is described. The technique is aimed at providing a cortical layer on top of the graft to ensure a reliable seal of the maxillary sinus and to achieve optimal stability of the bone graft in case of simultaneously placement of dental implants. Methods: The procedure was used in 200 patients (839 implants), using deproteinized bovine bone. The mean follow-up was 28.5 months. No inflammation of the bone grafts nor of the maxillary sinus occurred. The patients received implant supported overdentures or bone-anchored bridges. Results: The survival rate of implant restoration of this study was 97.6%. The total average of marginal bone loss in radiographs was $0.20{\pm}0.38$ mm. Insufficient primary stability, bony quality, and infection were thought to be associated factors in the failed cases. Conclusion: This study documented that deproteinized bovine bone, when used as a grafting material for augmentation of the sinus floor, may lead to proper osseointegration of a endosseous implant.

ALVEOLAR BONE LOSS & BONE DENSITY CHANCES DURING THE FIRST YEAR A COMPARATIVE STUDY ON FOUR TYPES OF ROOT-FORM ENDOSSEOUS DENTAL IMPLANT SYSTEMS (치근형 골내 임플란트의 초기 1년간 치조골 소실 및 골 밀도 변화에 관한 연구)

  • Regular radiographic examination has been considered an essential diagnositic method for osseointegrated dental implantation. This study investigated marginal bone loss through the measurement on periapical radiographs and changes in bone density through digital subtraction image radiographic method around 88 endoseous root-form dental implants in 43 human subjects. Four types of endosseous dental implants were investigated : Standard series, Mini series and Hex-lock system of Steri-Oss Dental implant system, and $Br{\aa}nemark$-type implant from 3i dental implant system, in a 3 month interval for a total period of 12 months. The results were as follows : 1. Rapid bone loss occurred in the first 3 months in all 4 groups, and the bone level stabilized at the first thread of the implant fixtures. Amount of bone loss for 12 months showed correlation with the length of the polished neck portion. 2. Most of the implant systems showed resorption of alveolar bone up to the polished neck portion although a long polished neck could delay the resolution. 3. Alveolar bone loss apical to the polished neck portion stabilized at the first thread of the fixtures with no correlation to either the time of exposure of the polished neck or types of implant systems. 4. No changes in bone density around the implant threads were observed throughout the experimental period. Bone density decreased at the marginal bone, and increased at the newly-formed alveolar crest. These results indicate that most of the alveolar bone loss occur within the first 12 months after installation of endosseous root-form dental implants resulting in the exposure of polished neck portions, and the bone level stabilizes thereafter at the first thread portions of the implant fixtures. The experimental period of 12 months seems insufficient for observing changes in bone density, and a long-term observation should be needed.

Assessment of trabecular bone changes around endosseous implants using image analysis techniques: A preliminary study

  • Purpose: The objective of this study was to assess the trabecular bone changes that occurred around functional endosseous dental implants by means of radiographic image analysis techniques. Materials and Methods: Immediate preoperative and postoperative periapical radiographs of de-identified implant patients at the University Dental Hospital of Manchester were retrieved, screened for specific inclusion criteria, digitized, and quantified for structural elements of the trabecular bone around the endosseous implants, by using image analysis techniques. Data were analyzed using SPSS version 11.5. P values of less than 0.05 were considered statistically significant. Results: A total of 12 implants from 11 patients were selected for the study, and 26 regions of interest were obtained. There was a significant increase in the bone area in terms of the mean distance between nodes (p=0.006) and a significant decrease in the marrow area in terms of the bone area (p=0.006) and the length of marrow spaces (p=0.032). Conclusion: It appeared that the bone around the implant underwent remodeling that resulted in a net increase in bone after implant placement.

COMPARATIVE STUDY ON THE MARGINAL BONE LOSS OF IMMEDIATE NONSUBMERGED AND SUBMERGED ENDOSSEOUS DENTAL IMPLANTS PLACED INTO EXTRACTION SOCKETS OF DOGS (발치후 즉시 매식한 Nonsubmerged와 Submerged 임플랜트의 변연골 흡수양상에 관한 비교연구)

  • The conventional osseointegration protocol calls for waiting up to 12 months for ossification of an extraction socket to heal before placing an endosseous implant. In this study, the possibility of placing a pure titanium implant directly into an extraction socket immediately after extraction was investigated. And the marginal bone loss of immediate nonsubmerged and submerged endosseous dental implants placed into extraction sockets was also compared. Pure titanium Nobelpharma Branemark implants and solid screw type ITI implants were placed into premolar extraction sockets of two adult dogs and allowed to heal for a period of 3 months, followed by functional loading of the implant. Radiographic examination was performed before implantation, immediately after implantation and 3, 6, 9, 12 months after implantation. The results obtained were as follows : 1. Immediately placed nonsubmerged ITI implants and submerged Branemark implants showed favorable radiographic osseointegration status and there were minimum marginal bone loss. 2. There were no significant differences in radiographic finding of osseointegration between conventional and immediate implantation. 3. Gingival tissue around implants showed more inflammatory signs than that of adjacent natural teeth. This study suggest that pure titanium Branemark implants and submerged ITI implants have the potential to integrate when placed immediatly after extraction of the teeth and warrants further investigation.

  • The restorative treatment with dental implants in edentulous patients has been a well documented treatment modality proven in experimental studies and long-term clinical investigations. The aim of this paper is to introduce the implant mediated drug delivery system as a novel application of endosseous implants. The system is composed of hollow cylindric implants which has multiple microholes for drug delivery. For this purpose, the general outlines of drug delivery system and drug delivery route is discussed briefly. In addition, this paper deals with the results of experiments done up to now and the future perspective of the system.
