Case Report of Temporomandibular Joint Disorder Patient (Functional Lateral Mandibular Displacement) with Many Clinical Symptoms Treated by Chuna & General Oriental Method (추나요법을 이용한 악관절장애로 인한 전신증상을 동반한 하악관절편위환자 치험 1례)

  • Objectives : This study was carried out to investigate the effects of Chuna & General Oriental Method on Temporomandibular Joint Disorder Patient(Functional Lateral Mandibular Displacement) with Many Clinical Symptoms. Methods : The therapies were Chuna, acupuncture treatment and herbal medicine. We estimated the effects of treatment by measuring the length from the end of frenulum labii superioris to the frenulum labii inferioris, Visual Analog Scale(VAS) and Patient Global Assessment(PGA) before and after treatment. Results : After treatment, we confirmed these improvements: the length from the end of frenulum labii superioris to the frenulum labii inferioris changed from 10-7mm to 0-2mm, the VAS was changed from 10 to 0-2 and many clinial symptoms improved. Conclusions : These results suggusted that Chuna & General Oriental Method effected for Temporomandibular joint disorders caused many clinical symptoms.

  • Neuroma is formed by abnormal, incomplete nerve regeneration after nerve injury. A painful neuroma in the hand can be psychologically and physically disabling. The goal of treating painful neuroma is to relieve pain and to restore nerve function. A numerous treatment modality was reported for alleviating the problem. These treatments include crushing the neuroma, ligating it, burying in soft tissue, bone, and muscle, injecting it with alcohol, phenol, and steroid, capping it with silicone cuff. But, none of these methods has been uniformly successful, although each has its advocates. No one technique reliably prevents formation of a painful neuroma. However, the principles of treatment is resection of neuroma and proximal stump of the nerve is transposed to appropriate adjacent tissue. Our current technique was resection of neuroma with partial normal neural tissue, and then the nerve ending was transposed and sutured to the side of the proximal stump with 10-0 nylon, so end-to-side neurorrhaphy was made. The nerve ending had to be placed and fixed into the proximal nerve epineurium like as a figure of a loop. We believe this technique is another useful method for the treatment of painful neuroma.

Comparison of the Effectiveness of Sling Exercise and McKenzie Exercise in Patients with Acute Cervical Whiplash Associated Disorder Following Rear-end Collision

  • Purpose: This study compared the effectiveness of sling exercise and McKenzie exercise in patients with acute cervical whiplash associated disorder (WAD) caused by rear-end collision. Methods: Thirty WAD patients were randomly assigned to one of three groups; a sling exercise (SE) group, a McKenzie exercise (McE) group, and a control group. Members of the SE and McE exercised three times a week for four weeks under the researcher's guidance. Three groups performed TENS treatment three times a week for four weeks. Diagnostic radiological equipment was used to measure whiplash injury. Visual analog scale (VAS), neck disability index (NDI), and range of motion (ROM) were used in this study. Results: The three groups showed a significant pre-post treatment difference in measures of VAS and NDI (p<0.05). The SE group showed a significant pre-post treatment difference in measures of flexion and extension changes compared to the McE and control group (p<0.05). The SE group also showed significantly greater improvement in the VAS, NDI, and ROM changes than the control group (p<0.05). Conclusion: These findings indicate that sling exercise is a stronger and more aggressive intervention for treatment of acute WAD patients.

ESTIMATION OF RESIDUAL STRESS IN CYLINDER HEAD

  • Residual stresses are introduced in aluminum cylinder head during quenching at the end of the T6 heat treatment process. Tensile residual stress resulted from quenching is detrimental to fatigue behavior of a cylinder head when it is overlapped with stresses of engine operation load. Quenching simulation has been performed to assess the distribution of residual stress in the cylinder head. Analysis revealed that in-homogeneous temperature distribution led to high tensile residual stress at the foot of the long intake port, where high stresses of engine operation load are expected. Measurements of residual stress have been followed and compared with the calculated results. Results successfully proved that high tensile residual stress, which was large enough to accelerate fatigue failure of the cylinder head, are formed during quenching process at the end of heat treatment at the same critical position. Effect of quenching parameters on the distribution of residual stress in cylinder head has been investigated by choosing different combination of heat treatment parameters. It was demonstrated that changes of quenching parameters led to more homogeneous temperature distribution during cooling and could reduce tensile residual stress at the critical region of the cylinder head used in this study.

A Case Report of Partial Remission of End-stage Ovarian Cancer Patient with Lung Metastasis Treated with Carboplatin/Paclitaxel and Traditional Korean Medicine (Carboplatin/Paclitaxel과 한방치료를 병행하며 부분 관해 된 폐전이 동반 말기 난소암 1례)

  • Objectives: The purpose of this study is to report the effect of combination treatment of Traditional Korean medicine (TKM) and chemotherapy on a ovarian cancer with lung metastasis patient. Methods: One ovarian cancer with lung metastasis patient was treated by TKM in conjunction with Carboplatin/paclitaxel since Feb. 2020. Repeat cycle every 3 weeks for 6 times. The patient has been treated with TKM at the same time. To evaluate the patient, symptoms were measured by Numeric Rating Scale (NRS), Eastern Cooperative Oncology Group (ECOG) and tumor size was measured by scanning with Computed Tomography (CT). Blood tests including cancer biomarker were conducted during treatment. Adverse events were evaluated by the National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), version 5.0. Results: After treatment with Carboplatin/paclitaxel and TKM during 2 months, the size of the ovarian cancer was decreased(Partial Response, PR), size and malignant pleural effusion at right lung disappeared. And no evidence of newly developed metastatic lesions. After 2 months, the tumor response was stable disease while improving the performance and other symptoms. Conclusions: This case provides us conjunctive treatment with Conventional and Eastern medicine may have substantial benefit for patients with end-stage ovarian cancer.

A Preliminary Study to Improve Death Education for the Public and Medical Students after the Enforcement of the Life-Sustaining Treatment Decision Act (연명의료결정법 시행 이후 일반인과 의대생 대상 죽음교육 개선을 위한 예비연구)

  • Considering the recent medicalization of death, the importance of preparing both laypersons and medical students to have meaningful end-of-life conversations, which is among the objectives of death education, will grow. The Act of Hospice and Palliative Care and Decisions on Life-Sustaining Treatment for Patients at the End of Life provided a new source of momentum to death education for both laypersons and medical professionals, as the importance of education on death is widely recognized. However, problems remain regarding how to prepare people for productive conversations at the end-of-life and how to secure the continuity of care. Different focuses and deficiencies are observed in death education programs for each category of learner. In education for laypeople, tangible information on how to actualize one's existential and personal understanding of death through real-life options is lacking, except for presenting the "protocol" of the Act. Conversely, basic medical education lacks an understanding of or confrontation with death on the existential and personal levels. Death education should aim to build a shared understanding that can facilitate communication between the two groups. The scant overlap between layperson education and basic medical education even after the Act's enactment is worrisome. Further fundamental changes in death education are required regarding its content. Topics that patients and doctors can share and discuss regarding death and end-of-life care should be discovered and provided as educational content both to laypeople and future medical professionals.

Circunferential resection and direct end to end anastomosis of mediastinal trachea on a post tracheostomy stenosis (기관절개술후 종격동기관 협착증에 대한 기관절제 단단 문합술)

  • A 37 year old male patient was suffered from severe labored breathing caused by post tracheostomy stenosis, which was localized at the mediastinal trachea [cuffed tracheal stenosis] and ranged 1.5 cm in length and approximately 3 ram. in diameter on tracheogram. After dilation of tracheal stenosis with dilator, endotracheal intubation was tried for induction of anesthesia and control of respiration during operation. A tube was placed just beyond the tracheal stenosis without respiratory difficulty. Under the endotracheal anesthesia, circumferential resection of the mediastinal trachea containing the stenosis, approximately 2 cm in length [4 tracheal rings}, was carried out and primary direct end to end anastomosis was performed with interrupted submucosal sutures [3-0 Dexon] and mobilization of trachea Postoperative tracheostomy was not performed. The patient was completely relieved from dyspnea immediately after operation. Post-operative convalescence was entirely uneventful and at present, about 3 months after operation, he is now conducting a usual life. From the literature and our experience, the etiology and treatment of post-tracheostomy stenosis were discussed.

  • This paper describes research about the realization of a diagnosis system for the surgical operation of the acupuncture. The system consists of the Munjin, Bokjin and Makjin software. The Munjin system diagnoses the pain spots or each Kyoungnats of the hands, root and neck. The Bokjin system diagnoses pressure-pain spots of the twelve Kyoungnak at chest and abdomen by following the displayed positions on the monitor in sequentially. The Maekjin system diagnoses radial pulse or inyoung and chongu according to the Inyoung-Chongu comparison in ethod. Compound diagnosis verifies the results which obtained from each diagnosis routine by the interface rule. This developed system contains the database which for the paitents diagnosis and treatment process recoding.

The Significance of 3-Dimensional Imaging in Tracheal Stenosis (기관협착증에서 3차원적 영상 진단의 의의)

  • Three-dimensional reconstruction of computed tomographic image(3D CT) is a well-established imaging modality which has been investigated in various clinical settings. It is commonly performed in case of congenital or developmental abnormalities, and traumatic fracture of skull and face that requires reconstruction of osseous structure. However reporting the 3D CT in laryngeal or tracheal stenosis is rare and its results are obscure. The authors performed 3D CT in six cases of tracheal stenosis and found diagnostic value of 3D CT. A Comparision of diagnostic information obtained from plain X-ray, 2D CT and 3D CT has performed in total six cases of tracheal stenosis. Surgical treatment of the tracheal stenosis was following in these cases : tracheal end to end anastomosis In 1 case, laryngotracheal end to end anastomosis in 2 cases. 3D CT information was compared with operative finding. In two of six cases, satisfactory information was not obtained from 3D CT in evaluating an exact stenosis of trachea. Future, it will be helped in evaluating of tracheal stenosis by 3D CT.

Effect of Sustained-Release of Adenosine by Elvax 40P Implant on Mouse Mammary Development (생쥐의 유선발달에 있어서 Elvax 40P Implant를 이용한 서방형 Adenosine의 역할)

  • To examine local response of adenosie(purine nucleoside) on the developing mammary gland, Elvax 40P implants containing adenosine were surgically implanted into mammary fat pad of the five week old female ICR mice. Inguinal(the 4th) mammary glands of anesthetized mice were exposed andplaced the implants for 12 days. One gland was treated with an adenosine implant, while the contralateral gland received a blank implant as control. For whole-mount preparations, glands were stained with alum carmine, and for histological observation, micro-selected mammary glands were stained with hematoxylin and eosin Y. Implantation with Elvax 40P did not affect on the damage of neighboring mammary tissue. Adenosie 25 or 250$\mu\textrm{g}$ per slow-release implant stimulated local mammary end bud formation of ovariectomized mice such as end bud size and numbers of end bud per gland in a dose dependent manner(P<0.05), and lower concentration of adenosie(2.5 or 25$\mu\textrm{g}$/implant) increased numbers of end bud(P<0.05) and end bud size(P<0.1) of intact mice. Adenosine treatment and intact ovarian function had moderate interation effects on the stimulation of end bud formation at 2.5$\mu\textrm{g}$ adenosine/implant(P<0.1). In histological observation, adenosine implants increased numbers of mammary epithelial type of cells at mammary duct in the presence or absece ofovary. These results indicate that adenosine should be one of regulators in mouse mammary ductal growth.
