Endoscopic Removal of Esophageal Foreign Body in a Moluccan Cockatoo (Cacatua moluccensis) (몰루칸 앵무새에서의 내시경을 이용한 식도 이물 제거 일례)

  • A thirty-month-old male Moluccan Cockatoo (Cacatua moluccansis) with mild anorexia was referred. Through physical examination, fireign body was palpated at the crop region. Radiopaque, lineal foreign body was visualized on the lateral radiographs of the thoracic esophageal region. The patient was definitively diagnosed esophageal foreign body which is ingested feeding tube. The foreign body removal was undertaken using a flexible endoscope and a grasping forcep without any other complications. This case report demonstrated that successful esophageal foreign body removal with endoscopy in birds.

  • A capsular endoscope (CE) for inspection of the large intestine requires a motor for backward navigation against the autonomous travel in the intestine. This study proposes an HF power system for generating a magnetic field and for delivering wireless power to the internal or implanted medical devices. The magnetic field is generated by a wound coil (L) around a wooden frame, and the current is driven to the coil through a resonating capacitor (C). The characteristics of the resonance frequency shifting of the L-C series circuit are analyzed. A stable magnetic field intensity in the field coil is maintained by a specially designed frequency tracking system that automatically follows the L-C resonance frequency. Testing confirmed that the oscillation system tracks well the parameter changes of the electric components caused by the operating conditions or environmental variations.

  • 본 논문은 자유 시점조절과 깊이감 조절이 가능한 단일렌즈 양안시 3D 내시경과 Head-mounted display 를 사용한 새로운 로봇수술 시스템을 제안한다. 최근 들어서 여러 양안식 입체영상장치가 의료 영역에 적용되고 있다. 그러나 3D 의료 장비들은 zooming 과 자유로운 시점조절, 그리고 접사를 하는 데에 한계가 존재한다. 입체영상 장비에서 필연적으로 발생하는 이러한 문제점들은 3D 영화를 찍기 위해서 사용되는 것과 같은 2 대의 카메라와 2 개의 렌즈를 사용하는 데에서 원인을 찾을 수 있다. 이러한 문제점들은 Da Vinci 로봇 수술 시스템과 같은 가장 최신의 시스템에서도 해결되지 못하였다. 본 논문에서 제안하는 새로운 시스템은 지금까지 제시된 문제점들을 해결하고, 현재 존재하는 로봇 수술 시스템에 몰입 입체영상 수술이나 증강 현실 수술을 가능하게 하기 위한 목적으로 제안되었다.

Design and Development of Sputter-evaporation System for Micro-wiring on Medical Catheter (의료용 도뇨관 표면의 도선용 구리 박막 증착을 위한 스퍼터링-열증착 연속공정장비의 설계 및 개발)

  • Integrating micro-machined sensors and actuators on the conventional devices with the copper power lines was incompatible to fabricate the mass produced micro electromechanical system (MEMS) devices. To achieve the compatibility of the wiring method between MEMS parts and devices, we developed the three-dimensional sputter-evaporation system that coats micropatterned thin copper films on the surface of the MEMS element. The system consists of a process chamber, two branch chambers, the substrate holder, and a linear-rotary motion feedthrough. Thin copper film was sputtered and evaporated on the biocompatible polymer, Pellethane$^{circed{R}}$ and silicone, catheter that is 2 mm in diameter and 700 mm in length. The metal film coating technique with three-dimensional thin film sputter-evaporation system was developed to apply the power and signal lines on the micro active endoscope. In this paper, we developed the three-dimensional metal film sputter-evaporation system operated on the low temperature for the biopolymeric substrates used in the medical MEMS devices.

Microscopic Imaging of Articular Cartilage using Polarization-Sensitive Optical Coherence Tomography

  • We construct and test the polarization-sensitive optical coherence tomography (PS-OCT) system for imaging porcine and human articular cartilages. PS-OCT is a new imaging technology that provides information regarding not only the tissue structures but tissue components that show birefringence such as collagen. In this study, we measure the cartilage thickness of the porcine joint and the phase retardation due to collagen birefringence. Also, we demonstrate that changes of the collagen fiber orientation could be detected by the PS-OCT system. Finally, differences between normal and damaged human articular cartilage are observed using the PS-OCT system, which is then compared with the regular histology pictures. As a result, the PS-OCT system is proven to be effective for diagnosis of the pathology related to the cartilage. In the future, this technology may be used for discrimination of the collagen types. When combined with endoscope technologies, the PS-OCT images may become a useful tool for in vivo tissue testing.

Two Cases of Jejunal Gastrointestinal Stromal Tumor Diagnosed by Capsule Endoscope (캡슐내시경으로 발견된 공장의 위장관 간질성 종양 2 예)

  • Small bowel tumors have been difficult to diagnose because of low incidence and absence of specific symptoms. There are no efficient and accurate tests available for diagnosis. Capsule endoscopy is an efficient diagnostic tool for small bowel disease and obscure gastrointestinal bleeding. We diagnosed two cases of small bowel gastrointestinal stromal tumor (GIST) diagnosed by capsule endoscopy that were treated by surgery. A 68 year old male presented with abdominal pain. The capsule endoscopy showed fungating ulcer mass at the jejunum. A 55 year female presented with melena. The capsule endoscopy showed an intraluminal protruding mass with a superficial ulcer at the jejunum. Two cases were diagnosed with GIST after surgery. We report these two case diagnosed by capsule endoscopy and review the medical literature.

Sex Identification in Cinereous Vulture (Aegypius monachus) from Feather and Blood Samples: A Case Report

  • Twenty-four cinereous vultures that had been taken to a wildlife center due to starvation and exhaustion were studied to evaluate approaches for determining sex. Coelioscopy was performed to identify sexes of two vultures, whereas, DNA testing was performed to identify the sexes of the 24 vultures. Testes and ovaries could be unambiguously identified with an endoscope and DNA analyses could identify sex sex in most, but not all of the specimens. Although the coelioscopy examination can unambiguously confirm sex, the approach is invasive and requires anesthesia. Thus, coelioscopic examination should only be performed when sex cannot be determined through DNA analysis.

Magnetic Actuator for a Capsule Endoscope Navigation System

  • The authors propose a magnetic actuator for use as a navigation system for capsule endoscopes. The actuator is composed of a capsule dummy, a permanent magnet inside the capsule, and an external spiral structure. The device rotates and propels wirelessly when exposed to an external rotational magnetic field. In this study we measured the effect of the spiral shape on the velocity and thrust force properties. According to our experimental results, the actuator obtained a maximum velocity and thrust force when the spiral angle was set at 45 degrees, the number of spirals was set at 4, and the spiral-height was set at 1-mmf. We also conducted a motion test in the large intestine of a pig placed on a 30 degrees slope. The actuator passed through a 700 mm length of the intestine in about 300 s. The device also managed to travel up and down the 30 degrees slope with no difficulty whatsoever. Our results demonstrate the great potential of this actuator for use as a navigation system for capsule endoscopes.

Anomalous Muscles of the Wrist Encountered During Endoscopic Carpal Tunnel Surgery

  • Objective : Anomalous muscles of the wrist are infrequently encountered during carpal tunnel surgery. Anatomic variants of the palmaris longus (PL), flexor digitorum superficialis, lumbricalis and abductor digiti minimi (ADM) have been reported but are usually clinically insignificant. Anomalies of the wrist muscles, encountered during endoscopic carpal tunnel surgery have rarely been described. I conducted this study to evaluate muscular anomalies of the volar aspect of the wrist, encountered during endoscopic carpal tunnel surgery. Methods : I studied a consecutive series of 1235 hands in 809 patients with carpal tunnel syndrome who underwent single-portal endoscopic carpal tunnel release (ECTR) from 2002 to 2014. Nine hundred seventy-three hands in 644 patients who had minimal 6-month postoperative follow-up were included in the study. The postoperative surgical outcome was assessed at least 6 months after surgery. Results : In eight patients, anomalous muscles were found under the antebrachial fascia at the proximal wrist crease and superficial to the ulnar bursa, passing superficial to the transverse carpal ligament. Those anomalous muscles were presumed to be variants of the PL or accessory ADM muscle, necessitating splitting and retraction to enter the carpal tunnel during the ECTR procedure. Other muscle anomalies were not seen within the carpal tunnel on the endoscopic view. The surgical outcome for all eight wrists was successful at the 6-month postoperative follow-up. Conclusion : Carpal tunnel surgeons, especially those using an endoscope should be familiar with unusual findings of anomalous muscles of the wrist because early recognition of those muscles can contribute to avoiding unnecessary surgical exploration and unsuccessful surgical outcomes.

Delayed rupture of a posttraumatic retromaxillary pseudoaneurysm causing massive bleeding: a case report

  • Posttraumatic pseudoaneurysm of the face is caused by blunt, penetrating, or surgical trauma. Although its incidence is low, pseudoaneurysm rupture can cause a life-threatening, massive hemorrhage. A 48-year-old man visited our emergency center due to a fall-down accident. Three-dimensional computed tomography (CT) showed a comminuted zygomaticomaxillary complex fracture of the left face. After open reduction and internal fixation, the surgical wound healed without any complications. However, the patient was readmitted 10 days after surgery due to pus-like discharge from the wound. Contrast-enhanced CT to find the abscess unexpectedly revealed a pseudoaneurysm in the left retromaxillary area. Massive oral bleeding occurred on the night of re-hospitalization and emergency surgery was done. The bleeding site was identified as a pseudo-aneurysmal rupture of the posterior superior alveolar artery in the retromaxillary area. Hemostasis was achieved by packing Vaseline gauze in the maxillary sinus using an endoscope. Delayed rupture and massive bleeding of posttraumatic retromaxillary pseudoaneurysm after a zygomaticomaxillary fracture is a low-probability, but high-impact event. Therefore, additional contrast-enhanced CT should be considered to evaluate the possibility of a posttraumatic pseudoaneurysm in cases of severe comminuted zygomaticomaxillary fracture.
